1 in a million chance your card got damaged....if you have another card, try with that. I would start though with a complete wipe and re-install and go from there. And if you use a program to restore apps, dont use it right away and try installing from play store first.

Wow it sounds like you really did a number on your phone.. maybe try clearing the cache & dalvik cache. if that doesn't work I guess resetting your phone is worth a shot. Goodluck with it!

also first time when I format it to exfat I put 4096kb than default maybe that it last time I dd do that. It's not the rom or phone it the cardim clear dalvik and cache right now to see

I have tried everything I know to do so I'm here asking for help....
I run Calks ROM and recently tried updating without wiping (normally never do). After a couple days of a buggy phone, I decided to wipe data, format, format, format, wipe data, etc and flashed it again. Start using it and it starts acting up again. Did my wipe/format deal and flashed stock...and left it stock. Within the first day it locked up and reset itself multiple times.
I don't know what to do...is there anyway to start any "cleaner" than formatting everything multiple times? I would really like a working phone again...
Are you restoring your information with titanium backup? If you are that's your problem.

+1. You can restore apps only, but don't restore apps + data.
Is this the only rom that's causing it?
I never, ever, ever use any "backup" anything. All apps get reloaded one by one and settings get modified one by one...
This is happening with every rom I try. I format EVERYTHING (except SD card, which is my next attempt at fixing...). This is why it's so frustrating. I can't blame anything. I try running the ROMs stock (no not-included apps) and the phone is still running like crap.
Tried deleting the ".android_secure" folder from the sd card?
If it's happening on every rom, and you completely wipe every thing correctly, the only thing they have in common is the sd card contents. Right?

Android secure is supposed to be deleted during the fActory reset. I will try getting the SD formatted tonight and start FRESH, FRESH....

Mines never been deleted from stock factory reset or reset with amon.
It started to cause problems with app downloads from me switching roms and reinstalling all my apps and moving them.
Just remember, if you have apps on your sd card (Not on a separate partition) move them to the phone before deleting it if you want to keep them.
Any luck fixing it yet?

I've been traveling for work and haven't had a chance to sit down and mess with it since the night I initially posted. Hopefully it is as simple as androidsecure not getting deleted...thanks for the heads up.
I just did a FULL FULL FULL wipe of the phone. "Factory reset," "format; SD, SD ext, system, boot, data, etc"....
I then flashed zip, rebooted and let it sit for 10 min before turning it on...upon unlocking, it locked up.
THIS IS GETTING RIDICULOUS!!! Is it possible the phone is just effed???

No good either...the last "good" backup comes up with "checksum error."
I'm not big on nand backups, since I usually do full wipes and restart from scratch. I don't really care if I can restore an old setup.
If you can't find a software reason for its acting up, it's a hardware problem. Full wipes take care of pretty much any software problem.
I would suggest unrooting and taking it by a Sprint store. If none of the suggestions you've tried so far have done any good, that's about your only option.

That's not an option either...as I bought it outright off ebay.

For the past 5 months I have been using cm7 with no problems. But in the last month or so problems have been slowly coming up. It started with phone freezes then it slowed down greatly, then I got fc on EVERY APP! So then I switched to synergy hoping to fix the problems but there still there. I need help quick because I can't even make calls with out it freezing.

have you wiped everything (data, cache, dalvik) in recovery BEFORE flashing?
I just went through the same thing with the stock rom (rooted without the verizon crapware).. Had it since Day 1 of availability.. then a few days ago.. force closing all the time..
Even when switching roms. Even when NOT restoring with titanium.. Did everything fresh (ugh.. you have no idea how many hours).... and photos were disappearing from it
My first thoughts were "frak... I really screwed up something" (my niece dropped in on the floor a few days earlier.)
Then the IT in me decided to fall back on my experience. K.I.S.S. (keep it simple stupid).....
If even changing roms didn't fix it.. what did everything have in common? The SD Card.
So I formatted the card making sure to take everything off I needed. Put it back in.. and put a fresh rom on.
No problems since. I'm guessing the file system of the SD card went a little wonky after a year of putting files on.. taking off.. putting on.. etc
Yep I cleared everything, data, cache
D cache and batt stats. Im going to try the SD card thing right now

Just gonna throw this out there. Try also formatting /system in the "mounts and storage" menu.
So far so good. The SD trick worked. But I want to put my titanium backup folder back on my SD card to get my apps back but I am not sure if that was the problem.
Also about the format /system l, I was thinking about that too. But if I can't get things working in the end I will format the system, data and cache.

Glad it worked.. after 3 very frustrating days for me and trying multiple roms and losing a few pics... glad I was able to at least past the info on to someone else
I am having this problem now with apps that are not installed on my SD card (which I just upgraded to larger size). All the Google apps are fc and some other ones here and there.
Were the fc apps all installed on SD?
I was thinking about a fresh gapps flash but may try the SD card first.

Glad I found this thread! I've been having same problem and couldn't figure out if it was a rogue app or not. It seemed like right after I flashed a new rom it would run great but after a few days I would start having trouble and eventually I'd have to flash another new one or try same one again.
I have a question though....I know most things on sd card are ok to delete since any app will put new folder on there if it needs to (obviously keeps pics, titanium backups, rom backups etc.)
But I don't have a way of formatting the sdcard OUTside of the phone. I don't have a card reader on my computer. Would it be ok to do this while the phone is plugged in? (something tells me no but...)
My only other option would be to put the sdcard in my old HTC ozone and try it that way...
You can just plug your phone into your PC, mount your SD card and reformat it that way. It works for me.

