then when i went to install from the SD card for juggernaut rom, it gives me error ever since.. it says (bad)
my computer can't read my phone anymore,
phone stops at the booting screen,
factory reset isn't helping either...
any suggestions? anyone?

When you are in recovery go to mounts and then mount usb storage. Then connect your phone to your pc. Try downloading another rom and transferring it over. (this would require an external sdcard)
Can you get in to download mode? If so just use the stock Oden and go back to stock. Then flash clockwork mod recovery via Odin, flash whatever rom you want to use. For what it's worth I am very happy with macnut

If you can get into recovery I would suggest reflashing whatever rom it is you want, i usually format everything except /sdcard whenever I flash a new rom to ensure theres nothing leftover (some might argue this but its never served me wrong), then follow the instructions the OP posted in the rom you want to flash. It sounds like a bad flash to me anyway.

As suggested above, try download mode (holding volume up and down while plugging in USB) and flashing through Odin. As to what's giving you the error, it could be a bad download (always check MD5 sums once its on your phone!), or if it was giving an MD5 sum mismatch then that usually is a result of renaming the file on your phone. Anything that you flash cannot have any spaces in it, otherwise it won't read. That goes for nandroid backups as well.

My current problem is that certain applications, not all, will dissappear after reboot if I move them to the SD Card. These applications continue to work and remain after reboot if I leave them on the phone.
The sequence of events involved a twlauncher forceclose that kept me from using my phone at all. I could do nothing other than boot into Clockwork mod. This happened after my work's Exchange Server certificate expired. Previously I had problems with a forceclose that I used the wipe cache, wipe dalvik cache and fix permissions to correct. I tried wipe cache, wipe dalvik cache and fix permissions several times to try to fix this, but it did not work.
I restored to a previous point on my phone. I had several back up points on my phone: an older one and a new one.
I had done several advanced restores: data, system, boot, to try to get the correct system to work and also to get the most up to date data. At one point, after I restored the boot data from my most recent backup boot restore, all of the applications that were on my SD card dissappeared.
I reinstalled the applications, but when I rebooted the phone, certain ones dissappeared. The applications will work and remain on my phone if I keep them on the phone.
After looking at the forums it seems I could have a problem with the SD card or the file system. The phone seems to work fine otherwise. Although, after I upgraded to CWM 5 and installed ROM manager, I got an error message regarding the first time I restored to an older backup point.
Phone configuration:
SPH-D700
Firmware: 2.3.5
Kernel: 2.6.35.7.nubernel_v0.0.2
Gingerbread.EI22
Clockwork Mod 5 (I was on Clockwork Mod 4 when I did all of the above; after I restored the phone to its current state, I upgraded to clockwork mod 5)
BTW, I would also like to upgrade to Gingerbread EL30, but it appears that I cannot simply flash the new ROM to my phone without restoring to stock ROM to upgrade.
First you can simply flash an FC09 ROM (latest). It is the modem that you need odin for.
Second: back up your sd card to a computer and reformat and restore.

Root the phone if you prefer to have it that way. All your issues sounded like data corruption anyhow.
Use Odin to flash the CWM tar file without reboot option checked, battery pull after flash, boot into recovery and flash a FC09 kernel from the dev forum. Lastly you'll just need to install Superuser from the Play Store.
That, or fkash the FC09 prerooted deodexed stock ROM in the dev section.

