[Q] Missing Device ID on VZW website

When I login to my vzw account, my galaxy nexus is marked as an unknown device in my account. does that mean my imei or esn went bad? is there a way to restore it? I am on aokp jb build 1

[Q] How do I remove my username and phone number from the GS3?

Tried a factory reset, data wipe, and flashing a stock rom and it still shows my username and phone number under about device and status. How can I get this removed(trying to sell phone).
