Hello,
My phone recently lost 3G and data with the error code 67. Registration Failure. Your PCS Vision username and/or password maybe incorrect. Please try againg.
I got this message a while ago when I tried a tweak that "improved my cellphone internet speed" by changing the PRL. But insted gave me this error and the loss of 3G and data. I fixed it by manually changing the PRL trough recovery (updating prl dint work because i had no data). All was good, and once againg without doing anything this error comes up and I dont have 3G or data, I only recive calls but cant make em, i get redirected into sprint payment or something wich i dont owe anything. I notice my PRI (Not PRL) version was 1.7 or something i changed it to 1.90 but nothing. Oh and before this happened I noticed that Car Panel aplication was turning on by it self.

I guess im going to have to do a hard reset.
Don't know how it could've happened by accident but I had the same thing when my MSID and MDN were wrong. Try this:
##{yourMSL}#, edit, enter the correct MDN and MSID, commit modifications. Then update profile.
If still nothing then try:
##3282#, edit, enter MSL, data settings, menu, restore. The phone will reboot into "hands free activation" and you could let it run.

BTW if that doesn't work don't do a hard reset while rooted. Unroot and go back to stock first. You will have to redo your MSID and MDN after a hard reset as well

Well I just unrooted and went back to stock (http://forum.xda-developers.com/showthread.php?t=780141) but in hboot i still have s-off.
Ok, I dont know whats MDN and MSID and lesss I know what are the correct ones for me, sorry Im a noob to this stuff.
Oh and im still getting the error after unroot/stock.
And I tried already the restore but it get stucks at "hands free activation" for ever with the error code 67 poping all the time.
I guess de MDN is my phone number cus thats what it was writen, but the MSID was a different number i changed it to mine but it dint do anything I guess il change it back to the number it was.

ok so mdn is mobile directory number or your cell phone number. msid is your connection to sprint. you need to call into to sprint and tell them that you got that error code. they will then have you go through the manual programming that was stated before. when doing this write down your msid and you lock code or msl.
You said your on a family plan... if you have another phone that was activated at the same time for the same area it will have the same MSID. You might find it on the other phone under menu, settings, about. Or, of its an HTC phone you can ##3282#, view, and see the MSID there.

Well called to sprint, they dont know how to resolve the issue they tell me to get my phone to the nearest store.
But I dont whant to, lol I know thers a way to fix this some how...

I just noticed the hboot loader still says s-off, dont know if unroot worked.

what method did you use to root? if you used unrevoked then you have to flash the unrevoked S-On tool then RUU back to stock...but if you have already ran the RUU then it doesnt really matter
if you have insurance, my suggestion would be to just have sprint replace the phone

Don't take in your phone with s-off!!!
If it still says s-off then you're still rooted and they may tell you you're SOL.
After I had my evo flashed to a different carrier then flashed it back to Sprint my Evo misbehaved the same as you're describing. Basically what I did was completely wipe and reset the phone back to stock the way it was at the Sprint store before I bought it. Then manually programmed the MSID and MDN and let it go through hands-free activation just like they did when I bought it. That's basically what I described doing. Don't waste $50 on insurance.
Redo the instructions from the same guide you referenced making sure you did it as described using the Unrevoked S-On Tool and
PC36IMG. If you used another method to root I got nothing. You'll have to google.
Now that you're back to stock go ahead and do a hard reset. I had first tried the hard reset while rooted and it retained many settings. Get it s-on first THEN do your hard reset. This will clear ALL the settings including your MDN and MSID which you can program in and follow with ##3282... like I described before. Your phone will contact the network and update all the correct settings when it goes into "hands free activation", update profile, check for prl and firmware updates all on its own. Just let it do it's thing.
Post back on where you're at.

