I'm not sure if anybody will be able to help me. Here is what is going on.
Since I received my phone (Unlocked GSM) the signal was never great. It got progressively worse in the time I've had it to the point where I can't get a signal any longer. Some details that may be helpful:
T-Mobile US
Running version 4.0.1 (ITL41F)
I started with the XXKK1 radio.
I have since unlocked the bootloader, rooted, and tried XXKK6, UGKK7, and UGKL1. None have made a difference.
I think the phone is defective, but I'm hopeful that somebody can find a solution for me. In the event that it is defective do I have any shot of getting a replacement now that I have unlocked the bootloader?
Any help would be greatly appreciated, because as it stands I am the proud owner of a very expensive paper weight.
To get a replaced phone simply... restore your radio back to XXKK1 and fastboot oem lock the phone again. I'd only do this if you can't figure it out. But let's do a few steps to check if everything's okay.
Have you tried resetting the APN? Try putting your SIM card in another T-Mobile phone? Is there service with your same SIM card in that phone? Do you have another SIM card (family member on the same plan or whatever?) try putting their SIM card into your GN and see what happens.

Good to know. I was under the impression I was screwed with an unlocked bootloader.
I've reset the APN and even tried new ones. I'm currently using my backup phone with the same SIM. No problems, great service.
I knew I left out a couple details... I'm a bit tired at the moment lol.
EDIT: I haven't tried another T-Mo SIM but I have tried an AT&T SIM with no luck :/

Okay so it's not a SIM card problem. Have you tried doing a factory reset? or even flashing 4.0.2 (using stock Google images)?

Negative on the factory restore. The phone was wiped when I unlocked the bootloader so I didn't bother. I haven't tried 4.0.2 yet either. I'll give those two a shot.

Try 4.0.2, you can search XDA to find instructions on how to flash it. It's pretty simple. Bootloader mode,
fastboot flash <whatever> <whatever>.img
If all else fails, you won't have to reflash XXKK1, you should just be able to just lock the bootloader and send it back in.

Purchased from Expansys USA. WiFi works fine, have not tried bluetooth.

Okay well before you do that fastboot flash business. Try putting WiFi on and going into settings -> about phone -> and manually search for update. OTA for 4.0.2 is back for GN, so you might be able to get it that way... after you update. See what happens, if it doesn't work well, then go into the recovery and factory wipe it and start all over. See if the results change.
And oh okay. Expansys is pretty good, I'm sure they'll replace it in a timely fashion.
Just did a factory restore and received 4.0.2 right away. Attempted to install and got the red exclamation point.
Is this due to having a different radio installed? I'm going to go back to XXKK1 and try again.

I pulled the battery and it booted fine but 4.0.2 failed.

Hmmm. You must of done something and it didn't like how your "Android" was set up. Probably because you did a change to it that wasn't stock-like.
Try putting it back to radio XXKK1 and try it again. If it doesn't work, download the stock images of Google and flash it via fastboot.

Bummer. Okay, flash those images... the radio.img one will probably bring it back up to XXKK6 anyway.

OK....long story short...finally decided to delve into rooting and custom roms. Flashed CM6 yesterday and everything was going great. Then suddenly yesterday evening as I was swiping between home screens on my device everything froze, and I have been trying to recover ever since.
Anything I flash now just freezes, and i have a touch panel-fail so even when i do get a rom to boot i cant actually get into set it to usb mode or anything.
using bootloader i can not get it to connect using the adb driver so that I can push via adb.
about an hour ago I was successfully able to run the latest RUU and it ran successfully, but device is still way laggy (takes 15 minutes to boot) and no touch screen.
Any help would be greatly appreciated!!! don't know where to go from here. From what I have read its not bricked because I can get into Bootloader and Recovery???
Just a shot in the dark here, but it sounds like since you're stock now it's an actual hardware failure.

Is there way I can verify that the RUU did indeed flash *EVERYTHING* back to stock?
It seen like you have a HARDWARE issues, Re-Boot the Stock Rom. Call Sprint and explain what's going on with your phone, please *DO NO'T MENTION YOU ROOTED YOUR PHONE* Tell them you try to wipe your phone and it was really laggy before you wipe, since you gonna Re-Boot the stock rom the phone is going to get UNROOTED, I know this cause I work at Sprint... So good luck...

