There are several reasons why this won't work. Any one of the following will prevent the OTA file from installing. You have all three of these problems:
1) You can't flash OTA with TWRP, you need stock recovery.
2) You can't OTA if the ROM isn't stock. Which sounds like a chicken-and-egg dilemma, since you are trying to flash the OTA in order to return to stock. But it really doesn't matter since:
3) The phone has already been updated to Marshmallow at some point, and you can't OTA update a phone that has already been updated (even if you met the stock ROM and stock recovery, of Items 1 and 2 above). I can tell the phone was updated to MM at some point (even thought the main version is blank), based on the radio number. Note that the OTA file you are trying to flash says "6.12.401.4_R-4.16.401.13" which explicitly expects and requires you be on 4.16.401.13 in order for the update to install.
What you want is the 6.12.401.4 RUU (not an OTA), which can be found here (and follow the instructions therein - no need to relock bootloader since you are s-off, which is says in the instructions): https://forum.xda-developers.com/showpost.php?p=64926626&postcount=6
Note that RUU will wipe all user data on the phone.
Or alternately, restore stock TWRP backup 6.12.401.4 from here: https://forum.xda-developers.com/htc-one-m8/help/tutorial-how-to-stock-stock-twrp-t3086860
The instructions are in post #1 of the linked thread, but the file you need (6.12.401.1) is in Post #3 of that thread (which often confuses people).

Thanks redpoint73, i will let you know after installing the RUU, however can you please let me know if there is any other mirror available to download the file "0P6BIMG_M8_UL_M60_SENSE70_MR_HTC_Europe_6.12.401.4_Radio_1.29.214500021.12G_20.72.4196t.01_release_464360_signed.zip" as the "https://www.androidfilehost.com/?fid=24369303960686952" is not working.

I suggest to ask @ckpv5, who is awesomely helpful, and can probably fix the download as soon as he can.
Already in bed .. I believe I have a mirror for this on Mega.. will check tomorrow.
Recently it seems AFH always down but it should be up in a few hours.
Thanks guys @redpoint73 and @ckpv5 for the help, RUU successfully installed and working fine. Yepiiiiiiiiii

Hello,
Apologies in advance for what is probably a dumb (and often repeated) question, but I'm hoping someone can sanity-check this before I attempt it as I am a bit confused around the version numbers mainly.
I have previously had Samsung Galaxy devices (S2, S3 and S4), all of which I regularly flashed with custom ROMs, but while I am delighted to be a HTC owner, the flashing process is very different.
I have a HTC One M8 from Three (UK) and am S-On; I followed the sticky guides originally to unlock the bootloader (using the code from HTC) and then installed the Venom ViperOneM8 (currently 3.2.1).
I am trying to return to stock so that I can update to the latest VenomRom based on Lollipop; I know there are inevitably a lot of questions about this so I appreciate anyone's assistance.
The output from my FastBoot getvar all is as follows:
(bootloader) version: 0.5
(bootloader) version-bootloader: 3.19.0.0000
(bootloader) version-baseband: 1.21.21331147A1.19_2G
(bootloader) version-cpld: None
(bootloader) version-microp: None
(bootloader) version-main: 3.32.771.3
(bootloader) version-misc: PVT SHIP S-ON
(bootloader) serialno: REMOVED
(bootloader) imei: REMOVED
(bootloader) imei2: Not Support
(bootloader) meid: 00000000000000

I understand that I need to return to stock, and with S-On the way this is achieved is via a restore of a Nandroid backup and recovery, followed by multiple OTA updates to bring me up to the latest version.
My issue is that there are only 2 available links for firmwares with the "H3G" CID (one for PhilzTouch and another for TWRP; I have TWRP), in ExocetDJ and Soldier9312's thread (http://forum.xda-developers.com/showthread.php?t=2701376) and these are completely different that my currently installed version (i.e. I have 3.32.771.3 installed, and the TWRP version is 1.12.771.17).
Is this the correct firmware backup I need (albeit a much older version - 4.4.2 rather than my current 4.4.4)? I have also picked up the corresponding recovery file (stock+recovery+1.12.771+recovery.img) from the same thread, so assuming the firmware is correct then is this the right one too?
If these are correct, I then need to follow the guide to restore the Nandroid backup (copy the extracted files to the correct location on the ext-SD card, flash the boot.img from that backup, full wipe from recovery, restore Nandroid backup), and that should leave me with a stock ROM.
Can someone please confirm the above?
Many thanks in advance
Don't go with 1.x version, you'll get stuck with no wifi. You only can go one version lower 2.x or same version 3.x