My phone is a US, Samsung Galaxy SII Epic 4G Touch. I Rooted it, Then I Flashed a Original SGS2 custom ROM on it. I think I'm eff'd, it seems like I can't flash over it. and when I do my activation, It tells me modem exception occurs. I don't know what to do! all help is appreciated. any Suggestions

I did the same exact thing tonight.. thought it was just a normal sgs2. can get to download mode but not recovery..
P.S. got it back up and running using the bone stock eg30.. but no service.. will figure that out tomorrow.. unless anyone has a idea for me

Try resetting the phone, first get your MSL by using terminal emulator, typing getprop ril.MSL . Then ##72786#, enter MSL. hopefully it will restore service.

I would say make sure you update your profile and PRL also, though it may not let you I guess... nevermind lol
Didn't come up with a MSL, when i flashed the wrong phone rom it trashed the modem.bin.. just need to get the right one i hope
P.S. Can't get kies to see it either on 3 pc's but the phone boots fine..
Having an employee taking it to the sprint store right now... suprised android doesn't have just full roms like iphones.. probably hard to do with all the different hardware though.. my own fault stupid modem.bin and me
With Samsung I have learned that Odin is your friend when flashing.
ok well i downloaded the new eg30 tar and tried to use mobile odin since i didnt have access to a pc right away. well it fubard on me,and so i got home and whent to flash through odin.well in my haste i flashed it in phone instead of pda.
Once i realized y i was getting countious fails i flashed it properly and yay the phone started and i thought all was good.well its not i put my phone back into odin mode and reflashed it again along with pit,modem,phone efs clear, and re partition hoping this would fix my phone. well ofcourse it did not im know stuck with everything working except for radio my phone displays this on my screen:

Edit: well sprint tried to get my phone to restore profile n that.I got my msl code but phone says its wrong so know they say I need to drive an hr to a sprint store nothing else they can do....

When i tried to place a call it stated welcome to verizon ....
Edit: I have used the trick found to upload cutom prl and i am know on sprint. and the msl code that sprint gave me works with qpst but not the software on the phone? Is there a way to edit the profile info so i can get my stuff set back up using qpst?

I believe you just need to grab one of the .bin files (like the new EG30.bin) and put THAT in phone. That's where I flashed my Modem.bin to, so I'd guess that's what you need to do.

Has anyone tied a FULL tar together that includes the ROM, modem, and kernel? Maybe that would get you out of that because it would overwrite everything in one shot. When I last flashed the stock tar it still wasn't complete so my apps and modem remained. A full tar might set everything back to out-of-the-box condition. Let me see if one is posted yet..

Ive used the one tar that acs released to restore phone comepletley back to stock...it was 527mb I think give or take a few mb....and it hasnt helped either....

Did that include a modem? If it did, it must have been EG12 because we couldn't get back to EG30 until tonight. I am tired so I may be missing something but there is a way to fix this. I walked someone who flashed a GSM radio on a CDMA Nexus and we saved that. I know there's a way but I may need to sleep on it.

i think i have figured it out y i cant use my msl code they supplied me... my phone doesnt register the true esn hex,esn dec.....so it has a diffrent number on the phone then what it shows up in their computers...know i have all the infowritten on a piece of paper here infront of me.How do i put my numbers back in....

This has happened to me and i have searched for anything on this but no luck... did the same thing with odin
I have the same problem on my samsung, probbaly caused by a bad flash anyone find a solution
i will post if i find one, you can install another launcher over it that will hide it.

I've spent nearly two weeks trying to figure out what happened to my phone and how to restore it. After OTA 4.1.1 (I forced this clearing gsf), in a weak coverage area toggled airplane mode. It wouldn't come back out.
Fastboot flash, CWM, TWRP, dd command, Odin, re-partitioning, nand erase all - any attempt to flash radio-cdma.img hangs or fails.
I can confirm that many of the dire warnings about hard-bricking your device... Well, let's just say that with everything that I've tried, even the bolder attempts in the end, did not brick my device. Odin would always get me back.