Random thought
You can try to repair the SD card before you format( if the repair doesn't work). Mount SD card to the computer via usb. Right click, properties, tools tab. Run error check. Just like a computer files get corrupted.

I've had my Inc2 since 2011 and have flashed different ROMs more than I can count.
In the last 2 weeks, I keep getting SD Card was removed messages.
I reboot and its back. I figured the SD Card was dying on me...
Since then, I've switched to a NEW SD Card and am having the same troubles, but more frequently, like everyday.
In fact it took and hour to get it to work again yesterday.
Anybody know whats going on?
I'm running the same ROM I've had for months, I've used a couple different ones over the weekend to see if that was the issue... It's not.
I've cleaned all the contacts and blown our the SD Slot.
Is she dead Jim? or am I missing something?
Have u tried using a different rom?
If u installed the original SD card incorrectly on the same rom maybe it's remembering that?

Assumed that you've already saved contents on your mSD card, wipe it clean on a PC/Mac/Linux OS (Fat or Fat32) then put it back on the DI2,
boot into recovery - wipe dalvik & cache, REFORMAT the mSD card - then boot up normally.
What Rom are you running? If above steps still do not work, try a Master Reset from the Rom before doing a Clean & Fresh Install of the Rom.
Keep a watch on error messages on the screen as you do all these & post your results, good luck !

Been Runnin Miui for the last year. No changes on phone.
SD card acts like it was pulled. Have to poweroff and remove battery to have it back.
A reboot into recovery still doesn't give me access to the card.
I've also tried different SD cards with the same results.
Anyone reading this with this problem, try returning to stock recovery and rom with the pg83.IMG(I think that's the name) and try doing a factory reset in stock recovery. My SD card works after that

This problem has gotten worse. And most of the time the SD card isn't even available in recovery either.

Yeah that's about what my problem was. Usually if you go for about a day without an SD card in, it will most likely recognize it again. If not, I could usually tap the back of the phone on a table, down near the bottom where the SD port is.
If that still doesn't work, you'll have to fastboot flash the return to stock image
My theory on this problem is that after flashing Roms/s-off and messing with stuff eventually corrupts some part of the filesystem. You might also try following the guide to unroot and s-on to see if that works, though that will be a little harder.
Flashing the ICS firmware didn't help.
Guess I'll have to try going the long way around...

Got the PG32IMG factory restore... that was a wash. Got everything but s-on done... and upon getting through the setup... SD Card removed unexpectedly.
So had to use ADP and FASTBOOT to get a recovery installed and back to square one.

Did you try doing a factory reset with stock recovery though?
I've read about some people with roaming problems on their HTC one, and flashing the stock recovery back and doing a factory reset fixed it.
Sooo give that a try if you haven't already
Edit: that probably sound un related but maybe after messing around with HTC devices they get mad and sabotage themselves haha. But seriously though. I messed around with the return to stock image a couple months ago, and I could get it work for a very short time, like you experienced. But just the other day I entered stock recovery mode from bootloader and it started doing a factory reset on it own. After it started up it recognized an SD card when I put it in and i then fastboot flashed a custom recovery(not cwm or twrp can't thnk of the name) then I flashed the andromadus mimicry ICS ROM that was still on the SD card and right now I've been playing music off it for about an hour and it seems to be fine
My note 3 reboots several times a week since the upgrade to kit-kat.
I tried searching around to see if anyone else had posted about this.. but I didn't really find anything.
I'm sure you all have had it at least once (not just kit-kat) - but its your classic, you hear the vibrate the screen turns off and then its booting itself up.
This is a stock Note 3.
I'm assuming if I don't hear any feed back on here I will just get it swapped - kind of annoying.
Could be a number of things. If it's not rooted, I would first suspect the sd card. Try pulling it if you have one and see if it stops. If it doesn't reboot with the sdcard out, try formatting the sdcard with the phone, or better yet, try a different card.
Another thing to try would be a full factory wipe and reflash with either kies emergency recovery or odin afterwards.

I think if the recommendation is a factory wipe I might as well get brand new hardware out of it.
Any suggestions that don't include having my phone wiped? Perhaps clearing cache? I haven't done it with this phone yet.. guess I will look that up.

phone is more laggy, some times it will be hang or reset, when you press power button it will take long time to turn display on or unlock and even can't play video file or music on sd.
i try france 4.4.2 official rom with odin (wipe every thing even internal storage) or custom roms with full wipe, but problem not solved
my friend tell me that this problem has begin when he remove battery and then reinsertrd when phone is working

first do full backup for your photos , apps etc then go to setting - backup and reset and press factory data reset

i do full wipe (data, system, cache, sd card, ...) from recovery. install 2 new Rom even samsung official rom. but problem not solved.
i think it's not so easy to solve
No bro when you do that in recovery this will just delete system apps and something else but when you do it from setting you will wipe everything in your phonee
Try it and i think that will solve your problemo
and if you have external sd remove it and test then if every thing is good put it again and see
this problem was on my old phone s2 ,, so don't worry

Your phone is not rooted right ?
If not , then go and root your note after that install twrp (custom recivery) and do full wipe (system , data etc) then install dn4 if you want stock and stable rom !
