Hopefully you guys can help me out. I'm suddenly having issues with my phone not being detected by my PCs when it is connected via USB.
When i plug my phone into my PC, i receive the error message: "UBS Device not Recognized One of the USB devices attached to this computer has malfunctioned, and windowes does not recognize it." from my Windows XP PC. My Windows 7 PC is unable to recognize the device plugged in.
The strange thing is that even though my phone is not being recognized by the PCs, the phone will still charge.
The phone is not in USB Debugging Mode, and I've gone ahead and flashed 2 separate Kernals on my phone, but i still am having this issue. I have tested this out with 3 USB cables, on 3 separate computers (Work (win xp), Home (win7), and friends(win7)).
2 Days ago the USB worked fine for me as i was able to flash Franco Kernal and JB onto my Gnex, but suddenly it's not being recognized by my computer anymore.
Stumped.....Hoping that this isn't a hardware issue and something that i can fix without having any downtime away from my phone.
Any thoughts?
Try other USB port and other USB cable.
Sounds like you're missing some drivers... Remove the device completely in Device Manager & plug it again. For MTP, Windows 7 should find the drivers online by itself. If you need fastboot access, you'll have to download the seperate drivers for that. I'd recommend the GNex Toolkit in that case since it comes with the drivers included (you'll still need to install them of course )

Yes, I have been using the GNEX tool kit, though not the 7.3 version that was released today. The 7.2 version was being used at home and on my work PC. I usually install the drivers from the toolkit then plug my phone in when done. The drivers have been recognizing my phone but not since 2 days go.
I'm currently trying to find the MTP checkbox in the phone Settings to double check if it's checked, but im having problems locating it.

Go to Settings > Storage > tap top right menu to display USB computer connection option & tap it

Ah yes, that is where it was hiding....But unfortunately the checkbox is checked.
This seems like a hardware issue, but I'm not certain since the phone still recharges when plugged in

Could be... Charging doesn't necessary have anything to do with data transfer AFAIK... Do you see something in the Device Manager when you plug it? You might have to right click & do a re-scan to force detection.

Device Managers shows "Samsung Galaxy Nexus ADB Interface". The drivers look to be installed.
Also another odd thing, is that after i charge my phone and unplug it from my PC, it will continue to show that it is charging on the lock screen and in the top right corner of my phone. However if i go into Settings ---> about phone. It shows Battery Status : Not Charging.

You get that when the phone is running normally? I think for me that one only shows up when I put it in bootloader mode. In MTP mode it's just a media device like any other AFAIK... I'll check tomorrow when I'm back on my Windows laptop to be sure . Have you tried putting it in PTP mode? If so do you see the device as a camera or is it the same issue?

Sounds like a software bug, but I'll admit it is a bit strange considering your main issue...
Hello i have a question too on Usb subject. forgive me for go off topic over here is there anyway to get my music be playing in the car radio tru usb with my first google phone it worked like a charm model Htc google nexus one just plugged it in en voila music came out..but with the gnex won,t work thanks in advanced

Google galaxy nexus nfc battery. either the battery or the port is shot. People having issues.
And get your drivers from Samsung and Android sdk. Qbking77 has a YouTube video on how to set it up.

Basically the way the Nexus One talked to other systems through USB is completely different from the way the Galaxy Nexus does. The GN uses a new protocol called MTP. Your car stereo probably can't read the files on your phone because it doesn't have MTP support.
Phantom Charging Issue
So I've gone ahead and Fiddled around with my phone for the past day trying to fix my USB issue, and I've discovered that this issue may not be due to USB issues.
Throughout the day when i was playing around and trying to fix my USB recognizing issue, it miraculously started working again. Now I am able to connect to my PCs and transfer the desired files.
Upon this issue being resolved, i noticed that periodically the screen would turn on, on it's own while being off in the locked state. I observed this issue to see what may be causing this, and it is turning on due to my phone thinking that i plugged my micro USB cable in to charge it. The charging indicator on the batter flips on, and even the USB Debugger / MTP Notification is present in the notification drawer.
Apparently this is a known issue with the Battery or charging port with Gnex's. (See below the 2 youtube clips for more details for problem/fix.)

