IIRC its a theme on launcherpro from a member on here. I *think* its either ultralinx or, more possibly, classicalgas.
Be buggered if I can remember, But im sure its one of those two

The apk is for KitKat only. Won't get installed on Jellybean

Just find an old hboot...3.16 for example and fastboot it. Then run the ruu. You should be fine.
Also, this has been mentioned a bunch of times. So just read up and you'll find your answers.
