Same here. It worked for me on 4.2 but 4.2.1 doesn't allow me to turn on BT
I had broken Bluetooth when I upgraded from 4.1 to 4.2. To fix it I had do a clean install of the stock ROM via fast boot (NOTE: it wipes everything).
Instructions to do that are floating around in here but if you can't find them let me know.

I just did a clean install of 4.2 via fastboot using the image from Googles site, then upgraded OTA to 4.2.1
If you are in 4.2.1 and your Bluetooth does not work here is what worked for me: IT WORKS ONLY IF YOU HAVE UNLOCKED BOOTLOADER AND FASTBOOT SET ON YOUR PC. Download the 4.2.1 factory image from Google yakju or takju depends what you have. Then with 7zip extract ALL the compressed files until you have the file named system.img . Put the system.img file in your fastboot folder and boot your Gnex in bootloader. Then open a terminal in fastboot folder and flash the system image with the command : fastboot flash system system.img. After finishing, reboot and the Bluetooth should be fine. At least that worked for me. WARNING! YOU WILL LOSE ROOT AND OTA ROOTKEEPER BACKUP IF YOU DO THIS SO YOU HAVE TO REROOT. You will not lose data and all your settings doing this. For further explanations and guides about flashing refer to efrant's guides and threads.

Awesome, this fixed it for me! Although your instructions were a bit to vague for a novice user like me, I managed to get the fix working on Windows with the Samsung Galaxy Nexus Toolkit V10.4.0 (www skipsoft net):
Download the Factory Image (https //developers google com/android/nexus/images)
Extract the tgz-file (with WinRAR or 7-zip.org)
In the extracted files, find a zip file (in my case image-yakju-jop40d.zip) and extract it.
Copy the system.img to the "put_img_files_to_flash_here" directory in the Samsung Galaxy Nexus Toolkit directory (in my case C:\Galaxy Nexus ToolKit\put_img_files_to_flash_here)
Now start the toolkit and select your device model (in my case "7. Android 4.2.1 (Build JOP40D)"
Choose option "11. Boot or Flash .img Files to Device [Fastboot Mode]"
Make sure "system.img" is displayed as available image file
Type "system.img" (or s* for short) to select it
Now follow the instructions. I can't redo them to document... They are clear. But give it some time, it will reboot once or twice and return to the Fastboot menu.
After flashing system.img, the device is unrooted, so you have to root again. You can do it your way, or use the toolkit option:
"4. Root or UnRoot your Device [Fastboot Mode]"
Then choose your favorite option. I chose option 1, because I already had it rooted before.
After these steps, you have your root back and a working bluetooth!

I also had bluetooth issues after reluctantly accepting the 4.2.1 ota. I lived without bluetooth for a while, but then finally had enough of the seeming joke google played. After considering aokp, for now the solution was reflashing the system.img from the google developers site. Works like a charm now, and i do not know why i had the issue in the first place. Again, a confirmation that a reflash of the system image via fastboot will fix your bluetooth.
Guys, this only applies to factory rom, right? Só I guess I can't do this using a custom rom...
I'm tempted to install Google factory image to see if it fixes the annoying issue...

I'd think a factory image on a custom rom would only cause problems. Perhaps you could unzip the custom rom zip, and find the system image to reflash. Either that, or its likely a known bug on that rom. What rom are you using, and is it a nightly?

i have similar problem, but BT was working more or less fine. Only hang up few times. But main problem is that i could not use BT in my car any more. Phone connecting to car with no problem, and i could receive calls from anybody, but as only i try to call from phone, not from car HF system, i get problems. While phone dialing, it happens in background mode, car audio system working, but as only recipient answer the call, Gnex turning his internal speaker, but not the car's HF system, like it was on 4.0.4 - 4.1.2. I flashed custom rom - the same problem, then i download and flash stock rom again, and nothing happens. Does it helps if i now reflesh system.img

I asking because i gave Gnex to the warranty service (with all above mentioned explanation) and they gave me back Gnex with 4.0.4 and claim them for that. now i don't want to flash 4.2.1 until they response.

