So.. i was installing RCMixHD Rom yesterday, i partitioned my card as follows (using Gparted in Ubuntu):
FAT32 14GB
EXT2 2GB
This Rom is awfully bad, it crashed few times and freezed that i couldnt even reboot so i had to remove battery when it was still running.
I decided going back to CM7 but i just realised that SD Card size is 12.84GB, and Gparted doesnt show any partitions on it, what the heck?
Use a card reader or connect the sd while in recovery and make sure all the partitions get mounted.
If android is booted the ext partition doesn't get mounted. Also if you created your ext partition while android was booted that probably messed up the ext partition and that's why the rom doesn't work right.
First of all a 16 GB cars is more likely 15 Gb in real life.
Moreover, the phone does not recognizes the ext partiton in the SD card menu (in settings), just the FAT32 one.
So 15GB-2GB-possibly some unpartitioned for data = 12.84 GB

Is that a class 10 card? i am trying to figure out which card should i get for my desire, after bad experience with kingston 16gb class 10.
Its Class 4 Sandisk SD card.
if you want to partition the card try ext 3 for a2sd+
Minitool Partition manager is the way to go to create ext partitions on a sd card
free too..
I've tested two sandisk class 4 SD cards.
If I format it to fat32 and partition the space(to use link2SD), my phone refuses to use the SD card and says it's not there. I use minitools for partition and the official SD card maker software for formating. I did everything by the book and tried multiple options(different labels, ntfs, ext2, etc.)
A fresh, unallocated space card works perfectly fine however.
My phone is rooted and unlocked bootloader running softMIUI rom with a modfied power saving blend kernel.

Was looking at your post and I decided to grab a few of my MicroSD card 1gb, 2gb, 4gb, 8gb and 16gb....
I formated all of them on windows .... right click format, all default settings... Fat32. These are MicroSD cards that have had information on them... they all run fin on my arc. All different brands.
This leads me to believe that there's possibly either something wrong with the MsD card or the reader on the phone.

Solved it
Did this:
Changed the partition id to this:
"0*06 fat16,greater than 32 mb"
and made the second partition fat32 as well.
Removed the 100mb linux swap file partition I created....not sure if that was necessary.
I'm running the Blazer rom with the Jellybomb domination theme. I have a 16GB SanDisk class 2 microSD card. I formated it, it erased everything etc... but as soon as it's done it still says SD card damaged, may need to re-format. What am I doing wrong, or is there a fix for this?

so lemme get this right u wanna format ur ext sd?
Well, perhaps the card is damaged?? Or is one of those fake knockoffs from China.
What I'd try first, is to put it in a PC.. use a normal Format on it (not quickformat)
See what happens... if the PC can format it, then go to step 2 below.. if not it's damaged/fake for sure.
Step 2.. Download one of the many SD card testing software I'd use H2testw.
If everything comes back fine.. then you should be able to pop it into your phone without an issue.. just be sure to format in Fat32 for the best compatibility.
Also, 16gig Class 2 is pretty much the bottom of the barrel card wise.. you can get 32gig class 6 or better for under $20 online at most retailers.. I've seen 32g Class 10 for 30-40, and just bought myself a 64 class 10 from amazon for 55 shipped.
Might be time to upgrade that card
Recently changed to a 32gb class 10 micro sd the other day and noticed in the latest TWRP that the option to format isn't there any more. Haven't partitioned a memory card since my og evo and was curious if things changed.
twrp format
twrp and cwm wont recognize memory cards in exfat format. it will only recognize ones in fat32 format. I used a program to convert my 64gb card to fat 32. There is a post on android central with link to the program. Its free too. Hope this helps.

This Is a Class 10 Card.
The 4th reviewer was only stating his opinion. Every Reviewer (111 of 112) received a Class 10.
And the 4th reviewer most likely also received a Class 10.
I'm actually using this card right now. Ordered it Saturday, got it Wednesday. Works like a champ.

What ROM are you using? Did you format the card?
senergy rom, and yes i formated it. i also transferd from old card to new card and still a no go.
went into setting /then to storage and tried to mount it that way also. it might be bad.

I can't see pics at work if you uploaded a pic with that, but does your computer read it? Does your phone say its corrupt? If everything says it's corrupt you can probably go to control panel -> hardware and do a full format (25/30 minutes). If nothing recognizes it at all, it's bad and you should request a replacement/refund.

Working Perfectly
This is what I did:
- I removed my SanDisk 32GB Class 6 card from my S3 after 'Unmounting' it.
- I saved All data on the card to a Folder on my pc
*the 32GB card is FAT32
- I installed the 64GB card into my S3, Formatted it, and Removed it after 'Unmounting' the card.
I transferrerd All data onto my New 64GB Card via my pc
- I installed the 64GB card into my S3 and Everything is Working Fine
*the 64GB card is exFAT
The 64GB card had 59.47GB Usable memory beforing transferring my folders over to it.

Your card arrived in the cardboard/blister packaging?
Don't know what the odds are of getting a defective card, taking into consideration the Seller, packaging, etc. But hopefully your replacement card will have no issue.
Have you tried to install the card into another device, if one is available?

You can but you can't format it using the Windows formatter. I used easeus software to format mine so that the card would be visible in clockwork recovery. exFAT is not supported there. Had a bit of a scare after formatting with the HP utility. Repartitioned in easeus but it would only read on the computer and it would FC system apps on my phone. Figured out that the partition needs to be primary not logical (what easus defaults to for new partitions) to work on the phone. All you have to do is pop the card in the computer, open easeus and format fat32. Don't mess with partitions

I didn't realize that Clockwork didn't read exFAT.
I just did a backup to my Internal storage cause as you say, CWM didn't see the ext. SD card.
I'm gonna reformat the card now.

I'm going to do the following:
- format 64GB card in my S3, which will delete all data
- install Easeus onto my pc
- put 64GB card into my pc
- open Easeus Utility, and format 64GB card to FAT32.
- reinstall Folders previously saved on my pc from my 32GB FAT32 card to the newly formatted 64GB card
- install card into my S3, and I should be good to go?
UPDATE:
- Formatted 64GB card to FAT32
- CWM recognizes card
