Here is a simple tutorial on how to unlock bootloader and root your Htc One V.
Do this AT YOUR OWN RISK and I don't guarantee it will work on all Htc One V (but successfully done on my Htc One V (PrimoU)).
First: Credits go to:
1) paulobrien for his tools & guide
2) bagofcrap24 for the stolen unlocking bootloader tutorial..
Second: It is good to have official RUU for your Htc One V version. You can find the official RUU HERE(credit : JMZ) & HTCSync in case you something goes wrong and you need to revive your One V.
Or make a nandroid backup after you successfully unlock the bootloader. How ? Read further below. So read fully before you attempt to unlock bootloader and rooting.
Third: the tutorial
Unlocking Your Bootloader
Note: After doing unlocking bootloader, all your data/apps are gone so make a backup to your PC first.
You will need to run the initial setup again.
1. Go Here "http://www.htcdev.com/" and create yourself an account on HTCDEV.
2. Download These Fastboot Files
3. Make a new folder on your C: drive called fastboot.
4. Extract The Zip downloaded in step 2 to the new folder
5. Go Here http://www.htcdev.com/bootloader/ and select "All Other Supported Models" and "then begin unlock bootloader"
6. Yes, Tick both Boxes, Proceed
7. Skip up to step 8 as we have all we need for the first 7 steps
8. Power off your phone by holding the power button down for 5-7 seconds (the three keys will flash a few times then it will power off)
9. Hold volume down button, the device will boot to HBOOT
10. Select Fastboot With Volume+or- then press Power once (you'll be in fastboot mode)
11. plug your phone into your PC and open a Command Prompt.
(windows XP - Click Start > Run > type CMD in box then press OK
(Windows 7 - Click Start > type CMD in search box then press Enter)
12. cd to your folder where we put fastboot files cd c:\fastboot
13. type this into your command prompt window fastboot oem get_identifier_token
14. select & copy the text as per step 9 on the HTCDev website
15. paste your identifier token into the box at the bottom and submit
You Will Very Shortly Receive an email from HTCDev with a link to the second part of the instructions and your "Unlock_code.bin" attached
16. download the "Unlock_code.bin" from the email and save it to the same folder that we extracted fastboot files to in step 2
17. type this into your command prompt window fastboot flash unlocktoken Unlock_code.bin
18. The Display Will change on your phone, press Vol + to accept and power to confirm.
Congratulations You Have Successfully Unlocked Your Bootloader
UPDATE: Read the IMPORTANT note added below
Rooting with Superboot for GSM Device (Primo U)
1. Download r1-primo-superboot.zip to your computer and extract in any directory.
2. Download the modded boot.superboot.img
3. Replace the boot.superboot.img in the folder that you extracted just now (r1-primo-superboot).
Or download one of the Superboot that relevant to your device HERE
4. Power off your phone by holding the power button down for 5-7 seconds (the three keys will flash a few times then it will power off)
5. Hold volume down button, the device will boot to HBOOT
6. Select Fastboot With Volume+or- then press Power once (you'll be in fastboot mode)
7. Connect your phone to PC
8. Double click ‘install-superboot-windows.bat’
Rooting with Superboot for CDMA Device (Primo C)
1. Download r1-primo-cdma-superboot.zip to your computer and extract in any directory.
2. Power off your phone by holding the power button down for 5-7 seconds (the three keys will flash a few times then it will power off)
3. Hold volume down button, the device will boot to HBOOT
4. Select Fastboot With Volume+or- then press Power once (you'll be in fastboot mode)
5. Connect your phone to PC
6. Double click ‘install-superboot-windows.bat’
Once it complete, the phone will reboot on its own. In app drawer you will see a SuperSU icon, click on it and update SU binary then you are done with rooting your Htc One V.
You can try any app with root access to confirm it is working.

