Ok , this is the deal my fellow hardcore developers .... My Galaxy Nexus has a rare ( first time i see something like this in all my tinkering years ) condition where the memory of the phone is like in a locked state , you factory reset the phone .... enter the bootloader --> go into recovery and wipe cache , dalvik , data , etc ... all the wipes available and once the phone restarts everything is back there !
I tried the odin method (Nand Erase All) , odin fails when its writing the Image portion of the files ....
I tried cyanogen mod installer , cyanogenmod installer reboots the phone , goes into bootloader , unlocks bootloader , injects clockworkmod and then the software continues "installing" until the point the phone restarts .... guess what ? original software comes back !
theres no way to restore the phone ! and clean/erase everything on it , i tried the regular phone reset ... didnt work , phone goes thru the procedure but when it restarts everything is back ! if you uninstall an application once you restart the phone the app is back there ! the phone is not encrypted either , i sincerely dont know whats going on here and what i can do to factory reset this device ! open up to any suggestions and methods !

Not a rare problem at all honestly. Happens all the time with flash memory.
Two things to remember.
1. Not all NANDS are created equally. Some may last longer than others.
2. A galaxy nexus usually has more reads/writes than most phones.(flash more roms etc..)
Plus the majority of the gnexus are getting pretty old.

Never thought I'd be the one to create a thread like this..
After I flashed the OKV4 ROM, strange things started to happen (this was after a full wipe). Apps kept force closing, etc. Later that day, when I picked up my phone, I saw the samsung logo flashing over and over. Making me unable to enter recovery.
Download mode still worked though and I proceeded to flash a stock firmware via ODIN. This did not help. Oh, and the Odin flash succeeded, without rebooting my phone though...
So, I'm still stuck with my phone which can't seem to get out of this strange loop.

I remember reading somewhere here that if you have flashed a diff FW now and then a possibility the last recent kernel is still in the system folder etc....this is maybe the cause of the bootloop.
Since you had confirmed going to download mode is possible..this may work logically... Why not do the following :
Get your phone rooted via Recovery (there is a thread here by The-E)..Once done flash arco68's CWM Recovery....Boot to recovery n do this :
Factory Format....Wipe n Clear including Delvik...Then go to Mount n Storage wipe every single folder...System...Data...Etc (this will wipe every single data left behind but I assure you the CWM will not be wiped) once done reboot n put the device into DL mode and use Odin to flash back to the stock FW.
From this moment the recovery will be replaced by stock version and the stock FW will be restored....once it boot up..Walla as I had done this over two times :

This isn't the ordinary bootloop where the boot animation plays. It resets literally 2 seconds after starting. This makes me unable to enter the recovery at all. Thanks for the effort tho.
Tl;dr:I can not enter recovery mode.

Hello everyone ...
Yesterday I ran out of battery and phone shut down . Today when I want to turn it on with full charge , it hangs on boot animation and won't boot at all ...
I wipe cache partition , dalvik and even factory reset and didn't help . So I re-flash my ROM ( RufDroid-TeamShogun v3.0) and it seems it's ok right eventhough I should re-flash many times to use this ROM ! whenever I get to setup screen after flashing , it pops up System UI Has Stopped and it doesn't go away after ok and say again SYSTEM UI has stopped !
Anyway, After two or three flash it'll be ok and I setup the phone . But right now the problem is I cannot turn the bluetooth on ! it won't turn on at all . I did dalvik , cache or ... wiping but doesn't work . I even re-flash the phone with this ROM but doesn't work again . The bluetooth was ok on this ROM Before yesterday .
I Need to mention yesterday before raning out of charge , Phone felt from my hands to the ground but it happened lots of time before . It can be because of hardware damage ? I think it's more about software ...

How about you try with changing the ROM? Just for the sake of it, to see if everything works on another ROM ... If it indeed does, you could REdownload your ROM and try to install it again ...
@OP, if you suspect hw failure, return to stock and check if it works. If not...

I helped him to unlock bootloader , flash twrp , and root this **** ( i know N7 is great im jealous )
But hew wanted so badly cyanogen rom so he dv-d cyanogen and flashed it everything were fine , till 1 day ago he had force closes in Settings when trying to enter wirless & networks more / mobile networks - he got " settings stopped working and nothing more happened
also he got that kind of fc when entering about tablet / status
and for the final problem he got his network unable to connect like there were no sim card " inser the sim and reboot tab ".
He asked me what to do sim was fine so i told him to flash cyan rom again via Twrp and here we go :
He , did backup, had cyanogen rom already and As im a GT2 7" user i told him by routine to wipe system, cache , dalvik cache , n DATA -.- my bad now he has unlocked bootloader , working twrp , windows recognize an "Android device" but Adb devices shows nothing what to do now , im not in to twrp im hard user of cwm so i dont rlly know what i can do with twrp to help him.
I think there are 2 options :
1. try adb **** on other laptop
2. Force flash stock image
What do you think ? If u can answer as fast as you can i hope there is an easier way to mount data part and just transfer rom.zip to dhis **** .

And silly q why twrp when trying to copy log to sd shows unable to write otg but "success saved to data/.... " ?

