I just received my Nexus 7 2013 16gb WiFi from amazon a few days ago, and it's been great. The recovery has had issues though, it tried to do an OTA update the first day I got it, and when it rebooted, it just stayed on a black screen instead of actually going to recovery, so I had to hard reboot it, to get back to the jellybean it came with. This wasn't a major issue at the time, since then I've just unlocked it and flashed kitkat via fastboot (I'll get lollipop once it has gravitybox or something similar). But since then, I've rooted it and installed TWRP, and TWRP runs fine, but has some weird issues. It seems to not actually see the OS at all, or even the storage filesystem. I have to do a hard reboot to even get out of TWRP, as trying to get out normally gives me the error "Warning blah blah no OS installed, are you sure?", to which I tell it I'm sure and it just reboots recovery. But doing a hard reboot boots into the OS just fine. Am I doing something wrong? TWRP was installed AFTER I installed KitKat and the updated bootloader with the official google factory images, is this a problem?
Some images of what I'm seeing:

Maybe it's because you're running TWRP 2.8.2.0 which is for Lollipop. If it doesn't work with 2.8.1.0, try with the lastest 2.8.2.2. Running Lollipop at least, everyone had the same problem with both 2.8.2.0 and 2.8.2.1.
where can I place the recovery image where twrp will see it?
Thanks for your suggestion LittleConfucy. I was wondering if you can tell me where to put recovery image that TWRP can see it?
Flashing is a problem without being able to access the image to flash in the stripped down rom file system that twrp boots into. No shared directory in /cache.
Nevermind I got it dialed in.

What did you do to fix this? I just had the exact same thing happen. I had a 5.0 rom and tried to flash a different one but once in twrp no files and the no os line when I tried to reboot. I'm on 2.8.2.0

So if I hold power down and force it to reboot it should boot back into the OS? And once in the OS will I be able to use TWRP manager to flash the latest?

I rooted my tablet and installed TWRP end of august.
Two days ago I installed 4.4 with an OTA update which broke the phone, it always booted into recovery. I learned that TWRP was incompatible with 4.4 older versions of TWRP might cause problems with 4.4 and flashed the 4.3 factory image using adb. The tablet is now working again.
I'm just not sure how to proceed now. It seems the TWRP recovery is gone, because from Fastboot when I select recovery I get the dead android with the red triangle. Can I just update by following this post?

Where have you read twrp is not compatible with 4.4? Many people are using twrp with 4.4. You should use the latest version which supports SELinux security contexts.
It is probably easiest if you just flash the KRT16S factory image from that thread.

It doesn't seem to happen for everybody, maybe it's older versions of TWRP. Mine was from August. I rephrased my original post to make that clear. There are some threads here about the problem and if you google for it you'll find some more warnings. It's always the same thing, the tablet will only boot into recovery after an OTA with TWRP:

Thanks for your reply though, so I'll try flashing the factory image. Should I use adb for that or newest TWRP? Also the red triangle means my custom recovery is gone, correct?

Personally I think it is easiest to flash the factory image but you probably need around the same number of steps either way.
The factory image is meant to flash in boot loader using fastboot, not using adb.
The fallen android means you are using stock recovery. It is an image that hides the recovery menu. press/hold power, press/release VolUp, release Power to see the actual recovery menu and any errors which might have occured

Thanks again, all worked well going along with your guide. Only thing to do now is getting TWRP and SU flashed.
It seems like I'm not really up-to-date anymore when it comes to flashing, I only knew of using recovery and never heard of sideload or using fastboot to flash any images.
I wiped the internal storage on my phone by mistake and now my phone has no OS or ROM to flash it back on. I tried to sideload with TWRP but the feature wouldn't start. I flashed the stock recovery and tried to sideload it there but I get an "error: protocol fault (no status)"... I tried to mount storage on TWRP but that feature didn't seem to work on it either. It's a good thing that I still have my OnePlus One to use, but I would appreciate your help!!

I had the same issue the first time I tried to add side load the stock rom, I tried it again and it worked. my suggestion is to try the side load again. Make sure your using the full rom zip.

I tried like a thousand times on both stock and TWRP to no avail. When I reflashed TWRP and tried to sideload one last time it told me that I needed ADB version 1.0.32 which is what I should have because I installed the "15 seconds ADB Installer v1.4.3" from the stickied guide here on the OP3 forums... Anyway, I got access to the TWRP folder and the internal storage by just being in the TWRP recovery, not sure if it was always like that, but I just dragged and dropped the ROM and other flashables onto the phone... lol Maybe I was freaking out too much to notice, but why would anyone sideload anything if you could just drag and drop stuff while in TWRP and just flash? Maybe reflashing TWRP fixed something or I was using an older beta OP3 version before, I re-downloaded everything.

