Hey guys I need help, I've been trying to fix my phone for days now with no luck.
(HTC Desire)
Basically this is what happend. I had an MIUI Rom for ages and decided I want to update it. So I used my SD card and downloaded the new rom and flashed it all worked fine.
I then read I should make a ext partition and in the process of it I broke my sd card some how and my phone stopped booting. So I followed a tutorial to put the Rom back on my phone by using adb push to put the rom in the data partition of my phone. But the data partition on my phone was only 140mb and the miui rom was 180mb so I got a smaller 120mb rom and pushed it to the phone and flashed it using adb shell recovery --update_Package=DATA:Rom.zip or something along those lines.
So the rom was working fine but I really didn't like it so my friend offered for me to use his sd card to put my old rom back. So i went through the process but when I clicked install file from zip it just said "no files found" but I know the rom was there. I tried using the adb shell recovery command except with /sdcard/ rather than DATA and I just got signature verification failed. I disabled signature verification in recovery and it still gave me the same error on the phone but on the command prompt window i keep getting these errors:
insmod: can't insert '/res/modules/crc16.ko': invalid module format
insmod: can't insert '/res/modules/jbd2.ko': invalid module format
insmod: can't insert '/res/modules/ext4.ko': invalid module format
After messing about a bit more my phone crashed so I had to remove the battery and my friends SD card became unreadable and needed formatting. He won't let me use it now lolol.
So now im in a position where my phone won't boot at all and when I try to use adb shell to flash a new rom I get those errors in command prompt and signature verification failed. I've also discovered im getting these errors regardless of whether the rom is there or not.
Sorry for the long post but im desperate and any help would be very much appreciated!
Does your phone still boot in hboot?
You need to find a working sd-card. What did you use to partition it? How did you break it?

I used minitool partition wizard I think. I followed some steps in a guide and when I put the SD card into my phone after it said "unrecognised SD format" or something similar so I tried the process again and which point my phone doesn't pick up the SD card at all anymore. So I put it in a memory card reader on my pc and it still picks it up so i deleted all the partitions and reformated it and still only my PC will pick it up phone just says insert SD card.
When I turn on my phone it says HTC for a split second then goes straight to fastboot as if i was holding down vol down + power when I power it on.
Why doesn't the push rom to DATA partition and recover from there work anymore?

You broke your sd-card by using Minitool. Why do you guys use it anyways?!? If you get a new sd-card, check the guide in my sig.
I think you don't need an sd-card to flash a RUU by the exe method. Get the Official 2.3 RUU by these means:

Extract the "RUU_HTC Desire Android 2.3 Upgrade (Gingerbread).exe" from within the "HTC Desire Android 2.3 Upgrade.zip:" then connect your phone to your PC in fastboot, and run the exe.

Thank you so much! Got the phone to boot using your method but now it says insert simcard =/ is this because I used the wrong rom or something?

I don't know. The RUU is fine. Maybe there's dust in the sim slot.
Or maybe, if it's locked, there's somehow a sim lock problem. Try unlocking it with the sim unlocking utility.

I can still acess fastboot actually and my phone recognises the phone when im in fastboot, but as soon as the phone boots the pc stops recognising it and cant find drivers for it anymore
Sorry for being a pain.
Edit: Unplugged my phone from the pc and plugged it in again and I got "Qualcomm CDMA Technologies MSM X No driver found"