Prior to receiving my GS2 from T-Mobile I did research on how to Root, Flash ROMs, you know, all the cool things Android phones can do, and I mean A LOT of research. When I finally did recieve my phone the first thing I did was root it, which worked flawlessly - at which point I thought "Hey, this isn't so hard I won't have any problems at all!" OH BOY WAS I WRONG.
Here's where everything went wrong: The first ROM I thought I'd install was AOKP Milestone and then I'd try CM10, so I put both .Zip files into my SD card. I backed up to Titanium Backup and to my External SD card through Clockwork mod then I installed AOKP Milestone and made it to the home screen with some force close messages. It was running, but didn't look very stable to me and the whole "Swagger" thing turned me off to it. I went back to Clockwork Mod and the website that I got CM10 from also told me to run DarkSide Wipe before installing the ROM so I did just that. Ran Darkside, installed CM10, then installed Gapps. I then rebooted my phone and CM10 started to load up, but after 10 minutes of just the splash screen I assumed it was stuck in a bootloop. I then restored the backup I made before installing Milestone, but I remember there being an error and it not being successful. It was at this point that Clockwork Mod started being buggy. Starting to panic, I tried a factory reset through CWM. It formatted data no problem, but it would hang at "formatting /cache" until I turned off the phone. So I panicked some more and formatted system, emmc, data. (Not a smart idea, I know.) but it would hang when I tried formatting the cache again. And its been stuck in a bootloop ever since.
I read somewhere that you could use Odin to revert your phone to make it completely stock by flashing a kernel. I thought my prayers were answered! I was finally going to just start over and forget this ever happened! No. My computer won't even recognize my phone is plugged into it. (Yes, I have all drivers up to date. Yes, I am using Samsung Micro USB, and Yes, I have Keis installed.) Since my computer won't recognize my Phone, Odin won't.
* I can access Clockwork mod, and Download mode, but my computer won't recognize my phone when plugged in.*
I know it's a lot to read but I have tried so many things I'm just so discouraged about this whole thing. I know I'm a noob so please explain your answers like I'm 5. I don't understand most Android modding slang.

If you have an external sd card, load a rom on to it from your computer and then get your phone into recovery mode and choose to install from external sd card. See if that works. Also sometimes a cache wipe can take a while. Also don't panic and start doing things you know nothing about. I would recommend and ice based rom then figure how to get twrp recovery then move on to cm10. . If you can get past this, I can help you. Pm me

EDIT: The funny thing is, when my phone is in CWM my computer recognizes it and I can mount USB storage, but won't recognize it while in download mode. Weird.
don't use superwipe with cwm 6+. it will break your recovery and cause a bootloop.
also it seems that a lot of people have problems with cwm 6.0.1.9 and it seems like it doesn't wipe properly so i would recommend latest twrp or sk8terwitskil's cwm. i think that's what happened with aokp and why you were having so much fcs.

Battery pull didn't work either. :/
UPDATE: I've tried installing 3 different ICS ROMs from the external SD via CWM but they all seem to freeze 3/4 through the install. After about 5 mins of being frozen the phone reboots and the bootloop just comes back. Once again I have nowhere to go. I'm lost.
If anyone has more suggestions, please share!
i don't know where i read it but i just remember now that someone said kies and odin are incompatible. so maybe that's why odin couldn't detect your phone? kies might have been running in the bg.
so i would uninstall kies and driver. restart comp. then download and install just the driver from samsung website. then put the phone into download mode and connect to pc. see if that will help.
if not, i don't know lol. usually i would suggest flashing to twrp but i don't know how well that's gonna go over a broken recovery.
I would suggest flashing TWRP recovery 2.3.3 and try and flash a ROM again after....CWM has gone downhill in my opinion.

You need to have computer install odin driver while you're in download mode before odin works. Try this wipe cache, wipe dalvik, wipe data/factory reset, format system, format cache and format system. Then flash new rom in recovery.

Funny thing is, I just plugged in my phone to a computer running Windows 7 and Odin recognized it! I guess my XP at home doesn't like phones. From there I flashed a stock kernel and VIOLA! Phone is out of the box new! Now I'm very weary about installing a ROM again.. I don't want to go through that ever again. Maybe i'll do some more research before I molest the software's insides.

Try recovery from sk8erwitskil, it's the only one that seems to work w/ my Hercules. I had similar problems to what you were experiencing and it's the only recovery that worked for some reason. I am on a GNex now so take that for what it's worth. I'll pull out the Herc. tonight when I get home to update to the CM10 latest nightly, it's been a while.