I screwed up when I tried to update to 3.2.0 OOS. I were rooted so the installtion failed and I restored a corrupt backup with TWRP that resulted in a stuck boot logo.
I did this too fix this. boot into fastboot (if you still can) then make sure its connection properly. "fastboot devices". Then in your ADB folder place a TWRP.img and type "fastboot boot twrp.img" wait like 10 seconds, it will seem like it doesn't work but if you are a functional TWRP img it should boot up just fine. Select allowed modifications and then you can also access storage. There is a post on the oneplus forums where you can download signed full zips of the ROMS. download any of them. I will past in the post if I can find it later. Not home atm. Just copy the .zip onto the phone and then do a factory reset and flash the full rom and then reboot should work just fine. You don't need too flash TWRP just boot it if you still wanna keep stock recovery.

Bro don't worry.
All you gotta do is to download a stock image.
Boot into fastboot.
Then use a toolkit like this one.
http://forum.xda-developers.com/oneplus-3/development/toolkit-oneplus-3-toolkit-unlock-t3398799
Then use his toolkit to download and boot in twrp.
Now copy paste a full signed zip onto the phone by connecting it to your PC .
http://forum.xda-developers.com/oneplus-3/how-to/mirrors-official-oxygen-os-roms-ota-t3410870
Do a system and cache clean.
Flash the zip and make it boot before rooting it.
Done..!!

So as the title says, after flashing cm14. 1 can't go into recovery anymore, it just a black screen. I tried to use flashify but nothing, a couple of apps show that I have twrp 3.0.2.1 but can't get into it... Help?

Same situation here. Neither booting the recovery via fastboot without flashing nor flashing the TWRP an then booting into it result in black screen. Flashify gives no results here. Please help.

I believe the official 14.1 nightlies are based on the CB which requires the modified TWRP, which is what @JKMB888 has linked you to.

Just re flash the TWRP image via Fastboot. Apps like flashify actually use TWRP or whatever recovery installed to flash image file AFAIK. I recommend using modified TWRP,Flashing some roms break the access. But the modified one can withstand almost every new rom installs.
Hope it helps.
Why i have always mount decrypt data when i boot in recovery mode which is TWRP 3.0.2-22 aand ask me to input a password?
Edit: I found the solution, i had a pattern lock screen and i removed it!

At some point your phone data became encrypted. While booted up if you search for encrypt in settings you'll see your phone is encrypted.
Same thing happened to me yesterday after flashing 3.2.7 and booting up for the first time before going back to TWRP to flash AICP. Only way to decrypt permanently, as far as I know, is to wipe all data including internal and completely start from scratch.

After I got the recovery back, I flashed CM13 again and ARM64 6.0 Nano Gapps for my Oneplus 3, but I'm getting an error after the "preparing apps' screen is loaded saying that - unfortunately the process android.process.media has stopped one plus 3 boot and then the phone switches off.

Hey, guys. I'm pretty sure I bricked my phone and there's no way to get out.
So I was on CM13 official builds on my OP3. Then, on the OTA update system they have, I got moved onto the CM14.1. I downloaded from here and went into my twrp recovery to flash. I made a clean flash and then proceeded to go back to system. I thought everything was ok until I realised I hadn't flashed a gapps package which I pretty much need to survive as all my contacts backups etc. are on there. I powered off my phone and went to go back into the recovery after downloading gapps 7.1 micro. It went past the *You're device can not be trusted'* and then went to the oneplus screen. After that, it stayed like that for about 30 secs and powered off. At first, I thought I messed something up and tried again. I repeated that for about 3 times and it didn't work. I tried going into the normal system and coming back to recovery and it didn't work still. In fact, now I can't even get into system. Now, i can only boot up to fastboot, where my computer still doesn't realise my phone is plugged in.
Anyone got any ideas on how to fix this? I wouldn't say I'm a noob at this because I have flashed many times before but I'm relatively new to the scene. I didn't see this thread before i ripped my hair out and i think wrecked my phone @JKMB888
EDIT: I sorted it out with help from Naman Bhalla's unbrick thread and this. A certain combo works and now I'm on CM14.1 enjoying life
I had CWM Recovery and an older cyanogenmod rom installed and I wanted to upgrade to 7.1.2 from here: https://forum.xda-developers.com/lg.../rom-android-7-0-nougat-20161008-r16-t3476750
I copied the rom to internal memory, rebooted into recovery, selected the rom file to install. It showed some messages about patching, then in a couple of minuted erred with smth like "Erro loki-ifying because of incompatible aboot image".
I then rebooted and now it boots and shows "ANDROID" and is stuck on it - looks like it partially updated the ROM and failed somewhere.
I am not sure what to do now - I tried to boot into recovery (Power off -> Hold Power + Vol down till menu shows) but it doesnt work - it keeps booting onto this stuck boot.
Any ideas?
As I understand it, being an amateur ROM flasher, all those 3rd party ROMs need a TWRP recovery to work. Provided your bootloader is unlocked, I'd flash the stock on via RSDlite, flash TWRP 3.1.1.0 (my best experience), THEN install whatever ROM you like in TWRP. This'll start you from a clean slate and wipe ALL data from the phone. I'm guessing you forgot to wipe dalvik and cache before restarting, to. Someone of more experience may chime in, soon.