I definitely wont rule out hardware issues...this is a refurb device i just swapped my original EVO for on Monday. I guess I just want to rule out any software issues for sure.
You mention that re-booting the stock rom will UNROOT the phone...however I hav no way to currently re-boot the rom??
Also looks like i can not get into recovery now, when I select the option in the bootloader it just takes me to the screen with a picture of a device and a red triangle

Get a Memory Card adapter for PC or use anything that will let you connect your SD card to your computer... As soon as you put the *ORIGINAL STOCK ROM* in your SD card your phone will do everything by itself...

It loads the zip then says parsing for about 1 second then just reverts back to the HBOOT option ???

I think so, on the bootloader it still shows "S-off" ...doesn't that indicate it is still unlocked?
OK, so I was able to flash the file in the bootloader. It looks like it won't load past the 'HTC EVO 4g' splash screen.

yeah, as far as i can tell. It said it completed and gave me the option to press the power button to reboot.
It did finally load up...after about 15 minutes, but touchscreen is still erroring out and not funtional.
Ok,my mom has a Dinc just like me. However,her's is completely stock except an unlocked bootloader. She installed the update and everything ran smooth. With the occasional freeze. But today she turned it to go to school and turned it back on and she was prompted that her Dinc has become unstable. Can anyone explain this or tell me how to fix it?

Something got corrupted during the update it sounds like. You can first try a factory reset from hboot, if that dosent work you can do an ruu. However there is no 4.08.605.15 ruu and im not sure if it will let you flash an older version. If it dosent let you do the ruu, your only option would be to try to root and hope you can get a working rom on there.

Not really a corrupt update my self and many others had no issues. It is more likley that it was just one of those rare times that things just mess up.

What shame! Seems like this phone is just on its last leg and can't handle updates of any kind since Froyo
Long story short, with above average ROM knowledge, and two previous successful flashes under my belt, I flashed Slimbean JB 4.2.1 Beta1 for my Sprint GS3 SPH-L710, and all was quite fine until I messed with menu/security/SIM PIN. Though I thought I was setting it for extra phone security, I actually implemented an unknownst-to-me feature that upon the following reboot, the phone requested a PUK code for a now-locked SIM card (my phone doesn't have a SIM card by the way).
After a long CS call to Sprint, including International Sprint, I was told to go to a local store. The short part of this story is that no one has been able to help. You see, since I DON'T have a SIM card, I have no ID to provide to Int Sprint for them to find a PUK code for me to enter. Although, I've had many troubles in doing so, I have since been able to reset the phone to stock factory settings; HOWEVER, this has NOT removed the PUK code screen, which prevents me from using the phone at all.
I am out of ideas of things to change, alter or fix to get rid of this issue. I have tried CWM and TWRP solutions with no success. In a sense, although I can do anything with the phone - recovery, download, flashing - except actually use the OS, I would consider this phone as useless as if it were bricked at this point. Any ideas from out there would be appreciated. Thanks.

I have successfully returned boot, system and recovery to stock, but this has not resolved the overriding issue. I don't believe I have tried to re-root since doing so. Perhaps that is worth a try.
EDIT: rerooting after Odin stock flash did not help the issue.

I am afraid none of these tasks worked. This issue must reside somewhere recovery can't get to, and only the OS can. The software is likely built so that a regularly performed task can't override a security issue - even if it is a false issue.

This happened on my nexus and I had to get a new phone because your phone is basically locked at that point and I did try random codes and one of them worked but my lte didn't work after that neither did MMS so I just kept calling sprint non stop for a replacement and they finally gave in and gave me an exchange for free

Did you have to contact International Sprint CS or just regular? How long exactly did you badger them? Had you flashed the phone, increasing your Binary count? If so, did Sprint not hit you with a bill after replacement? Just curious, as I have really upped my count due to recovery flashes to try to fix the phone. I think I can still get a Samsung E-ticket through Sprint store for about $37.00. But again, I am concerned about the flash count, and being unable to reset it. Perhaps I should do what I can to really brick it once and for all before I turn it over.

