Can't update rooted Defy with 2nd Init installed - Defy General

I get this message in "Android system recovery <2e>
E:Can't open /cache/recovery/command
.
-- Install from sdcard...
Finding update package...
Verifying update package...
E.failed to open /sdcard/update.zip (No such file or directory)
E:signature verification failed
Installation aborted.
I had downloaded the update and the system rebooted to thge "Stock Recovery" section to install the update from sdcard.
Furthermore, even after the download of teh update, I can't see the update.zip file in the sdcard using the File Manager

I was trying to update from the update .3 to .6. I was on Recovery Mode and this is what happened:

E: Error in /sdcard/update.zip

[Q] Installation aborted recovery of Gingerbread 2.3.4 (Atrix)

I have an unlocked and rooted Atrix. When I try to install Gingerbread 4.5.91.zip from ext-microsd card using CWM recovery I get the following:
--Installing: /sdcard/Gingerbread-4.5.91.zip
Finding update package...
Opening update package...
Installing update...
Installation aborted.
I am using 'choose zip from sdcard' and not 'update'. Installing update is immediately followed by 'Installation aborted'.

Getting "no /sdcard found" Errors

First, My phone config = Verizon LTE Galaxy Nexus 4.0.2 Stock ROM/ Unlocked/ Rooted/ ClockWordMod/ RomManagerPro/ RootExploere-Paid/ Titanium-Paid
Second, when I try to run the zip I get the below.
What did I do wrong?

Pretty sure that's a bug in CWM. Try installing straight from recovery rather than through Rom Manager and see if it works for you.

I powered down the phone and did the Volume +/- & pwr and chose the file.
Why did you post this in development? This belongs in Q&A.
Did you try going in recovery and select "install zip from sd card" and then select "choose zip from sd card" and point to it manually?

I rooted my HTC One X. In order to install a firewall. I'm using Clockwork Mod Recovery v.5.8.4.0 and the HTC RUU.
A few months ago a new software update was released (4.20.415.4) but I can't install it. After downloading the zip file my phone reboots, but the recovery shows the following messages:

you're phone has to be stock for the update to work, unroot it and flash the stock recovery

HTC One M8. Is it possible to update to Marshmallow?

Hello i currently have HTC One M8, Software Version: 4.18.720.8 & Android Version: 5.0.1
I have OTA_M8_UHL_L50_SENSE60_MR_hTC_Asia_India_4.18.720.10-4.18.720.8_release_449296.zip but when i try to install this update from recovery by using "Apply from phone storage".
I kept that file in internal storage as well as tried to rename it to update.zip but error keeps showing:
