Owing to circs I wasnt able to start repartioning and installing ROM until today. I followed the instructions to back up the SD card and then partitioned the SD card as follows:
tools/
partition sdcard/
remove all partitions and start from scratch/
chose "1024"
This completed okay.
I then did
wipe/format/
format all partitions except sdcard
install from sdcard/
choose zip from sdcard/
then selected the rom to flash it.
The Rom I chose was Android Revolution SD which said it had installed but when I rebooted from Recovery all I could get was Recovery no matter what reboot option I chose. So I went back to wipe/format and reformated all partitions except sdcard and tried the ROM suggested in previous threat to OP but it refused to install. So repeat with Oxygen and various other ROMs all of which aborted without completing.
In addition 4EXT started rebooting continuously in Recovery. When I run Recovery I get a message that says it hasn't been configured but don't know if that is a problem or not. I take the battery out and power up with sound down, which brings up Fast Boot and then I go back into Recovery and try again. So far no ROM (other than Android Revolution SD) has even pretended to install.
Any ideas? Please?

have you tried to wipe the devlik cache ? or do a full wipe ,

Hello, tonight i tried some thing (i just wanted to change rom), but i made some mistakes... now i cannot boot my phone, it keeps rebooting. I have Leedroid ROM and it keeps rebooting after PC-boot like animation. The only thing i can access is the back button + power recovery. What can i do?

I have tried to wipe the cache and system, and solo deleted storage (hard reset)... but same point. I think that if i can put a update.zip on the sdcard i can flash it... but at the moment i don't have a sd card reader. Is the phone dead?
No your phone isnt dead
1. If you have a nandroid on your sd card just enter hboot and recover your system.
2. Otherwise you can perform an md5 check of the rom you downloaded and wish to flash. (just to make sure its not corrupted, i suppose your trying to flash leedroid) Enter hboot wipe everything >system wipe>cache>dalvic and then relash the rom. Oh and make sure your sd card is partitioned correctly ... a2sd, D2EXt ...
3. If problem persists go into hboot mount your phone as storage copy another rom you would like to try and then flash this, you can just follow the steps in no2 abouve again.

The thing that i have done BAD is the format of the card... or better, i have done right things but i'm almost sure something was wrong. I formatted it from gparted, then created a 3GB FAT32 partition, and with the remaining space i created a ext4 partition. Then, i moved the files needed for the rom that i wanted to flash (MIUI) in the fat32 partition, as i seen that the ext4 part was mounted read only (dunno why).
IMPORTANT: I have done all this mounting the SD card via Clockwork recovery.
When i selected the voice "flash a zip selecting from memory card" (or something )... it said "cannot find a valid zip".
Then the only choice for me was to reboot and... that is
Now i cannot mount SD, as i don't have (at home) a SD card reader and i don't have access to CWM recovery.
I have the following problem:
-some days ago I root at the phone and followed a tutorial and I wanted to put GingerVillian 1.4 rom.
-I encountered during flashing sdcard problem in the sense that it did not want to partition, after countless attempts on win then on linux failed. It ends almost immediately with full option and run ext2 to ext3 getting error 'ext3 fs' via adb!
Finally (I did not explain why) PartitionSD worked in recovery mode. I realized that I have to go for it took a few seconds (before it ends almost instantly (I say only one point for the work phase)) Then I upgrade ext2 to ext 3.
I installed the custom ROM.
Everything was ok / functional. I moved and dalvin-cache to sd: a2db cachesd.
My problem is yet to come, do not know why I got the impression (or so I understand) that the nand backup will contain information that I needed on the phone (numbers phone, msgs, etc.) and I could choose what I want to restore only. So I reboot in recovery mode and I did the restore.
Unfortunately this has restored my htc phone-sense and goes into a reboot loop as long the card is inserted. I removed the card and I could enter the system.
Unfortunately does not recognize any card (I tested with the default sdcard that came with, it boot even though he just does not mount)
I also entered the recovery mode: and tried to format (same error as at beginnin no chance error "run via adb") and I tried to mount from the recovery options:
Error Can not mount / dev/block/mmcblk0p1
I note that the root has been preserved a2db I no longer recognized in the terminal emulator (somewhere logical since now is htc sense again)
Can anyone help me?
OK. I suggest you should start from scratch. Just take your sd card out your phone.
First fix your sd card:
-Boot a Gparted live cd.
-Format your SD card to one big fat32 partition.
Then do this:
-Download a rom without A2SD or anything like that. For example: Oxygen 2.0.1 (in fact it has kind of A2SD support, but if you don't have an EXT partition it doesn't matter. nothing can go wrong on that)
-Put that rom on your SD card and boot in recovery. (Vol down + power button, you'll enter fastboot. wait 10 seconds. select in the fastboot menu: 'recovery'.)
-full wipe. Everything. data, factory reset, dalvik cache. everything,
-install rom.
Post your results.

I had the phone rooted and a custom rom installed it was a copy of the stock virgin rom without the annoying programs, everything was working fine, but when I needed to change the number on my phone i tried the ## some number # and it didn't work, so I went to the menu and tried a reset to factory default, whe it booted all I get is a clockworkmod recovery console, i can navigate to the sd card where there is a backup of the original rom and info, it lets me restore but when i boot I still get the clockworkmod recovery console it will only boot into this console, when i try updating or intalling a new room it always fails, can anybody please help me?

