Check to see Perso SHA256 is on.

I was on SlimRom and it was not working. I finally went back to the official rom for one final try and it worked.
This is what I did. I'm not sure if just trying the unlock code would've worked but I did follow these steps (it's a bit weird since you don't actually change anything, maybe it forces the phone to refresh something.)
Just make sure you are on official ROM or it won't work (I tried only the code with the SlimROM and it did not work).
After unlocking I went back to SlimRom and then CM and it was always unlocked.
1. Make sure that the phone PROMPTS for the code with a NON accepted sim card and that your IMEI is correct by dialing *#06#
a. Go to the Dial pad and dial *#197328640#
b. Please click on number [1] UMTS > [1] Debugging screen > [8] Phone Control > [6] NETWORK LOCK
c. You will see 3 options
d. Please click on number [1] Perso Sha256 INFO, the screen will change slightly
e. Please hit the MENU button then when the menu pops up please carefully hit BACK to return to the previous screen
f. Next Please click on number [2] Perso Sha256 ON, the screen will change again to a one line message
g. Restart the phone on the screen by holding the power button and click RESTART
h. When the phone prompts for the unlock code simply enter the unlock code provided to you
i. Your phone is now permanently unlocked!

I had tried two times to unlock my phone with a code I had bought online, but it wouldn't work. After doing this it worked perfectly! I also turned off GPS and Wi-Fi (for some reason they were on). I don't know if turning them off helped any, but it never hurts to try to turn them off if they are on before putting in the code. Hope this works for you!

One of the users in our company has locked himself out of his i780 by forgetting the 4-digit PIN he set on the device (not the SIM PIN).
The problem I have is that it is running one of the older ROM's that did not have the hard reset button combination on it so I cannot reset it at all. It also won't enter bootloader mode using the usual combination (although this may be because of the countdown timer on the device not accepting input).
Anyone got any ideas on what I can do next?

I wonder if you can short certain pins on the flash chip to destroy the checksum and thus cause the device to reset itself - this method was used in the LinkSys WRT54G router and some MP3 players..
did you try reflashing the phone with new firmware? might help since the pin is set on operating system level (afaik), the firmware upgrade is done on bootloader level.

But he can't get into the loader mode, so Mirage is a no-go.. I suppose the phone doesent even connect to ActiveSync, hm? If it would then the official HL1/2 updaters are able to reset the phone into the loader mode and flash it
Correct, I cannot get it into bootloader mode because the usual key combo does not work and activesync refuses to recognise the phone until the PIN is entered so I cannot use Mirage or the official updaters!
Looks like I'll have to send it off to Samsung!

Yeah, I've sent phones to them before, they do take a while.
They say they need them back to take the lock off so looks like it'll take a while!

Doesn't it wipe itself after the 50th attempt?
How old is that version? before Nov2007? Can you post the version number? something like "i780XXHA4"

2. type your MCC/MNC code (list here: http://en.wikipedia.org/wiki/Mobile_Network_Code), e.g.26003 for Polish ORANGE
3. type your new NCK (12345678), then OK

read a few threads about this problem, but they seem old - is there any way to unlock the tp2 ?
i have used the same pin for over a year and suddenly it said "incorrect password" when trying to unlock.
now it doesnt even give a "response" when tapping the unlock button.
i can tap in an emergency number and call that but nothing more.
when i try to connect to pc i get error that device is unlocked and pls unlock to sync.
any tips ?
Assuming that you are referring to the WM OS pin code, no. There is no way short of a hard reset to clear that out.

I have been looking for the past couple days and have not found an obvious answer for my issue. So hopefully this isn't an answered repeat but if so pleas link ^-^.
I received my HTC One M8 dev edition the other day and was hoping to get it unlocked with probably cynogenmod installed while it was fresh but I am having issues with the bootloader unlock. So far I am able to get my unlock code from HTC and when I flash it the command window shows success but I never receive the prompt on the phone to commit to the unlock. I found out the hard way that it didn't auto accept since I soft bricked installing twrp and no longer have access to the recovery even after reflashing a stock one (but at least I have phone access again).
Has anyone had and found a solution to this problem? I was really looking forward to having a nice custom rom but seem to be stuck with the factory dev edition which is still loads better than the carrier edition. I will consider a non HTC bootloader unlock if I have to but the dev unlock seems nice and easy.
Hopefully some one can just tell me I did something retarded and all will be good ^-^.
Also I have tried Squabbi's toolkit but it ends up the same way, but less command prompts.

You should be able to enter recovery by going to fastboot and using the volume keys to navigate to the recovery option, and picking it by pressing the power button.
To enter fastboot with your phone on, hold down the power button and vol. up until your phone turns off (20 seconds or so). AS SOON AS IT TURNS OFF, switch to power button and vol. down. Then go to the recovery option, and you are in.
And while you are in fastboot, you might as well try running the unlock key again. If it doesn't work, go back to HTCDev for another. I'm not sure how that works, but I think you could get another code if you can't get this one to work.