IMPORTANT
As some people forget to make a nandroid backup for their stock ROM, after they successfully unlocked their device,
when a soft brick happens, it will be a big problem to be back on stock when they are unable to find the official RUU for their device.
So make sure, after successfully unlocked your device, don't do anything else yet, flash the CWM recovery and make a nandroid, make nandroid and nandroid of your stock ROM.
Download the Recovery for GSM Device:
CWM 5 - http://forum.xda-developers.com/showthread.php?p=28153112
Direct download link : http://download.clockworkmod.com/recoveries/recovery-clockwork-5.8.4.5-primou.img
CWM 6 - http://forum.xda-developers.com/showthread.php?t=1871243 (the latest version as at 05 Sep 2012)
Download the Recovery for CDMA Device:
CWM - http://forum.xda-developers.com/showthread.php?t=1749798
How to make a nandroid backup ?
Once the CWM recovery installed:
Press power once (to go to hboot)
press volume down/up to navigate the menu to RECOVERY, press POWER once
Once in the RECOVERY menu:
click backup and restore
click backup
the backup process is running. It will be a few minutes to complete.
once complete, check whether the backup is successful
click backup and restore
click restore
you will see your backup files, e.g: 2012-06-12.0.13.04
click Go Back or use the back button
click reboot system now to boot your device
once fully booted, mount your sdcard to PC, browse to \clockworkmod\backup and copy the 2012-06-12.0.13.04 folder to your PC as a second backup in case something is wrong with your sdcard.
2012-06-12.0.13.04 is an example only, you will get a different no.
Alternatively, you also can use TWRP recovery:
GSM version - http://forum.xda-developers.com/showthread.php?t=1721749
CDMA version - http://forum.xda-developers.com/showthread.php?t=1764183
Now you are ready to do the rooting process.
Once your device is rooted, you are free to do whatever you want to do with your device, to flash a custom ROM, a custom kernel... whatever.
If soft brick happens and there is no official RUU, a stock nandroid backup may helps you
It doesn't work !!!!
It stucks at -waiting for device- in
11. type this into your command prompt window fastboot oem get_identifier_token
It works perfectly when s off and root. But why cant i gain root access when i already have SU app?
I tried to update the SU but it failed on gaining root access and busybox list.
When i used root uninstaller app, it asked me for adb access which i dunno. What do i have to do?

I don't see why rooting is a development issue. Development is for, well, software development like ROMs and recoveries, etc. Rooting is required to develop, but isn't development per-se.
I am planning on buying this phone.
I just have a few questions.
What is S-off and S-on?
Also, what is RUU?

I just give a basic explanation:
S-Off - security off - you can change everything including hboot partition layout, radio, system, data & recovery
S-On - opposite of the above, all cannot be change (which normally with a locked bootloader)
S-On + unlocked bootloader - you can change only system, data & recovery
RUU - ROM Update Utility - and exe file which consist all the OS files to be installed to the phone.

How do you toggle S-On to S-off?
Why is it recommended to have the RUU you said one should have?

We have to wait until a developer finds an exploit.
To root, it is mandatory to have S-OFF?
Can I just make the root without the S-OFF?
Im here again:
How can I make S-OFF in my HTC One V, the HTCDev teaches seeks to remove the battery, but the HTC One V has internal battery (you can not remove)?
Someone who has made S-OFF on your HTC One V, could you help me?
(sorry my bad english, im speak portuguese only)...

You cannot do restore factory settings when you moved some system apks out from nand.
In order to flash a RUU:
1) you need an official RUU for your device.
2) you have htc usb drivers installed on PC.
3) relock the bootloader
in fastboot mode type fastboot oem lock
4) flash RUU
Do you have some kind of note of your actual software version number ?

Maybe I have to push system files again with adb?

You have a very old h-boot so you need to upgrade it first!!
So you need to flash the stock rom for your hboot or the ruu of your hboot and then when the hboot is 1.36 or 1.39 are the two best, you can flash a rom without problems!!
You dont have to unlock bootloader because is unlocked!

Ah i see in the Sprint section. Maybe it's good to mention that too in the first reply because this is the international section and therefore doesn't hold Sprint ruu files. So that confused me a bit. Thanks for the link
My bad I didn't realize that
Thanks both. Really appreciated your advice. I tried steps in below link

Flash Firmware via fastboot

But both ways failed.