I was having trouble with a previous ROM that I was using (Paranoid Android 4.2) after several months I decided to reflash...
With a pretty full internal SD so I decided to delete some of my old files etc. I picked a new ROM (JediMindTricks 4.2), updated my CWM/Rom Manager, wiped with SuperWipe, Installed the rom, and now I am stuck in a real jam....
Of course I am boot looping and as a result of that I am trying to revert to a backup, however CWM freezes almost instantly when trying to do anything. Whether it's wiping, reinstalling, or restoring a back-up it freezes. So with a similar previous issue I decided at this point I need to revert to stock ICS using ODIN. Inside ODIN (which I have never had any issues with) when I put the phone in download mode ODIN never recognizes it and will not allow me to revert to stock...
At this point I seem to be out of options.... What do you guys think?

Did you try to boot in recovery and install CWM from internal SD? Wipe data / factory reset and wipe cache in recovery mode... I'm not an expert but it won't hurt to try!

Unfortunately it is completely unresponsive when attempting to install any version of CWM.zip that I try to install in recovery.

Yesterday I had trouble with a bad upgrade through Kies and my cell kept telling me to plug it with Kies and do an emergency recovery. Nothing worked as the emergency recover didn't work with my cellphone and I wasn't able to boot in recovery nor download mode. I had to plug it in another computer and re-ODIN an earlier version of my stock rom... Not sure if it will helps you...
Plug in your phone to your computer, open kies, select "emergency firmware recovery", enter serial number from phone (write down beforehand) then cross your fingers and hope for the best. Good luck.
Also, be careful with those wiping scripts.

After literally wrestling with this for the last 7 hours ODIN has finally recognized it...
You DONT super wipe with the latest cmw recoveries. Superwipe was made for the early 5.0.x.x version, it breaks the new recoveries.
So i reset my GS3 yesterday. I am running a stock rooted phone.. My phone has always been fine but after a reset through the phone the phone never loaded back up. It was stuck on the samsung screen. I tried pulling the battery out a few times. I went into my CWM recovery and tried wiping both caches with no luck. I tried restoring the phone and that didn't work. I finally tried wiping all data and now the phone won't boot at all. It says phone can not boot and will go into odin mode. Odin will recognize my phone though. What should i do? Should I try to restore my phone to factory through odin? I can't find the stock sprint version tar file. I could only find the verizon one. Can someone help me and tell me what is the best step for me? I can get into CWM but the phone won't boot.

If you can get into recovery mode, you can just restore a nandroid backup (if you have one made). If you don't have one, you can wipe /system, /data, and the caches and reflash your Rom (provided you have it saved on your SD card).
If none of the above work, download mode is your best friend.
You can get into Download (Odin) Mode. That's a good thing. Sextape has the md4 TAR file for your over in their thread (warning, outside link). They have the stock unrooted as well as stock rooted file. Since you have already wiped your data, you might as well flash the "Restore" version:

If you know how to use Odin, you can just downlaod the TAR file, but I recommend using their auto-extractor (the exe file), since it's well tested and there's less chance of user error.
All that being said, I'm curious to how your phone became "softbricked." Did you install any apps or mods recently that could've caused it? Custom Kernel?
It sounds like you eventually ended up wiping /system, which would be why you can't boot up now. I haven't heard of it automatically going into download mode because of that, but I haven't tested it on my phone, either.

You can also take the sd card out plug into a computer download a rom zip put it on the sd card pop it back into the phone and flash through the custom recovery which should be there.

Thanks for the help everyone. I guess I didn't soft brick the phone I just wiped it and there was no rom on it.
T989 without any SD card in it, the one I had died apparently new one on order. Went to flash the latest PA rom but kept getting error 7 using CWM. I updated my recovery to the latest TWRP and still will not flash. I tried updating to the newest radio UVMC6, but that wont flash. I am currently on CM10.1 nightly. My process for flashing is recovery, superwipe, flash, which is when the flash errors out and I re wipe and then I can flash my CM10 back on. So I can flash CM10 all day long but nothing else. I searched on here and found that the ATT version of this phone has this problem and you have to flash an AOKP and then flash whatever you want, other phones with similar problems say use ODIN and flash stock and then go from there. I am just looking for any advice on this situation. I could not find anything specific enough in my search to help me and I am hoping someone here can. Thanks in advance.
Note: I have also tried flashing CM10.1 RC5 and has the same problems.

