I have had this happen twice now on two different roms over the last few weeks. Phone will not come out of sleep and I have to yank the battery. When it reboots, it acts as a fresh install and apps are gone, no shortcuts to anything. Does not see my google account so all contacts and texts are gone. Everything is there on the SD card when I take a look at it.
Any idea what is going on?
Possibly corrupted NAND, bad wipe or partitions?

My girlfriend's stock DInc is making apps disappear and reappear. We've done a "hard reset" by pulling the battery. Nothing has helped. And now, some of her apps have randomly been moved to the sdcard showing a silver/gray icon that says SD on it. What can we do to fix this?
Try moving everything back to phone from SD. Problem shouldn't persist. Reboot to confirm. May have to put widgets back. Then move only the largest apps on SD, reboot, set up widgets if needed.

The apps weren't their normal names and the only option it gave us was to uninstall. The names came up as "com.***.***.***" She found out what apps they were and reinstalled them, then deleting the one off the sdcard. We have one called "com.Google.zxing.client.android" and we don't know what it does. Lol.
Not sure what causes it, but at least 3-5 times a day my phone will freak out and start restarting in an endless boot loop. Even when i pull the battery for a hard reset it still gets back into the boot loop!!! I've ran multiple roms and still have the same problem.....any thoughts???
Full wipe between flash? Not restoring stuff with TB?

I am having the same problem. It's getting really annoying.
I have the latest clockworkmod and tons of space on my SD card. Before flashing any ROM I always "wipe data/factory reset", "wipe cache partition", and "wipe dalvik cache"; sometimes multiple times. I only use TB to restore my angry birds, lol.
I just got finished trying to install CM7 and it rebooted as soon as it tried to download apps (g. maps, gmail etc.). Prior to that I was running Virtuous and it was mostly stable, but would start boot looping while visiting certain web pages (newegg sale pages and also some mazda3forum.com pages).
Just restored back to stock and it's booting now, hope it works.
Good news! CM7 appears to be working now.
What I did was all the usual wipes, but then also formatted boot, system, data, and cache. I must of had some bad code hanging around in one of these partitions even after wiping multiple times.
So far so good and I have been able to download and install all my google apps as well as browse mazda3forum.com w/o crashing.
I am happy.

I'd say go back to a stock rom... or, better yet, unroot, then root again. Give yourself a fresh base to start from and then worry if you are still getting those loops.
Make sure you're not constantly flashing the same kernel. Some kernels are known to freeze up and cause random boots.
Other than that, my first thoughts is that you haven't properly wiped your phone.
Tried cleaning the battery contacts? I got a rash of these sudden reboots; then noticed it was only happening with one of my 2 batteries. Cleaned the contacts with a pencil eraser and bam, no more rebooting.
Might not be your culprit, but it's easy to eliminate!

I am having the same issues. Yesterday, I ran RUU and got it back to stock everything and didn't even install one app. I ran the youtube app that came installed with it and it rebooted. I decided I wouldn't put up with this crap anymore and drove straight to Verizon and they ordered me a new (probably refurb) model that should arrive in a day or two. I have seen many threads on the internet of people having rebooting problems on the Incredible and it seems to be on random builds.
I just want a phone I can use without it rebooting. Making me think I should just get an iphone so I can have some consistency with a phone that stays on. At this point though, I'll wait until September for the new one to come out with LTE support.
Happens to me too...No matter what rom I flash what kernel i flash......or if i wipe date/devlik/cache........I always get random reboots time to time.....other than that CM7 is running great since i have flashed 2 days ago(only 1 random reboot since then) Again I am extremely happy with CM7
I def can second the battery issue, had a slim extended battery which would power down randomly. I found tapping on the back cover where the contacts are would cause a power down. OEM battery no such problem over time or by tapping. Sent extended in for replacement all will be good. I suggest you try another battery or cleaning before trading in your phone.

Maybe I'm using the wrong term, "icon", but I've attached a picture of my screen that shows my problem/question. Why do the real icons go away and the default android icons show up? Anything I can do to fix this?
That will happen if your apps are in your sd card, and the card is unmounted from the phone. Or it can happen if your phone is low on memory and kills Sense/whatever homescreen you are using.
Well, the apps are on the sd card for the most part, but perhaps the problem is the "unmounted" issue. Guess I better do some research on that.
I'm using the V6 turbocharger so I don't think it's a low memory deal.
I'm using the SNS gingerbread ROM, but this problem was apparent from day one with the stock rom, too.
I run into the same thing after rebooting the phone from time to time,for some reason it justdoesnt like having some apps on the sd card.