So today, I was upgrading from modaco 2.7 to 2.7.1,
after the upgrade (using recover mode and the signed zip file (core chinese)), I did a system wipe and reboot the device with the USB still plugged in my PC, and everything seems to be normal until my pc's tray notification pops up with the "unknown device found" message - i thought it was a little bit odd, so then I waited for the hero to boot up completely - it reboots after showing the HTC logo once, and then it boots normally to the setup wizard (select language, etc);
I then followed the normal procedure of typing all the wifi, flickr, gmail stuff in, and when i got into Rosie, it shows a notification icon on the hero saying "SD Card removed". I can't access the SD card, nor my pc can recognize the hero. I ejected the SDCard and re-insert it but it's not detected either. HTC Sync on the device does not give a notification nor the mass storage notification shows up either.
I tried rebooting into recovery mode, (Hero 1.2.3) and i can see the menus just fine; however, when i try to mount SD card as usb, or flash update.zip or any zip, or fix ext3, or clear dalvik-cache or wipe the sdcard's ext partition, it keeps giving me errors saying
error: run 'whateverprogram' in terminal
when i try to set 'USB Mass storage on', it says "Error enabling USB mass storage! Run ums_enable manually from console.
I then try to login the device using adb shell through window's command prompt, it says device not found as well.
giving up, i tried to reboot teh device into fastboot mode, the PC recognizes the USB device just fine. it's really odd.
Anyway I can do a recovery on this to make these device work again?
p.s. when i try to choose zip file in recovery mode, it says

Can't mount sdcard, aborting".
after every reboot (shutdown, start), the setup wizard shows up again as if I have not written anything on my device after any setup.
I can't download anything to run on the Hero as it requires the SD card. I can't flash any recovery images either as again it requires the SD card.
Any ideas??? I'm left with a half-brick now
Update1: using ConnectBot I can login my own device using root,
it shows the following when i run df -h:

Thanks for the quick reply.
I have tried doing so, in fact, i have 4 SD cards to try and none of them are readable in the Hero; they are readable using a card reader in my PC though.

Update2: the only way that my pc can detect my device as "My HTC" is when i boot it into fastboot mode; unfortuantely there are no way for me to update the device in this mode, so i can't do anything to modify the rom, or to check it using dropbear or any shell client. =\

damn man. its freaking. i am not going to update to 2.7.1 untill this issue is resolved.
still no go.
im left with a half-bricked device that:
1) clean wiped, normal boot, cannot detect SD card nor the Hero is detectable by XP.
2) clean wiped, recovery mode, can't run any of the script except :
Reboot System Now
Wipe data/factory reset
Fix apk uid mismatches
Fix rotate
3) can't download anythign from browser as it needs to save onto sd card.
4) android market doesn't have flashrec so i can't reinstall the recovery image.

When you are in the shell check out the file /proc/cmdline.
Would be interresting to see if the HBOOT send a command line to the kernel to disable the SD card.

can't. It doesn't even boot into normal mode, nor boot loop. it just holds at the HERO logo.
I think I have the same problem, i can charge my phone but trying to mount my phone to either my pc or mac it just won't be picked up
tried using RUU with a goldcard, no go. remote not allowed (fastboot flash system system_rel.img, nor fastboot boot boot.img or fastboot flash boot boot.img. no go. sigh.)
now i have a brick :\
wonder if there is any way to go into fastboot mode and flash ANY part of the blocks (device, recovery, or system, if at all).
Some progress: using hboot mode, i placed HEROIMG.zip into the sdcard and it loads successfully. it works! and im now rolled back to official 2.7.405.5; however, the USB and the sd card is still dead ;\
I guess I'll be bringing it back to HTC to see if they would do anything about it (fortunately It's now back to stock recovery and 2.74.405.5)

I am having the same issue... what I don't understand is: it has to be a software problem, because, from what I've read, everyone of us have been installing radio and firmware updates to the htc hero, and since some of these updates the USB and sdcard mounting is not properly working... I believe it's not possible to break a USB port just by installing software...
so there should be a way to fix this other than sending the phones back... If anyone has any suggestion I can try it on my phone