For onw those superwipe scripts are not needed with twrp, u might be getting the error from that, are u trying to flash from internal SD card since ur external died? That might cause it but I don't think so, I would suggest trying a manual wipe of all partitions, data, system, cache, dalvik, 2x , then flash, if that doesn't work try using Odin, go back to stock and start over, that should fix it definetly

Thank you... I wasnt sure about superwipe on TWRP since I just put it on and came from CWM. I am trying to flash from the internal card and the new external card will be at my doorstep tomorrow so I will drop that in as well. I will drop back in once I get all this done and hopefully all goes well.

Ok so I got my new SD card. Tossed that in, just figured I would try flashing from the external, no luck there. Same issues.

Got CWM back on but now I cannot flash SuperUser
When I go to flash Superuser it says

Quick update... I am a moron... Did not realize that root is kept with this restore... Got Superuser from market and am good to go.
Installing TWRP now and will have an update on the flash soon.

I'm not sure what your overall problem is, but if you are going to install TWRP do not use Goo Manager or the img file. They have been messing up peoples phones. Odin the tar or try the zip from the TWRP thread.

Well that could be a problem... I just got GooManager to do what I wanted it to do before I saw this message. If I overwrite it with the tar will that be ok or am I better off starting this process over and Odin back to factory stock again?
Along with that after I got TWRP working through Goo Manager again I tried flashing PA again and I had the same error as I did in the first place.
Maybe my phone just hates anything but the nightly I had before...

If you can boot into TWRP, then they must have fixed the Goo problem.
It seems like you have tried most everything. Maybe you should Odin back to stock. If you are comfortable doing it, that is.
i went to flash with goo last night and it bricked the phone kind of, i had to ODIN and reflash the CWM again and bam the phone rebooted no problem

I did Odin back to stock before. I am still on stock gingerbread right now with root access and am getting the errors I have been. I followed the tutorials in the sticky thread that I posted before that got me back to stock.
Within TWRP I tried installing the newest Cyanogen Mod and got
E:Error executing updater binary in zip
Error flashing zip
When I try installing the PA rom I get the same error
Any thoughts? Would going back to stock through Odin and doing this again potentially fix it? or is that a shot in the dark?
Lucky for me I kept a backup saved on my PC from before I did anything here. So I am back to the CM 10.1 Nightly I started on for the time being.
Is there any other method of flashing a rom that I might be able to try?

It would be a shot in the dark. I did a Google search for your error and a couple of people said they were able to fix it by using TWRP 2.3. That was for different phones though. Maybe try that before going stock.
I am by no means a pro at this and am still learning as I go. Just to make sure of what I am doing, can I Odin TWRP 2.3? I also found this

Just a thought here... I keep gettin emmc errors... I was reading about upgrading and downgrading TWRP through the Terminal Emulator and it said use emmc or sdcard depending on what the phone reads it as.
When I first got the phone i recall seeing emmc when i would go into any file browser. Now it just says sdcard0 (internal) & sdcard1 (external)
So how can I get recovery to stop looking for emmc and start looking for sdcard0?
Unless I am way off in left field here with this.

Yes, use Odin and the 2.3.3.0 tar from the techerrata website. I like using the Odin method because I have never had a problem upgrading recovery that way. I don't know about the emmc errors because I have only used the dd method like twice.
Edit: Maybe your internal memory is corrupted? I'm not sure how to test for that though.

I think u must be getting bad DL then because none of that sounds right lol, if u got twrp to load into your device and all is working fine then the only other option is a bad DL, try checking the DL with a root browser or something to see the size of the file, status 7 errors are from the zip, usually bad install script in the ROM, which could be caused by a bad DL, check the DL size and let me know

If the DL is good, I don't know what else to tell u that is very strange indeed, but interesting too, I've never not been able to install at all, I'm curious..