I have the exact same issue with my car kit since 4.2.1 and am about to reflash my system.img... I'll let you know what the results are.
I actually re-imaged the system.img and it seems a bit better now... It still loses it's connection after a while and it could be after 2 minutes or after 60 minutes. Once the connection is flaky or broken, I cannot connect to my carkit anymore and the weird things is, no other phone can connect until i completely reset my carkit. It seems like it's messing up with either communication or whatever as it affects the way my carkit behaves afterwards.

Try the following. It worked to fix my crappy 4.2 Bluetooth sound quality. I did the manual install. Don't forget to change permissions.

2 days had passed since I bought SGN and already I had loved it. I had rooted it using Nexus Root Toolkit and pushed the 4.1.1 JB update. Last night suddenly and without a particular reason while I was out I was unable to make calls. The mic and speaker wouldn't work. Also the screen opened with no reason and closed after a few seconds. I came home and wiped data and cache via CWM. The problem was not solved. As a friend suggested I also formated everything, system etc. But due to the fact that I was used to my HTC Desire having an SD card I forgot to push the ROM. Since SGN does not have an SD card I can't do it anymore.
I tried pushing the ROM via Nexus Root Toolkit but I was unable due the fact that I must have USB Debugging on. Also every other feature of the Nexus Root Toolkit must have USB Debugging on. Well, I have no operating system to do that so... The only thing I want to do for now is to push a ROM in my phone to install Android again. Any ideas or directions on how to do that?

Can you get it into fastboot? Maybe try pushing the factory JB image through fastboot flash boot.img?

You could probably push the ROM though fastboot using adb commands, but besides that do an Odin (the easy way out) restore but it would completely wipe your phone.
Firstly, thank you all for the quick response. I did use Galaxy Nexus Toolkit and managed to return to ICS stock. More of that, mic and speaker now work (!). I wonder if that problem knocks my door again though... One more (smaller) problem this time. For a peculiar reason my SGN does not mount when I connect it to the PC. I tried the laptop, no luck there too. The phone charges and I have enabled USB debugging. Any suggestions/directions to that?

Until few weeks back my bluetooth was working fine. Since OTA update from google, my bluetooth does not work
The issue is that I cannot even slide the Bluetooth radio button to the right to turn it on. It keeps popping back to off.

I had the same issue. I'm not sure if it happened with 4.2 or 4.2.1, but i know bluetooth worked fine on 4.1.2. My solution was this: First backup anything you might hate to lose, then download the stock factory image from google developers, unzip twice i believe until you can find the system.img. Move it to where fastboot is in your path, and reflash the system image : in my case, "./fastboot flash system system.img". YOU WILL LOSE ROOT, EVEN IF YOU HAVE OTA ROOTKEEPER! All in all, with an unlocked bootloader, it was easy enough to re-gain root and re-apply system mods. Worth it to be able to play with / learn some bluetooth capabilities. I just couldnt believe my ota broke bluetooth, and again this should work for you if you can compromise and weigh your options.
I have the same problem with my Nexus 7 and GNex. The 4.2 update sucks. Breaks more that it adds to JB.

I can confirm the same problem with stock after accepting the latest OTA update, only a reboot fixes it. Going back to Xylon.

I've had nothing but problems since updating to OTA 4.2.1 Android....Actually it started with OTA 4.2 update. I see that Android changed the Bluetooth stack starting with 4.2....

So last night I plugged in the phone to charge in my car and I started to smell electrical and heard noise from phone. I pulled the charger off and found it was starting to get real hot...well now it wont charge and I cant get the computer to see phone when connected... I'm trying to get my info off and relock it for warrenty....if I cant get it to be read by my PC, is there anyway to get it to be at locked state???
If not is there anyone that has a current Gnex factory rom backup with CWM they can share????

