This doesn't seem to be app specific or kernel specific either. It happens in even the settings app and I've tried pretty much all kernels and different settings but this issue seems to persist. This does not happen in Custom Roms, I tried really hard to replicate it.
I've reported it to Oneplus via the feedback app with logs, but have little hope for a reply from them. I'm just taking a shot at here if anyone else noticed this or maybe even found the reason for it. I checked logs but couldn't find anything specific.

Extensively used CNP and Omni, even intentionally tried to replicate this but couldn't. Seems it's restricted to just OxygenOS.
When Ivrun stock oos, I always use renderkernel. It is the smoothest I've ever seen on this phone. Not sure how to read the GPU bars but it never exceeds above the navbar area. It usually stays right in the bottom half of the navbar.
Give render kernel a try and you really wont be disappointed.

Using render right now and yes it is by far the smoothest kernel. But this issue still persists. The only common factor is OxygenOS, I'd assume it to be a hardware or screen protector issue but it does not happen with custom ROMs.
After talking to @joshuous this is what he suspects could be the reason for the issue. Thanks again for explaining.

I experienced the same issue for the first time on OB4 and it persists from time to time. Solution: Reboot
Ok so as a temporary fix I found enabling 'Disable HW Overlays' from developer options helps a lot.
This maybe similar to a fix for YouTube lag found on Nexus 6.

Hi, occasionally I have a similar problem where I experience intermittent lags or heavy frame drops during scrolling. My setup is OOS 9.0.3, stock kernel, unrooted.
Turning on "disable HW overlays" definitely helps a lot, but in my case just turning the screen off and on seems to get rid of the quirk at least until I turn the screen off.
Also there was a thread on XDA apparently talking about the same kind of lags, and the OP there said to have solved it by tinkering with some custom kernel to crank up the CPU frequency whenever s/he touches the screen. I didn't try it myself though.

Noticed today a lot of screen artifacts ie. Letters being dropped and unreadable,
I've factory reset the phone and all seems OK now,

I think I might be having the same issue. It seems if my phone is running and I lock it for a while. Then unlock it after it's been sleep. All of my widgets are showing garbled pixels all over the screen. Then when I reboot the issue is gone. The next time it happens I'll take screen shot and see if you or anybody else is having the same issue as me.
Also using BW but reading this I'm scared to use their LWP, let us know how it goes, did BW cause any reboots? Fishing here because I'm not sure if BW is to blame or not.
Mine has rebooted a few times but I've found nothing to blame it on yet.
BTW no artifacts.

Hello mate...it was probably more of a coincidence than anything, I'd had bw on for 24hrs then this happened so very possibly a very big coincidence
I found after a factory reset it spot on again....
Maybe I'll try the bw lwp again and see how it goes.

Hey. This is kinda old but I am having similar issues.
I also have BW but Ive had it for about 3-4 days and just started experiencing this today. Only changes I have made today was auto-flashing franco-r116. This could just be a coincidence I suppose.
I managed to catch a screenshot. Its tough because it comes and goes rather quickly, mostly when im scrolling through stuff.

If you are running R116 and you are getting artifacts then maybe your gpu can't handle the overclock, one version is 384mhz and the other is 512mhz. If 512 gives you artifacts use 384, if 384 gives you artifacts then you will have to use a milestone Franco kernel because that is stock gpu, 307mhz.
This was my first thread (screenshots included).
Since then it's been an intermittent problem on Apex and Nova Launchers. Not on launcher 7, however, and not on the new miui launcher, and (as far as I can remember) not in any app besides the launcher. The problem also seems to be exacerbated by having lots of widgets, though naturally it's hard to measure since it only happens intermittently.
aLogCat also comes up with "OpenGL is out of memory!" when these things occur. The problem seems to transcend kernels and GPU overclocking (my device seems to be bad at CPU overclock, and I've never tried it seriously).
I think that narrows it down to: launcher issue, widget issue, usage issue (maybe it's being strained too much at a particular time), or some combination.
My current best hypotheses (or suggested solutions, sort of):
1) http://stackoverflow.com/questions/9412662/how-can-fonts-cause-opengl-out-of-memory-error i.e. certain widgets have fonts too big. This fits with your usage of BeautifulWidgets and my large clocks. But it doesn't always cause an issue, or for everyone.
2) Large grid (in my case, 5x4). One of the common denominators for Apex and Nova, but not MIUI, is resizable grid. As such, I'm currently in the process of testing 4x4; it seems to be able to handle more strain, but since it's intermittent I'll have to try for longer time to be sure.