Same here, using EvoledRoms. The only "icon" after reboot that changes to that would be the xda app. Out of all of my other apps this is the only one that happens.

If you kill the launcher, the app icons should come back when the launcher reloads. The launcher is up and running before the SD card is fully mounted and readable when you boot the phone. This is the same reason why widgets can't be stored on the SD card.

Looks like those apps are in a folder. This has been an issue since the Gingerbread update for the Evo. The fix is, after each time you reboot, go to menu/scenes and pick a different scene (doesn't matter which one, as long as it is not the current one you are using). If it asks you to save the current scene, do so. Then, after it finishes loading go to menu/scenes again, and pick your current scene, or the one you just saved. When it finishes loading your app icons in your folder will be back to normal.
Sucks to have to do this every time you reboot, if they ever release another update, I hope they fix this annoying problem, but I am not holding my breath.
Not sure what my phone was doing when this happened tonight but at some point it froze (does that from time to time), I went to turn it on again and for some reason it was taking longer than usual. When the phone finally booted up, my current launcher (Go Launcher EX) was filled with mostly widgets that said app not installed and popups for a dozen or so of FC's. After a little digging, it turns out that the apps on my SD card no longer appear on the phone when I go to the Applications section in the settings. Is there anyway I can see if the apps are on the SD card when I connect my phone to my computer, or through another method to verify that they are there? Has anyone else experienced this issue? Is there another solution to this?

Just about. Seems like any app that I try to open that I've downloaded are force closing instantly. Even market does the same. I'm thinking possibly a corrupted file on the phone. I know that my last chance is to do a complete wipe and reset. Which in that case if I have to, I might as well install a rom since I've been meaning to do so but have been putting off for a while. But before I do this, I'm trying to find the reason for this happening,
What rom are you on? I've had this issue on newt and Joel's Runnymede/Sensation XL 3.5 ports. Not sure what causes it, but the only way I know to avoid it is kind of unacceptable, simply don't put apps on the sdcard.

I was wondering if someone could help me out. I am having issues since the past few days with making and receiving calls. The phone FREEZES while on a call, (there is no set time as to when this happens) and I have no choice but to take out the battery and put it back to restart the phone. It is becoming a quite a nuisance and was wondering if this was due to some application or something else I'm unaware of. I had a phone protector and I've taken it off since the past few days but it still freezes while on a call. FYI...I use the GO Dialer EX for the calls, have avast mobile security installed as a virus protection. I have *TheCollective* AoCP - CP5.3 installed with AOSP JELLYBEAN 4.2 Linux 3.0 Ktoonsez kernel.
Any help would be appreciated. This freezing is driving me insane...

Such problems can be something hardware. I had a similar issue with my old cell phone, freeze for 10-15 seconds maybe, and then reboot. There was an error in the Sd-card. My suggest is to use some app for check/scan your sd cards for errors. If your sd card is corrupted for some reason, or has some files corrupted, read or write from it can produce unpredictable results, like a freeze.
Yeah, could be any number of things, including a corrupted file on your SD. You can check for errors on your SD by following these steps:

I did a fresh install of a new ROM today (AOKP) and it did the same thing today. While making calls it was freezing up at random times. Would formatting everything and doing a fresh install fix it?

so you're saying that when you use the included android dialer you have no issues? so we can conclude that go dialer is the culprit?

and formatting everything might fix things. if you're going to make the effort, i'd wipe data, cache, dalvik, format /system, flash rom/gapps, wipe data and cache again.
its also common to have issues when not coming from stock when you flash an aosp-based rom
I am having the same issue. Phone freezes sometimes during calls. I can finish the call, but then the phone is frozen, I have to take the battery out or wait for phone to restart after a while...Also, sometimes looks like proximity sensor doesn't work, as it doesnt turn on screen in call when I remove phone from ear.

Same issue here on a GS4 with latest CM11 nightly and no custom dialer. Been having the issue for a couple of months. I've tried different ROMs including back to stock with the same issue. I though perhaps it was a defective proximity sensor but several appss show the sensor working correctly. Open to any ideas!