You can use an app called Mobile Odin Pro on the play store. I think it's $5.
If you're on a 4.2.2 ROM then download the 4.2.2 factory images from Google's servers.
Extract everything so you have the System.IMG / Boot.IMG / Userdata.IMG / Recovery.IMG files
Before using Mobile Odin MAKE SURE you have the 4.2.2 radios installed. You can find them in the sticky in Verizon Galaxy Nexus Development section.
Once you're sure you have the newest radios installed use an app from the play store, BootUnlocker, to relock the bootloader. You can reboot to make sure the padlock is gone from beneath the Google logo to be sure.
Then when you have the radios installed and the boot loader locked run mobile Odin and select the IMG files you extracted. It will reboot and you'll be totally stock.
Note: If you happen to be on a 4.1.2 ROM then use the 4.1.2 factory image and radios. Same for any android version. You have to use the same factory images of whatever android version you're currently on.
I've done it before successfully and I actually just helped someone with this the other day.

I am new to this forum but not to DIY (and messing things up).
I have Cyanogenmod 12.1 on my Nexus 7 2013 WiFi.
I've installed my N7 in my car dash and it is working fine except for GPS (Use N7 for navigation was the main reason behind my install). I've tried this kernel but with no luck to fix GPS issue.
I have gone through so many posts here and found out Cyanogenmod is not worrying about fixing the GPS issue. And for this reason I want to go back to stock android so that I can use GPS in my N7.
I have little problem here. Becasue the way N7 fits in my car, I had to change the USB port end from bottom to side (by opening back cover and bending the joints of charging port, I know its stupid mistake). And in doing so I screwed up royally and left with broken USB port. Now charging using wireless charger.
So long story short, how can I can get stock android without using computer (or before that if anybody knows the fix for GPS issue, I want to give it a try)

Assuming you have some kind of network connectivity, Just download the image to your device. If you prefer to download from a PC, transfer it by using a cloud storage solution like Dropbox. The free version will have enough storage for a custom ROM.

Thanks @ezas, I have few more questions.
So can I download the 5.1.1 image from developers.google.com page which is a .tgz file and push to tablet and use TWRP to flash? Or do I need to extract this .tgz file and push only some of the files and flash? I am sorry but this is the part where I get confused.

my nexus 7 2013 wifi edition is stuck in opening wifi set up because my wifi cant connect to any router. Is there any way to bypass this one like for example using ethernet cable instead of using the wifi, is that possible? or any other way to disable or skip the wifi set up and how can i flash custom rom for this one without using twrp ? My device is not rooted. fresh flashed from stock.

I'd recommend using Nexus Root Toolkit - it has full ilustrated instructions for tunning your device. You can flash a custom rom using it. cheers.

Nexus Root Toolkit is a dead simple app that lets you flash content through fastboot mode. If your device is softbricked, it can also fix it. http://www.wugfresh.com/nrt/

ive done reverting to stock rom using nexus toolkit but my wifi still not working. i also use adb to push a file but it doesnt recognize my device though fastboot mode is working it does detect my device.

I don't know how to help you with your wifi problem, I just helped you with flashing through fastboot & NRT. Hope someone else knows something about it. Cheers.
Do you mean that it has no internet connection when connected to WiFi? I also experience it. You just have to reset the router or google how to fix it.

its a wifi failure cant connect to any wifi devices due to i changed its mac address but its okay now.. already fixed it. anyway, do you know how to free my system memory in my nexus 7 flo fresh flash from stock rom marshmallow 6.0.1 MMB29O, i noticed after i flashed it, a little small memory left in my system memory, how do i delete those google apps like hangouts etc maps i dont need them except playstore.

You have to root your device for that. In order to root your device, you will be needing a custom kernel then flash the superSU 2.60+
From there, you could easily delete the system apps.
