Issue with in-call volume not adjusting, how to debug? - G4 Q&A, Help & Troubleshooting

Hello,
I have noticed that for some time now whenever I try to adjust the volume of a call (during that call, using the volume buttons) the Bluetooth volume is adjusted instead of the in call volume, leaving me to either not hear well enough or too hearing too loud.
I have tried rebooting and noticed that for a few seconds after each reboot (about 15) whenever I place a call I can adjust the in call volume just fine. There is a definite point in time, though, when the in call volume in replaced with adjusting the Bluetooth volume during the same call after boot.
Notice that I do not have Bluetooth enabled and have never used a Bluetooth headset.
How can I debug this? This looks like some service starting and messing things up. How do I find which service is that?

When i get or make a phonecall my speaker turn automatically on.
Even on the lowest volume it's still loud.
I also have to turn off the volume myself.
Is there any solution to this.

Volume bug in call

It seems that when you are in a call, the volume is FULL VOLUME, and you cannot turn it down without turning the volume UP and then DOWN. Anyone else seeing this?
tested mine(grey) on inbound/outbound calls....volume worked as it should
It seems to be the first time if you restart the phone. The phone earpiece is full volume, then you press volume UP and it goes quiet. It seems to be OK after that. So a minor issue apparently.

Ringer Volume Low While Media Playing

I recently received a replacement Galaxy Nexus (my original power button broke). I am having an issue with the new phone where my old phone worked fine--and I think I have found a solution that may help others if they are having this problem.
When listening to media, either music via PowerAmp or podcasts via DoggCatcher, the ringtone for an incoming call is reduced to a point that is almost inaudible. I can tell there is an incoming call, because the media stops, but that is unacceptable. If I stop the media but keep the ringer at the same volume, an incoming call rings at a much louder (and satisfactory volume). For some reason, playing media is reducing the volume of the ringtone.
I found a solution that seems to be working for me. I noticed that while adjusting volumes in the volume settings control panel, the volume would be reduced but adjusting the volume would return it to normal. To solve the problem I set up a tasker profile that will adjust the volume on the event of an incoming call. The tasker profile looks like this:
Context: Event > Phone Ringing, Hardware > Headset Plugged (otherwise I think this would make vibrate worthless)
Task: Ringer Volume: Level 2 (anything else seems much too loud, but I will have to try it out).
Anyone else having this problem? Know the cause? Other solutions?

Call volume low, slider doesn't do anything

This hasn't always been the case, but maybe it cropped up after the last update.
My in call volume is really low and adjusting the volume doesn't do anything. Maxed out its low. If I go down lower, to its lowest it's still the same volume as maxed. No change in volume up or down.
This is really obnoxious, anyone else have similar problems? I tried googling around but no dice. Found some old stuff circa 2013 for the nexus 5...
EDIT: Only happens in stock dialer app on my mobile phone number. Google voice/hangouts number in call volume works correctly... So it's not a hardware issue.
Yes I have this 50% of the time when on my handsfree in the car. I can ring someone, hear the dial tone is low, hangup and immediately redial and its fine. Its driving me crazy! Changing volume between min and max makes absolutely no difference to call volume.....

Bluetooth Audio Volume Jumping Around On It's Own After Call/Assistant?

Haven't seen this specific issue answered yet - if its a duplicate somewhere a point to the right thread would be appreciated.
For whatever reason over the last couple months, the volume on my pixel 2xl will jump way up (or way down) when on bluetooth either after a call or after using google assistant. It doesn't seem to happen everytime, but just now I was listening to music on my bluetooth headphones at my desk and got a call. I took the call on the headphones by answering the phone and then as soon as I hung up, the volume on the phone shot up to 100% which was not enjoyable.
This also happens when connected to my truck (Ford SYNC) or my running earbuds, and it can also crank the media volume way down (happens frequently in the truck) after a call, at which point I have to manually adjust the volume again. The same activity also happens if I activate google assistant and issue a voice command, but in both cases it is not 100% consistent. It appears like the system is trying to dynamically change the volume for some perceived situation, but it sucks at it.
I also use A2DP app for volume control settings and app launching for all my connections, but I have used that since I got this phone 2 years ago and none of the settings have changed.
Anyone had similar behavior and found a fix? It is really annoying when my music goes so low that I can't even hear it in the truck after a call, and equally annoying when I have headphones on and it cranks the volume up and tries to blow out my eardrums.
