[Problem]Having trouble with phone (calling) application - Samsung Galaxy Nexus

Note: this has happened with versions 4.1.x and 4.2.x both, always on a rooted stock image (I do not use any custom roms on a regular basis).
The Problem:
After a call that I initiated (regardless of the call duration, happened after 1minute calls, aswell as after 20+ minutes calls) then phone stops responding after I move it away from my ear (instead of turning the display on and letting me terminate the call). After that the only solution was to pull the battery or to wait for a "application not responding" message. On the ocassions where I was waiting for the ANR screen after force stopping the application everything went back to normal.
The occurrence of this seems to be totally random, and I was not able to reproduce the problem on purpose. I've tried to do a manual force stop + clear data/cache for com.android.phone but that didn't resolve the issue.
At times it seems like its fixed, not happening for days or weeks, and at times it strikes after 3 calls in a row.
The particular error was happening even after full factory resets and stock image flashing.
Any ideas?

Phones freezes for 2 second then reboots, once a day, immediately after waking up

I've had this problem sometime over a month now, can't remember if I did anything to make it like this. I made a post somewhere about it happening the 17th of May, so it started somewhere before that.
My phone will freeze for 2 seconds and then reboot itself.
It started while I was on stock 4.0.2 and now I'm on stock 4.0.4 IMM761. I have a GSM Galaxy Nexus.
It only happens just after the phone has been woken up and something is started. The most common (as it's my go-to-way of forcing it, to see if it's still there) is after waking up the phone, sliding down the notification bar and pressing "settings", which doesn't open but the phone freezes and reboots itself. I first discovered this when I wanted to go in there to change screen brightness. It's also happened when I went into the notification bar and pressed a battery app there. It doesn't matter if I'm doing it in the lock screen or after the lock screen.
It's also happened when I wake up the phone and enter the camera through the lock screen.
It also regularly happen if I set an alarm in the morning. When my alarm is suppose to play it freezes and reboots, and when it's rebooted, the alarm has been cancelled without ever playing.
It doesn't need to wake from deep sleep to do this, I've had it happen mere minutes after trying to see if it's there.
I don't know if it has something to do with this, but the 21st of April I tried to do the "clear google services framework" trick to force 4.0.4 ( I was on yakjuxw), but it did not work and I remember my phone acting janky and I was scared it messed something up, but I seem to remember it getting better. I can't remember what exactly it did though.
I've read that 4.0.4 would fix the reboots that a lot of people were having (don't know if it was the exact same kind of reboots as mine) so I did this to make my phone a yakju (translated from Danish):

I did that the 8th of June, and it worked flawlessly and I was on 4.0.4 IMM761 in no time. Unfortunately it did nothing to stop these reboots.
As part of the process of making my phone a yakju I, of course, did a complete factory reset, although I did make a copy of my entire SD and transferred everything over again after the process.
These past two weeks I've tried the following to fix it:
Clear cache partition in recovery.
Clear google services framework.
Clear media storage.
Tried to uninstall many apps, 2-3 a day, to see if they were causing it, re-installing them when I still had the same problem. If it's an app causing this I've yet to find it, but I find it weird that an app could be causing this.
It happens once a day, but never close to each other. It seems that some time has to pass before it can happen, and it only ever happens just after the phone is woken up, either by my alarm or by me opening the notification bar and pressing something there.
Please help me, this is unbearable.
Another thing, yesterday I tried to let it run without me pulling down the notification bar and pressing "settings", which would set off the freeze-reboot. I found that my phone would not freeze-reboot but very often it would "reload" app icons and widgets when I went around my homescreens. Where I can only see my wallpaper for half a second and then the icons/widgets appear. It would do this very often throughout my 1day 5hour uptime, and a couple of times I would try to force the reboot (by pressing "settings" in the notification bar), after the icons/widgets did the invisible/load thing and it didn't reboot. This must have something to do with it?

experienced this on my nexus s back then what i did is from recovery>mount and storages>formatted system, boot, and everything else except sd card.then it was all gone.btw you will have to flash your rom and kernel again as this is a full wipe.hope i helped and do this on your own risk

