edit: can you clarify if you're CLOCKWORK recovery, or STOCK recovery when you see those errors? If the former, just jump to the last step of my walk-through and proceed.
You're in good shape, and we should be able to get you sorted with relative ease.
Assuming you have ADB setup, if not, get it setup and working before following these steps.
- Download fastboot from here: http://developer.htc.com/adp.html#s2
- Place the fastboot.exe file in your adb tools directory (typically C:\Program Files\Android\android-sdk-windows\tools)
- Download this file: http://download.clockworkmod.com/recoveries/recovery-clockwork-5.0.1.0-vision.img
- Rename it "recovery.img"
- Place it in your adb tools folder (C:\Program Files\Android\android-sdk-windows\tools)
- Connect your phone via USB to your computer, and boot into the bootloader, you should now be in fastboot mode (phone should say "FASTBOOT USB" in red)
- Open a command prompt and switch to your adb tools directory where you moved "fastboot.exe" and "recovery.img".
- Run the following command "fastboot flash recovery recovery.img" (leave the quotes out)
- Once it's complete, you should be able to boot into recovery (try it a few times if it hangs on the white HTC splash), mount USB, copy a ROM over and install it.
Let me know if you have any problems/questions.

hey thanks for your reply but i fixed it already thank to nipqer.. i forgot to mention it here. but thnks anyways..

[Q] need help getting S-ON / unroot

After reading around all day I seem to be suffering the same problem that a growing number of individuals are experiencing, my phone froze and now it will not pass the HTC screen.
I have tried to flash a new rom - no success
I have re-flashed the recovery (v3.0.2.4) thru fastboot
I receive the following errors on recovery boot
E: Can't mount /cache/recovery/command
E: Can't mount /cache/recovery/log
E: Can't open /cache/recovery/log
E: Can't mount /cache/recovery/last_log
E: Can't open /cache/recovery/last_log
I have formatted the SD card
I can't mount or format /data /cache or /system
All three partitions are blank thru ADB shell
I can't flash any backups (error formatting /system)
Using the factory reset option in fastboot only causes the phone to hang
I have my part7backup but it does not restore S-ON
I have tried to manually restore it using gfree as well to no success
I have HBOOT and can connect thru ADB
I have tried a few different PC10img's but they all fail on the bootloader
I am trying to get this to a point that I can send it in for a replacement unit but I seem to be getting no where, any help would be greatly appreciated.
Current kernel - 2.6.32.28-cyanogenmod-g4f4ee2e

You can wipe cache in fastboot from your pc. Enter fastboot, plug in your phone, then wipe (or at least I can with Fastboot Commander, never tried Android Flasher, but should be the same).
@Pipusz:
You should write at least what the fastboot command for erasing cache is.
@OP:
When phone is in fastboot mode, connect phone over USB to computer. Open cmd in windows, navigate to android sdk folder (I assume you installed android sdk, if not, see my sig) and enter
Code:
fastboot devices
The output should show a long number (this is your phone's serial number). If it does, then fastboot sees your phone and you can enter
Code:
fastboot erase cache

Fixed my phone :)

I had errors in my previous recovery which showed:
E:Can't mount /dev/block/mmcblk0p1 (or /dev/block/mmcblk0)
(No such file or directory)
E:Can't mount /sdcard
Also errors such as error formatting system and mounting system.
So i flashed a new recovery, amon_ra's recovery, this one included the option to move the recovery log to sd. Then re-installed my rom.
Here's how i did it.
Plugged in the phone, booted to fastboot.
I opened the cmd and typed:
cd C:\Users\Francis\Documents\ANDROID files\Android SDK Windows\tools
This is the path that contains the fastboot.exe. I then typed fastboot.exe.
Downloaded recovery-RA-desire-v2.0.0.img and cut n pasted into the tools folder.
I typed: fastboot oem enableqxdm 0
I then typed fastboot flash recovery recovery-RA-desire-v2.0.0.img
The recovery file was obtained from this link: http://files.androidspin.com/downloads.php?dir=amon_ra/RECOVERY/&file=recovery-RA-desire-v2.0.0.img
I then went into bootloader, then recovery and re-installed my rom.
If you use an EXT, you would be better off with 2.0.1
It is common problem with PVT4 and Clockwork recovery 2.5.0.7 which is installed during rooting with Unrevoked
I used AmonRa 2.0.1 but recently switched to 4EXTRecovery.
OP: Are you seriously that stupid you had to create third thread for it?!

[Q] HTC Droid Incredible (first edition) internal file corruptionish

Issue a little resolved.
Info below.
Thank you.
Perhaps you did a factory reset? It usually takes a few minutes longer than usual to boot after doing one of those.
If you didn't, it might be worth trying now.... turn off phone, hold volume down and power button together, using volume button scroll to 'factory reset' and select it using power button.
You can add back a stock 'OS' by following the steps here (#33 contains the stock OS)
http://forum.xda-developers.com/showthread.php?t=1571404
I performed a Factory Reset. It rebooted and went to the HTC Droid Incredible screen, which only lasted a view seconds. Then it went straight to Clockworkmod Recovery.
And by that restoring the OS part you said, I can't find it. The link you provided sends me to a fix-the-screen forum post. There's no #33 there.
The phone has HBoot unlocked, btw if that helps anything.
Have you, by chance, tried flashing a stock RUU? PH98IMG on the sdcard and all that jazz?
I have, as a matter of fact, tried doing that.
I started with images at HBoot on a sdcard but all it did was read it and then throw back an image-not-recognized even though the image IS recognized.
Then I tried doing a few RUU's but all of them throw errors back at me.
This is getting frustrating.

I somehow got the phone to startup it's OS, somehow...
But now it is freezing almost every time I go from 1 app. to another. How do I fix the app. freezing?
And now it is getting stuck in boot loops again! Please help!

Just do a full wipe in recovery, and then flash a rom of your choosing. That should get you up and running. Or you could do this ruu http://dinc.does-it.net/Stock_Images/4.08.605.15/PB31IMG.zip thru hboot.
Thanks for the file! I downloaded it to a FAT32 formatted sdcard and went into HBoot and let the file update. The update was successful, but I still got the random reboots/freezing/etc. I can't access the market/google play because it keeps saying there is no connection but I do a wifi connection. It also keeps getting stuck on the HTC Incredible white screen.

Could not mount /data to setup /data/media path
E: Can't mount /cache/recovery/log
E: Can't open /cache/recovery/log
E: Can't mount /cache/recovery/last_log
E: Can't open /cache/recovery/last_log
E: Can't mount /cache/recovery/last_install

When trying to flash a stock rom, did you try different usb cables and usb ports?
Did you confirm the model number in download mode?

I think it's Hail Mary time. See your PM for info using Heimdall and pit file. I have put what you need on the ftp server. ~metafizik
I had this happen with mine once and had to flash older version stock ROM & Pit .from there I had no signal, after rooting & philz...flashed new modem..then updated to updated ROM. Oh and mine also failed adb until I got a new battery. Also I would suggest to uninstall and reinstall drivers on computer...both Sammy & Heimdall.
