I have CWM installed but can't see a way to wipe the virtual sd card (/data/media). I goto Advanced > Mounts and Storage > wipe data but it doesn't wipe the virtual SD. Also, factory defaulting doesn't either.

Boot into clockworkmod and select "mounts and storage" when you scroll down you find the options "format /emmc" (internal storage) & "format /sdcard" (self explanatory)
After that run a full wipe from here

Whenever I flash any rom on my gsm nexus my previous apps and info still show up on the new rom, even if I did a full wipe. Is there something wrong with my nexus?
If they're on your virtual sd card then no, nothing's wrong. CWR doesn't touch your sd card partition nor does doing a factory reset. The only way to delete everything is to format them (mounts & storage -> format /data). If you want you can format all of the partitions, however you won't be able to boot until you flash recovery and adb push your ROM's zip onto the 'sd card' and install it.

I am selling my phone to a friend and would like to format everything and restore to factory default.
By doing the 'factory reset' function via the settings > storage method, it seems only the internal(system) storage is cleaned.
so how do i format the 'phone' storage?
can i simply just usb it and format that partition?
or can i just usb it and just DELETE everything there?
whats the best way?

Hi, my question is, what of way do you format device to flash a ROM. In picture A there is swipe to factory reset and in picture B you have to type yes to start. While flashing a ROM which option do you choose? Factory reset or Format data. I forgot which one I used to format data.
With TWRP if you wipe "factory reset" clears, / data, cache / and / dalvik but in / data does not erase / data / media / or your internal storage.
But is you wipe /data from Advance wipe, it will delete everything in data partition including internal storage.

I used TWRP to "format data" on my phone and then loaded CyanogenMod from an external sd card, but it is stuck in the load screen. Do I have to use a different ROM after a "format data"?

Before this i am using phone with micro sd extrrnal, so when i full wipe, i know sd not wiped.
So, in gnex, it use internal, so if want to flash new rom, what i need to avoid so data in internal not wiped? Does full wipe erase sd also?

Both "Wipe data/cache" in CWM and "Wipe data" in TWRP excludes internal media (/data/media), so you should be good to go.
CWM is verified on my Nex4 and TWRP is verified on GNex.
