Hi,
I am currently running AOKP build 31... I know there are many more recent builds but I was happy with this one.
I decided to try JB and was trying to wipe the device and I'm facing something strange:
- I tried to wipe the phone (data, cache, even data/factory reset) from the recovery menu and it has no effects at all...
- the first couple of attempts I did not notice that the wipe was not happening and when I tried to find the JB zip file on the phone from the recovery menu, it said "can't mount /sdcard"...
- I tried to trigger the factory reset from the settings menu on the phone and the phone comes back exactly as if I had done a simple reboot...
I'm a bit lost... Am I missing something obvious? I did search and was not able to find something that would look like what I'm facing.

Maybe try Super Wipe Lite? Idk... it might be of use. Just flash it and it should wipe your device (except SD card of course).
Did you wipe system?

Well, i have no specific menu entry to wipe system, I think... I tried wipe data / factory reset... where would I find wipe system option, please?

Would be interested in testing this, but I seem to have no way to flash anything since when I'm in the recovery menus, when it tries to access the /data or /sdcard folders, it does not find it...
I think I'll have to find a way to get recovery to "see" /data and /sdcard to be able to fix that.
I found a "format system" entry... Just did it... Not too sure where this is going to take me...
I would reflash recovery if I were you. You can do it from fastboot mode using the gnex toolkit

I had tried it earlier today and it did not complete the operation (was blocked at something like "waiting for the device" something).
I have just done it again (after formatting /system) and it worked.
But it still doesn't "see" /sdcard... so I can't load anything...
Now when I enter in fastboot, I see : Fastboot Command Read Error - 21474833647
EDIT - I reflashed Recovery from the toolkit and Fasboot status is now okay... I guess I now need to find an img file and to try to flash it from the toolkit?
I'm stuck here... What would be the next step? I tried to do a Nandroid restore from Gnex Toolkit but when I start it, the main window of the toolkit just vanish and that's it...
Okay, so I used GN Toolkit to reflash entirely the stock config and now I'm installing 4.1. Don't know exactly where things went wrong but I'm now back on tracks, it seems.

Probably because you didn't read the instructions and were in recovery instead of in fastboot.

Yes, that most probably explains the second problem where the phone was not detected.
But i'm not sure it explains the first problem where there was no access to the /data or /sdcard...

CWM says : E:Can't mount /sdcard
Okay, so now I am trying to get back to my previously backed up ROM, after having tested JB...
And I am back to where I was earlier: CWM can't mount /sdcard...
I must be missing something very obvious but I don't know where to start looking.
I have just wiped entirely the phone, thought that this problem would be gone.
Any hint on what the issue could be?

I might be having a related problem, but it wont even let me go back to the stock rom... it errors out when flashing the stock rom but the recovery flashes a zip already on my internal storage..
My problem is that with JB and now clean CM9 RC1 both of them have problems booting and freeze and reboot during the bootup sequence..

SHG-T989 on TMO. I was running CM 9.0 but wanted to update to 9.1. NOW I CANT DO ANYTHING!!! Here is what I did:
1. Downloaded the CM9.1. 2. did a backup through CWM. 3. wiped data through CWM, "wipe data/factory reset. 4. wiped cache partition through CWM. 5. did a Darkside Super Wipe through "install zip from internal sd card" (last time installing CM9 i got stuck in the boot screen and had to go back using Darkside Super Wipe, reinstall CM9 and it worked fine). 6. Reboot to recovery. 7. installed CM9.1 through "install zip from sdcard". 8. installed gapps through "install zip from sdcard". 9. Reboot.
It came up and was stuck in the CM9 boot screen. So I went back in through CWM and tried to "wipe data/factory reset" and it wont work! Tried to go back to stock through Odin and it wont work! When i try to wipe data, the process starts and completes the "formatting /data, then gets stuck at formatting /cache. Does the same thing if I try to wipe the cache.
Im completely stuck and have no phone.
Which recovery version are you using? It might a corrupted recovery.
Try this recovery TWRP is cwm flashable:
SGH-T989_TWRP_v2.2.2.0.zip - 6.72 MB
Then TDJs super wipe and then install the stock ROM or any other ROM.

