Its driving me crazy! I open a book and it immediately starts reading it out loud.
I press "stop reading aloud" and it turns off, but when I exit the Books app and get back in its reading aloud again!
I have Talkback turned off in System settings and don't know what else to do...

I have this problem too and find it annoying.
Do you use the Light Flow app to control the front LED? I use this app, and I've read the accessibility access it has (seen in Settings-> Accessibility) seems to cause this bug for whatever reason. I haven't tried to reach out to the app developer for comment so I don't know if he's aware or fixing it.
EDIT: I sent an email to the developer to see if he is aware of the issue...
i use google books to read lately, but i didnt even know it had an audio read out loud function. so no i am not having any issues. i just open books and start reading normally with no audio strangeness...

Wow, you're the man!!! Thanks for the heads up and for looking into this and contacting the Light Flow developer.
I do indeed use Light Flow - my phone was set to "not allowed" for web scripts as well...I selected it again as you did and will report back after I get home from work if it stops reading out loud.

Yeah my fingers are crossed too. Keep in mind that I think it takes a reboot for the problem to re-occur anyway...
Well, I starred this bug report on Google Code, and followed one of their suggestions - I disabled light flow in the accessibility settings (and light flow still works), and this stopped the 'read aloud' from happening. Not sure why enabling light flow in accessibility is necessary in the first place but glad it seems to have fixed things.

I am really really happy that the galaxy nexus has a notification light. That was the thing preventing me from moving on from my nexus one.
Of course I wish Google would make the notification light more customizable because as it stands its really worthless by default.
I'm using light flow to help but its not perfect. Maybe its just not ready for ics yet or I'm doing something wrong but these r the problems I'm encountering:
In Google talk when I type a message and the screen goes off, the notification light comes on as if the person has responded but its being triggered by my own message. The only way I've been able to help with this is to hit menu>end chat after typing my message.

Thirdly it doesn't seem to stop the sounds from the notifications in the night time mode...it turns off the light, but even though in that option I have it set to do the sound as well, it doesn't do it. I had to go into the sound settings and just turn off the notification sound (which actually didn't help, I had to go into each apps settings and turn it off there).

Personally, I have the paid app and it has been hit and miss for me on ICS... sometimes I can't get the light to go away unless I FC the app. I uninstalled it for now until the niggles get smoothed out.

I was having same problems - light won`t stop flashing even after I checked received sms or missed phone call. Had to uninstall app But once its fixed, I will be first to install it - fantastic!
Anyone else know if there's any other app that can configure the lights?
I've bought a few over the years for my other devices but haven't tried any for Galaxy Nexus yet.
I've noticed though in Google Voice, when I get a text, the light is green.
I used light flow for a bit. but killed at after a day or so. Its still a little buggy in ICS as others have said
i had the proble with the light not turning off too. I had to turn off the notification light for no signal and low battery, and its been perfect since then.
I guess knock on wood, but I have never had a problem with the light staying on with Lightflow paid version.. I feel like if you swipe to dismiss a text message, it will stay on, but if you just open the Messaging app, it will stop blinking. Also, if you use the "x" on the notifications window, it will stop blinking without opening Messaging.

With regards to the above post, I have never had the "no signal" light on, but I do have "low battery" and it works fine.
Strange, my problem is actually the opposite to you guys. With light flow my notifications clear just fine, every time. However, it seems like about half the time it refuses to notify me at all. Drains a bit of battery too.

I'm working on improvements for ICS. There's a few things I'm working on for a release in the next few days.
Firstly, at the moment gtalk just doesn't work correctly for identifying notifications due to changes in the gtalk app. I've pretty much rewritten the notification detection for gtalk now and it seems to be working fine in the test builds.
There's a bug I've fixed where for some people the app won't work after a reboot.
As for swiping away notificatons, that won't clear them. It's really frustrating that the accessibility service is so limited in this regard and won't report anything useful back on what's happened. Because of this, I'm adding a new option to clear notifications additionally when you pull down the notification bar as this is something I've managed to hook into. Therefore it'll clear when you either open the app, or pull the notification bar down (but will then be off even if you don't actually view the notification)
I've had a few crash reports (thanks to those who send them in) that I've been trying to fix also, but some of these can take a few iterations to completely stop.
As for battery use, after the next market release adding the above, I'm going to try and add a new feature to just display the most recent notification that's come in. Although it wouldn't be my personal choice, it'll use less battery than the notifications where it's changing in succession around all the notification colors.