On second thought, your phone may not even allow you get as far as being able to dial the activation number, but just thought I'd throw it out there.

Yeah, I don't have any access to the phone OS whatsoever. Sprint tech has it now; doing some kind of process which includes a 40-minute file extraction and a 30+minute flash. Perhaps that will finally reset everything. If not, Sprint has already said it will then replace the phone.
Received a new GS3 today and I'm back in the game. Thank you to everyone who tried to assist me. In the end, no one on any forum, not Sprint, and not even Samsung could correct the original phone problem. The moral - be very cognizant and careful when flashing any CMDA variant of a native GSM ROM, as there could be settings within them regarding the SIM cards that GSM's do have, but CMDA's do not, or perhaps other harmful settings if messed with or changed. This warning is really nothing new to anyone flashing custom ROM's; it's simply another hard-learned lesson by one person whose adding more truth to the usefulness of such warnings.
I bought a Galaxy S Glide (I think the same as the Blaze) from an outlet in Hong Kong on import from Canada (on boot it says "Rogers" so I can assume it's from Canada).
After using it for a week it suddenly stopped using the network and said that I need a lock code.
I went back to the shop where I bought it and they said they couldn't do anything immediately and would I like a replacement, which seemed like a nice thing to do.
However, after a couple of days I notice a glitch on the screen happen a couple of times.
I refused to believe it was a big problem (because it was my new phone right!) within a week the screen started to totally crash to the point that it would freeze in the boot up process.
You can see a picture attached of how the screen looks after I managed to do a hard reset.
When it starts to go like this, you cant do anything with it.
The guy at the shop wont change it or refund it, he claims I dropped it.
Samsung Hong Kong don't support it because it's from Canada and the only shop that knows what to do with it is the guy that sold it to me.
I'm taking the guy to the small claims court, but I'd rather have a fixed phone.
Any ideas please?
Thats happen to me, is a problem with the roger version. Everything in internet say the problem is hardware, but now mines works perfect. So, is like a bug in the rogers, maybe hardware problem, but if can be fixed with software i dont think so.
If you flash ICS with superoneclick (will remove rogers bootscreen) the problem will be fixed, of course you can flash it with a tar image with odine and will have the rogers bootscreen too. With that the phone will work again.

I strongly doubt this, that looks and sounds like a hardware fault. But hey, if there's no warranty option, you've got nothing to lose...
-E- FWIW, I would boot it into download mode, and let it sit on that screen for an hour. Then plug it into your computer and make sure Odin sees it. If it hangs while in download mode DO NOT FLASH ANYTHING. You REALLY don't want your phone to hang mid-bootloader-flashing, that's an automatic fullbrick...

That was all say, but, in odin my glide never freeze, only freeze while booting or charging too. But not in odin. And right now is working without any screen freeze.

If the device is no more under warranty, I would say trying this would not harm the device anymore. Also this is a samsung released update so nothing unofficial about it.

if you search in internet about freeze problem in glide you will see. Can be roll back with odin. But, if you have no warranty on your device, you dont have nothing to lose, and much to win if the problem solve
My brother had this same problem on his glide (bought at newegg). I got him to install galaxym unlock (galaxsim? Something like that), and you purchase an inapp unlock for $3, and it runs, reboots your phone and you regain unlock. They claim its permanent. So far my brother didnt get locked again.
Is cheap, fast and you can also see in the reviews that there are more glides that used it and worked.

This helped with the frozen screen as shown in the picture?
thats for unlock, is possible that your phone will lock again when update to ICS. my phone was too from newegg and is rogers, thats why i say is a problem with rogers version.

Thanks for thinking about my problem, but this solution misses one very big point, the phone that was locked was changed for the phone that has a screen that doesn't work.
I do not have the locked phone any more.
probably will lock again after you flash ICS via odin, thats happen to me. Is not a screen problem, again, if you want to try to solve, do what i say before.

