Camera unresponsive - Samsung Galaxy Nexus

My camera for my Galaxy Nexus has been broke for the last several weeks.
When opened, a black screen opens before shortly thereafter giving the message "gallery is unresponsive" and force closes.
I restored to AOKP build 3, updated the ROM to build 4 and flashed that (after corresponding factory reset, wipe data/cache/dalvik).
Now the camera opens, shows the "take picture" etc buttons but still has a black screen and force closes.
I assume while flashing different ROMs I had restored using Titanium backup a system setting/app that caused this problem.
I understand this question has been asked, but I have to find a solution to my problem (I have done battery pull etc.)
Any help will be appreciated.

I should mention that I have had a couple messages over the previous couple weeks where it says "SIM card invalid" and phone restarts.
I have not had this message is several days.

have you tried a different ROM?
use RootBox 4.2.2
ive been using it no problems and amazing battery life, not to mention the smoothness and pure awesomeness of the ROM

Yes. Originally this problem happened shortly after flashing codename. With this ROM, a black screen would show and then "gallery is unresponsive."
I reverted to AOKP and flashed updated version. Now camera app opens, shows the take picture etc. buttons on bottom but where the actual picture should be stays black and gallery goes unresponsive.
Have also tried deleting camera apk and re-flashing a downloading camera zip file. Same problem.

have you tried clearing data and cache for the gallery app? try that

Cleared data and cache for the app. Same issue.
Very frustrating issue.

Fast boot flash back to stock. If camera is still acting up then its more than likely hardware issues

do you get any messages after boy saying something like: com.xxx.xxx has unexpectedly stopped. this has happened to me to. you have to format your sd card. try that
remember to backup all your important file from the sd card to your computer EXCEPT the DCIM (where a'll your pictures and stuff are. anything related to the gallery don't backup)

Why not try a complete wipe.
fastboot erase boot
fastboot erase recovery
fastboot erase userdata
fastboot erase system
In that order
then fastboot back to stock as mentioned by namtombout.
If that doesn't work it is indeed a hardware issue.

I don't think I've got any of these messages..
I will attempt the complete wipe per instructions in this thread (simply working up to the time that I actually will go through with it). I'm thinking I must have restored some system settings/camera settings from titanium backup during one of my recent ROM changes.
My battery life has been awfully poor lately as well....wondering if the restore of some system setting screwed with that as well...
Hopefully the complete wipe will get things back in order.

after you complete the wipe, DO NOT use titanium backup to restore your apps (even user apps). try fresh ones from the play store and see if that helps. if not im afraid you will have to go back to stock and then root again, flash a new ROM again, and then download your apps from the play store.
this problem has happened to me with my old phone (Samsung Captivate) but the formatting of the SD card fixed that for a while, when it started again i did the steps above and the phone worked better than ever.

[Q] CM 7.1 -- Google Apps crash and/or not working

A few months ago, using ROM manager, I upgraded my ROM from Revolution to cyanogenmod 6. I then updated frequently, following all of the nightlies with great success. I jumped onto CM7 when it came out and it worked. When I tried to update past the first release candidate, however, I noticed my stock Browser, Gmail, Maps, Google Documents, and Google Reader now crash. I even cleared all caches and data upon updating.
BROWSER and MAPS problem description: Both programs begin to open up and refresh themselves and then almost immediately disappear--sending me back to the home screen of my phone.
GMAIL and DOCs let me see myheaders, but when I click on one, it starts to open, blinks and attempts to refresh a few times, then sends me back to the list of message headers (gmail) while Docs sends me to the phone home screen.
Again, I am not getting force close messages. I have tried using ROM manager to fix permissions w/ restart to no avail. I tried redownloading Google Apps from ROM manager for 2.3.4, reflashed the Gapps, still no go.
I am completely baffled. Does anyone know what is causing the problem?

Thank you so much for your reply. I'm sweating over here.
I just wiped data/factory reset, format boot and format system, reinstalled ROM w/ Gapps from ROM Manager, fixed permissions.
(1) The stock Browser still closes itself w/o opening any pages (no force close message). Just has Google's web address in the bar at the top, looks like its going to open it and POOF. Gone. No force close.
(2) Maps - Opens, zooms in on my location, looks good and then POOF. Gone. No force close.
(3) Gmail - Shows me headers, seems stable, click on header, looks like it's opening, white screen swipes in, sends me back to headers.
Basically, not much changed after the wipes.
ADDITIONAL INFORMATION: I am using an Seiko(sp?) extended battery. Could this be a factor?
If you are using cm7.x.x, make sure you are installing the correct gapps. make sur you are installing the gb-gapps for cm7. Hopefully that wil work for you.