Just to clarify, I assume that the version 3.32.771.10 in your post newer than my current version 3.32.771.3?
Therefore I would be better off using the 2.22.771.5 one.
You should be fine using 3.32.771.10 if available (plus I think the Lollipop OTA requires this version to update)

ok, thanks chap; i'll give it a shot.
I can always restore a Nandroid of my current firmware (taken before I start) if it goes tits-up
Thanks for the help guys.
I ran through the process last night with relatively little drama and I'm now running Lollipop and Venom 4.0.0.
Much appreciated as always.
Hi All,
I'm slightly confused and I was hoping someone could help. My phone is European with SuperCID, so I'm guessing I can install any RUU from the EU that has stock lollipop and it will work? An RUU will flash my entire phone back to stock right, including recovery?

Just did it before your reply! Sorry I forgot to include it initially.
I saw it ... you have T-MOB US version-main on EU device.
If you want it to be stock EU lollipop 4.16.401.10, this thread is for you : http://forum.xda-developers.com/showthread.php?t=2735235
Or you can simply run RUU.exe - https://docs.google.com/file/d/0B17smFr95pleeTQ5U1NzNkZzM0U/edit?pli=1

HTC One M8 - OTA Update Error
Hello,
Could you please advise me on this,
Recently i purchased HTC One M8 from Aliexpress,
And it is a TEST Device, Hong Kong Edition.
I tried to update through OTA but after downloading it shows, " Your phone running modified version of Android, So installation will be cancelled"
I read some threads and i find it more complicated, I don't know what is the actual problem itself. Help me brother please.
Locked Bootloader
Software No: 2.11.708.2
Android Version: 4.4.3
S-OFF
CID: 11111111
Post your fastboot getvar all result (minus serial no. & imei no.)
OTA depends on your CID/MID and version-main and most probably you have a wrong MID for the software version
You should have 0P6B63000 for your current software version

Your device MID is wrong. You need to change it to 0P6B63000
See this thread on how-to : http://forum.xda-developers.com/showthread.php?t=2708581
and the command that you need is here : http://forum.xda-developers.com/showpost.php?p=54909204&postcount=136
after you change MID to 0P6B63000, install the stock recovery and try the OTA again.. it should work this time provided you have a pure stock 2.11.708.2 with all system files intact.

Thank you so much,
I have two questions ( I am sorry if it sounds dumb, i am a complete novice)
1. Does this process require Unlocked Boot-loader or Root?
2.Install stock recovery? Currently i have Stock Recovery, Is that necessary to install again after changing MID? If Yes, Could you share the thread link please?
Thanks for your time. Appreciate your help.
1. It requires root. If your ROM currently no root, then you need to unlock bootloader and install custom recovery like TWRP to install root/SuperSu
or .. unlock bootloader, install TWRP recovery .. no need to install root and you can run the command to change MID while in recovery.
2. If you already have stock recovery installed then no need. But I believe you need to do as no. 1 then you need to reinstall stock recovery.
All the files needed are in my thread as linked in my signature : http://forum.xda-developers.com/htc-one-m8/help/tutorial-how-to-stock-stock-twrp-t3086860

Guys,
I fixed it by flashing Stock Recovery,
Thank you.
Greetings,
So i updated my m8 to marshmallow and then i did these steps to root
1)unlocked Bootloader
2)Flashed TWRP
3)Flashed Superuser.zip
When i did the above, the device stuck at boot screen and so i did the following
4) Recovery mode TWRP
5)Wiped data,cache, system(By mistake)
and then it said "No OS Installed"
I tried to flash the official RUU.exe (RUU_M8_UHL_L50_SENSE60_MR_hTC_Asia_India_4.18.720.8_Radio_1.24.21331147A1.09G_20.68.4196.01_F_release_417228_signe)
which i use to previously do it in case i mess up like this but that RUU is not working any more, it says please run another rom utility ..Image error..
I think the Radio version is updated and also the Hboot too(Not sure)

You can't run an older version RUU unless you have s-off.
If there is a Marshmallow RUU for your CID, it will work. You will also need to relock the bootloader (if you haven't already).
Otherwise, find a TWRP backup of the proper MM version number. Or just flash a stock/custom ROM to get the phone running again.

