SD Card Problems With Clockwork Recovery
Hi there XDA, basically everytime I have an SD-EXT partition on my phone (i always, always make them using Linux manually) if I format my SD-Card (which should only format the Fat32 partition, I run into a big issue. My SD-Card will un-mount, possibly format i don't know, then it won't remount. If I connect my phone to windows the SD-Card has all its partitions removed. Very strange?
Anyway I decided to switch to Oxygen 2.0.1 this morning, as it's a much more stable rom, and anyway I performed what I said above on Froyo 2.1 (DeFrost) and the SD-Card wouldn't mount, when I noticed that the SD-Card didn't have any partitions I formatted it using Windows. I then placed the update.zip file of Oxygen onto the root of my SD-Card.
I proceeded to Clockwork Recovery and selected the option 'Select .Zip from SD Card' and I was shocked to see something saying No Files Found. We I have tried everything to sort this out, and absolutely nothing seems to be working? I have used a Ubuntu Live CD to try and completely wipe and re-format my SD-Card which completed sucessfully, but no joy, i've tried all the options on Clockwork Recovery, nothing works, I used a 2GB SD-Card to install Oxygen and this worked fine, once the phone was booted everything would work including the 8GB Card if I switched them. I know I should just put up with this, but I need to fix this.
Does anyone know how I can get my card to work again thanks.
Right now there is no ROM installed on my phone because I just wiped my phone, but when I went to install a new ROM, it doesn't seem to recognize my external SD card. I tried 2 SD cards and neither of them work, but they work on my other phone. How can I fix this issue? I went to mount and it says "E: Unable to mount '/external_sd'.
Update twrp if available, if not try to reflash it and if it still doesn't work then try cwm

The latest TWRP doesn't seem to recognize my SD card either. Also both of them are saying that my device is not rooted and it asks if I want to fix root. I'm trying CWM now but it isn't finding anything either.
Edit: CWM says my device isn't rooted as well.
Edit 2: Flashed http://www.androidfilehost.com/?fid=23212708291679687 with Odin and when the phone boots up, it's stuck on the Samsung logo. Any thoughts?
Factory reset. Incompatible data probably causing boot hang.
This will wipe apps and data.
Thanks for the help everyone. I flashed back to stock and re-rooted. However it still won't read anything from the external SD. Does this mean the SD reader is broken?
Can you plug it into the computer? Does it see it there? If so what is the file system type? Can you format it?

Yep, the computer only sees the phone's harddrive, not the SD card. On my other phone, the SD card is working fine.
No, I mean plug the card into the computer. Not the phone. But if another device uses it ok, then you msy just need to reformat using the computer.

Reformat the card? I will try that. But I have two SD cards and neither work on my S3, and both work on my S4.
That tells me the cards are ok and something is probably wrong with the device then. If you format one card to fat32 and it still can't read it, its probably hardware.
@bjornturoque Here was my problem. Exactly like yours. I am BL unlocked, Rooted, TWRP 2.8.7. I am having a problem with the ext sd card not being read in TWRP. I have had ver 2.8.5 / 2.8.7 / 3.0.2 of TWRP and none of them will read the card. I have tried formatting the card with my Note4, GS 5, Tab S, and my PC using SD Card Formatter. Still getting the same result.
Here is what I did to fix it.
Install Xposed, Download Wanam Xposed, activate the module, open and go to security hacks and check the box for SDcard RW permission, then reboot.
I just did this to mine and it fixed my problem. Hope it works for ya.
Format SD-card via Windows (FAT32/Default Allocation).

No, doesn't work
I have a 128 GB SD with exFAT, it worked without problems in TWRP 3.0.2 recovery under CM12.1
Now I am on CM13 and beside the bluddy permission google-bug it works in the system but not anymore in TWRP 3.0.2 recovery.
I tried to fix the permissions to write to external sd (this 128 GB sd) by adding stuff to the /data/system/packages.xml file, see this post.
But I didn't change the wrong thing I am pretty sure and doubt that this is related, as TWRP comes without using this file, I guess.
I flashed 3.0.2 again with Odin, no change.
Fact is, that I installed CM13 with another SD-card, a 64 GB FAT32, not this one. Maybe it never worked under CM13. But anyway, what has TWRP to do with CM13 and its permissions? TWRP is previous to any CM system, which is not running at TWRP times.
What could that be?
I flashed now CWRM v6.0.4.7 (ClockworkMod Recovery) but the problem persists:

What is it? The sdcard is working flawlessly in the running system and another FAT32 sdcard works in TWRP. What do I miss here?
Is it that TWRP and CWM doesn't know exFAT?
I think to be quite sure that this was working, I have exFAT since a while now.

Hey all!
I have a SD Card dilemma... I use philz_touch_6.57.8-m8 and in recovery... it can't mount my external sdcard if its formatted to xFAT... If I format it (in windows) to NTFS it works! But the issue is... neither sense or GPE read NTFS.. it says card is blank and needs to be formatted.. so what can I do?
Its obviously better to have it in xFAT so I can do all backups and recoveries in recovery... but everytime I need to send new roms or do backup, I need to unplug the microsd and plug it in my laptop (with an adapter)...
Also... I did root, unlocked bootloader and s-off... converted from sense to gpe, and sometime along the way my sdcard is mounted as /storage/sdcard1.. it was originally /storage/sdcard0
Anyone got some ideas?
edit: in recovery.. I can't mount /sd-ext or /storage/sdcard1
nvm.. I made a "default" format in Philz and now its working in both rom and recovery
I tried both CWM and TWRP, but neither mount my SD card if it has an ext4 and fat32 partition. It works if only fat32. Android can see the SD card just fine.
SD card is Samsung EVO 32GB
CM12 with persistent recovery.
After that failed, i changed whole card back to FAT32 and tried to partition in TWRP (CWM didn't have option.) Twrp to partition.
Used windows 8.1 with minitool partition wizard.
Maybe you can try FOTA CWM and TWRP recoveries for custom ROMs

Thanks for replying, but I have not pressed thanks, as I stated in OP that I'm using those recoveries. The actual problem for those interested was that I had the partitions as logical, not primary.
However, it still takes ages to Mount, and it's a UHS card. Is it due to size of partition or something else?

One: nobody comments here to get thanks.
Two: both partitions must be primary. I repeat, BOTH.
Three: with 4.3 stock, ext4 usually don't work for xperia m, but fat 32 works.
For CM12 EXT4 definately works, fat32 don't, apps disappears after reboot if you have fat32 on cm12.

TWRP / Wipe / Advanced Wipe / Format Data
Reboot Recovery
Flash a Rom
If that doesn't work you'll need a RUU.zip
rename the RUU.zip to 0PJAIMG.ZIP, place it on a ext sd card formatted in FAT32 (a 16 or 32gig one should work), reboot into DOWNLOAD_MODE, agree and it should flash.
I tried to install a RUU but i have not renamed it to 0PJAIMG and pushed it on the device
anyway, i got help by the #twrp IRC channel, i formatted the /system partition with ext4 via adb again, because it was emmc (there was no filesystem on it) and not f2fs.