I know it's weird! Funny thing is, official updates includes radio + recovery + pretty much every part of the rom is being re-flashed (including hboot, from what i understand); how ever, ONLY in hboot mode, the device is recognized properly as "Android USB" by windows.
Some of your problems I had to. To be exact: that my pc wouldn't recognize the phone when connected with usb except for fastboot and charging.
For weeks I searched and found some people with similar problems, but never a solution.
I had no problems with my sdcard however.
Now my problem is gone. How? I had the HTC Generic 2.73.405.5 ROM (pre-rooted and packaged as update.zip by MoDaCo) and decided to update to Paul's ROM v2.5.
After I was finished with the installation wizard and connected the phone with usb to my pc everything suddenly worked again!!!
So I got the usb and HTC Sync notification at the top of the screen and can mount the sdcard using the notification bar.
This tells me that in my case it definately wasn't a hardware problem as I always thought, but a software one.
I hope you can something with my experiences because I know how frustrating it is when this doesn't work!

I tried this but it didn't seem to work for me, so I just put the official rom update and sent the phone back to my network saying that it was broken, now i've gotta wait for another 2-3 weeks to get my phone back , but i'm hoping they'll give me a new phone as i did kinda drop it and there two scuff marks on it

today I tried going from 2.8 to the HTC generic ROM you told, and then to 2.5. no results.
I also installed the official HTC / Orange Rom update, which I have now. It has improved a bit because now when I plug the USB cable the PC detects something, but it does'nt work... it says 'unknown device' (tried this on two computers)
when I boot to fastboot ( BACK + POWER BTN ) my computer detects Hero ok (My Htc)
I really don't know what else to try...
I saw another post from someone who had the exact same problem and got it fixed by installing SPL and ROM from fastboot menu

I tried it too, but when trying to use the SPL zip file he says I get an error ( I guess it's because he has a T-Mobile and I have an Orange Hero)
[update]: I finally got the SPL to flash correctly and then installed a modaco 2.8 ROM, but I still cannot connect the Htc Hero to the PC via USB.

hi i try to install one of the custom ROM last night but when i update the phone with ROM manager it will not start i can get in to the boot and clockwork recovery but that's it i try recovering it but nothing i tried using one of the ruu_bravo.exe but i get so far then it say error 107 what do i need to do to get it back to normal ?????
Which ROM did you try? What version of cwm do you have? Did you flash in Recovery? Did you do nandroid backup first? Did you use correct RUU for your device/carrier?
More information please (and full-stops, commas and capital letters, so I don't go blue reading one long sentence...)

Is USB debugging enabled? I'm not sure it matters if using ruu, but it does for all other operations involving connecting via USB!

hi i did check that and it was so i did try with out it enabled and it did not even start with it enable it start then give me a 107 error so ???? it because i flash the new rom or is it my windows 7 drivers or even settings ??
Don't use Rom-manager for flash,update,backup.it gives u more trouble then it solves.experienced user recommend not to use it.
i am also in same situation like you few days ago usb bricked and stuck on stock rom..and i solved my problem by doing this steps.
after i unlocked my sd card via fastboot command"fastboot oem enableqxdm 0" sd card recognized by phone and i can browse it via file explorer.
then i copied the visionary+(it gave me temporary root access which is needed to work this whole thing out).
root explorer,terminal emulator apk and those usb bricked update file(image_flash,mtd0.img) to sdcard from pc via card reader and installed them.
before copy those update file on sd card i edited the mtd0.img to put correct cid n rom info via hex editor.
after that i copied those update file in phone memory
/data/data/ and set permission for r/w and execute with the help of root explorer as i got root permission
after that i simply typed this command in terminal emulator
"su
/data/data/flash_image misc /data/data/mtd0.img"
Rebooted my phone and voila usb brick is gone htc sync,usb tethering,sd card mount,charge everything is working
i fixed it with a little help
OK my wife has the same phone as me so all i did was use clockwork recovery to make a back up img of her phone and then i flashed it and it had all her data and the phone went in to safe mode so i factory reset and its now all fine and working with the original rom
So Ive been having problems with my SD card as posted here http://forum.xda-developers.com/showpost.php?p=16689682&postcount=286, and my question is, can someone explain to me how to flash back to stock without the use of my SD card ? Or am I screwed ? Thanks.
What you can do is download the RUU exe for the hboot you are on, if 2.10, its the 3.70, if 3.16, its 4.24 [the new ota], then connect the phone to the pc via usb, boot into the bootloader, select fastboot, then run the exe on the pc.
Thanks for the reply, and sorry for the noob question but how do you determine what hboot you are on ? Cause if I boot to the white Revolutionary screen (volume down + power) the third line down says "HBOOT-6.16.1002", is this right ?