Tried both ways below
download 2.16.654.4 RUU.zip
extract it and found 0P6BIMG.zip
move it to sd card and reboot via bootloader
Nothing happends
rename the RUU.zip file as firmware.zip
place it in adb directory
boot the phone into hboot mode and plug into pc and choose fastboot. it turns to fastboot usb
right click on the adb directory with shift and open the command promt
typed Fastboot oem rebootRUU
when see the black screen w/ HTC, typed Fastboot flash zip firmware.zip
it always gives error 99
Please help me to sort out this thing

Hi
I relock the bootloader and did the prrevious steps. Now its worked.
Now it shows "RELOCKED" in the bootloader menu.
Is it the normal sign ?
Yes as you are s-on that's normal. Only with s-off you can change those flags to LOCKED
Hello, I have root access on my HTC one Phone and that's it, I kept it as stock as possible
everytime I get an OTA I flash the stock recovery instead of TWRP to get the update then I reflash TWRP
for the 1st time I couldn't update to the latest one, kept on getting a red triangle with an exclamation mark
when I tried many things I found out that the recovery is corrupt (each time I try to enter stock recovery I get the red triangle with !)
Any suggestions on what I should do? I can't even flash RUU because of the recovery.

Transfer the OTA file from your downloads folder to your PC and open it with 7zip. Then open the firmware zip that is in it and find the recovery.img
Flash that to your phone. Then run the OTA again.

Interesting solution, will try it out.
Just to clarify, the OTA you've mentioned should contain the recovery of the new firmware I.e. for the Lollipop update.
Because I am still in kitkat and the OTA I want to flash is the Lollipop one, is it okay to flash a lollipop recovery on my kitkat ?

I did what you've mentioned and I couldnt update, I do not get a red triangle with an exclamation mark, and I can also load into the stock recovery.
but when I try to OTA update i get a message says there is an Error in the file (the OTA file) and also some kind of verification error.
I also tried to flash RUU but it gave me the following Error:
sending 'zip' (546333 KB)...
OKAY [ 21.860s]
writing 'zip'...
(bootloader) signature checking...
FAILED (remote: 12 signature verify fail)
finished. total time: 42.661s
Any idea whats happening? I relocked the bootloader to flash RUU.
Post result of fastboot getvar all (delete serial no. & imei no.)
"verification error" suggest that you may have wrong ROM on wrong device version.

Sure I will soon,
but I did not flash a ROM since I got the phone, only root access and TWRP recovery, iv been getting OTA updates with no issues before this one.
Even RUU doesn't work (although I never tried it before)

I tried RUU again, this time with a different file (I downloaded the earlier one from HTC dev page, but I still got the same error <(bootloader) signature checking...
FAILED (remote: 12 signature verify fail)
finished. total time: 115.740s>
Attached is my Phone Details and the error I get when I try updating by OTA.
In Summary, this is my situation:
Phone was rooted, but never flashed a ROM
Flashed recovery extracted from OTA file
UnRoot the phone from SuperSU settings (although my root checker still says i have root)
Relocked Bootloader to flash RUU
Can't Flash RUU I get the mentined error.
Cant update by OTA, i get the attached error
Im trying to resist flashing a ARHD rom, but kept it as a last resort
Any Ideas???
RUU from HTCDev page ? I guess that's not RUU but kernel source zip.
Your problem is not corrupted recovery but you have system files modified/removed as shown in your screenshot.
You have two choices here :
A. Restore system part only from a Nandroid
1. Unlock bootloader
2. Install TWRP 2.8.5.0 - http://techerrata.com/browse/twrp2/m8
command fastboot flash recovery openrecovery-twrp-2.8.5.0-m8.img
3. Download TWRP backup 3.28.401.9 - https://mega.co.nz/#!VQgnECRS!asb9fd5ZHF3lf0e0iqmw0QIEc3sYS7JDTjjzi-hXvuo
4. Download stock 3.28.401.9 recovery - http://sourceforge.net/projects/htconem8/files/Recovery/3.28.401.9_recovery.img/download
5. Extract the downloaded 3.28.401.9_ckpv5.zip on PC
6. Boot to TWRP recovery and make a backup of boot only, this is to see where the backup goes on your device.
7. Connect device to PC, select Mount, enable MTP then open Internal Storage, TWRP/BACKUPS/SerialNo./
8. Transfer the extracted 3.28.401.9 folder (not 3.28.401.9_ckpv5 folder) and its content to the backup path on your device
TWRP/BACKUPS/SerialNo./3.28.401.9
9. Restore the transferred backup - tick only boot & system - swipe to restore (this way you can keep your data)
10. In reboot select bootloader
11. fastboot flash stock recovery that you downloaded - command fastboot flash recovery 3.28.401.9_recovery.img
12. reboot - check for OTA, download, install (you should have one OTA to 4.16.401.10)
13. done
OR
B. Use a correct RUU to restore everything and even wipe your internal storage
Check @EddyOS thread for all files needed : http://forum.xda-developers.com/showthread.php?t=2735235
You can skip step 1 & 2 - but relock bootloader as your device is S-On.