Ok so I've searched what feels like a million places and finally am comming here as a last resort. I will try to be detailed in everything i have tried up to this point so sorry if this gets a little long. I hope this clears up problems on this phone for anyone else with similar issues.
So the problem is that i am experiencing major lag on this phone. Let me specify. Mostly during games, which some are unplayable. Now that being said, I have not tried many games but here are some specific ones that have had some problems:
Nova 3 (which I am unsure of compatibility due to mixed reviews), The lag makes the game unplayable, online and offline
Modern Combat 4 (lag isnt too bad but still makes the game hard to play and not fun),
Shadowgun Deadzone had a wierd blur effect which made it unplayable --- fix : I had tried chainfire which fixed it but also disabling hardware overlays fixed this issue --- this is just some background info maybe it helps maybe it doesnt.
I have even noticed some lag on vector, a non graphicaly intense 2d game.
Some lag occurs even on the home screen as well (mostly when the device is first turned on and the home pages have not been loaded into ram yet, but this still shouldn't happen).
I know this phone has a weaker graphics chip and processor than the international version, but with the adreno 225 and the snapdragon s4, this device should be able to play these games and not lag on the home screen.
specifics: power saving mode is not turned on, I have tried to disable hardware overlays and force gpu but neither did any good (execpt shadowgun listed above). I have tried rooting and installing chainfire, this didnt fix the lag. I am on stock android 4.1.1 right now (rooted).
This phone is nearly new, about 2 months old.
So I guess my next step would be to overclock? does anyone think this would help? My point was that this phone stock should not have these problems and maybe I just got a bum phone.
Ok so thats the main problem, another, I have recently experienced battery drain. With all radios off, the phone not used (in my pocket, turned off) I am experiencing about 8-10 % drain per hour. - I dont know if this is normal but if any suggestions please let me know. While in use (browsing internet) I get about 40 -50 % drain per hour.

After I used my S3 for a few months, maybe 3 or 4, I also began to experience great lag and choppy performance. I was disliking the bad battery and the general slowness of the ROM so I flashed another ROM. I would suggest you flash an AOSP ROM such as the M snapshot of CM10.1 and see if that runs any smoother. I noticed a huge battery life increase and a much better performance. If you still have lag issues after an AOSP ROM, you most likely have a problem not related to the ROM. If you find an increase in performance with the AOSP ROM, but you prefer Touchwiz, look for a different custom ROM that is Touchwiz based. These ROMs will likely have a smoother performance and a better battery life than stock ROMs. Also, about your battery drain issue go to Settings > battery and see what is using a lot of the percentage.

Alright thanks for the help everyone. The solution is that Gameloft did not optimize this game for this phone. So change the buil.prop, and the game runs like butter. For anyone wondering, the solution is to edit build.prop and change the:
ro.product.model=Galaxy Nexus --- delete watever you have and type in "Galaxy Nexus"
ro.product.brand=google --- be sure this is lower case.