Bought my phone from Amazon US marketplace about a month ago (but live in the UK), and suddenly the phone did that on start up one day. However, I agree with lorddavid, it is not a hardware fault as it does not freeze in download mode. Samsung service centres won't repair as it does not have a UK warranty, and although I can get a refund by returning I would rather have a working phone.

Was very happy to read this, seems like I'll have to try it. Have been putting it off as it will wipe my phone which I did not want to do just yet - I have a lot of photos and clips which I can't exactly back up as it does not boot normally. Really annoying. Any way to access these/back up before flashing?
And another question, that ICS update is the first leaked one which carried a few bugs (as did the official UCLH2 one) - can I install the fixed UCLJ3 straight away? And do I need a rooted phone in the first place? In Rawr0w0's initial post it seems to suggest I do. Sorry for all the questions, and thanks in advance.
dont remember, i think is the UCLH2, and i think that there was a odin flashable of the latest version (with bug fixes), you can try that (but dont know where download it :/ )
Do you have the photos in your internal storage? i think if you flash with one click, it will delete it, but if you flash the firmware via odin, it doesnt delete it (but i am not 100% sure)
And no, you dont need root to install roms via odin

YES WAHOO!! My phone works! Thank you for the advice! I mainly got it from the other thread but without having read your attempt at flashing I would never have tried it - I'm so happy! I managed to make a back up and copy my media via CWM anyway, but everything was still on there including all my texts and whatsapp messages, and now I have ICM 4.04 too!
Phew I thought I would have to send it back, get a refund and order another one which is a massive pain and lose everything on my phone.

My problem started some months ago, when a friend tried to root my phone, he told me that he rooted it but every app that checks for root told me that it wasn't rooted and i couldn't use apps that needed root also, the downside to all this is that i couldn't use apps that do not allow root, like the banking app, or Pokemon Go, but i used the phone like that for some months until last week when i decided to try and fix it. I thought that if i flash a stock firmware everything should be fine and the phone should be as new. On the phone, it says that the model is SM-N910F(behind the battery), in the settings it also said SM-N910F, so i tried flashing a rom for this model at first and it kept failing with me not knowing why. After trying different thing and googleing some time, i finally noticed that in "Fast Download", it says that the phone's model is SM-N910P, so i started searching about this, i didn;t understand why there were 2 different models on my phone, then i found out about searching my IMEI on imei.info, so i tried it and here it was SM-N910C, i had no idea what all this meant, but at this time i already got stuck into bootloop. Eventually, i found this thread: https://forum.xda-developers.com/note-4/help/help-sm-n910f-sm-n910p-bootloader-t3299285 and did what cam_bob22 said, it worked, i finally flashed the firmware on my phone and it worked, i could use the bank app, pokemon go, everything was fine except the fact that i couldn't select or change my mobile operator, when going to mobile networks, the mobile operators options was greyed out and couldn't be pressed(i want to mention that i did have service and that data worked but the internet would work only in roaming and i couldn't get 4G). So i tried updating the software from 5.1.1 to 6.0.1 like it was said in the instructions and hopping that this update would fix the issue, but unfortunately for me after the update i somehow lost service completely, the signal sign was in the shape of a triangle with no bars, and the mobile operators setting was exactly as before.
So far i tried going back to 5.1.1 but besides the Hands Free activation that i can't make to go away, i still can't use the mobile operators setting and my data only works with roaming activated and doesn't get 4g. One thing that i forgot to mention is that the IMEI with the 5.1.1 firmware was of a samasung Galaxy S5 not a note 4, i checked it on imei.info.
I was wondering if there is any way for me to go back to the OS i had before my friend tried to root my phone, when it said in the settings that it was SM-N910F and everything worked as it should have.

There is no way to go back unless he made a backup. Which is why you should always do this before you flash anything.
I'm not sure if this will help but you could try flashing a international custom Rom after you have correct firmware. But if your phone is a Sprint version, you'll have to use a special data fix and kernel.
What firmware did you end up using to flash? Was it Sprint? If so, and you use gsm service, try flashing my Gsm stock Rom for Sprint phones. It's in post 2.