I would rather use option B sounds much easier, plus i backed up my internal SD already. but I just want to confirm what you mean by correct RUU? I used the HTC Dev one first (like u said its not an RUU) but then I used one I downloaded from this XDA thread http://forum.xda-developers.com/showthread.php?t=2701376
the RUU file name is: 0P6BIMG_M8_UL_L50_SENSE60_MR_HTC_Europe_4.16.401.10_Radio_1.25.214500021.06G_20.68.4196t.01_F_release_417809_signed.zip
it gave me the same error I mentioned above (where it says verification error), should I try downloading another RUU file?
Check the thread that I linked. You need one more file htc_fastboot.exe and how to run it,
there are two different commands, 1- to put device in RUU mode, 2- flash the zip
You also can use RUU.exe instead of RUU.zip : https://drive.google.com/file/d/0B17smFr95pleeTQ5U1NzNkZzM0U/view
Just boot to bootloader, relocked bootloader and run the RUU.exe (select run as administrator in Windows Explorer context menu)

I have Fastboot installed, and I know the commands to flash RUU (thats what I used to flash it the time it failed)
Anyways, not I tried using the exe file you provided and it gave me the following:
ERROR [155]: UNKNOWN ERROR
the ROM Update Utility cannot update your Android Phone.
Please get the correct ROM Update Utility and try again.
This is very weird, thanks for your help, any other ideas?
Note: My bootloader is relocked using Fastboot oem lock command, I did it before running RUU but still get an issue!
ERROR 155 - bootloader is not relocked
Did you relock bootloader before you run RUU ? Is the RUU run in as Administrator ?
If you did relocked, then I'm run out of idea.
Left you with the nandroid way only
You can ask in that thread about RUU problem ... I'm more on nandroid type.

Yes I did, I relocked before trying both zip RUU and exe one.
When I go into bootloader it says **Relocked** on top.
That's why I'm confused why RUU isn't working.
Your problem is only one "reboot" file that you need the original one. I don't keep that version file on my PC, if not I can give you a flashble zip to flash only that one particular file.

To flash the RUU.zip you HAVE to use htc_fastboot.exe - NOT the one that you get from Google as it doesn't support the larger ZIP file size. If you try and use the Google one you'll get an error 12

I see, It will be great if I can get a flashable zip file.
but If you cant, do you reckon if I try these steps it might work?
1. Unlock my bootloader.
2. Flash TWRP
3. Flash Revolution ROM version 34.1 (lollipop).
4. Flash stock recovery (the version which is extracted from HTC's latest OTA).
5. Flash RUU using fastboot zip flash method.
What do you think?
Or should I follow the method you mentioned earlier?

No point ...
If you're going to do step 5, then no need step 1-4, what you need is use that htc_fastboot.exe not google fastboot.exe
Or
If you do have lollipop ROM, just extract out "reboot" file from system/bin and later replace this on your stock ROM. Use TWRP File manager to replace it and put permission chmod 755 (but I guess this is harder for you)
Cool, I'll try flashing with HTC_fastboot, didn't know there is a difference, hopefully it will work ?
