After flashing the t-Mobile stock image T989UVMC6_T989TMBMC6_TMB downloaded from Sam Mobile, and Odin did say PASS
Now, the phone is displaying "Firmware upgrade encountered an issue, Please select recovery mode in Kies & try again."
The phone is connected to the computer, Kies is stock on "Connecting" for very long time then it says the device is having a problem connecting and to solve the issue by restarting the device.
The phone displays a yellow rectangle with exclamation mark in it.
Any idea how to get out of this?
Solved
Even with the exclamation mark on the phone screen, I started Odin, it did see the phone, reflash the MD5 stock image file, left auto reboot checked, and everything is back to normal. Phone started fine.

i just got this phone and tried to root it. i installed the kernal using oden and it went over without a hitch... but then after the phone turned off it got stuck at the boot screen with the yellow triangle and wouldnt go any further. after some time i unpluged it and turned it off and on. same thing happened. so i flashed the kernal again. this time when i turn it on it sais "firmware update encountered an issue- connect to keis in recovery mode.
i tried connecting to keis but it sais the device will not respond. i cant enter clockwork recovery mode cuz it just goes to the same error screen. i can only get into oden mode. i have a i9100 mugkg2 on tellus. nothing is working. i also tried using oden to install the stock rom but it fails every time. please help! how can i just get it back to factory
It sounds like you've bricked your phone, have you tried a USB jig to try and reset it yet? Try ebay, look for i9100 usb jig

Install KIES, then use the Unlock Root

Hi teck2100,
If you can get into odin mode (aka download mode) then you're fine. You should be able to flash a stock rom using odin.
Look here for a list of roms: http://forum.xda-developers.com/showthread.php?t=1113928 and pick one that you can root using D00ml0rd's method (zergrush), it's the simplest way I've found for root and Mobile Odin is the best app for custom rom's in my opinion.

Used Odin 307 with PDA and only Auto Reboot checked each time. Flash went fine completed successfully with Pass each time. Unfortunately that's where the
success ended. The phone rebooted with the TMobile Logo and Samsung logos flashing on screen and immediately went "Samsung is Upgrading 155 applications."
After this process it rebooted once again Flashing the TMobile and Samsung logos. Once booted, the phone screen is black with no icons. At the very top of the
screen, I have a battery icon, the time, 4 bars despite having no sim card in and that little eye icon. At the bottom, the back and menu buttons light up when
touched. The phone is recognized by windows and does allow me to browse the file folders. On the off chance that my file was corrupted somehow, I downloaded it
from a second source the 2nd and 3rd times I flashed it. Now I'm stuck. Nothing I do changes this condition.
Please Help!

The TouchWiz is back and all the icons, it goes through the setup process but it's unable to turn wifi on and it doesn't seem to see my sim card. Aside from a hardware failure of some type, I don't know what this could be.

Have you tried going into recovery and doing a "Factory Reset" and reboot, it might fix the problem.
Also if it does not fix all the problems you can try to boot the phone up just to get into the SETTINGS
screen, then goto settings>accounts tab>backup and reset> and select "Factory data reset" then
reboot the phone if it does not reboot on it's own.

I got this phone from a friend that was downloaded with Mean Bean. I then flashed a factory rom using odin that passed and ran correctly. However, after doing an over the air update it crashed. Now I am unable to get to get to recovery (power, volume up, home button). when i turn the phone on it is stuck in a Samsung Galaxy Note II logo boot loop. Once i do a flash it goes through normal routine and when it resets the phone shows that it is downloading (movement of the blue bar at the bottom) but after it reaches about half way it resets shows the logo then starts the download process again. However, odin says pass..
I have tried using a re-partition pit file, re installed drivers, tried other factory firmeware files.
If anyone has a suggestion I would appreciate it
Recently, when trying to flash a CM rom on my Epic4gTouch, something went wrong and I now have the 'Firmware upgrade encountered an issue. Please select recovery mode in Kies and try again.' error. Knowing how to fix this, I open up Odin and attempt to flash a stock samsung rom for the phone. I hit start and things seem to be going well. Both progress bar on Odin and on the phone started to progress. Then all of a sudden my phone rebooted! I got the 'removed' message in Odin and the flash failed. It rebooted right back to the same screen as before and now every time I plug the phone in it restarts after around 5 seconds. This makes it almost IMPOSSIBLE to fix my problem!
Yes, I have tried removing the battery for hours and trying again, I have tried different USB ports, two different USB cables, restarted my computer, reinstalled Samsung drivers and nothing works!

EDIT: When holding power and volume down, I get in the upper left corner in red letters: Low battery!! Can not download!! (Even after having it plugged into the wall for around an hour)
I have a Samsung Tab 3 (SM-T210R). I am currently stuck on "firmware upgrade encountered an issue" screen. I have connected to Odin 3.07 and re-installed my stock ROM. It is successful multiple times now. The issue is that whenever the device restarts, it reboots to the same screen "firmware update encountered an issue". I got to this state by trying to flash a backup in TWRP. It failed, and now I can't use the device whatsoever. The device also will not turn off. It will try, but promptly reboots to the same screen. I have also tried re-partitioning with the proper PIT file along side the firmware ROM. This was also a "PASS". Any ideas?

Have you tried forcing a reboot to recovery using the button combination? HOME, POWER and VOL UP buttons.

Yes, I have. It just turns off and then reloads the "firmware update encountered an issue" screen.
Take your battery out and put it back in.
I think you can flash with Odin in state "firmware update encountered an issue".