As for the sounds not stopping, have you set the native app to "silent" for it's notifications. Just want to see if it could actually be the main app that's triggering the sound.

The paid version of the app also supports google voice.
I'm having a really bad issue where once the screen is off, I can't get it to turn back on. The only way to get it on is to reboot the phone via ADB. Then, once I turn it off, it stays off again. If I press the power button SOMETIMES it will flash my lock screen for a 10th of a second, but then turn back on. This started when I installed the app. I was able to test the LED colors, and it did that during the test, too. Hopefully it's an easy fix!

i got this problem too, turn off foreground, uninstall, install, turn off foreground, turn on. You should be good to go then. App is a bit buggy, but better than what google is currently offering us.

The good news though is that I can get my phone to work again by entering an ADB shell, entering "su", running a "ps" to list the running processes, then running "kill (proc number here)" to end the process, allowing me to turn my screen back on.
EDIT: Ok, so after killing the process using the method listed above, I re-entered the app, turned the "run in foreground" off, then turned it back on, then rebooted. Phone seems to be ok now.

hmmm i have the app running almost perfectly on my phone right now too...thats what I did...are you rooted running stock 4.0.2?

You can also pull the battery if you want. There are several of us that have had this issue with light flow. The developer has stated that he is working on a fix. I would suggest removing the app until there is a confirmed solution.

Yea, battery pull works too, but considering how finicky the battery cover of the GN is, plus the case i have, it's easier to adb reboot or kill the process.
See my above edit, though. I seem to have gotten it to work.

I stand corrected; app description says root is not needed. I was thinking of another app I saw that does similar functions that requires root.
Hey guys. Im having a problem with Light Flow. Im sure a lot of you use it? Although i couldnt find many threads on it. I even searched Google for this issue. So hopefully you guys can help.
Anyways im using Light Flow 3.0.2
And it works great but a lot of the time ill get a notificiation (screen will be off) the LED will flash its appropreate color. Ill check the notification (Lets say Gmail) read the email, delete the email, respond to the email w/e and turn the screen off again.
Then the LED will still be flashing for that notification even though i checked it and erased it.
The only way i have found to fix this other then reboot ofcourse is to go into Light Flow > Notifications. I notice when i do this LightFlow gets granted Superuser then i exit turn screen off and all is good.
Any ideas?

Did you make sure you don't have any other app notifications that have the same color?
Are you using any task killers or killing the app with the hold of back button?
Same issue here, disabled it until it gets an update to support JB.

for me
when removed the app it disabled the vibration for new sms, gmail ..etc
tried everything without success (disabled settings & re-enable it, rebooting, ...etc), so I didn't bother to install it again after a factory reset (stock ICS by the way)
if anyone had the same problem and knew how to correct it let me know.

There is a lightflow widget that clears all lightflow notifications when tapped.
I am just using that while waiting for a fix (which couldn't come soon enough).

So i found out a little more.
So for me before 4.1.1 SMS was my biggest issue that notification would never go away unless i manually cleared it with the app or the widget.
I noticed that is was working on 4.1.1 after i went to accessibility and turned it off and then on again for Lightflow. after i did this i noticed that the notification for SMS was clearing on its own. So i thought it was all good.
UNTIL
I reset/tuned off the phone and received another txt. The notification would not clear on its own. So i was messing around with it and went back to accessibility and turned it off and on again and now the SMS notification clears itself until the next reset of the phone.
Hope it works for you guys. I dont mind toggling a setting until an update comes out.
lightflow issues not fixed on 4.1.1 derp :silly:
Gtalk and Gmail both clear correctly. Gonna see how it is with SMS and Missed Call when I get one of those.

I just downloaded the lite to try it. I feel like its hit or miss. Too much customization and easy to get lost. For example, right now, I have two different notifications going off for like SMS and email. Because within the SMS and email app themselves I've defined a notification sound and then within light flow settings you can define a sound. So I get x2. I think the trick may be to go into the app settings themselves and set them to silent. Then use light flow and set the sounds up. That seems to be the KEY. However, there is a learning curve it seems to this APP. Once you set it up, your good...but dam it takes awhile.
Colors are working good though, but the dismissal of notification via "opening the app" dont work worth crap. You have to set it to "on screen" dismissal
Plus I found myself disabling and re-nabling the app from withing settings --> Accessibility often.