The reason the flash failed is simply the fact you put the ROM on the internal storage, flashing from there wiped the ROM before it gets written. If you had stored the AOSP on an external SD, it would have worked.
As Lineage is based on AOSP, it should have failed also...
After a bit of trial and error, I managed to find a combination of files and actions that allowed me to successfully root my 64GB Pixel C on 8.0.0 by installing SuperSU via TWRP.
Steps:
Perform a clean installation of the official release, found here
Use the unofficial 3.0.0 TWRP release found in this thread (direct download link here) and boot to recovery ("fastboot boot [TWRP filename].img") - the latest official 3.1.1 TWRP did not work, either via flashing or booting
ADB sideload SuperSU 2.82SR3 found in this thread (direct download link here)
Hope this helps!

Thanks. My problem was my pixel kept booting to TWRP even selecting to boot the system, kind of strange. I think I missed your first step, the pad was OTA updated to 8.0 from 8.0 Beta release. I will try again with flash the factory image of 8.0 first.

Still works for me as of the September 2017 update. About to try it again with the October update.
It's driving me absolutely mad. I just got a brand new Pixel C today. It's updated to the latest Oreo but nothing else has been installed yet. I unlocked the bootloader and copied the SuperSU zip file onto the device via USB file transfer from my PC, then I set up a password. I'm booting (not flashing) TWRP because all I want is root.
When I'm prompted for a password in TWRP I'm told it's invalid. I also tried "default_password" and that's invalid, too. I looked all over for a solution and found a modified TWRP that doesn't prompt for a password but it won't decrypt the internal storage.
I'm really pulling my hair out here. I can't figure out why it's working for some people and not others.

Try format userdata, then boot straight into twrp
Command is "fastboot format userdata"
You also have to flash SuperSU while in twrp to stop the tablet from encrypting again.

I'm confused about "stop the tablet from encrypting again". I know TWRP needs to decrypt the data partition to access it, but are you saying encryption won't be re-enabled when I reboot the system? I had to ask because it sounds strange that installing the SuperSU zip would mess with encryption.

No just use flashfire to update and keep root, it won't encrypt the tablet. You can still boot official twrp as well after using flashfire to update as your tablet shouldn't encrypt.

Well I'm finally rooted. I didn't change the encryption, just tried a different method and after several tries it finally worked. I used adb sideload inside TWRP after canceling the password screen. Odd that it took several tries, though, to get through it without generating errors. We'll see how it goes next month.
I have had a lot of trouble with rooting Oreo...
I started out trying to use Magisk but gave up after many attempts. Every version, and every kernel I tried resulted in booting to recovery and not system. I could have tried the ADB method but I actually want to have TWRP so I didn't try that.
By the time I tried SuperSU my factory image had had an OTA and many apps & files installed so I can't say my system is "clean" but I have Fastboot flashed the stock boot image many (many) times to recover from whatever Magisk does to the kernel. I never looked all that hard for this unofficial TWRP people talk about so I used 3.1.1-1. At some point I converted to File Based Encryption because it is so much nicer to use, and so TWRP cannot decrypt my internal storage, but the workaround is to put zips and images on a USB drive. It also does not ask for a password. So far so good, I have my workarounds. I do also have this weird error where TWRP cannot unmount the system but I can flash kernels without problems. I have chosen to keep system read-only for superstitious reasons. I think it will fail if I look at it funny sometimes lol.
From this state I flashed the latest SuperSU and it booted! So I do have root. I still would like to try Magisk for the modules but it seems this isn't ready. However, my trusty old terminal app hangs at the su command... and several kernels will crash my tablet... so it's "working" but there is more to do. And literally just now I see I have an OTA. lol.
I followed the original instructions and got rooted with the latest version of Oreo. But, TWRP still wouldn't flash correctly without causing boot loops.
I finally got it resolved by using the original instructions, then loading the Official TWRP app from Google Play. I used the app to flash twrp-3.1.1-1-dragon.img.
Everything is working properly now. I have 8.0.0 (OPR1.170623.032, Nov 2017) loaded and rooted, and the Pixel C will boot into TWRP and see the encrypted data.
Is it posible to flash latest Oreo stock factory image https://developers.google.com/android/images ryu-opr1.170623.032-factory-020f1cf9.zip using stock TWRP or do I have to side load it ?
I'm getting a Invalid zip file format error in TWRP when I try and flash it