I believe my warranty is void since this phone was purchased in a different country and has had the bootload locked/unlocked a million times. Anyone else have this problem that they have been able to fix manually? Or am I going to have to cross my fingers, ship my phone to get it fixed, and hope the post man doesn't f**k up?

Interesting problem I have had on and off. When I plug my phone into my desktop, it will connect normally, then a few seconds later it acts like I unplugged it. It's not the rom as it does it in recovery as well(both with usb-ms activated and without). I have HTC Sync installed. Rebooting either the computer and/or phone doesn't fix it, it just goes away after awhile and starts working again. I've experienced the problem FAR less when connecting it to my laptop, it will typically work there and not on my desktop when the problem crops up.

I have the same problem. just keeps discontenting over and over it sees it for a few seconds then disconnects reconnects then off again.

Mine doesn't reconnect, it 'disconnects' and stays that way(as if i unplugged it) until i actually do unplug it and reconnect it.

I was having the same problem on my MacBook until I realized the MacBook must be plugged into power. Once plugged in everything was good. I wonder if the Evo demands more power through USB then other devices.
Yeah, sounds like a power issue to me. If you're using a USB port on a hub or on a keyboard, try one on the PC itself. Also, it could be a slow USB port or a bad cable if you are continuing to have issues. Try another computer if you can, and if you still have the problem try a different cable. Still have the issue, reflash (if you're running a custom rom) or contact Sprint for service.
Same issue here, but on a desktop PC (Dell Studio XPS) using the front-mounted USB port (not a hub). Keeps disconnecting/reconnecting, but often takes 10-15 minutes between cycles. Using stock ROM (for now, at least). Will try other ports & another PC and will report back.

If it works on a laptop, but not the desktop, then it appears to be an issue with your desktop. Any other USB devices doing it?
I would check your motherboards manufacturer and make sure you've got the right USB drivers, a bios update might be in order as well.
But before all that, I've got an idea. First and foremost, try this:
Go to device manage (either from control panel, right-clicking my computer and going to properties, or searching for devmgmt.msc in the start menu search.)
Navigate down to Universal Serial Bus controllers, double click on USB Root Hub (you probably have 1 for every USB Port, so you may have to do them all.)
Click on the Power Management tab, and uncheck the "Allow the computer to turn off this device to save power." check box and see if that solves your problem.
i had this issue on my hero. it was corrected for me after i did a wipe of data and cache. i restored from titanium afterwards. YMMV

one thing you might try is disabling sense and using the android launcher. then reset the phone. once it boots up try and connect it to your pc. This was how I got mine to connect. when I was using the sense launcher it somehow blocked that options screen and the default I selected wouldn't stick. once I got the connection to my pc working this way I was able to re-enable sense and it's worked perfectly ever since.
I do not take credit for this fix, someone else on here told me to try this n it worked perfectly.

it was nothing with the rom, since it was doing the same thing in recovery.
it turned out to be the short little usb extender cable that was going a foot and ahalf from behind my pc to the side so i could plug it in easier.

Hello all. I'm TK, newb who has been lurking for a while. The forum has led me to some awesome changes with my Evo, but that may be catching up with me. I'm still rooted and running CM 7 with SZ 1.01, and none of my computers are able to recognize the USB drive on my phone, regardless of which cable I use.
I can't get ADB to work on my PC, so I'm not able to perform any commands for the phone.
Can anyone help me with this issue? It would be much appreciated. Thanks!
Do you have debugging(in application settings) on when trying to use adb? What about allow mock locations? Try turning debugging off and then turn it back on(while plugged in USB). If that doesn't work you could always go to recovery mode and enable USB toggle and get it connected that way. Also make sure you're actually pulling down the notification bar and selecting "mount".

Does your phone get recognized at all on your computer when you plug it in? If not you may have a hardware problem -> usb port is loose.
On my original cord that came with the phone I had to trim off about 2 centimeters off of the plastic to go in far enough to have my Toshiba laptop to read it. I have had to do the same thing to several cords since. Maybe your usb?