I downloaded the newest gapps in Rom Manager and applied. Same issues.
Just to be sure, this is what I consider a full wipe:
In recovery:
Wipe data/factory reset
Wipe cache
Under Mounts and storage:
Format boot
Format system
Format cache
Format data
Under advanced:
Wipe dalvick
Back to recovery:
Install rom/gapps
Use cwm 3.0.0.7 for amoled
Use cwm 3.0.0.8 for slcd
Do all of this from recovery, not through rom manager.

Argh. When I booted, punched in my Google account, the Market installer came up, checked all, ended up in endless Market white-screen loop. Let it sit for quite a while, the beast never stopped, restarted. Got to the home screen and tried opening the browser... still not working.
Between this and last message, I restored my old 7.0 ROM and like I thought, it was still working. *MIND BLOWN*
Not sure what else to try at this point. I've never had a problem installing gapps on cm7. If I think of something more, I'll certainly repost.

Severely handicapped my OS

I should mention I'm running the dark_hd CM9 rom, rooted S-OFF.
So earlier today I was in the bootloader and accidently hit Clear Storage. It brought up a black screen with an image of an open box and then my phone rebooted normally.. fat fingered it somehow. Oops.
Upon loading the OS it immediately asked me for my Google info and I knew something was wrong. All of my apps icons are there on the launcher screen, however none of them are installed except for the basic apps. Also, when I tried to install Titatium Backup again, it says 'Unfortunately, .. has stopped'. I tried to reinstall a few more apps and each one says 'Unfortunately...has stopped". Before I restored, I wanted to see if I could get back some of my data and make a fresh Titanium backup of it all.
I have a few apps like a a logbook program and my Heytell conversations that I would like to see if the data is still there before I do a full restore or a wipe. Is there any way I can fix this "Unfortunately" error? Also, if I do a restore, will my app data be saved or will it revert back to the info that was there when I did the backup? Thanks.

So you hit factory reset from the hboot menu? You could check /data/data and see if the settings for your apps are still there, but i doubt it. You should also check /data/app and see if the apps apk is still there. If it is there, delete it and then try reinstalling it again. If you do a backup, it will restore the data that was backed up. You can do an advanced restore and restore everything except data.

Did you have any apps moved to sdcard? As it almost sounds like that could be the case if some of the data for the phone like the launcher homescreen data is still intact but the apps were on the sdcard which would be part of storage.

I'm sorry it was CM9. Fixed. I had CM10 on the brain as I was getting read to flash the Pons CM10 ROM. I didn't have any luck recovering any of the data so I just wiped it and flashed a new rom. I was able to run Titanium and recovery the data but I still lost some since my last backup. Thanks anyways.

Having some bazaar issues

I'll cut to the chase with my issues and explain where I am now. Then I'll go through some if the previous issues leading up to this...
I got tired of the problems and flashed back to stock FC09 with Odin.
Booted up and made sure it worked...
Flashed CWM 5.0.2.7 with Odin.
Rebooted
Flashed CWM 6.0.2.8 with CWM...
Booted up to make sure nothing broke.
Data, dalvik, and cache wipe
Flashed a CM 10.1 nightly from a couple weeks ago.
Flashed goo.im's latest jelly bean pack (2013-03-01 iirc)
Booted up and went through the normal setup.
Only google apps that showed up are search, play, talk, and voice search. No Gmail or Maps or others. Gallery I'd there but force closes right away. So it and the camera do not work.
I haven't tried recreating the rest of the problems yet since I ran into a problem there.
The rest of the problem started after finally deciding to try the nexus radio hack. I got it working then had a hell of a time getting it working again after flashing a newer nightly. What should have worked didn't. Got it working then decided to revert back to the factory modem and CM 10.1 kernel. Wasn't worth the hastle for me.
After all that I got it working again. Then after the next nightly I lost all my apps, Google apps, and contacts.
Since then I was able to manually flash the Google apps and get it working but even then, it would not sync automatically though I set it to do so. It would forget my contacts and would not sync with gmail unless I did so manually.
Looked at to goo.im zip... realized gmail and maps were not in there.
While I was confused... that does not change the fact the phone seems broken in bazaar ways.
Losing my contacts randomly, refusing to automatically sync to Google, losing my apps after flashing an updated rom (also changes my background to an old one) and other weirdness.
Restored my latest nandroid backup after realizing the camera was broken last night. Everything worked perfect before making that backup. Now the camera and gallery force close...

Go to Settings->Apps->All : Find and click on Gallery then hit the button that says "Clear Data". That should fix the force close problem.