No, I was fine on 4.0.4 but when i went to 4.1 it stopped working, I'm now on 4.1.1 and it's still not working.
It is a fantastic app though, I'm sure the developer will update the app eventually to work with 4.1.1.
So clearly I have done something. Whenever I open the stock email client and hover with the spen it reads the email out loud. Help please.

I've heard light flow actually messes up accessibility settings so that may have something to do with it. It may have something to do with samsung's software rather than lightflow though as many devs have run into this hurdle and have already contacted samsung about it.

You are correct I see that Andrew has quiet a bit of detail on this issue with Talkback randomly kicking off. It appears to be a Samsung issue that is effecting other apps.
There's an issue with samsung phones and 3rd party Accessibility options. LightFlow installs an Accessibility option, as does Tasker and a few other apps. When those 3rd party Accessibility options are turned on, it cases the "talkback" to come on in certain situations (stock touchwiz launcher opening folders is another example) , and also other weird things like disable the "flow" input on the stock Samsung Keyboard. Devs have complained about this to Samsung, and we can only hope they decide to cooperate. I opened a ticket with Samsung's "customer service" and their response was basically that they "don't support 3rd party apps" , despite that it works just fine on any other android device.
For me, I was able to stop this horribly annoying behaviour by disabling "Custom Notifier plugin" under the accessibility section (as talk back was already disabled). This Custom Notifier is for the Sony Mn800 live view watch... it basically takes all notifications and pumps them out to the sony live view. Turning it off solved the problem.

Tried Google Now voice search, and while it has no problem recognizing my speech and giving me answers, it will not speak back to me. I have language set to English and Speech Output set to Always Speak. Apps volume is set all the way up too. Will anyone here have any suggestions to solve this issue? Thank you in advance.

I guess it depends on what u asked I tried a few things. Only got the webpages without a response. I said "talk to me" and Google voice talked to me. Try it and see if that works for u.

Thanks. At what point did you say "talk to me"? My wife's Galaxy S2 talks back with any simple question (like "what is the capital of Illinois?") The OGP just shows the answer and the map. I set Google Now to always speak back (and the app volume is up), so I am trying to figure out why it's not working. It's a bummer.

I don't know excatly. My guess is that it might has to do with lg software not fully working with all the Google now features? And I said it like after 3 tries.

You have to make sure your phone's sound is on. It will not work in Vibrate Mode, even if you have app sounds on and Speech Output set to Always Speak. Just tested and it worked with ringer turned on, but not when set to Vibrate.
Phone sound is on. Google Now speech output is set to Always Speak. Media volume is all the way up. Still, zippo, nada.
Worst part. My wife keeps on egging me that her two-year old S2 does it, and my "big and fancy-schmancy" G Pro does not.
You need to turn the System Volume up, not media volume.

System volume is on.

Just in case someone else runs into this issue.
My alarm (default app) would get snoozed immideately after going off, thus becoming useless.
I have discovered that if I disable the "OK Google" detection from any screen option, the alarm functions fine.
Hope it helps someone not to miss any important appointments.
The ok google from any screen feature also kills the video camera. You get a "can not record while on a call" warning.

Glad I happened to see this post ... For the past week I've been trying to figure out what was automatically dismissing my alarm ... Uninstalled and reinstalled so many apps and xposed modules and never figured it out. Thanks!

Running in to a similar issue. But instead of the alarm snoozing. I get a pop up message stating "Alarm/Clock has stopped working" and then a flashing notification light. The problem started just this week. I uninstalled the latest Google update and also turned off OK Google" detection from any screen, but that doesn't seem to have had any impact. Odd that I've had no problems with Google Now up until this past Tuesday.

I was unable to replicate your issue. I only get the "Snoozed" notification.
However, I moved to Timely alarm app since and it works just fine.
Thank you for the response. I was hoping I'd be able to figure out what's the issue is and continue to use the stock app. But looks like I'll have look into a third-party app.

The recent Google Search update seems to have fixed my issue.