Yes, my computer just says that it doesn't recognize the hardware. it charges just fine by the computer.
I'm running ClockworkMod for my recovery, I'll check for USB Toggle there.
No joy on finding USB Toggle.
It's called something different. Can't remember but it's there, I believe. I've only been in Clockwork recovery like once.
For the last 3 months, whenever I have connected my phone to the computer with the USB cable, I would get a little pop-up screen on the PC with about 5 options to select from: sync my phone, see the folders and files, etc. I have not seen that little box in about 4 days.
Luckily, when I bring up Windows Explorer (file manager), the PC does recognize the phone in the USB port and I can proceed as normal. I really have no need to sync my phone anyways but I am curious what happened. I have removed the battery more than once, using the same USB cable I have used for 3 months, and reinstalled the drivers just in case. Rooted with the LIH Jellybean leak. Any ideas where the USB pop-up screen has gone? Thanks.

Computer glitch not phone glitch =p It usually goes away when you accidentally press "always use this" or something similar. I cant remember how to reset that
Hi all, I've been lurking the forums since bout September of 2012. I haven't really been posting here, mainly just gathering ROMs, kernels, help etc.
Anyways, my Nexus 7 isn't being recognized by Windows 8, Windows 7, Windows Vista, or even Ubuntu. this leads me to belive this is caused from the tablet. I have installed numerous drivers, but with no luck. Recently, I noticed when I boot into recovery, and the tavlet uis on the Google screen, my computer recognizes it as, "Unknown Device." Soon after it passes the Google screen and boots into TWRP Windows can't detect it anymore. My tablet says it's connected via AC, not USB - which is probably why I can't transfer files. I have MTP checked, and I tried PTP too. I've tried enabling and disabling debugging with no luck still. I'm using the OEM charger for the device, and I have tried using multiple cable with no dice.

They had me turn of my Nexus 7 and then hold the Volume Up + Power button for 30 seconds and then hold only the power button for 60 seconds. I was told this is to remove static from the mother board. USB Charging, USB Transfer, and Wireless Charging (which is what made me notice the issue) now work. The only problem I have now is that the cord that came with the tablet seems to be a bit loose and will cause it to stop charging when I don't just let it sit on a table."
That was suggested on a different forum, but that didn't work for me neither.
ADB nor fastboot cannot see my device as well.

What drivers are you using? Try the generic USB ones here, which I use with no problem at all: http://developer.android.com/sdk/win-usb.html

Gotta love Asus' tech support.
Try Nexus Root Toolkit, it has a nice walkthrough that shos you how to first uninstall any existing non-working usb drivers. Then just try its options one by one and you'll have working drivers in minutes.
I gave up even trying to get my original N7 and my N7 (2013) to be recognized by any PC. What I do instead is use a free app of the day I got from Amazon a while back called Wifi File Explorer. Run the app and you can access the file system from your PC. Upload, move, copy and create folders.
It's worked for me for a while now. I'm sure there are other free apps that will do the same.

Have you also tried usb ports on the back of the PC? Those are directly on the motherboard, whereas usb ports on the front of a PC are almost always via a usb hub connected to the MB. I know of instances where this makes a substantial difference (device being recongnized or not). You might try it (if you haven't already).
Edit : also try disabling AV software and possibly firewall. But admitedly I have personally never had any issues regarding device not recognized due to this.
This was my solution.
Bought the 'official' usb to usb3 lead from the Play store. When I tried connecting to my Macbook Pro the Android File Transfer can't find the Nexus. I thought it might be the USB Debugging needs enabling so I did that. Nope. Maybe the Android File Transfer needed updating so updated it. No joy. I'm a bit lost now, can anyone shed any light on my problem?
Mine would say something about the screen being locked even though it wasn't.. When you plug it in, pull down the notification drawer and tap the Choose More USB Options, or whatever it says. Make sure File Transfer is selected and try again.. took mine a few times of switching between FTP and Charging but it works with my Mac now

More thanks. I had to switch it between charging and MTP 5 times to get it but it worked in the end.
Is there any way to making the MTP setting sticky for trusted devices? When you connect with usb debugging, it asks you to verify and save the device. If something like that was possible with this setting, you could have the security of not auto enabling when charging from random devices but the convenience when connected to your own remembered computer.