Your not fully bricked if you can get to the clockwork mod. What version of clockwork do you have... 3.0.2.4? have you tried plugging phone into computer and see if it connects via sd card adapter not usb (looking for errors dead sd card)
try wipe data and sd card If this doesnt work try these steps found by ldking93 on androidforums.com ( I can't post link yet do too xda spamming rules :-(
1. Put your SD Card into your computer.
2. Download CM7 from here.
3. Download the google apps (Market, etc...) from here
4. Put these both on your SD Card, still zipped. (DO NOT unzip these!)
5. Start up into recovery (Hold volume down, home, power. When you see the LG logo, release)
6. Click wipe cache partition (you will need to use the camera button to select)
7. Click Advanced>> Wipe Dalvik Cache
8. Hit the power button to go back to the main recovery screen
9. Click Mounts and storage, then format
A) data
B) cache
C) boot
D) system
10. Click install zip from SD card
11. Click asopCM7-VM670-20110601-signed.zip
12. Once it finishes (it may be a while, and you should see no errors. If you do, PM me and I will help you.) Click your GApps Zip.
13. Reboot your system. You will now be using clockwork mod 7 (aOSP CM7).
update
download cm7 and gapps (google apps) from cyanogenmod's website, and if it doesn't work try googleing "lg optimus v clockworkmod boot loop" and click on link by androidforums or androidcentral

I used clockworkmod recovery to allow the use of the sd card via teh mount usb option, downloaded xionia recovery, it allowed me to flash xionia recovery, rebooted into xionia recovery and was ablo to install new rom without a problem.

About 2 weeks ago I suddenly received the SD card damaged message in the status window.
I ended up buying a Kingston 16GB to replace the original SanDisk but I'm still getting the same problem. I'm not seeing a problem with the Sandisk or Kingston on a PC.
Presently;
*If I boot into Clockwork Mod Recovery and mount the sd, I can create partitions, FAT32 and EXT3 and the phone will refuse to boot.
* If I delete the partitions and create a single FAT32 volume, the phone boots but complains that the card requires formatting. If I format the card in android it appears to work.
* I can then mount it inside of android and windows can see the contents ok.
* If I reboot into clockworkmod recovery, and mount the sdcard which was formatted inside of Android, Windows can't see the volume and gparted reports that the volume is corrupt.
I've read that this may be a hardware problem, but it seems more like software to me since clockwork is reading the card differently to android?
Many have said after replacing the phone the problems went away. Can anyone comment on this problem? I had been using Gingerburst and Data2SD for over a year prior to this issue suddenly developing. It just occurred while trying to play a song in an mp3 player and being told the file wouldn't play. I rebooted the phone and then the memory card problem developed.

When you finish flashing the recovery reboot to it and repartition your sd, first make it whole in fat32, then make partitiins as you wish.

Hi thanks for the suggestion. It seems I had a very old version of clockwork mod recovery installed, 2.0.5.7. I used Clockworkmod Recovery Manager to try and update it but it never mentioned the version of CMW Recovery which it was obtaining and attempting to flash, or that the flash was failing.
I tried to install your 4EXT recovery but it kept failing, I noticed Superuser was outdated, so I tried to upgrade that but it couldn't flash the upgrade binary. I was suspecting there was some sort of latent rooting issue present, so I then used 'revolutionary' (available from http://revolutionary.io/) to S-Off and root the phone again. This updated clockwork mod to 4.x.x.x. I used Clockwork Mod Recovery manager to flash the CWM Recovery and it reported success this time. I then used the partition manager and it seems to have successfully entered CMW Recovery and created the partition. Looks like the phone is stuck booting at the moment however, seems to hang on the HTC splash screen every time you add that second partition.
I will delete the EXT partition and go back and try to flash 4EXT again now I have rerooted and confirmed S-Off. This has been a right pain, I hope it isn't a hardware issue.
Fixed!
Partitioned directly from CMW Recovery 5.0.2, installed the rom. Booted into android, rebooted into CWM Recovery. Installed the data2sd script. Booted, waited about 20 minutes for the boot to finally complete.
Checked from both CWM recovery and Android and the card is visible on the pc and in android on both. No idea what went wrong, maybe the original volumes did become corrupt but in addition looks like something was wrong with the old version of CWM which was causing the inconsistency in the volumes being visible from android and not CMW Recovery (and vice versa).
Steps performed to fix:
* Installed Revolutionary which offers the CWM Recovery upgrade to 4.x.x.x.
* If necessary use the CWM Rom Manager from the Market to upgrade CWM Recovery to 5.x.x.x as the upgrade does not seem to flash correctly from the 2.x.x.x version I had installed (I could boot by deleting then creating a single partition with fat32).
* Use CWM Recovery to partition SD card directly with EXT partition.
* Factory reset/wipe cache/delvik cache
* Install android rom.
* Boot into android, wait for the boot to fully complete.
* Reboot into CWM Recovery and Install Data2SD.
* Boot and wait bloody ages (approx 20 minutes).
So I installed an update to CM 12 and rebooted without wiping any cache(s) etc. The phone was stuck in the CM boot animation loop for hours - this has happened before and I was able to rectify the problem with the below process - so I decided to boot into CWM recovery and clear Dalvik cache and perform a cache partition wipe. Before, this has sorted the initial problem but this time I was again stuck at the CM boot animation after rebooting.
I booted into recovery again and tried to access my interal storage to locate a CM zip but couldn't access the partition. I kept getting an "E: Can't mount SD card" error when I tried anything that involved accessing the internal SD card/
I rebooted and entered recovery again and decided to perform a "wipe data/factory reset" after deciding that I wanted to go back to stock Android. However, the internal storage wouldn't format because it wasn't mountable.
Now when i reboot I'm stuck on the white HTC screen with "this build is for development purposes only..." at the bottom - I can't even get into fastboot!
I really want to restore and flash stock Android. How can I format the internal SD card and flash stock ROM?