I found some threads on here mentioning CDMA tools and I've downloaded and attempted a couple, it seems that my pc has to have all the supporting programs to make a connection to my phone. The problem is these posts are not specific to my device but the S3 and it seems like a whole lot of trouble when there isn't a 'known' path and specific files for my VZW GNX.
I have also ran emmc check several times and memory checks out good. Sim card has also been replaced.
So I sit here today with 4.1.1 installed (less the radio-cdma.img). I can Odin the older stock images found here on XDA by deleting the radio-cdma.img within. Then use fastboot to flash the 4.1.1 images individually to have a Wi-Fi only device.
I'm willing to try anything. Any takers?
Your meid is smoked. You will need to completely reprogram phone. A restore won't do it Cuz its the serial of the device from factory. You can't fix unless you have msl

Could you point me in the right direction? Is this done using CDMA tools? I've seen places that mention it can be done via the handset dialpad (other devices). I've found threads that give suggestions on how to contact your carrier to get them to give you the msl... and I've also seen posts that state that Verizon's MSL is almost always 000000. My brain is turning to mush! :cyclops:

Verizon msl is usually 000000 you will need either the box device came in or the numbers under battery. The sticky part us going to be the MSID number you'll have to put in. If you have the ORIGINAL nandroid you made immediately after you unlocked bootloader, that one might work but no guarantees. The dialer code is *#*#msl#*#* with msl being the 000000. I believe that's it...could be *#*#spc#*#* with spc being spc and then have to enter msl. The MSID is mobile subsidy identification. Sometimes its NAM. Your MIN would be your phone number..mobile identification number.
If you get into the MEID it will have a couple numbers to enter. The numbers you will need will be your ESN hex and dec. I have all the dialer codes and can pm you a link if you like. Once you finish with the MEID repair that's where the NAM MIN thing will be needed. Unfortunately, the NAM/MSID isn't the same as your number and won't accept all 0's but once the meid is back you might be able to restore but am not positive on that. Can call Verizon and have them refresh phone I believe once meid is restored.

The phone did nothing although as soon as I completed it went the field cleared. I'm on my own. Verizon told me a week ago they were sending me a refurb. Called yesterday and there was no record of another phone being sent out. Rep told me my Sim card was not activated in their system and a ticket was opened to have it unlocked on their network. It should be fixed on their end today. However, I still think I'm going to have to get radio-cdma flashed on my phone. Using getprop (from app market) it appears at some point I may have flashed GSM radios. I say this because there are GSM listings from the getprop output.
Any suggestions?

I'd probably return everything to stock first. ##XXXXXXX# and *#*#XXXXXX#*#* codes depend a device's factory ROM.
I'm not convinced it's your MEID though. That alone wouldn't stick your phone without signal and you'd still be able to flash a radio. I'd also be careful not to mess with IMEIs or MEIDs unless you're absolutely certain that's the problem. If you're hell bent on it or you're doing it for fun, not caring of you wreck it, look up MEID repair and read up before messing around.
It wouldn't hurt to try and install a new radio via CDMAWS or a similar program. Just don't change anything you're not familiar with. You can really screw things up with programs like that if you mess around.

Does he have the samsung drivers installed? If so does his computer recognize his phone when plugged in either adb or fastboot? Cause if he does then you can easily go back to stock 4.0.4 IMM76k using my link in above post
MEID definitely smoked. *#06# yields nothing. I have tried flashing back to stock IMM76K but cannot get past radio-cdma flash... it hangs. If I remove the radio-cdma image it stock flashes no problem... but I still don't have MEID when doing that. I believe I flashed GSM ROM or radios at some point. I say this because when in terminal emulator and type getprop, the output has several gsm entries. Verizon is sending me another SIM card. But before I put it in I believe I need to have the GSM stuff off the phone. I'm not quite sure how to do that.
My screen is cracked, Verizon and Samsung can do nothing for me. I'm on my own on this. I have two rumor touch (sprint) that work fine. I also have HTC EVO 4g (Sprint)... been looking at activating on Verizon, but have had no luck getting CDMA tools to recognize phones... even after wiping drivers and re-installing multiple times.
I'm stuck using a very basic Samsung Knack flip at the moment... Sucks big time. Cash is short so I'm trying to make do with what I have.
Still needing help guys and gals!