using cwm 5.0.2.7 did a darkside super wipe, flashed on a JB rom (P.A.Cman), flashed gapps (20121011), wiped cache, wiped dalvik cache, rebooted then got stuck on boot animation. rebooted back into cwm to try and wipe again and reinstall, no luck got stuck on formatting cache.
loaded up my nandroid which went fine, ended up installing twrp instead to see if that would help, rebooted into recovery and was stuck on the "teamwin" blue screen and wouldn't go further multiple times. so, put cwm back on (6.0.1.5) to see if the updated version would work. nope, same thing- can't do anything with the cache partition.
any ideas??
luckily my phone still works fine with the backup i made. running aokp ics build 40, streamline #27 kernel, uvle1 radio. been running that setup since july with no real problems, just wanted to try out JB finally and seems now i can't

Ok, the wipe scripts in CWM recovery v5 are broken! Got experience there because I got stuck on the PACman ROM boot animation.
So, for factory reset, use Darkside Super Wipe. Install zip from sdcard and choose the wipe. From there, install rom, then gapps, and then use the Darkside Cache Wipe. After that reboot and it should work (hopefully)
However the most important thing is that the wipes don't work in CWM recovery v6! It will do bad things!
So yeah, use super wipe to factory reset. Use cache wipe for cache. But follow the instructions to install the ROM.

got it all working again.. ended up odin flashing twrp 2.2 and i was able to boot into it, wipe everything, and install the new rom stuff.
Similar problem, but cannot make zips to work - HELP NEEDED

I hope you guys can help me. My T989 got stucked in the splash screen when I tried to update to a nightly CM10 ROM. I entered into Recovery MOde (ClockworkMod 6.0.2.7), then tried to use Darkside Super Wipe... but apparently the phone cannot open zip files. When I hit install zip from sdcard and then select one of the zips (I have tried different ones) it says "---Installing: /sdcard/sgh-t989_darkside.supe.wipe.zip"... but it does not move from there. After 3-4 minutes it reboots by itself and gets back to the splash animation, but never goes beyond that point.
I tried to sideload using ADB, but the phone is not recognized by my PC.
As you said, I think that the recovery mode is corrupted... but I cannot install a new version using a zip
Is there another way to reinstall a version of CWM that allows me to install zips so that I can follow the process you suggested (super wipe + installing ROMS)?
Help will be very much appreciated

Do NOT use that darkside superwipes with CWM recovery v6.X.X.X. Does bad things.
Use the scripts only for CWM v5.X.X.X.
Anyway, the damage is done and bad things have happened... So maybe check here

Use Odin. There is a tutorial on how to revert to stock GB (might be ICS) in the T989 development forum. It should be a sticky in that forum.

Thank you very much for the answer. I actually spent hours trying to find something before I posted my question. Even before posting I found and tried the method using Odin... but my phone is not recognized by Odin. I followed the instructions but on step 4 I actually do not get any "warning" screen... and then the phone is not recognized.
In the meantime, I can get into recovery, but I cannot install any zips. I have tried to complete a factory reset, but the process gets stuck at "Formating /cache..." and then reboots. When I try to install a zip (any zip, the radio, a new ROM, gapps,... any zip really) it gets stuck too.
Any other ideas? I really appreciate your help.

The only other possible option I can think is to try and install another recovery....
However!
I have no clue how to do it, and in the end you might just end up making the issue worse.... Like hard brick it.
I might think of something later, but I don't know what else I can do for you. I will search around, but no promises...

Ok to be honest, try restoring a backup first and see if anything happens there, and then try this thread.
However like before, this may make the problem worse! Proceed with caution and read and understand everything before attempting.

A couple of days ago I tried to install a functional backup that I had from CWM... but it does not work It runs for a long while, then reboots... but it still freezes on the splash screen (my theory is that the backup is not recovered, as I know for sure that the ROM I backed up did work fine)... I've heard about the jig method as well, but have no idea on how to go about doing it. Might be worth a shot. I don't think the jig costs that much anyway. I will see if I can find anything later.

