After I got my nexus 5x, I unlocked bootloader and root, and finally I go back official stock 6.0.1. But the camera didn't work fine(sometimes black screen, and then phone reboot). Have to flash CM13.
Following the guide, I did those steps strictly. Yet failed.
- When I tried to install CM13.zip, the TWRP told me I had to decrypt , and need to enter the default password which I don't know/have.
- I tried another way, flash official recovery.img . Failed again: the official recovery told me "can't mount SDcard", update zip file from sd card failed.
How can I install CM13 now? Or, how can I solved the camera issue?
I feel terrible about this, could you guys help?

Update TWRP to 3.0.0-1
You can't flash anything worth flashing with stock recovery.
Make sure you are using the latest stock version "J" as it has alot of fixes for this like the camera error.

Yes, I'm using the MHC19J build. I still got the camera issue, and tired for that (this is the reason why I want flashing CM13). Do I need to format userdata while I always get the red warning at booting ?

Flashing the stock firmware using TWRP should do it for you. Just perform a FULL WIPE (formatting all partitions) before flashing the rom and keep in mind that you need a modified TWRP version to flash Oxygen OS based roms. The official TWRP won't do it.
Can you explain what you mean by "formatting all partitions"?
I have a modified twrp recovery. When I click on wipe there is two options " advance wipe" and "format data" is the 2nd optioption the one your referring too as well? I flashed ROM.zip with the modified twrp without formatting data (I rather wait on your reply to be sure) and instead of getting stuck on the boot logo it is doing a bootloop now. OTA = on the air update // for upgrading your android version e.g. OOS 3.2.6 --> 3.2.8 // NOT for solo installation!
You can either use OOS 3.2.6 or the very awesome Open Beta 7 (Don't be irritated because there is "OTA" in the filename, both are complete roms)
These are full roms officially from OnePlus.net and I'm pretty sure that they will work together with TWRP 3.0.2-22.

i literally downloaded those files you linked to me and still no luck. it shouldn't be a problem that I'm using android file manager to transfer the rom over to my phone right? I'm going to wipe everything and go to stock recovery and use adb push to flash stock rom. any other suggestions you got?
Anyone please? I've rooted moto g 1st and 4th grn so it's not the fact that I'm being a complete noob. I've downloaded the modified twrp flashed 3.2.6, beta 7 and cyanogenmod. No luck on anything.

thanks for your reply. i managed to fix it. the other guy's link for a modified twrp didn't work (the custom recovery not the link) so i downloaded another and ended up installing beta 7 which worked. for future reference the link (mega unbrick) you provided me would that work with a mac?

I doubt that though there is no specific info on this .
My problems started when I was on version 3.2.4 and could not update to 3.2.6 or 3.2.8, the update downloaded (OTA) and got stuck on the installation page after my phone rebooted. I previously has TWRP installed but uninstalled it before trying the OTA update and have again managed to install it. When I click "Install" in TWRP and look at /sdcard the folder is empty with the exception of a TWRP folder. I have tried to use adb sideload toOnePlus3Oxygen_16_OTA_008_all_1606122244_e0cfc5ffc8bf411a (cant link since this is my first post, sorry) but got an error 7. Any ideas on how I can fix this?

Sounds like your encrypted? Not sure what version of twrp you are on but this is the one that most have found luck with.

If thats the case running the command fastboot oem lock
And then fastboot oem unlock will help to wipe everything and remove encryption

Are you trying to get him into more trouble that he's already in?
Locking the bootloader while having custom recovery will result in a brick!
And there's no reason whatsoever to lock the bootloader on OP3 once it has been unlocked. Everything works with unlocked bootloader (including official OTA) so just keep it at that.
To remove encryption you have to format data partition - command: fastboot format userdata.

But still, many people bricked their devices when they tried locking bootloader without stock recovery and ROM. Better not to push your luck, especially when there exist better ways of doing stuff (wiping internal storage in this case).

As soon as I got the OP3 in July, I somehow managed to successfully perform all the steps to get it rooted. I was running OnePlus3Oxygen_16_OTA_013_all_1608061823_765c081a5c8e4dad.zip
OnePlus has been showing me a "new version available" since September, but I very patiently and calmly didn't install it OTA because I didn't want to lose the root.
Finally had a bit of time today, decided I'd finally upgrade. Looked around for the "most recent stable version" and picked this one: OnePlus3Oxygen_16_OTA_035_all_1612310359_e10cadfb2af7.zip
Followed the steps for flashing using TWRP (twrp-3.0.2-1.28-oneplus3) here:

It worked. Yay. Then (god dammit **** ****) I still saw this "new version available" and couldn't swipe it away... I accidentally clicked it and it downloaded the OTA. Fine.
Now I'm stuck in a TWRP boot loop. I tried re-installing the OTA_035.zip from TWRP -- I tried restoring my Nandroid Backup (only had enough room to save "Boot / Recovery / System" partitions, NOT data) -- and I tried ADB Sideloading 16_OTA_013.zip.
I'm stuck rebooting in TWRP 3.0.2-1.
Is there anything I can try before going ballistic and wiping my data?