Also dont know if this will help but when I am in recovery, if I try to mount my SD card it says "E:Can't mount /dev/block/mmcblk0p1 (or /dev/block/mmcblk0) (I/O error)"
Well I don't know if it was changed, but since you were on 2.3.3, you had to be on the 2.16, so for ruu purposes, look for that.

I didn't know which one to use so I decided that since I have only had my evo for less than a month then I would use the latest version and it seems to have worked. So again, thanks a lot.
Okay, so I guess that I have a hardware problem and I will have to have it replaced. While my phone was just sitting there a notification came up that said "SD card unexpectedly removed"
Do I have to change the phone back to S-On before I get it replaced or can I keep it S-Off ?
Glad that you got it working, and you need to be s-on.
If you're s-off, re-root, then download the unrevoked s-on tool, flash the zip in recovery, make sure the bootloader reads s-on, the unroot again.
okay, so when I try to use the unrevoked s-on tool it fails and says unsupported radio type. any fix for this ?

try the 3.70 radio
You need to go to the development thread, then download the 2.1...11.19 radio and flash that.
Thanks for all the help guys. I've ran into another problem now. Before this new problem, if I wanted to put a new file on my SD card I had to flash to stock which gave me a 30-45 second window to mount the card to my pc and copy any files I needed to and power off before it unmounted it. Then I had to reboot, run revolutionary and get a recovery before it unmounted. Then I could flash a zip from my SD. BUT NOW, even if I reflash stock, it auto unmounts the SD at bootup.
Frustrated but still trying to problem solve.... Will update if anything changes.
Okay. I think I got it fixed now. After spending all day trying and flashing my phone at least 12-15 times it seems to be doing okay.

I have done days worth of research in my spare time, before finally caving in and asking for help. I'm still kinda new to the custom rom scene, and somewhere I must've messed up.
I was updating my ViperX rom to a newer version, but got an error code during the install, which rendered the OS useless. So decided to wipe and reinstall the older version again, which failed. So I tried to install CM 11, which also failed... Somewhere a long the line my SD Card stopped being detected by the phone AND my PC. So now I don't know how to get an OS on it.
Here's some info I might need to share.:

(Bought in Sweden, before moving to the UK)
I have access to bootloader
I have access to recovery which is CWM 6.0.4.8.
My SD card WONT MOUNT (I think), so I don't think I could do "side loading", since it's not recognized in ADB . If I reboot the system, it bootloops at the CM update logo forever. I've tried reinstalling it several times, but it only installs for a few seconds, then restarts. (I'm guessing it's corrupt?)
(Edit: Earlier today it ran out of battery, and despite charging it for hours, it's not turning on and the little LED on the top is blinking RED... What's that all about?)
Is there ANYTHING I've missed, or can do to fix this? I don't care what kind of ROM I have to use at this point, I just want a WORKING phone.

Flash Philz 5.15.9 recovery and you have working sd card mount there copy what you want to your sd card and if you want to install kitkat rom flash lates CWM or TWRP or Philz and inatall it

You must've missed the part where I said neither my Phone nor my PC can detect the SD card, so can't put anything on it. :/

NO I not missed nothing you don't listen me. flash Philz 5.15.9 recovery after this go in recovery connect the phone to computer and choice mount sd card after 1-2 min you will see your sd card and if you think not do this I will stop help you. It is your choice to flash revovery you dont need sd card only fastboot commands, use exactly the same recovery where I write you not newer not older have shears from me on googledrive use search function and download it take the recovery.img from zip archive then place in your fastboot folder the connect the phone in fastboot and run the command fastboot flash recovery recovery.img. Simple thing to do