I have tried to flash 5 different roms that are known to work and flash right but they all do the same thing on my phone which is hang at the Samsung screen and not boot. Im using the TWRP recovery with exfat support from the dev section. This is what ive tried....
Ive tried flashing from both internal and external sd . I never ever never dirty flash. I always wipe cache, dalvick, factory reset, and format system every time times 3.
Anyone have a clue why I cant flash anything?

Did you say "format system"? You don't need to do that. That will wipe your entire OS. When you flash a ROM, it already wipes and replaces the system, so there's no need to format it beforehand. If you're doing it AFTER, then that's your problem. You SHOULD do the wipe cache, wipe Dalvik and factory reset after flashing though.

This is usually what I do when I flash a rom...
1. Wipe cache times 3
2. Wipe dalvick times 3
3. Factory reset times 3
4. Flash rom
5. Wipe cache
6. Wipe dalvick
7. Factory reset...
Thats my normal way I flash any rom even if the dev says just wipe caches. I dont never dirty flash...

Im really starting to wonder if its the twrp Im using with the exfat support. Its not the official one on gooim its one a dev modded to work with exfat. And that would make sense but I cant flash from internal storage either but I can restore a nan backup from internal or external. ..

Okay then. I read "format system every time" so I was worried about that. I didn't notice that you have 1400+ posts, which implies that you have some experience.
Yes, trying a different recovery will help narrow down the problem (or fix it). Did you read through the status messages after flashing to make sure that everything looked okay?
Also, how long do you wait at the Samsung logo after flashing? Sometimes it takes a while (I think it took 10 minutes on my Tab 2 before - S3 is faster though).

Yea I have a little knowledge and usually dont ask questions cause normally i can figure it out on my own but this has me stumped.
I got no errors and twrp said flash successful every time. Wait time normally I give it 10 minutes but got aggravated last night and set the phone down and left there for 30 minutes or so and still stuck. And every rom ive tried has been a one known to boot with no problems. I read the threads to be sure.
Im gonna try flashing the twrp thru gooim when I get home just to see and if that dont work im gonna try cwm just to eliminate the recovery but I got a gut feeling its the recovery. I would like to get it fixed soon because im working on building a rom and cant flash it to test it.

Just to let everyone know in case anyone else ever has this issue I did finally flash a couple of roms. The only thing I can figure out is I was using rom manager to boot into recovery instead of the three button combo. Shortcuts never work when doing this kinda stuff and I should have known better

I use ROM Manager to boot into recovery all the time, never had any issues with it (I don't like mashing the keys too frequently, I ran into issues with a stuck power button a bunch of times on my S2 Skyrocket and I assume it was from doing button combos).
If you run into further issues I'd recommend backing everything up to a pc, then do a full wipe, then format your internal and external sd cards in cwm to get rid of any bad data that might be causing this, and then get into download mode and use Odin to go back to stock (has to be Odin of course since formatting the sd cards would remove any roms or backups saved there).
This has worked for me when I've had weird, unexplainable things going on. It's a pain in the butt, but can be worth it if it makes things work properly.
PS -- This was seanz's suggested method for dealing with strange issues on the sr, as I'm sure you recall.

Thanks bud. I use Odin to go back stock and it didn't help but I didn't format the cards either though. Im using twrp and never had any issues with it but the official version dont support exfat. [Q] Help? I get this when flashing a new rom "Unfortunately setup wizard has stopped"
I have a Galaxy s3 and I have twrp installed. And so far I have tried flashing 4 or 5 different custom roms and every time I end up getting this message "Unfortunately setup wizard has stopped" and then some message about ".com.goolge gpapps has stopped".
I am flashing the rom's and gapps from trwp. And I get the success comment after both the rom and gapps flash. I then restart/reboot and the new rom's custom boot animation comes up, like with cm10 the blue clockwork logo spins, and then after 10 or 20 seconds I get the two error messages and thats its. And when I flashed liquid smooth, I got the Liquid Smooth boot animation.as well and the the two messages. I eventually get my phone tuned off and I get it booted back into twrp and restore my backup to my stock rooted setup which is what I currently have running and so far is the only thing I can get to boot up. I have tried cm10 nightlies from the past week and cm10 stable and liquidsmooth, and a couple others. I downloaded several from goo.manager but i most recently tried a cm nightly i downloaded directly from clockworks site.
Any Idea's. I have seen a couple other threads on other devices with a similar problem but most seemed to have the wrong gapps, and I am using the most current gapps, i even downloaded it from multiple sources to double check.
Does the fact my phone is android 4,1,1 and most these roms are 4.1.2 or newer matter? And if so what is the fix?