Thank you for the reply, i wanted to say that with the Sprint firmware i used, after doing all the updates it unlocked the mobile operators option, but it couldn't find any.

No, the network operators is unlocked but there's still no service, when i try scanning for an operator it just stops with no message, and nothing happens.
I forgot to mention this, but before all this happened, I made a back-up of the phone with smart switch, is it possible to get back to how it was using that? And another thing is that when using the backup, it only restores music and photos, no contacts, no apps.(From what i've noticed so far)

You would have needed a system backup with twrp.
What country you in? What network you on? What firmware you on?

Ok. About the best advice i can give is, because your on pure Sprint Rom, something is messing up with csc or something.
All i can suggest, is maybe try updating to QC1 with Odin and then try flashing my gsm Rom. Which is essentially just a tmobile rom.
And if that don't give you the function you you need. Then maybe try an international Rom.
It's hard to say what the last person did to the phone.

Also. You stated your phone first showed as a 910f. So maybe flashing a international F rom, will provide the correct csc settings for your phone to work. Only problem your gonna have, is because it's Sprint hardware, you'll have to pair the Rom with a kernel that'll work and some lib files for data.
I can help you by putting together some files to flash. But the main question would be, Do you want a stock note 4 Rom?
Also before any of this happened, do you know what firmware you were on? Marshmallow? Lollipop?

I would really appreciate if you helped me put the files together cause i have no idea how any of this works, i'm new. But i wanted to ask, is there any chance for thia to brick my phone?
When it had sm-n910f on it i couldn't update it after 5.1.1, when i pressed update it wouldn't do anything.
And yes, if possible i would like it to be a stock firmware, but if it is not possible i don't mind using something else as long as it works properly.

Well, first, let me state,about bricking device. I have to state that it's not impossible to brick. But after hundreds of flashes never had problem. These devices are hard to brick. What hard bricks a device is messing with boot loader and partitions that shouldn't be messed with, like trying to unlock lte bands. But just flashing roms is fine unless you flash a Rom not for your phone and it over writes wrong partition.
All snap Dragon note 4s have the same layout so all you need is correct kernel, csc, and some ril lib files to get data working.
Now, mine main worry is that the phone is still unlocked,because I don't know what possible changes were made to it.
But with all that being said,I'll move on. With another post.

Before we go too deep into this and upgrade and stuff. I'd like to just test a custom Rom on your phone and see what works.
So what firmware are you on right now?
5.01, 5.1.1, or 6.0.1?
I'm gonna take a guess and move on. You last stated you were on OG5 which it's 5.1.1. So if that's true, let's try the following.
First I'm gonna assume you already used Odin to flash that firmware. So now we need to flash Twrp. So download this on pc https://eu.dl.twrp.me/trlte/twrp-3.1.1-0-trlte.img.tar
1 . Put phone in download mode. Open Odin and plug in phone.
2. In Odin options make sure everything is unticked except Reset Flash Counter.
3. Now click on AP and select the Twrp file.
4. Flash it. Let it flash until it states it's completed. Once done. Unplug phone. Pull out battery. Put back in and boot to recovery by holding power+home+vol.up
5. Now you should have Twrp. Just reboot phone so we can continue to next post.

Ok, here you go. If your where we left off, you should be on stock QC1 and have Twrp installed.
Just download this file, boot into recovery, and install. This file will delete dalvick/cache, so phone will take a few minutes to boot up.
I actually recommend before flashing, you do a factory reset in recovery then install.
Anyhow here you go. Let me know if it fixes what your looking for. If not, my only others option is to install a custom n910f Rom. I'll gladly walk you through if need be. But try this first.

Alright so i just tried it and when starting my phone, i can't even set it up because i keep getting a message that com.android.phone has stopped. After about half an hour of trying to start it, i finally managed to get to setting and to app manager, there i looked for com.android.phone and it was nowhere to be found. Apart from this i noticed a N with some signal bars where my phone signal should've been and i didn;t get the hands free activation anymore, but that's all...