Anyways I flashed blu-spark with ADB. Booted into blu-spark. Wiped Dalvik/Cache, re-installed OnePlus3Oxygen_16_OTA_035_all_1612310359_e10cadfb2af7.zip, wiped Dalvik/Cache, rebooted -- keeps booting into blu-spark TWRP.
Get the "device unlocked" error screen, then the dm-verity error screen, then the OnePlus logo for about 2 seconds, and then back to blu-spark. Repeat ad nauseum.
What state is my device even in? I wouldn't call it bricked... I assume it's still rooted, bootloader works, I can flash different TWRP versions, ...
Any other ideas ... ?

After you've flashed the full zip, flash also the latest stable version of SuperSu (2.79) and latest TWRP from @eng.stk (the v11, it solves the dm-verity error). Then reboot to recovery, it should appear TWRP and the error should't appear.

There are quite a few versions of TWRP from eng.stk (bluspark). Which one did you install? Try version 11 which is supposedly for N or 3.0.2-19 or 23.

I saw a thread regarding the OnePlus 3T update some post above, if you've downloaded from there you've made a mistake if you own a OnePlus 3.

I tried flashing my old images I used last year (I just wanted the phone to boot into *something*) Oxygen_16_OTA_013 & SuperSU-v2.74. Managed to boot. Updated 4 Apps (??) But as soon as the login screen came up, it would reboot ... and keep looping. So I couldn't use the phone, but at least I managed to boot into something other than TWRP.
Next I tried to reinstall OOS4, 16_OTA_035 with SuperSU-v2.79 SR2. Still get the verity error, but at least I managed to boot! But - the phone is *super* buggy/unusable... SIM cards aren't working - I can't access my options screen - and "Developer options are not available for this user".
Now from my "About" screen I have OOS 4.0, Android 7.0, Kernel version 3.18.31-perf+ [email protected] <-- (that's from my TWRP version. right?)
Did the OTA I install from OnePlus directly screw everything up? For that brief moment before I installed it, I tasted a seemingly properly working OOS 4, so I know it's possible...
A few lessons I've learned are -- I can use any recovery (TWRP) to use MTP and flash files and stuff... but in order for the phone to work properly, I have to a) flash the SuperSU (not just the ROM) and b) make sure I have the "correct/proper" TWRP (kernel?) matched with each ROM. Is that kind of right? That's where I'm stuck I guess.

I'm also a bit confused about the best version of SuperSU-v2.79 to use, because blu_spark was writing somewhere (can't find where) that SR1 and SR2 were the cause of battery issues ... So yeah, I've got other things to try, but should the ROM from OOS4.0.1 work with any of them?

In my opinion, the only thing you can try is to make a data backup from recovery to any external storage (like a pendrive) by Otg, but it takes some risks. In case this partition has been damaged, you can get issues when you install the rom.
The safest you can do is clean installation in spite of you lose data so, full wipes and a new full rom.

Yes, I was on f2fs before this happens, so I only have to install all without formating data. You should do a manual backup to do that, once on recovery, you be able to enter phone data partition from windows. Copy all folders you want to save before formating to f2fs in recovery. Other setup and configuration like ringtones and stuff, all cache and configuration of apps, wallpaper, led, colors and themes... you will lost it. I don't know how to backup that, but its the worst part of formating data... google backup doesn't save all the config
You will start with the android phone start menu, like new, with new dactilar and patron security, etc...

Try TWRP 3.0.2_19 and Supersu 2.79 stable. For some people this works and according to eng. stk, only Supersu 2.79 stable is compatible with N.

Tried flashing twrp-3.0.2-19-oneplus3.img and from TWRP installing:
OnePlus3Oxygen_16_OTA_037_all_1701041839_aa64b084a40a4.zip
UPDATE-SuperSU-v2.79-20161211114519.zip
Same issues as described above: boots into 4.0.1, but buggy, SIM cards don't work, no Dev options, etc...

I had my OP3 for 1 year and had tested lots of differents ROM and I get used to change kernel too.
I bought an OP5 and have found someone to buy my OP3.
I went back from RR 5.8.3 to OOS, reverted stock recovery and locked the bootloader.
Everything went fine, except I had the dm_verity screen, even with locked bootloader.
I flashed TWRP and OOS 4.0.2 to use the fastboot commands to end with this. Then, just to be sure, I flashed OOS 4.1.3 to try OTA.
What a surprise, I can't to it. I can download OTA file but I can't reboot to apply the OTA.
I tried to upload the file on the internal sdcard an do the manual update, tried to apply it in the stock recovery or to sideload the full ROM but everything doesn't work.
I can't understand what I've missed.
Can anyeone help me ?

I too had that dm verity isssue after i had locked the bootloader. So i just asked oneplus to flash my device remotely. Bam..all good.

reboot PC, delete your previous adb tools, try the one attached below.
Download latest 4.1.6 build and recovery from downloads.oneplus.net
flash recovery first, boot into it, wipe everything, then immediately do adb sideload.
I will have to add that sometimes adb sideload can misbehave and you get a message that you're blocked, delete the folder you unzipped, close adb, reboot computer and try unzip and run commands again. Once sideload works and dm verity is gone you know the deal

It worked !
I had to wipe everything twice before it worked but now it's all good !
I downloaded OOS 4.1.3 and stock recovery from oneplus website and was able to do the 4.1.6 OTA upgrade.
So you guessed well, it was a previous upgrade failure and corrupted folder that blocked me. TWRP wipe hadn't helped as stock recovery did.
Just to add, the dm_verity warning is also gone.
I relocked the bootloader and can now sell my phone.
Thanks again, my problem is gone.