Are you wiping system and data before flashing? This is most likely your issue.

Ok what you have to try is this.
Boot into recovery and wipe system and data (removes settings so you will need to reset up everything) They wipe delvic cache and normal cache, then flash the rom and the proper gapps for the rom.

On the wipe screen in twrp there is a system option but for data it says format data? Do I need to wipe "system" and "format data"? or does the wipe system option take care of what i need to do? If I need to also select "format data" which do I do first ? format data or system wipe? will i need to wipe cache or dalvik as well, or will those be wiped in the system wipe? Sorry for all the ?'s I am a real noob at all of this.

No wiping the system doesnt format the data partition. Wipe system then format the data and then wipe the caches and then flash the rom.

What gapps should I be using ? I hav gapps jb -20121011. I havnt wiped the additional areas yet either but thats because I want to make sure im installing the correct zip files

Most rom threads have a download link to the gapps that work with the version of android that specific rom is on. Look in the rom thread for the one you're currently running. There are different gapps for each android version (4, 4.1, 4.2)
I have that same problem too
I was trying to install new roms for my phone i tried liquid smooth and avatar and CM 10. I also tried install a new kernel and also different recoveries but nothing seems to work. I never "dirty" flashed and i use the right gapps. Someone please help me here i hate stock roms!
No matter what Gapps version I've tried I still get this same error. This occurred after trying out the Dark ROM modeled after CM11, but now no matter what ROM I try, this error still occurs. The only time I was ever able to get passed it was using a backup, but that has mysteriously gone AWOL. With that in mind, has anyone actually figured this out? Is there anyone with any USEFUL input? So far all I'm seeing is a bunch of suggestions that don't work.

That's a good point. You should be on TWRP 2.7.0.0 that's the latest version of that recovery.

I use the latest CWM. I'm a creature of habit. The first Samsung I ever put a custom ROM on was done with CWM, so I stick with it. I finally got it to go, but it constantly froze, tried to reset, stuck with the same issue. Never used TWRP, so if someone could actually point me in the right direction, that'd actually be helpful.

I have no idea how to use TWRP...it only lists a .img file and no instructions on how to use it. zip files can be used from the bootloader or recovery. WTF am I supposed to do with an .img file. And it can't be to use the terminal emulator because to do that I would have to be able to get passed the Setup Wizard...which is the issue.
I finally figured it out with CWM. Thankfully. The phone would've went flying otherwise.
If anyone else stumbles upon this thread (so they can find the answer for CWM also), you have to format the device from the CWM Recovery to get rid of the crap that is blocking it. I did the following:
Once in CWM Recovery, Remove your SD card to avoid formatting it by accident.
Use the Volume Key to scroll to "mounts and storage".
Format all existing options (this is why the SD needed to be removed); this will blank out the phone's system which was causing the Setup issue to begin with.
Put your SD card with the ROM to be installed back into the phone.
Follow the guide as normal after this (install ROM, then reboot, then KK_gapps, then wipe the two caches, then reboot.)
I think I got myself a Maguro Galaxy Nexus (on ATOM ROM, Android 4.2.2) with a corrupted /data partition.
(Not 100% sure though, so if you think I'm wrong, please tell me)
I can't wipe the /data partition in CWM, nor perform a partial restore for data from nandroid backup. When I try that, CMW crashes and my phone reboots.
I can however still boot the phone normally. Everything else works, just can't do a factory reset in CWM, which I want to do to flash a new ROM.
Any ideas how I can solve this? I'm alright if I loose my data, I got backup. Would like to avoid erasing the virtual sdcard though (takes ages to copy).

Try using what command? I can flash roms n gapps no problem, but I can't do factory reset. Is there a command for factory reset in fastboot?
P.S. We are almost neighbours, Schiedamer hier.

