I've discovered a pretty big audio bug. Certain apps will keep playing loud audio even when system volume is muted. A couple of days ago I accidents hit play while downloading an album at work from Amazon music. It started playing loudly. I quickly muted the volume (yes, system not ringer) but the song kept playing. Today I pulled up Sentinel 4 and it was blasting. Again I quickly adjusted the volume down but it didn't lower it or mute it.
Has anyone else experienced this?

AFAIK the system volume slider only controls system things.. notifications and such.. use the media slider to control music..

I had the media slider muted too. I chatted with Samsung support and they had me do a soft reset which resolved the issue. They couldn't say what caused it though. It was like Windows support telling me to reboot.

Weird. . But glad you got it resolved.. I wonder if the problem will come back. Perhaps it could be related to an app you have installed..

Similar issue here. Sometimes when I use the hard volume buttons the software slider doesn't respond. Reboot usually resolves this but lately I'm finding myself rebooting alot. Hopefully they will push an update soon.

First Lockscreen shortcuts, then after noticing the thread here, I checked my phone for lightleak and noticed my phone too had the problem but didnt make too much of it since it dodnt interfere too much in my use.

But now the Volume controls dont work. Works nowehere, Media, In call, Ringer volume everything is stuck at whatever level they were at.
When i press the button the screen shows as if the volume decreases, but actually there is no effect. Even if Mute media volume, music plays at full volume, my in call is stuck at mimum volume and I can hardly hear people.
Any known fixes?
P.S: Havent installed any audio apps, just using stock music player

Try to bring it to the HTC Service Center for then to check... Maybe its a faulty device?

You have the issue with volume from beginning or after some days of using? This can't be a hardware problem, try to restore factory settings...

This has happened to me more than once on various ROMs from CM9 to CM10.1 (I haven't used a stock ROM in a while): I'll be watching YouTube (or playing a Flash game - I'm not sure if it's restricted to Flash videos though) with the volume at the lowest setting or muted, and all of a sudden it blasts to full volume!
This is most likely to happen at night, when it's conveniently the most annoying. I hesitate to wear headphones now because I don't want to kill my ears - it seems louder than full volume, although that may just be because it's quiet otherwise. I'm using Volume+ at the moment, but from what I can tell the issue is still present without it. Unfortunately the jump is completely random, so pinpointing the cause hasn't been very easy.
Anyone else have this issue? Searching didn't turn up any similar-sounding threads, but I may not have used the right terms. I'd just like to know I'm not the only one...
While I'm no expert, i do have a few thoughts.
The first thing that came to mind was profiles. You mentioned that you've been using cm9/10, could you have set profiles with different volume settings to trigger at different hours of the day?
Also, are you certain you haven't accidentally held down any volume buttons? I've had multiple instances where i accidentally hold the volume button and my volume will go from 0 to 100% without showing or any of the volume levels in between.

I was just listening to music with the phone on the floor and the volume jumped randomly halfway through the song, so it's definitely not an accidental button press.
I'm going to uninstall Volume+ and Voodoo sound (I wasn't using the latter, and the former was probably killing my poor speaker and my family's ears) and test it again, but I'm fairly certain the problem started before I installed either of them a while ago.
I'll check back if it happens again with no volume modification apps installed, which if my hunch is correct, should be fairly soon.
It seems like a random hit and miss glitch. For instance, if I'm watching a video on YouTube while my phones volume is on silent, I can't raise any sort of volume. Not the phones, not the media. So I have to press the settings button, manually slide the phone volume and then media volume raises. This is a big issue for me because I want my phone on silent while I watch videos.
My old HTC One S never had this problem. Is this exclusive to Samsung? Cause I noticed the S3 also did it when I had it.

I play music off my phone in the car. After I answer a call, the music resumes at a much lower volume, despite the fact that I haven't touched the volume on the phone or car. I have to press volume up 3 times and it doesn't go gradually up, but instantly from the low volume to the high it was at. Is there a setting to get the music volume to maintain? Using poweramp. Thanks

I continue to experience this same problem on stock 4.1.2 ROM. I suspect it is a bug in the ROM. I don't know how to duplicate the problem because I have gotten calls and the volume was correctly restored. I've also tried making calls and the volume was correctly restored.
check all your sound setting and make sure that the volume is up on the system part of the sound menu...i had the same problem ...that seemed to fix it for me...

I'm having a strange problem where notification sounds will spontaneously start playing at super low volumes. And the only way to rectify this temporarily is to do the hardware check under device help that was mentioned in another post. However this solution vanishes after the phone is restarted.
I noticed this with the stock notification. So I installed other notification using Zedge. They seem louder.

It's not a stock notification issue. I tried using an external sound. Same problem. It's really annoying and obviously a software glitch. I'm just surprised no one else is having the issue? Again the fix I'm using while effective at making the sound louder for notifications, stops the volume of media from being able to be controlled via the volume buttons.
I am having the sound issue as well.
The first time I noticed it, I was playing with the JBL speaker and when I took it off the volume was low...both for media and notifications. After a reboot, media and notifications seemed fine. Later I was showing a co-worker the JBL Speaker...this time, stopping the music before removing the Mod...the sound volume sounded fine. I think it may have something to do with removing the Mod while its being used??? Can anyone else recall what they were doing when they noticed the difference in audio levels?
I'm not doing anything... when I reboot EVERYTIME, the volume is messed up... It takes me messing with "device help" to get it working again.
Facing the same issue.
Let's hope they will fix this issue in next software update.