The build.prop is located at /system/build.prop
The only things that would might be affected (for this specific build.prop change) are other apps that read your device name / type, such as the play store
The play store would be affected in that it would show more apps possibly, and apps compatible with the nexus
I think it crosschecks your device name against other stuff too though, so dont quote me on that

Hi there,
Was hoping to get a response from Ayysir or Gideonx, but not sure if they're monitoring this thread (obviously everybody's expertise is welcome!). I've been having a bit of a problem with 3.69 (and had the same problem with 3.68). After a clean install on 3.69, including only a very few data restores from TiBU (and none on the apps that are giving me trouble), I've been consistently getting random but inevitable freezes on nearly all the games I play on my phone, sometimes to the point that the phone actually reboots. I get a screen flicker, and then progress slows to an absolute crawl and I even see frames flickering back and forth at times (i.e., the unicorn in RUA2 moves back and forth a frame or two on screen). When I can fully close the app, it usually reboots fine, but then freezes eventually as before. Everything else about the ROM is absolutely rock solid - I actually just commented to my wife that, other than the freeze up issue, I've never had such a clean ROM on my phone, awesome battery life, smooth as silk, etc.
I've tried pretty much all the kernel options available, in addition to stock, to no avail - the problem persists. No undervolting at all in any case, and the highest CPU temperature I've seen recorded through trickster has been 54 degrees, which shouldn't (I think) be enough to trigger heat-related issues should it? The CPU is generally sitting around 42 degrees under load, which seems great to me.
Anyways, if you have any thoughts about what could be causing the issue, I'd appreciate it. I'm assuming it's ROM-related because of the steps I've taken to eliminate it, but who knows, maybe the issue is hardware related and there's nothing to be done but replace the GPU or something.

EDIT: Should also say that this is the second clean install I've done and the problem has continued in both cases (i.e., doesn't seem to be a bad install).
Just to add to my previous post: I've been reading that some problem on Nexus 4 devices have been having problems running games built with the Unity engine since the upgrade to 4.2.2. Anyone have any thoughts on whether this could be the culprit?

You may want to try this... Goto Settings --> Developer Options --> and check Disable Hardware Overlays (I haven't seen this mess with Halo functioning) and Force GPU Rendering. I think there was another related setting you need to turn on.
I got a warranty replacement and haven't been able to switch back from TouchWiz due to a baseband upgrade... So I apologize if that isn't as thorough as I would have liked.
I turned these settings on and game performance went back to normal.

I'll give it a shot if it reappears - the problem hasn't occurred in two days now, after having been a very consistent issue since 3.68. All I've done in the meantime is some clean up of the file system via sd maid. Truly bizarre.

How to install
Preparation
Ensure you have a working copy of ADB for your PC. A simple search should find versions for all major operating systems.
If you are running "stock" (ColorOS), the first thing you will need is a recovery. There are many recoveries available. The most popular ones are CWM and TWRP. Please consult other threads to find and install a suitable recovery.
Once you have a suitable recovery, download from download.cyanogenmod.org to your PC.
If you wish to have Google applications available such as the Play Store, download a current copy of gapps. CM recommended gapps are shown at wiki.cyanogenmod.org.
Reboot your phone into recovery.
Installation
If you wish to save any of your current data, back it up. Details vary by recovery. The app "Titanium Backup" is also popular to backup and restore your installed apps.
Wipe your internal data. Again, details vary by recovery. The option is usually called "wipe data / factory reset" or something similar. This is absolutely critical and required if you are coming from a stock ROM or a different third party ROM. Note! If you are coming from a previous nightly build of CM11, you can often get away with keeping your data. But if you encounter any issues, please wipe data and retry.
Find an option in your recovery that looks like "Install from sideload" or Install from ADB" and select it. On your PC, run "adb sideload cm-11-yyyymmdd-NIGHTLY-find7.zip". ADB should show a progress indicator. When the file has completed uploading, it should install. Repeat with your gapps package if desired.
Reboot your phone. The first boot after install can take a few minutes (usually no more than 5 minutes) so be patient.

Has anyone noticed that with the 0716 update, xposed throws a systemui error and you cannot get to Modules (it restarts prior to showing Modules).
Tried uninstall xposed and all modules individually. Reinstall not fixing issue

Working on the black screen issue today. I can repro it easily now using "stability test" but the phone needs to be pretty hot to trigger it. Usually about 38c battery temp. Don't know if others are seeing the same thing or if it happens at lower temps for some.
I've eliminated user space thermal engine as a cause. I deleted it and the problem still happens, and goes away after a couple minutes.
Ill be going over the kernel tomorrow.
My temperatures were cooler, probably closer to 30c. Despite cooling down, the BSoD wouldn't go away and I could only access my screen following a boot.
We know the Gummy kernel works. Can we reverse engineer it to see what the fundamental differences are?