Lol, feel a bit stupid now. I didn't know there was the fastboot command "erase". I erase userdata using fastboot, however, now I got a whole new problem... It got rid of the whole folder, not just the contents. Now there is no data folder. And I forgot that the virtual sdcard is also inside the data folder... Now I got no data at all, nothing to flash...
I'll try to find out what to do, but if anyone reads this soon, perhaps you can help me.

Alright, I tried something I've wanted to try a long time now - I connected my USB memory stick (OTG mode) while in CWM. It worked wonders. I have all my zip files (ROM,Gapps,Kernels) on it, so I flashed from there.
EVERYTHING WORKS NOW.
Though my sdcard data is gone, but luckily I backed it up this morning.
P.S. For those of you who don't know, I found out you can restore nandroid from, and backup to the USB memory. I think I won't even put my nandroid and zip files on my phone anymore. I'll keep it on my usb memory.
Hi all, today my phone was acting very strange so I decided to do a factory wipe.
Afterwards I tried to boot my phone and it just sat on the samsung screen.
I then decided to wipe everything and restore a back-up of a different rom.
The install of the back-up failed.
I tried to install a fresh rom and it failed.
I am getting these errors in twrp.

You need to read the last few pages of the thread.
1. Possibly billiards factory reset
2.philz recovery
3. Lots of wiping..

Twrp does not backup or restore tw 4.3 or any 4.4 properly. Best bet is install philz 6.07.9 and flash fresh.

The thing that is probably going on is that bits of data are left over. We need to clean.
Process A is this link. And we should still do process B but, this is the deep clean

I was finally able to install the latest smooth rom w/o aroma but I am getting this error when the phone boots up.
UIDS on the system are inconsistent, you need to wipe your data partition or your device will be unstable. with a tab that say im feeling lucky. I selected that and all seems fine but i cannot turn on my wifi.
sorry to be a pain, but what should I do?

I found the wifi fix and it is now working, but I am still getting the error on start up about the data partition.

Follow the steps he provided. Proper wiping is super important.

Here is how to do a clean wipe, just remember you r doing like format the comp. hard drive and install new OS. I see u have Philz now. Boot into Philz then go to Mounts and Storage and pick format /data and /data/media (/sdcard). Once done pick: format /system. done pick format /preload. Then go back and chose Wipe Cache Partition. Once it finish, pick Wipe Data/Factory Reset. First, pick Wipe Data/Factory Reset. When finish, pick Clean to Install a New ROM. Then Reboot back into RECOVERY and now you can install any rom of your choice in the external sdcard (showned as sdcard1 in Philz). If it's still not working, use Philz to install the back to unrooted stock MC2 by Billard412 and follow my steps in the other thread. The reason I said this because I have to note 2. Once is fine with whatever wipe. The other had to go back to MC2 and upgrade to MK4 then rooted through CF Auto Root and install TWRP then Philz then wipe everything in the above steps then everything is fine with a hitch.

Thanks for the help. I now have my note2 back up fully and I am back on a jellybomb rom. not sure if i will go back to a 4.3 rom as I never had this many issues on ANY phone on any rom on any OS.

Remember not to use TWRP to back up 4.3 and install it. It wont work. I always have the back to unrooted stock MC2 in the external sdcard just in case flashing the new rom wont work. I am glad you got your note 2 back. You should try out the Smoothest Note 2/3 1.3 tester. It is very stable and has alot of Note 3 features.

I actually installed smoothest rom with TWRP with no issues until I started to use TWRP with Smoothest.
Ultimately it was my fault for not switching to Philz. I just had to many backups for TWRP and as I said I was able to install Smoothest from it so I thought all was good.
Is Philz compatible with the older roms or do I need to keep switching recoveries?

you r fine to install any rom with TWRP. but if you use it for back up and then restore the MK4, it just wont work. I am not sure about Philz and older rom. So cant tell you.

Philz is good for everything. It will work for every rom and it will even restore your twrp backups. If you used 6.07.9 the option for twrp is under backup/restore then misc options.

I was last night, but now not so much. My phone is trying to hands free activate unsuccessfully and my data connection does not work. My GPS may not be working also.
I have tried a factory and data wipe and reinstalled the rom. I also called sprint for help but nothing they suggested worked.
I'm at a loss as what to do next. What do you suggest?