You should be able to transfer a ROM.zip to the tablet with the adb push command then flash it thru twrp. Just factory reset thru twrp before flashing the ROM, a standard wipe should suffice, no need for any advanced wiping.
Make sure you have ADB, and the SDK+ the proper drivers setup on the computer.
Also, make sure you are in fastboot mode to use adb (volume down+power), you should see an android with his insides exposed, and the word start with a green box around it.

I know i should be able to do so , and i know that i need to enter fastboot mode. The only problem is that adb dont see device , ill try it on my clean and "everything works" laptop and weell see mayby he did smth to drivers or other ****

Lol totally not trying to say you sound/are dumb, don't know you from Adam, sorry if I came off that way, just going thru the basics, you never know with the people on the interweb
Since adb won't recognize it, I would just use the wugs fresh toolkit. There is an option to "flash stock+unroot", it should get y'all back to square one, and it will also reflash twrp and root the tablet, with a couple clicks.
That was rather a silly joke by me I know u did not wanted to say im dumb cuz some ppl re just forgetting about simple things like turn your phone on to call a friend im thinking about wugs toolkit but i dont like (personally ) toolkits that are doing smth by 1 click i have briked that way my 1st phone with wm by instaling rom throught 1 click installer and since nexus isnt mine im just carefull about what im doing and saying now but thx ill jump to mah friend morning and try to fix his problem
I've used wugs toolkit a few times and it works like a charm.
Hello guys i need help with my galaxy nexus. I'll explain what happened - I was running zmod 5.1.1 since quite some time without any problems, but the phone had become slow due to loads of apps on it so i decided to do a fresh install of zmd 5.1.1 again. I did the fresh install and the phone was working absolutely fine for 2 days, but yesterday the phone restarted randomly and since then the whenever i try to boot the phone it restarts when it reaches the lock screen. Some times it doesnt restart and stays on but it becomes highly unresponsive and eventually restarts. I have twrp recovery and sadly i forgot to do a backup after the fresh install of the zmod rom. I am able to boot into recovery and the phone is recognized by my pc when i connect with the usb cable. I tried to place the rom and gapps again into folder when i connected my nexus to pc but when i go into recovery the files are gone.
Then i tried to troubleshoot with nexus root toolkit but it says some drivers are missing and for complete driver installation i need my phone to work which isnt working so i left it. Then i tried to manually flash stock rom with help of adb and fastboot. Every command worked perfectly with adb and fastboot and i managed to flash stock rom 4.3 yakju jwr66y factory image, but as soon as i booted the phone it began booting with the zmod animation and restarted at lock screen. I am clueless as to what has happened and need experts to please help me out as this is my only phone.
P.S i do not post here regularly so if i have made mistakes posting this query please forgive me.

Your eMMC chip may be beginning to fail .One little sign of it is when you said you copied files to it but they weren''t there . Here's what I mean . You copy 20 files to the phone and place them in downloads . You then boot to recovery but you don't see the files . In comparison you delete 10 files using the pc . You boot into recovery and the 10 files are still there . So you hook it to the pc with a toolkit and successfully restore it to a factory image and when you reboot it's not the factory image but the old rom . That' is a clear indicator that the eMMC chip has likely failed or is failing . The chip is sort of locked in a snapshot , or last picture of the phones state before it failed . No matter what you do nothing changes . Google bad eMMC to learn and get a better explanation than I'm giving . I have one nexus with a bad eMMC . Mine won't boot at all , and can finally get into twrp after about a 15 minute wait . Then if I click on anything in twrp it's another 5 minute or more wait . I tried everything . My suggestion to find out is get wugfresh nexus toolkit . Follow all the directions on restoring the factory image to a bricked phone . If you are successful in restoring and you reboot and you see the zmod animation you might as well toss the phone in a drawer and get yourself another phone . The reality of it as best i can tell is you really are adding and deleteing files and you really are restoring the image but through the eye of the eMMC you haven't changed a thing . It has to function and see the changes . Compare it to looking at a webpage . Everything you click actually works but all you see is the same damn page . The connection is broken between whats displayed and what's really happening in the background . Hope this helps . It's the understanding I have of it and some of it might not be exactly right . If anyone with the knowledge disagrees with any part please feel free to correct and explain any part of it . Thanks

Faulty motherboard. Try to find a second hand one on eBay. I had the exact same problem and a "new" motherboard solved it

my nokia xl firmware version : 1.1.2.2 model rm 1030
after i installed an app called super voice ,, << it should be called super virus
my phone started to hang then restart after some minutes
so i tried factory reset and flashing rom using (nokia care suite and factory reset)
but every time i do that when phone starts again i find that every thing came back even super voice app ,, all the photos in storage etc ,like i didn't even factory reset or flashing rom

IS your device rooted? did you granted superuser permission to that app. if you did those you have to void your warranty to restore your device.. follow guides on xda to flash TWRP and wipe the system partition. if you are a noob in this thing message me and i will help you as best as i can. wiping the system partition only will brick your device. if you did a a wipe you have to flash a rom to boot into your device

my phone now is stuck in the nokia screen
when it was working i tried wiping data using TWRP ,, didn't work and everything goes back to normal like the phone memory is freezed and won't change ,,
i tried rooting it using TWRP but then it gets stuck in black screen until u remove the battery ,, then u will find that the phone isn't rooted ,,
,,
i am sure it is a hardware problem ,, but i am not sure which part is causing this weird problem
