So today i rooted my desire, and tried installing the Sense 2.2 HD.
The installation failed and now i cant boot up, get stuck at the white screen with HTC logo. Connected SD card through USB in recovery mode, and formated it multiple times successful into all kinds of formats (fat16, 32, ext2, 3) in both windows and ubuntu.
Now when i try to run the update.zip or any rom file from recovery i get the following error
E:Cant mount /sdcard
(No such file or directory)
or some other problem
Any help would be nice

I've did a wipe data/factory reset, is it still rooted after that?
Cant get this crap to work. It boots straight into Fastboot usb now. But it wont find the sd card. Its the stock sd with 4 gig. My pc finds it and it works

Yes you should still be rooted, a wipe/factory reset won't unroot your handset.
All you need to do is the fastboot command to get your SD card recognised again. If this doesn't help, I'd try a new or different card.
Wow took awhile. Downloaded an RUU and went into fastboot and updated.

I rooted my HTC EVO 4g using Unrevoked, it worked fine, then I decided to flash a new rom, followed all the steps and I backed up my stock and wipe data/factory reset and Davik, now my phone doesn't read the sd card, is not even connecting throw usb, not even for charging.
Took the sd card out and did a FAT32 format (4 times already), downloaded the new zip to it and still not reading.
Can't do a restore because I re-formated the sd card and is not reading anyways.
I have another SD card and same thing happens.
As soon as I check install zip from card it reads

E:Can't mount /dev/block/mmcblk0 (or /dev/block/mmcblk0p1)
(No such file or directory)

You should probably use Amon RA recovery. Just go to ROM Manager and flash it from there.

I don't have access to my phone, I can only go to H BOOT or Recovery.
If I connect my USB it reads HBOOT USB PLUG, and because I deleted the stock ROM, I can't send any info to the phone.
It is possible that the debugging is not even on and I can't turn it back on without access to the phone.
I have noticed the the sd card in me EVO is kinda funny. If I push it in all the way my phone won't read it. If I push it in then close the little latch then kinda pull it straight out with my fingernail until the back is right up against the plastic in the slot it reads it. Try that...it might be your problem. If not I would RUU back to stock and take it back to Sprint and get a new one.
That's funny, after all this time, I had to buy another EVO, this time got rooted and flashed myn's warm two point two, played with different kernel's and my new phone is working great.
Still try to figure out how to fix the problem, I think I'm not the only one that had to purchase a new phone after screwing with it.
If anyone has any ideas, I'm all ears, I would like to fix this so I can help someone else.

Hey, well I made a mistake (supprise). I was following a guide on here and mixing it up with cyanogen's own guide for flashing their rom and well basically I whipped my phone without having the rom on my ssd, i took the ssd out and put it into another phone to put the rom on and now when i put the ssd back in i'm getting:
E:can't mount /sdcard
when I try and install the rom from it.
Suggestions? Please tell me i've not bricked my phone. I only just realised I should have done a rom backup but it wasn't said it the guides I was following and i was so fixated on doing it word for word It only just poped into my head.
Cheers. Fast replies would be nice, I don't like sitting here with my phone in clockwork without anything on it.

I have another ssd card current set up as a gold card. Could I use that? Both are htc supplied ssd cards.
Ok so i just pluged my gold card in and it could open it. Should I just use that or reformat my other ssd and use that instead?

in the process of backing up my ssd and formating it, going to see if that fixes my problem. Figure I should probably start a new rom on a fresh ssd anyway.
Be nice to know for future if wiping everything is actually needed for roms
You said your goldcard was recognized correctly by the phone? Did you use another card before?
Where does the error occur, when downgrading or did you already downgrade and are already rooted and want to flash the rom already?

I've done everything but the flash so far. I was on the final bit and I had "full wipe" zip on my ssd and it opened fine and then i reset to factory settings and cleared cache and then my ssd couldn't be read any more so I couldn't flash with my rom.
However my other ssd which I had to previously format to turn into a gold card can have the zip's read on it. So I figure if I reformat the ssd I wish to use (not gold card) and put the rom back on it will be fine.
Also can you charge the battery while in recovery? I think my battery must be getting quite low now as it's been stuck in it a long time and was only about 40% when I started. I presumed 40% was enough to install a rom, didn't expect this delay of a few hours.
I'm also presuming I can't boot into anything any more as I think i've wiped the previous rom no?
Ok now after formating it and putting the rom on it says "no files found" :/. Does it even matter if I use a differnt ssd to flash as to the one I wish to use long term? i was under the assumption it would install other things to the SSD but now I realise thats probably totaly wrong.

