you should flash CWM first before rooting. if you already have CWM flashed just search for super su then flash it via CWM. http://download.chainfire.eu/339/SuperSU/UPDATE-SuperSU-v1.41.zip

the img isnide of the rom, with CWM??
I assume you have a JB hboot ? Higher than 1.31 ?
Flash the cm10.1 boot.img (located in the Rom.zip)
Fastboot erase cache
Fastboot flash boot boot.img
Fastboot erase cache
Enter the recovery and perform a full wipe
* factory reset
* wipe cache partition
* wipe dalvik cache
* format cache, system, data
Then install the cm10.1 rom.zip and install the gapps (Google apps) zip.
Reboot and done.

Did u flash the boot.img with fastboot commands ?
If you want to flash a new rom you also have to do a full wipe on recovery.
Now its messed up maybe
Flash the boot.img from the desired ics rom and do a full wipe in recovery and then flash the rom
Either because you didn't do a full wipe or you didn't flash the boot.img......so try doing both of these

Yes I did flash boot.img with fastboot commands, then flashed CWM Recovery by ADB, then flashed the ROM but NO LUCK yet
I really don't know what people are doing to their hoxes
It's not that hard. Make sure you do a full wipe and start from scratch. If you do a full wipe and flash the boot.img and then the rom a bootloop is for 99% out of the question !

I will do this now, but what about no signal, imei & baseband "unknown"
A last resort is a RUU
Put the phone in fastboot mode and do the next commands
fastboot getvar version-main
This will say the build number (2.17.****
Fastboot OEM readcid
This will give your cid code.
The combination is important for the ruu.
Can you post them here.
Why the hell did you format the sd card ?you had your nandroid backup and the Rom in it ..
Dude if you don't know what your doing then why are you playing with your device ...start from the beginning
Go to fastboot >flash kernel
Go to recovery>mount SD CARD and put the Rom in it >do a wipe for data/cache and dalvik cache only!!!
Then go to install zip from sd card
And install the Rom

Flash the new boot.img from the new rom, perform a full wipe in the recovery and install the new rom.zip !

Why do people mess around even before reading !!!
You flash a boot.img via fastboot commands on the pc. The boot.img is located inside the rom.zip file !