I haven't tried that yet. I'm trying a "RAM booster" app to see if that helps, but that doesn't sound too stable even if it does.
I'm going to do a full factory reset and installing nothing, or moving anything to the SD, and just let it run like that to see if it works.
Just writing an update for anyone googling themselves here with the same problem, I had it resolved.
I found out that if I didn't force the reboot (by turning on the phone and quickly pulling down the tray and pressing something there) it could last a long time with just the launcher resetting, but it would lead to complete freezes (anywhere I was on the phone) and would require a battery-pull.
My plan was to wait for jelly-bean before doing another factory reset, I got the JB update and it completely removed the problem. I still don't know why the problem suddenly started or what caused it, all I know is that the JB update fixed it, and the update is fantastic, my phone's never been better.

Some issues of the stock rom, is anyone else having them too?

hi guys, i got a d855 and i have some issues which i think they may be stock rom's bugs and i d like to hear your opinion. I have the latest ota V10h-EUR-XX btw.
1) Every time i restart it appears in the notification panel the "voice mate" shortcut. I remove it using the edit at far right but still it reappears every time i reboot the phone.
2) Sometimes after an unanswered call or message the screen turns on as usual but it doesnt turn off. Ofc i ve selected to shut off at 30 seconds from the settings. Even if i change the settings to like 15 seconds the screen still wont turn off unless i manually lock it. This problem fixes if i restart the phone.
3) Every time i boot my phone i get this message: "unfortunately the process com.google.process.gapps has stopped"
So is there someone else who experiences these kind of issues?

You're having permissions issues.
I'd bet my left testicle that a fresh flash back to stock will fix all 3 of your issues.

I've encountered the screen on issue - although not only did the screen stay on after a missed call, the phone also became and remained completely non-responding, no screen input registered. Had to reboot manually. It happened only once in 3 weeks though.

[Q] Stability and text message issues with multiple ROMs

I've been having various issues with my T999L for the last several weeks:
1) the phone will just crash without warning. This seems related to text messaging somehow, or to Handcent, as it most often happens while I am in the middle of typing a text message. However, it does happen on occasion at other times.
2) My phone will sometimes just lose the ability to send text messages. I've tried uninstalling Handcent and using the stock app but it doesn't help. Rebooting does nothing as well. I'll try to send a text and it will just spin forever. It can last anywhere from 5 to 20 minutes, after which I can send messages by deleting all pending messages and then resending them. I don't lose LTE data or the ability to make calls. only texting is affected.
Both these issues have happened on CM11 and PAC-man, so I'm not sure if my phone is somehow damaged or it's a kernel problem or what. I'm kind of hoping it's a known issue and there's a fix, but I haven't been able to find anything.

Ive had this happen a few times as well. When it starts spinning I turn on airplane mode and then once it comes on I turn it back off and usually the message will go through.
Try using a stock based rom and see if issue persists

Lost sound during calls, process isn't responding...

Recently my LG G4 H818 (dual sim) started causing troubles after 8 monthes of use. Everything started a week ago, when during the call I couldn't hear the other person. The other person could hear me well. It didn't matter if I was calling or I was receiving a call. It didn't depend of which sim I was using, call sounds from both sims were silent. After reboot this issue was resolved. Temporarily... After couple days everything started again and again reboot helped.
Yesterday, after regular reboot due to this issue, my phone showed an error - "Process isn't responding. Do you want to close it?". This is where nightmare started. Everything froze or started lagging badly, I couldn't unlock the phone, I couldn't launch any app, even notification drawer was not coming down, the phone was useless. I could receive calls, but I couldn't pickup (phone freezes instantly). When I tried to launch an app, it would say "This app isn't responding. Do you want to close it?" before freezing for couple minutes. It was happening to every single app (Phone, Contacts, Shazam, Whatsapp, etc.). I barely managed to save my data to pc using usb cable and decided to hard reset (erase all, format data, etc.).
After hard reset, during the first launch everything seemed ok, no lags, no errors... BUT this morning I received a call, picked it up and sound was missing again. I did the reboot and now I'm back again to freezing, huge lagging, errors, and phone is useless.
Everything is stock, latest Androd 6.0 is installed. Not rooted, bootloader is locked - I have not done any changes to the firmware at all. I did drop it once on the hard floor (only once) after a month of purchase, but until last week everything was working ok. I must mention that battery wasn't lasting long enough, but it might be due to my heavy use (9GAG, Chrome, Corporate email app, gaming sometimes) - charging 3-4 times in 2 days.
Has anybody faced same issue? Can anybody help me? Couldn't find any results on google and on forum here. Please help.
P.S. rooting and installing a different ROM is not an option, since dual sim LG's have limited available custom ROMs. Also, I use a corporate email app that cannot be installed on rooted phones (it is really annoying).