Objective: Attempting to return to out-of-the-box stock android after using a custom ROM, LineageOS. I mainly want to return it to a condition where I would be able to hand it back to the carrier. It's also much more convenient for me to use the phone in stock. From my understanding, this is possible without much difficulty.
Issue: After many days of searching forums and following guides specific to the phone's model, I've put myself in a confusing position. The phone's current state is as follows: Installed OS[None], Bootloader[Relocked], S-ON[True], Boot to Recovery[Unsuccessful], SIM Card[None]. I'm unable to detect the phone on my computer(running Windows 10), and therefore can't utilize adb commands.
If anyone has any advice or guidance I would really appreciate it; and if you need me to provide any more information I would be happy to. I'll also try to respond as quickly as I can. Thanks!

What is your CID/MID and software version? Just use an ruu for your device that's at your current firmware version or higher. Put it on the root of your sd card and run from download mode.

I only have access to the CID(TMOBO10) and software version(2.51.531.32).
I've attempted to use an RUU, although after many attempts I couldn't get past the application setup screen. After the progress bar finished installing the RUU, it would close and never prompt me for anything else. I tried it multiple times.
I don't have a SIM/SD to use, unfortunately.
TWRP was installed as my Recovery, although trying to access it returns a boot-loop.
In terms of using adb, my phone will charge if plugged into the computer but it's not being recognized for file transfers.
I'm gathering that the only option left is to use something like a SIM card as you mentioned?

I only have access to the CID(TMOBO10) and software version(2.51.531.32).
I've attempted to use an RUU, although after many attempts I couldn't get past the application setup screen. After the progress bar finished installing the RUU, it would close and never prompt me for anything else. I tried it multiple times.
I don't have a SIM/SD to use, unfortunately.
TWRP was installed as my Recovery, although trying to access it returns a boot-loop.
In terms of using adb, my phone will charge if plugged into the computer but it's not being recognized for file transfers.
I'm gathering that the only option left is to use something like a SIM card as you mentioned?

You could troubleshoot your RUU issue, it should work if you have a windows computer. Try googling it. Unfortunately I can't help you much there, it's always worked fine on my machine.
Else, yea, the SD card method also works well. Find an ruu zip version first before trying to find an SD card. The ruu.exe you tried using won't work in this method.

PC method
As @Tarima has mentioned the RUU is not running because of a Windows/PC issue.
Your best option is to try and run it in another computer or a friend's computer.
You can put your phone to download mode (volume down+power)
And run RUU again. Here is a link just to make sure you have the correct RUU. It is for your CID and its 2.51.531.32

For microSD card method
Use a friend's microSD card. It will need to be formatted to FAT32. So backup the microSD card data. Hehe!
To create an installable zip for that RUU, you again need a good computer.
Here is how you can get the installable zip from the RUU since it is encrypted you just cant extract it.
Download this decryption tool from here https://forum.xda-developers.com/chef-central/android/tool-universal-htc-ruu-rom-decryption-t3382928
Choose 32bit or 64bit as this is vital to its working. Also make sure dotnet framework 3.5 is installed on your machine or else this tool will not run.
Once decryption tool is ready to run, just drag and drop the RUU on top of HTC_Decrypt_Tool.exe
It will give you some options. From these say yes (Y) to get new keyfiles AND to create installable sdcard zip file. you will know it when you see.
All other options such as uploading new keyfiles to community and getting boot.img and system.img is optional.
Once you get the zip rename it to 2PS6IMG.zip if it already isnt renamed.
Transfer to the root of your microSD card (dont put in any folders, just put it direcly on microsdcard)
Put microSD card into the your htc 10.
Enter download mode (volume down+power)
Download mode should detect the zip file. Press volume up to install.
