Use AmonRA instead of CWM.
Your phone is a PVT4 model probably. Or use newer version of CWM.

Okay so I have a DInc2 that will boot into recovery only (There is no rom installed)
Windows7 and Ubuntu see my 16GB SD that came with my DInc2 but my device doesn't.
I have tried formatting it Quick and FULL OVERWRITE (Over 2 Hours) and it sees no errors in Windows or Ubuntu
any time I try to access the SD card via recovery is says E:Can't mount /dev/block/mmcblk1p1 (or /dev/blocck/mmcblk1) (Invalid Argument) E:Can't Mount /SDcard
I have Amon Ra Style Recovery for Incredible 2 3.0.3 I was able to flash via Fastboot, but now Im lost.
Is there a way to flash a rom via adb/fastboot or how do I get a rom back on my device I am lost without my phone.
Help?
------EDIT------
Okay so I partitioned the SDcard In Amon Ra
Not 100% sure what I did but it worked
I set
Swap:1024MB
EXT2:1024MB
and Remainder automatily went to Fat32 but it sees my SD in recovery when I go to flash a new ROM
I had to format my are sd card from bone stock sense rom when mine did this

So my desire is stuck.
Rooted, running Redux 1.2, went to use it yesterday and the screen wouldn't turn on.
So I took the battery out, replaced and tried again, and it was stuck on a loop on the Redux boot screen.
Took the battery out and replaced again and booted phone into recovery, and tried to backup (or restore) and got the message:
E:Can't mount /dev/block/mmcblk0 (or /dev/block/mmcblk0p1)
(No such file or directory)
Can't mount /sdcard
So now can't boot into the phone and can't do anything in recovery it seems.
Anyone have any ideas please?
Your SD card might be corrupted. Test it to see if it is ok.

I installed CWM-based Recovery v5.0.2.6, but once i boot into the recovery mode, I get the following lines after the options:
CWM-based Recovery v5.0.2.6
E:Can't mount /cache/recovery/command
E:Can't mount /cache/recovery/log
E:Can't open /cache/recovery/log
E:Can't mount /cache/recovery/last_log
E:Can't open /cache/recovery/last_log
And I can't make a backup and get a message "Can't mount /system" followed by the same lines above.
What can solve my problem ?

Thanks for your reply, is there a link or something ?
Because I want a trusted tried link if you have one
Use CWM 5.0.2.7 cuz itachisama's cwm 5.0.2.8 is ext4 and i think the OP is still in stock rom. Stock roms filesystem is RFS

I found this one in this thread:
http://forum.xda-developers.com/showthread.php?t=1421650
"signed_cwm-5.0.2.7-flashablezip-ext4+rfs.zip (Flash from Recovery)"
It should work perfectly ?
And should I use "apply update from sdcard" or "install zip from sdcard" ?
Done, works perfectly ^^