I'm S-ON....currently it says "No OS Installed"
Will be much appreciated if i can get assistance to get back to either marshmallow or lollipop.
and i just updated my TWRP to 2.8.7.0

Hello, First of all thanks to CKPV5 for the backups for my CID...
So when i try to restore the backup, the screen is stuck at "Restoring System" for 2 hours.. i guess this is not normal..
The following is what happened
1)Downloaded TWRP Backup ..This one-->> 6.15.720.2 https://mega.nz/#!mxZUADbA!y5Nc2PFk9...K5YlsZHdorQgXM
2)Extracted the backup on PC
3)Booted to TWRP recovery and made a backup of boot only(To check where the backup goes on my device).. it was in external SD in twrp/backups/Serialno.)
4) I enabled MTP on TWRP and transfered the 6.15.720.2 Folder to the twrp/backups/serialno.
5) Then i did a wipe of dalvikcache,cache,data,system(Did x3 times as i read somewhere)
6)Restore-- selected the 6.15.720.2 backup and swiped to restore
i waited for 2 hours+ and gave up.. So i downloaded another TWRP backup for my CID HTC__038.. i.e 4.18.720.8 Stock Unrooted ----- http://forum.xda-developers.com/show...&postcount=707
I tried the same above process.. The same problem persists... It's stuck at "Restoring system"
Did i do something wrong? Please assist.. Thanks a ton
Use correct TWRP version 2.8.7.0 or latest
TWRP 2.7.0.2 is outdated, won't work on MM.
Read again all 14 steps..
Also, don't install root/SuperSU offered by TWRP.
Later after all are ok, use only SuperSU 2.67 to root

TWRP version is really really old, like KitKat old.
I think you mean SuperSU version, as that number scheme is for SuperSU, and doesn't make sense for SU. And that number doesn't work for MM. That is the reason you got stuck after rooting.
Good rule of thumb, is always check on updated root methods after an update. The old root methods almost never work after a major OS update.

you have to get OTA.zip from previous version 4.18.720.8 then extract it and find firmware.zip. you have to flash it see this video how to flash firmware https://www.youtube.com/watch?v=ybu7fZa41so
the install twrp and restore 4.18.720.8 stock nandroid. then again install stock recovery instead of TWRP.

Greetings,
So i updated my m8 to marshmallow and then i did these steps to root

He solved his problem 5 days ago ..
OK I'm going to apologise now if this seems long but I think it important I enter as much info as I can. I have a problem with my M8 which is it will either not connect to my network provider H3G (Three here in the UK) or keep dropping off the network. It started three weeks ago I had happily been on GPE converted using enewman17 thread and all had been fine until I started getting messages telling me either my sim had been removed OR sim card added please restart your device these were numerous times a day. Things I have tried, a new SIM has not worked, I flashed back to a stock sense rom which has not worked either I still get no signal when my sim is inserted. I currently have my sim in my old Nexus 4 which is working perfectly. Now my daughter also has a M8 which is also on three network and working perfectly but I notice when in hboot all her info radio, OS ( x.xx.771.x) is different to mine. I had thought flashing a radio might be the way to go but I'm struggling to find the right one AND the right guide to follow. I'm superid, s-off, unlocked bootloader (don't think root as it's sitting with a newly flashed stock sense on, I know I'd appreciate a little guidance if someone is willing to try and help this poor old duffer out.. Thank you for reading. I have attached an image of my daughter's working m8.
Bit confused here .. yours is the one with problem (as I understand) or your daughter's one (as you post screenshot).
From the screenshot, that's running lollipop rom and correct radio and other details.
What you explained above (if it is not hardware related), looks like firmware mismatch problem.
Can you post your problem device fastboot getvar all then we'll see what you can try.

