I got the native Hotspot working on gsm-unlocked RAZR M with Straighttalk/ATT sim!
(Not sure if this has already been posted, but I struggled with this for awhile, so I wanted to share).
It would always fail, and tell me to activate this feature call ATT.
All I had to do was edit the APN settings to include "dun"...and boom, it worked!

If you encounter issues with this apn, try (posted by MaverickHHH):
"APN TYPE : INTERNET + MMS" should be "APN TYPE : DEFAULT,MMS".
Also that thread set the following proxy, port:

Edit..after multiple test with different apn types am getting the same speed around 10-12 mpbs
Mine is Rogers LTE works on and off aftet flashing custom rom and fixed it by deleting all apn settings and just reset it to default. It pick up my networks apn settings automatically.

For the longest time while on Fido (Fido is a Rogers Sub-brand), I could not get the Tethering to work. It seems that (at least for Fido guys) the the APN for LTE Tethering should be 'ltemobile.apn' just like the regular data one.
Telus APN update
Thanks OP for starting this thread.
I finally got my LTE working. You can remove the following:
"Also that thread set the following proxy, port:
Proxy: 74.49.0.18
Port: 80"
I left that as <NOT SET> and it was fine.
I made the mistake of not checking those before flashing a new rom and it seems my performance is not as good now as it was. But then again, I am not testing on the same place.
I just entered this from another post:

I recently rooted my Nexus 4, which I use on the Aio network, and despite what I thought was a complete user data backup my network settings were reset. Aio doesn't have them posted anywhere on their website, so I had to call and get them from some dude in India. Here they are. Not sure if any of this is device dependent, but I doubt it.