Currently when I try to enter recovery it enters a boot loop and freezes on the HTC screen. From what I read this is from trying to install a custom recovery with a locked bootloader. That part I can live with for now since it does not seem to be an issue, just the outcome of my mistake. My biggest issues seems to be the bootloader unlock not working. unless the two are related which could be as I am fairly new to this process.

So, have you tried the code again? And/or have you gone back to the HTCDev site for a new unlock code?
I have tried two unlock tokens so far and tried both of them several times. It's almost like I'm submitting something wrong and don't know it. When I submit the token I am including both the start and end brackets which is what it appears to be asking for and not just the string between them.

Are you copying and pasting the very first part of the code?
The part that looks like:
"(bootloader) < please cut following message >
(bootloader)" ?
IMMEDIATELY after that is what you copy and paste.
The copy should start when it says "<<<< identifier token start >>>>"
I am only copying the <<<< identifier token start >>>> and <<<< identifier token end >>>> along with the string between and leaving out the bootloader tag that is on each line.
I guess one question I should be asking is are there any settings on the phone that I need to set? I have developer options activated, usb debugging on, and the fastboot option unchecked in the power settings. Other than that the phone is stock settings. I remember my s4 needed to be in a specific usb connection mode but since they are completely different phones im not sure if that will apply here.

That's what I was afraid of haha. I don't know why but while I am no expert in this area I do not feel I'm ignorant either yet I always seem to have problems even when I follow the steps correctly >.<
I do appreciate the troubleshooting greatly thank you. I will keep trying things and see what I can find and if I figure it out you bet I will add it here because I can't be the only one getting stuck haha.

Maybe I'm missing something here. But the Dev Edition comes already bootloader unlocked, and therefore should be no reason to do so.
I think the prompt to unlock never comes up on the phone, since its already unlocked.

Yes, this will tell us a lot.
Or boot into bootloader and just see if it says LOCKED, RELOCKED or UNLOCKED.
I bet it says UNLOCKED.
Trying to unlock my phone, currently stock running 4.4.4 Ifound this website that has steps towards unlocking the phone. There is also a few youtube videos. However, when I try it, I get stuck on options 7. After Ii nput 1, it stays in the Servicemode screen. I can not see the screen where I can select {1}UMTS. Any ideas why? Everyone who unlocked it was running 4.4.2, does that have something to do with it?

If this is a T-Mobile phone (SGH-M919) , off-contract, contact T-Mobile. As of yesterday, they have to unlock it. However, if it is not off contract, that is not fully paid for, we aren't supposed to help you.

My G4 gives me warning about unlock attempts remaining when using a pattern or knock code (See attached image).
However, it doesn't say that it's going to wipe my device.... so I'm not sure what it's going to do if all the attempts are used. I don't want some idiot to pick up my phone and be able to wipe the device just by messing around with the unlock screen.
Can anyone confirm what happens when all the unlock attempts are used?
Is there any way to disable the unlock attempts limit `feature`?

Why on Earth would you think it wipes the device? It, like most any other phone, has a longer wait period to unlock the device if you incorrectly enter codes in.

On page 41 of the Verizon LG G4 Manual it states:
After 5 unsuccessful attempts to unlock your phone, you're prompted to enter a text phrase to confirm that you're trying to unlock your phone.
You have 10 opportunities to enter your unlock sequence. After 10 unsuccessful attempts, your phone will automatically perform a factory data reset and all of your
personal files will be erased. You will be warned after the 9th unsuccessful attempt.
If you've set a Knock Code, after 6 unsuccessful attempts, you're prompted to enter your Backup PIN to unlock your phone.

My Mom's Galaxy S5 for Verizon wipes the phone after 10 unlock attempts by default but has an option to turn it off (she discovered this the hard way when she wiped her phone because she couldn't remember her pattern).
Also, I know that the HTC One M8 does this.
Thankyou woozzy101 for finding that information, its very helpful. Now I just need to find a way to disable this idiotic 'feature'.
I just tested using an invalid Knock Code 5x times to make it use the backup pin... However when entering the backup pin it doesn't tell me that there are 'x attempts remaining' , but I only attempted to enter my backup pin once (and did it correctly) as I'm afraid of wiping my device by testing entering invalid backup pins. Is anyone brave/crazy enough to test what happens if you use all of your invalid knock code attempts then enter the backup pin incorrectly multiple times? I want to know if this will result in wiping the device... if it doesn't then I'll be using this unlock method instead of the pattern.
Also, does anyone know if this 'feature' is something that is part of Android, or a custom addition by LG or Verizon?

I downloaded the G4 manual from Verizon to familiarize myself with the phone before I got it. When I read about the factory reset I was like, ruh-roh, that is an accident waiting to happen. Way too easy to wipe someones phone messing around with it. Or if you have kids and your kid gets it and plays around with it....
I keep re-reading the Manual and the factory reset falls under "Changing Your Screen Lock". It might only do factory reset if you already have the phone unlocked and in the settings to remove the lock screen? Best Buy wont deliver my phone till June 5th (this Friday) so i cant really test it
On my EU origin H815 at the 10th attempt for a pin it simply tells me to wait 30 seconds just like it did at 5 attempts. At 15 attempts you get this;