Tried that and also cleared the cache and dalvik in CWM.
Neither helped.

Well, if anyone else runs into similar and finds this via a search here is where I ended up...
I re flashed to stock again with Odin. Only I set it to re partition this time. Then flashed can via Odin.
Finally, I flashed CM 10.1 and gapps.
Everything worked...
Restored the data out of my latest nandroid backup.
Everything was good.
Ran cyandelta to update to the latest nightly and all hell broke loose again.
Think it is because we are butting so close against a full system partition. I had less than 100k free.
I ran my "remove useless junk" script via a CWM flash. I now have almost 100mb free and everything is working as it should. I'm betting I won't have any more issues if I set cyandelta to flash it after updating from now on.
The flash removes a bunch of stuff from CM that I don't need and/or never use like wallpapers, sound files, videos, a few apps and other random things. Need to update it for 10.1 since I made it for 10.0 though...
And today it forgot my Google account...
Noticed my contacts were missing and then saw gmail wouldn't open.
Settings> accounts did not have Google listed.
+ Add account did not have Google listed to add it back.
Had to flash gapps again and set my account back up...
This is one of the original problems...

Warning Recording Failed - Rooted Galaxy Note 8.0

Yesterday, I opened up the camera app and tried to record only to find it with the error, Warning: Recording Failed, then the app shuts down.
I have a rooted Samsung Galaxy Note 8.0 and I have used pimp my rom to edit camera quality, but I believe my camera was working after that. I hadn't backed up original pimo my rom settings before config so nothing can be done there.
I have tried the following things:
Cleared cache and data, reboot
Cleared cache and data, wiped cache partition
Installed other camera apps (They showed similar errors)
Factory reset
Plain reboot
Edit camera settings (resolution lowered, quality lowered)
The only thing left to do would be update to the latest firmware, but my rooted tablet doesn't allow it, so I would need to do it manually.
Is there a way to fix this problem? I have looked for answers to not prevail.

lets see here... you rooted your phone, then pimped it out to peeps to make money when they need a camera?
The hardware is not gonna get any better by tweaking something in android. 720p is as best as you will get.
I bet your SD card is corrupt try refomating to allow camera to save to SD.

Nope, didn't work.
Formatted SD card
Tried with every single setting on the camera.
Nothing works.
I'll try taking out the battery and sd card and putting em back in like I saw on a similar thread, but it's proabably not gonna work.

Flashing from 8.0 to 8.1

Can I just flash up to 8.1 using the "-w" change in the flash-all.bat? Would prefer not to wipe my entire phone clean if possible, but if I must, then I'll do it.

Now, i am experiencing some apps constantly crashing, and i dont know if its because of it... or if its just incompatibility of 8.1 with apps that arent updated.
Gmail, Google App, Play Games, Calendar, Newstand, Wunderground, G Docs&Sheets have issues retreiving data....
So i am debating wether or not to wipe the device and start with a fresh image to see if it helps... im trying to find if this is a known issue before i do....

I mean I don't really care about having to reinstall apps, it's my data that I'm hesitant on since it takes a half a day just to pull everything off, flash, and put it back
factory reset fixed all my constant app crashing issues..
well, found out after reset it started again and found the culprit.... it weas Chrome v63 when i enabled Chrome Home... did it again with the constant app crashing... after resetting chrome back to normal, it went away... SIGH

Do it, I did it that way. Most folks have. Could there be an issue...yea, maybe but I doubt it. If it goes bad you just do it again with the -w in there. But it won't.

First time I flashed 8.1 I was in the same mindset as you. I removed "-w" from the script and flashed the preview a few weeks back. My SystemUI crashed upon first boot and got an error screen stating one of my partitions had been corrupted. Only way to save the phone was a factory reset.
Due to this, I will never dirty flash again. Additionally, I'll never recommend it to anyone else either. I did it out of laziness.
If you don't have it already, i highly recommend to back up your apps with Titanium Backup. I personally purchased the pro version. I then use an app (free from store) called "Z Archiver." In my opinion, it's the best zip/Archiver app available.
Using z archiver, I select all the folders I want to back up on my SD card. I conveniently zip them all together in one zip file on my SD card. Typically, I'll include my TiBu folder, DCIM (photos) folder, and various other files/mods I have on my SD card that I wish to keep. I use ADB to pull it from my phone onto my PC. After I flash the factory images, root, kernel etc and get my phone set up, I then use adb to push the backup archive file back to my SD card. From there, I unzip it and restore all my apps with TiBu. Replace my DCIM folder so I have all my photos back as well. It really only adds a few extra minutes into the update process but you can have peace of mind you did it the right way.
