Our device does not have an sd-ext partition. Use the latest TWRP 2.6 and erase system, data, cache, and dalvik cache.
Sent from my One V using xda premium

Ohh, I have been using TWRP 2.3
Thanks! I'll try flashing TWRP 2.6

After trying to wipe it in TWRP it says unable to wipe sd/ext

Sd_ext is just a partition on you sdcard that does not exist unless you have created one.
It's just stating could not format sd_ext because it's not there
Do not fret my friend, it still factory resets like it's supposed to

Seems like you got the partition tables messed up.
I'm no pro so I don't know what could be done.
Hey All, Thanks.
Fixed it some how.
First downloaded FOTA bricked. followed the steps.
Then tried to flash latest GB with his latest/old flash tool (linux). but that didn't work
tried older Flashtool windows Version 0.2.9.1 with firmware 4.0.A.2.368 (This was my rescue)
Finally phone booted

Do you know if you have the updated radio (v1.00.00.0928)? If so, then you need to know that there are only a select few ROMs and kernels that will work with the updated radio. If you also have the updated hboot (v1.57, I believe), then you're kinda stuck (like me). If it's older than that, then you can try to do the RUU process to revert back to the old radio.
Good luck!

This problem generally comes due to mismatch of kernel(boot.img or bootloader) and ROM. Check weather you are using correct kernel image or not.
U said u are wiping everything that's fine. But are you flashing a boot.img before installing custom ROM.??

[Q] Cyanogen mod endless boot, keeps on the blue circle...

I was setting my overclock with setcpu and it shut down my galaxy gio, and when it rebooted it just kept on the blue circle of cyanogen mod.
I already flashed stock and it would keep on something like the blue circle of cyanogen mod. I flashed cyanogenmod a couple of times too. Already partitioned my sd card again and i already wiped everything a couple of times too...
Someone help, i have readed like hundreds of posts and nothing seems to work
When i flashed stock i used a flashable cwm recovery, because odin dont detect my gio.
Flash stock rom and than go into stock recovery and wipe everything.
Hit thanks if I help

Try to flash the stock rom with odin, and when the infinite boot starts, enter in the recovery and wipe everything!
This should solve your problem :victory:

Friend, my problem is that i can't format /data or mount or nothing that involves /data like wiping dalvik cache and it makes my phone to dont boot
Descobri noutra thread que tambem es portugues
I'm also having the same issue
(Also im a newbie this was my first attempt at any flashing etc.)
installed signed_cwm-5.0.2.7-flashablezip-ext4only.zip
then installed Jelly Bread v5.0
was ok for a while and working but then i wanted to play with other ROMS (in this case i now wanted to try cm-10.1.4-GT-S5660-gio++.zip)
I enter recovery, wipe data/factory reset, wipe cache, wipe dalvik cache, wipe batt stats
Then install cm-10.1.4-GT-S5660-gio++.zip
Then it hung for 35mins at loading blue circle screen.
I then took battery out and enter recovery, and attempt to wipe data/fatory reset and it says:
--Wiping data...
Formatting /data...
E:format_volume: make_extf4fs failed on /dev/block/stl13
Formatting /Cache...
Formatting /sd-ext...
E:format_volume: make_extf4fs failed on /dev/block/mmcblk0p2
Formatting /sdcard/ .android secure...
Data wipe complete
Also i cant wipe dalvik cache it just does nothing blinks 1 second then back to menu.
Anyone know what i can do to fix this please?
Thanks for any help in advance.

YOu have the right CWM (cwm-5.0.2.7-flashablezip-ext4only.zip)
Make wipe, and format system wipe dalvik cache.
And try again.

i have tried flashing stock roms+recovery then go to CWM rfs and wipe and still get same errors
did you use cwm-5.0.2.7-flashablezip-ext4only?
Do you think its possible my /dev/block/stl13 could be physically damaged?

Yes I only use that CWM nothing more.
Maybe is damage. Becqau format he system I can do with any CWM onçy wipe the dalvik cache isn't possible.
So if you can do nothing for sure you have a hardware problema.
That happen's when your try put a custom ROM or the phone broken and after you try to solve it.

Wipe t999

I flashed route 66 4.3 update via Odin last night, however I would like to do a clean install. How can I wipe the internal memory so I can re-flash via odin? I'm trying to remove all apps etc. Any help would be greatly appreciated
Sent from my SGH-T999 using xda app-developers app
There is a script that can be flashed through a custom recovery that will completely wipe your internal memory clean. I have it on my S3. I'm on my nexus 5 now so I don't have access to it right now. If someone doesn't have a better suggestion I'll share it with you when I can.
Typed from Nexus 5, ZFFZ
Go into recovery and format /Data. If you are on TWRP, there should be an Erase Internal in the Wipe settings. Not sure about Clockwork.
Boot to recovery.
Wipe /Cache
Wipe /Dalvik-Cache
Factory Reset.
Format /System
Format /Data
Profit!!!
In recovery Format Data =/= Wipe Data
Format is erasing your internal
Wipe is wiping your app data.
Edit: If you want to flash what Jorge said above, just search for XquiziT's Superwipe and you might be able to find it.
superwipe doesn't work, i get an error and reboots back into 4.3

SM-T210R can't mount /system

Okay so today I tried to flash Inspirekat v2.1 rom on my galaxy tab 3 via latest TWRP, and after I wipe system, data, dalvik cache and cache I reboot into the system and I can't get passed the samsung galaxy tab 3 splash screen after some time I found out that the problem is the unability to mount the /system file, then I tried CWM and still the same thing.
Unfortunately I didn't make a backup because I didn't have any precious data there so I just thought I would skip that, I've also tried formatting all data on internal storage but with no avail...could anyone light up a candle for me?

If you wiped "system" after you flashed, then you no longer have a rom installed. Re-flash the rom.

Sorry I messed it up a bit in my previous post, I didn't wipe system "after" I flash but before I flashed
Every time I try to flash it just takes a minute then I reboot into the system; same thing happens then I get back into recovery to see that the /system is unmounted and can't be mounted back also when I try "repair partition" it shows that only 54mb of /system partition is used and around 1300mb is free, any insight on that?
Some errors about not being able to read storage file context and can't find path to /sdcard also appear sometimes on the log window or however it's called at the top right.

TWRP does have some issues mounting system sometimes.
Once you boot into recovery, choose the reboot recovery option and try the flash sgain.