If you did a full wipe you have to get it going again by flashing a new one ^^ So yes, go ahead with flashing. The Goldcard was especially necessary for the DHD to load the downgrade-zip, because when I did that originally it gave me an error (had wrong goldcard). Of course, I now have a working one, still using it.
You could try twice, like 1. reformat the sd you wish to use and put the ROM onto it or 2. use the other one to load the rom. Think about it, worst would be "yo man, wrong card"
Edit: this is confusing, indeed. So you actually did a new format of the SD and tried to open it in CWM and it says NO FILES FOUND?

My suggestion, would be to format the first SDcard in your pc, copy ROM to it, place in the phone, reboot recovery.

Well aparently neither of my SSD's work. Is it possible to boot into clockwork without going into the software? Ie can i get into clockwork straight from boot? Because I might need to restart clockwork to get this to work now.

Exactly, but tryed and the moment I put this rom on it apperas it cant read the ssd.
Its cyanogen 7.02 and google addon.
Ok then, my formated ssd (which is apparently empty according to clockwork) formatted and was empty on windows, put the roms on it. When i plug it back in to windows it has all my old **** back on which was several gig so there is no way I accidently copyed that over with the roms and didn't notice.
You can boot directly to the bootloader by holding volume down then pressing power.
Did you download the ROM from ROM Manager or direct from here/cyanogen

Sure, Power off the DHD then hold Volume DOWN and press POWER. It then boots into Recovery.
I dont think you will have bricked it. As far as I know you cant brick a phone by flashing a rom. Bricking is as a result of radio installation gone wrong. If your SD card is being recognised, just wipe it and put in in empty. Then install the desired rom using ADB shell on your PC. About charging in recovery I think it will work. If not just turn the phone off and plug it into USB.
EDIT. This is sounding above my level of understanding now. Ignore my advice. I was assuming clockwork is behaving itself
Downloaded from CM7 them selves, ah ok I will reboot. Yup now neither of my ssd's have data on them apparently.
Rebooted recovery and now not working. I click to use an option and it just goes straight back to the top of the list, even reboot option.

Or by using CWM 3.0.2.5 to format the SD as I did 6 days ago - have a new DHD now

I would suggest power charge instead of USB, because the real charger loads your phone faster due to lower power values over USB. Afaik.
OK, worst case scenario here is having to flash a 1.32 RUU and rooting, ENG S-OFF-ing again. I don't think your at that stage yet. Did you Radio S-OFF before this?

The light on the dhd only comes on when the phone is turned off and charging for me atm.

I've unlocked my Evo 4G using Unrevoked3, then ran UnrevokedForever. (was that dumb?)
After that, I tried to update to Cyanogen 7.0 using ROM Manager two days ago, and learned AFTERWARD it wasn't a very good idea, that it's not wiping cache and the dalvik cache etc. fully and this has burned many people. I got stuck in a boot loop (waited 30 minutes) and nothing. So I booted to the Clockwork recovery, and get the following errors:
Android system recovery <2e>

apply sdcard:update.zip
wipe data/factory reset
wipe cache partition
------------------------------------

Doing research this usually means the Micro SD card or the slot itself is boned.
So I've removed the Micro SD, and carefully cleaned and reseated everything. Now I get:

So it SEEMS I might have fixed the SD card, and it's now reading, but the backup is gone?
Just letting it boot, (and re-trying several times) it now sits at the white EVO 4G screen and goes no father.
I don't have an SD card reader or micro-adapter at this time. So I've been trying to push an update or flash through fastboot.exe connected over USB.
I've been reading the stickied threads, getting fastboot, and ADB (says device not found, but fastboot finds the Evo just fine...) with the Android SDK up and installed on my Win7 x64 PC for the past two days. I don't even know where to start anymore, I'm not even sure if I'm trying to fix the right things. I just need a working phone at this point, and hopefully haven't lost all my Titanium Backup info on the SD Card either.
I hate my first post being a question like this, I've been searching and reading until I'm blue, I just don't know where to begin.
It's ok, you're not dead in the water. Clockwork Recovery, in MY opinion, is just not reliable. Nor is ROM Manager. I've seen too many people have problems. If I were you, I would get the pc36img for Amon RA, which I have attached here.
Make sure it is named PC36IMG.zip (all you have to do is delete everything in the name between PC36IMG and .zip
Exactly like that, and put it on the root of your SD Card, then boot into the bootloader. (Vol down/Power til boot loader appears)
When it does it's scan, it will find the file and ask you to update. Click to yes and let it flash it, then the phone reboots back to bootloader.
Now, it'll scan the file again and ask you to update, but say no this time, then go to recovery.
If you do boot up into recovery, and you should, go to wipe and wipe EVERYTHING, except the SD Card. You have a good start by doing the SD Card already. Do a factory reset/User data, and then each sub category, except the SD card. Then, hit return and flash and flash your ROM. Now, you probably know, it will take some extra time to boot up, but you should be OK.

I've been trying to get Amon_RA recovery loaded. I could tell this was the best (too late), but am still struggling to push it to the root of the SD card so the loader will pick it up.
Now I'm thinking I can't push a pc36img.zip file to the root of the SD card with fastboot.exe correct? (I know I need to rename it to exactly the right name, saw that messing up lots of folks in my endless searching.)
I'm trying to push it with ADB, but I haven't gotten that to connect yet. I might not have the right USB drivers that ADB likes, and I can't get it to recognize the device, or open up the shell. I installed HTC Sync which is supposed to get ADB to connect, but no dice. Did the USB Debugging mode get blown away when I manually cleared cache and user settings? I'd thought that the Loader screen would have it, but I'm not so sure.
If I can't get ADB working within the next hour, I'm going to swing through Office Depot and get a micro SD card/USB reader or the little shell that makes it regular SD-sized and stuff the file on that way if I have to.
You don'[t have a way to just copy it to your SD Card? A card reader or a different phone you can put the SD Card in for a minute and connect to your computer.
You don't need ADB, but if you can connect via ADB, just copy the file to your sd card.
Yeah, I just got home from work, and picked up a cheapie USB/Micro-SD adapter on the way.
I'm loading the .zip right now.
ETA: Well, that's fixed. SOOOOOO much better than Clockwork.
ETA #2: I wiped/erased everything except for the SD Card....
Now what did I screw up? I got Cyanogen 7.0.3.1 (newest listed stable release) and copied it onto the SD root as PC36IMG.ZIP It recognized it, read it, and then I went into Ammon_RA recovery, and chose the option to flash a zip from SD card, and it spent time loading, prompted me to reboot, then it rebooted and still sits at the white HTC EVO 4G screen. I let it sit for 15 minutes.
ETA #3: Re-loaded everything once more and it's now working great!
The cyangen is a ROM, you just go to flash zip from sd card or whatever it's called,. browse to the rom and flash it. You don't have to rename that. Just go back to recovery, wipe everything again and then flash the ROM that way
Well, last night, after doing a complete wipe, INCLUDING the /system partition, my sd card would not eject properly. (Using Lion OSX 10.7.1)
So, I manually ejected it; when I tried to reboot the phone, it wouldn't boot.
To make a long story short, basically, in CWM, when I tried to flash anything, or recover my backup, I keep getting the "E": SD Card not recognized, won't mount" error, no matter what I do.
I tried to format my card, I tried a different card, but no matter what, my phone won't recognize my sd card. Also, it used to give me the option of flashing from my "internal" sd card, but that option does NOT show up in the CWM menu anymore, Probably because I formatted my system partition..
I had formatted the system partition a few times before, when I had fc's after trying a different theme or rom, but, I was always able to get back into recovery, and flash something that fixed it.
Is there anything else I can do at this point? My phone isn't "seen" by my computer anymore, so I can't get anything on to it directly, and it won't recognize my sd card, no matter what I do..

I just had the same SD error in CWM, ODIN'd a fresh CWM 5023 and back to working again! I was worried it was going to be a serious issue (had similar problems with a Vibrant that rubbed me the wrong way) But it was an easy fix
fortunately, the good folks at Best Buy had no problem swapping my phone for me..

Not true. Heimdall does the samething as odin does. I use heimdall instead of odin most of he time. Its better.