Could the mic issue when on speakerphone with 7s be please added in the known issue post.
So i spent the entire day yesterday trying to track down the black screen issue. I went through the DT, the kernel thermal driver, the userspace thermal engine, the thermal engine config, and everything else I can think of. Nothing is wrong with the CM kernel.
The current theory is that the stock/gummy kernels have more conservative GPU frequency/voltage settings that are preventing them from overheating. I will test that on Monday (I'm off work today). If this is the problem, we will need to solve it with the thermal engine config -- we will not degrade performance of the bacon and find7a for this issue.

I don't think Find 7a users are experiencing BSOD though, are they?
I have had 5 blackouts today. It does appear to me that many times these are heat related. Can a govenor be adjusted to not go above a certain threshold.
I love CM11. I hope this is solvable. (Nooo Mr Wizard I don't want to go baaak)

I have had no SOD on CM and one on Gummy. It is kind of a tossup between Gummy and CM which is better. No real difference I can see. I think I might like CM better. My point is SOD is not limited to CM.
I got the SoD issue twice so far in the 3 days using cm11. I also noticed notification light issue. I also get insufficient space in titanium randomly when trying to back up everything, if I just do recent apps it works. Anyone else have this problem.

I have been on CM11 for 3 days now (x9076) and had no BScreen issues - started on 717 and now on 718
Just ran benchmarking tools as not into games to try and get BSOD - Device got hot after reruns but feel confident of it was going to break it would of....

Weird when using any root file manager after say, copying then pasting action will get a crash in the file manager BUT on reopening it has completed the action that caused the crash..... just an observation but it is a constant occurrence

Switched to Franco kernel. I was sailing smoothly w/o Black Out, but late in the day Boom. 1 a day is a least better.

Did somebody notice, that after Nougat update device had become more laggy, for example, lags apps transition when multitasking, lags home button, especially, when waking device on fingerprint. If using some app for a long while, opening another app from recents cause noticeable delay on press, even if this app is still there in memory.
I know the lag perception differes from one to another, but still, I think this lags are really noticeable. I used to set 1,3k/1,3k cores on input boost with 300ms on Mm to get IDEAL fluidness in every situation, now I need to set 1,3k/1,9k with 400ms, but it still lags sometimes. If somebody faced this too, plz respond..
I'm really having trouble. I was deleting some messages from Gmail and the lag was too big!

I'm not really sure, if in app smoothness decreased for me with nougat. Maybe some apps became little more laggy, but another became faster, for example EX kernel manager works really faster, YouTube is smoother. On the other hand Telegram chat transition become crappy, kate mobile is a bit slower when browsing news. So if talking about inapp experience it's fair enough. Something improved, something got ****ty.
But still, multitasking and recent apps became MUCH slower. Maybe it's coz of op **** coding, mb it's coz multiwindow feature, Mb some app is causing this, and maybe it's just my crappy device. Who knows... But if somebody really knows, please answer)
I noticed that if a VPN app is active or the Keeper app, for example, is running in the background, laggy is much bigger!
You could do a clean install, overall custom roms are smoother! We sadly don't have this features since they can cause instability and you won't try that with 7 mil sells

The stock kernel has always been sluggish, and Nougat has been quicker than marshmallow for me, and I'm the type of guy that gets annoyed if there is 1 microstutter. I hate any and all lag so I tend to use performance based tuning and not battery life. Try FreedomOS 2.3 (not the one based on OB11, OB11 sucks a**) with Franco, Blu spark or render kernel (Franco, Blu spark are my favorite) and I guarantee you, all those lags with the multitask button and home button will be gone
Use akt app and use any profile in that according to your usage to remove lag