Sorry, yes mine is the problem, I thought a pic of my daughters firmware would help as it seems hers is just fine. That said, hers is running MM not lollipoop but that is another issue.

First - edit your post to remove serial & imei no.
Second - you have two options :
a) Revert to H3G and see whether this will fix your problem
1- install the latest H3G Marshmallow 6.13.771.4 firmware
2- install latest TWRP
3- restore the 6.13.771.4 nandroid backup
You can get the files and how-to from my thread - http://forum.xda-developers.com/htc-one-m8/help/tutorial-how-to-stock-stock-twrp-t3086860
Note : Your device is S-Off, no need to relock bootloader
b) reinstall stock 6.12.401.4 with RUU (as your current firmware is 6.12.401.4)
You can get the file and how-to - http://forum.xda-developers.com/showpost.php?p=64926626&postcount=6
Note : Note : Your device is S-Off, no need to relock bootloader
I suggest option 1 first to see whether the problem is fixed or not.
Usually flashing the firmware alone may fix your problem too.

Hi,
I have the exact same issue. Bought a PAYG similar from Three and not by network is detected with it. Getvar all shows exact same as lisamybeth.
I follwed the guide above but am stuck on the last hurdle... Part B returning to stick RUU. It says to install and run Htc_fastboot.exe but I'm on a mac and can't install. Exe files. I tried to fsstboot RUU without the Htc_ prerequisite but I get an error message. What can I try as a work around?

if you have troubles flashing the RUU through fastboot using PC
you can flash the RUU without PC by renaming the RUU to 0P6BIMG.zip and place it on your sd card ( formatted as Fat32 ) then reboot to bootloader ( Hboot ) it will ask you to update just follow the on screen instruction
Sent from my HTC M8 using XDA Labs
I was able to HTC_factboot using my work PC which fixed the issue! fantastic guide thank you!
Now question is can i re-flash TWRP and install HD-revolution ROM and still have Three network access?

Hi Guys,
I am new to the forum and trying to get my HTC working. The timeline of steps is as follows:
- Restarted the phone and found it is stuck in bootloader loop
- Somehow managed to install TWRP 3.0.2-0 using Squabbi's Htc One M8 Toolkit
- Installed Darkness Redefined ROM DND-N-v1.5-20170526-m8-Unofficial
Everything worked fine except for the battery was getting drained too quickly
- Flashed and Installed Android_Revolution_HD-One_M8_53.1_c8bf38 but the Wifi and SIM wont work
- Flashed and Installed 6.12.709.4_ckpv5 backup but the Wifi and SIM wont work
- Flashed and Installed 4.24.709.3_ckpv5 backup but the Wifi and SIM wont work.

I'm afraid your device has hardware issue.
You can try restore a version lower than current firmware i.e 3.28.709.6
Restore this 3.28.709.6 stock backup, reboot then see whether wifi & sim are detected. If yes, install stock recovery 3.28.709.6 or 4.16.709.9 to do OTA.
If still not working, nothing much we can offer to help.

If it is a hardware issue, RUU won't be able to fix it. If you're willing to try you can install this RUU -
0P6BIMG_M8_UL_L50_SENSE60_MR_hTC_Asia_TW_4.16.709.9_Radio_1.25.21331147A1.06G_20.69.4196.01_F__combined_signed.zip
from this site - https://easy-firmware.com/index.php?a=browse&b=category&id=1160
As your device is S-On, you need to relock bootloader before you can install RUU.
You can follow this guide (but use the above RUU, not the one in the guide)
https://forum.xda-developers.com/showpost.php?p=64926626&postcount=6
Hi,
I tried the lower version 3.28.709.6 and it still doesnt work. The RUU which you gave is not available on that site
I doubt if its a hardware issue because Darkness Redefined ROM DND-N-v1.5-20170526-m8-Unofficial works just that the battery gets drained too quickly
Sorry, my bad. Found the RUU on the site