Do you have the box that came with device so can reapply meid? You'll need your msl in order to edit. If have a backup of original stock after you unlocked bootloader..that one might reapply your info. Otherwise you need that mls

I just started having this issue the other day as well (baseband/IMEI, etc all unknown). Phone was just sitting on my desk when I noticed it didn't have service. After being unable to flash the cdma radio, I am trying to get the phone back to stock. However, I am unable to flash the stock image because there is a version check for the cdma radio. Is there a way to flash the stock image without the cdma radio check?

Any luck with yours. I just started having the same problem. My phone freezes and I have no service. I am trying to return to stock using this GUIDE but my phone freezes whenever I try to Flash the CDMA Radio. If I skip that step it doesn't install the stock version and I'm stuck in Paranoid 3.2 and unable to return to the store in the current condition due to warranty issues. If I can make it stock I will go get a new one.

Just wanted to let others know that I was on a JRO03O rom, decided to return to stock 4.1.1 and must have screwed up something because after getting back up and running I had the same issues as OP. No baseband, IMEI, *#06# was blank, etc. I was about to embark down the path JediNinjafication linked to, but it looked extremely intimidating. Jaymazz's link to Vabeachfc3s reset tool worked like a champ, put me back at at 4.0.4 and all the radio stuff started working right away! Phone updated itself to 4.1.1 shortly thereafter, and it's still going strong.
Flashed my e4gt from a rooted ics to Jellyverse ROM.
Worked flawlessly, but now i need to reprogram my data settings with DFS CDMA Tool... But the damn 16 digit password doesnt work anymore !
Ive flashed this phone to page plus myself and 2010031619780721 worked as a 16 digit password to unlock the phone last time... now no such luck
any suggestions ?
I'm not certain about pageplus but I don't think you need to reprogram the phone through the utility that requires the password. What are you trying to do? Do you have data and the ability to make and receive phone calls?
Call and text are working, there is zero data.
I know when i got the phone all the data profiles and EVDO settings were set to us cellular settings from the previous owner. I initially had to change all the values to my new number and the verizon towers.
I believe these settings got changed when i installed the new GB27 modem for this ROM ?
I spent alot of time reading/learning about flashing phones and I have repeatedly read that flashing new roms to a phone will require to it to be reflashed to the service provider.

And that last may be correct. I wonder if you need a specific modem for pageplus. For instance, I have to flash GC01 modem for Boost on Jellybean because the regular Sprint modem doesn't work (even though you think it would since Boost is really Sprint's prepaid provider). Maybe try the one for virgin mobile I can't recall what modem corresponds to pageplus and I don't see one specific to that carrier. Flashing the modem alone should not wipe out any of the phones carrier info that is programmed in that I've ever seen but it is important for it to use data.

Also, not sure if this applies but it looks like you have to have the correct prl installed for the data to work properly too. I posted in this thread about that and the modem for JB on PagePlus to see if anyone knows for sure:

Ok, I got a reply.
Must have a Verizon PRL flashed to the phone.
This means, do not use the update profile & PRL option in the ROM or it may wipe out the PRL. He didn't specify which JB modem to use however.
Edit: You will need your MSL to update the prl on the phone and unless you already have it, I understand its not easy to retrieve on a non-Sprint phone if that is what you've got.

Im pretty convinced that either the data or evdo settings got changed, or I need to update the PRL again, like you said.
Flashing the ROM changed one of those settings. We will fix this together XDA !

In case anyone else runs into this...
solution: Sprint 16 digit password worked. The ROM must have been written for a sprint device? either way it works