Some update. It turns out speakers and microphone works if on speakerphone. If I switch back to headset, sound dissappears. Seems like more of a software problem, than hardware.

Have you been able to fix it? Since I updated my H815 firmware's, I have been having same issue.

Phone Call Sound Problems

Hello. Does anybody have an idea why there is no sound whenever I send/receive calls? Also, the UI/related app crashes whenever I do the same.
No problems when I uses SMS though, so I don't think it's a signal problem.

Maybe a theme issue?
Nope. Just reformatted the phone and did not install any theme/launcher apps

I've been having the same issue and I have no idea why. Performed a factory reset. Tried running the phone in safe mode. It seems like it works briefly after a restart but then cuts out after a few minutes. I never had any issues like this before the update.
Problem for me goes as follows.
Receive phone call
Try to answer
Either the phone answers the call and there is no sound (mic or speaker) but otherwise acts normal or the phone refuses to answer the call and the "swipe up to answer" UI keeps reappearing. When the later happens the phone crashes and goes through a restart that takes longer than a normal one.
Make a phone call
No sound (mic or speaker)
The issues with the microphone seems to affect other parts of the system as well, such as ok Google and voice typing.
Speaker seems to work fine other wise
Putting the phone in safe mode seems to delay the problem sometimes.

Me too. Bumping in hopes of a fix.
Same. I'm running a custom ROM and haven't upgraded to 8.0 yet. Does the issue persist with 8.0? Google told me today to revert to stock and upgrade to see if that fixes it.

Same thing started happening for me yesterday!
I was sending a voice message on Whatsapp.
The msg was sent but no audio was recorder.
I received a call but there was no audio (both in & out)
I tried making a call and the same happened.
The "Phone" app would even hang after hangup and would crash. I get a white screen with the G logo and a loading bar beneath it, then it would start.
Starting the phone app is also sluggish.
If I make a call (and get no audio) I try to switch to loudspeaker and I could hear the other party but for a couple seconds then it's muted, then comes back for a couple seconds, and keeps doing it intermittently.
Sound from music app and videos works fine.
SOMETIMES after a reboot phone audio&mic would return but temporarely.
SOMETIMES when I try rebooting and it doesn't work, I try safe mode and it work (not all the time though)
SOMETIMES switching from LTE to WCDMA only fixes it temporarly.
This is really unpredictable.
I reverted back to a 7.1.2 stock. At first the issue persisted then it was fixed for a couple hours.
I updated back to O and audio worked for some time but after a few minutes, the issue returned.
Now I am on a clean stock install of O. without any apps installed. The audio comes back for a few minutes, then it's gone for hours.
Hopefully this thread will grab the attention of someone who might be able to help
Having the same issue on a regular Google Pixel. Also Google Assistant is not working using the "Ok Google" voice command and the phone app - answering and ending calls lag - same issue mic and speaker do not work. In Safe work, they work but on rebooting same issue persists.

I've starting having this problem too... its very frustrating. Im on Stock O (OPR3.170623.008), Verizon,with unlocked bootloader.

I have been having these same issues with my phone. I noticed it happened after I downloaded the what's call app to make a WiFi call. I noticed another user had the problem after installing what's app to her phone. Could this somehow be the problem? I am not super tech savvy but I am convinced it is related to my downloading the what's call app. I have uninstalled the app. Still the problem remains. Any ideas? Thanks.
Has anyone that posted in this thread was able to resolve this issue? I also have this problem, I'm currently waiting for it to happen again so I can pull logcat but no idea what I should be looking for since I have no idea what can cause not being able to hear the caller so randomly.
I was originally suspecting V4A, but last night I flashed stock system img, did not install V4A and same issue occurred.
