What i am trying to figure out if what causes the bug and whether other people are having the same issue
As you can see from both images, there is no cell signal but WIFI is on but somehow the phone thinks airplane mode has been turned on
This is the 3rd time is past few days and i have missed loads of calls as a result.
From what i can see from my apps, there is nothing that should be causing the issue.
Also, there doesn't seem to a root cause, It has happened while i was charging the phone and while the phone had been on constant use
Any ideas?

I'm usually in low signal areas and when I move into an area where I know there is signal, it sometimes takes the device 30-60 seconds to recognize there is a signal.
This makes me think the radio polling interval is set fairly slow, perhaps in an attempt to save battery life.
I've used the airplane mode to toggle the radio and "wake it up" quicker than waiting the 60 seconds for the radio/phone to recognize the signal.
By any chance, are you in or around a low signal area when this occurs? I wonder if our two experiences might be related.
Just a thought. Have you flashed a different radio at all? Maybe its crashing?
Not that this helps you, but the one thing I noticed from your pics is they spelled "aeroplane mode" wrong... lol
Why is your Android Build #?
I had this happen once before, but it was before I upgraded.. so not sure if that is related or was just a coincidence that it has worked fine the times I used it after updating.

Not 100% sure but this happens a lot while i am at home and the signal ain't too great. You might have a point though.
I just don't know why it won't let me switch bad.

This happens on my Galaxy Nexus too. I can leave the phone somewhere with a very good 3G signal and come back to find no signal and that I'm unable to toggle the radio on or off. Restarting fixes the problem.

This happens to me as well. Airplane mode that doesn't turn off and slow reception, sometimes signals just goes away and doesn't come back for some minutes!!! I hope they fix those issues soon.

I just did a factory reset and haven't reinstalled any apps. I'll see if the issue reoccurs.

I have had a similar issue, except that I am able to enable connection again by selecting to turn airplane mode off. I sometimes have to do this 2 times, but the phone does not freeze during this process. I have not been able to determine a cause as it happens in use and idle. This happened using stock 2.3.4 Samsung ROM (with being rooted) and with a recent flash to VillainROM 2.5. Through that I would assume it is deeply embedded bug that is being carried over with ROM's or, perhaps more likely, one of the apps I got is causing this.

well, my nexus experience just keeps getting weirder. the phone just restarted itself and hasn't gone past the google logo. it restarts (hear it vibrate) at that point and wont go past that screen. no idea what could have caused it as phone was right next to me and i wasn't using it
past the point of caring to be honest. i will try a factory reset when i get home tonight

I've been using it for the past 8 hours and have had a consistent 4-5 bars. It's supposed to be a later build of the ITL41F firmware.
Unforturnately, it's got compatibility issues with most of the mods. If you flash mods on top of this, you'll lose the the status bar and the 3 buttons.
Will continue to monitor and update.
I can confirm that the problem happens to me on a completely stock phone - no apps installed (just the ones included with the system), no root, no tampering whatsoever. It has to be a Android 4.0 and/or SGN problem.
Here another victim of the sudden loss of signal. Toggling airplane mode on and off does solve the problem, but what worries me is that I am incommunicado in the period just prior to the moment I discover that the signal is lost.

I also recently encountered this issue. My phone was working fine while rooted and bootloader unlocked until today. Usually when I wake up, I check my email and I'm usually on 3G because I don't get 4G at home. Gmail had not refreshed, couldn't get on browser, had to reboot. I left my phone alone for about two hours or so, turned it on to check email and again, no signal in a place I typically get 4G. Had to reboot again to get signal. It doesn't look like my phone is going into airplane mode like some people are reporting as I don't see an airplane icon or notification.
I have not tried toggling airplane mode yet to get signal back. My phone does not run custom roms or themes, only has some root apps. I have the Verizon version.

Hey guys... as it says in the title, I am having issues with my G-nex randomly losing connection/signal but then coming back again. From what I have read, this is a common problem... anyone have any tips? Also wanting to know if I flash a custom ROM if that would possibly fix the problem. Thanks!
Not necessarily the ROM, but the radios. Try that.
There are a few 4.0.4 ROMs for the LTE GNex out now you could try as well if you want to go the ROM route.

The signal issues seem to stop when the phone is awake though... :/ But I'm missing a lot of phone calls since my phone is "off" when people try to call me.

yeah, that's exactly what's happening... it does seem to happen more when the phone is off, but I think it still happens when it is on. it is so weird! I went into the verizon store, one of the reps said that he had just been having this issue today and that he put it into airplane mode and back off again and it seemed to fix the problem... I did it, and it seems to have helped, but that was only about 20 min ago lol. see if it helps you!

Hmm.. interesting. The problem here is that this started on March 24th... :/ But, still trying Airplane mode, even though I'm not optimistic..
I emailed my provider, asking them for info, but a friend of mine has been working beside me for the past few days, on the same network with the same phone, without problem. So I guess it's time to bring it in..

I'm having a similar issue with my GSM model, It started when my phone was updated to 4.0.4. Phone signal suddenly drops and comes back again in less than 10 seconds. Sometimes it happens with 1 minute intervals between signal losses, sometimes 5 or 20 minutes, it's very random. After noticing the issue thanks to "Light Flow" that blinks the notification light in white when there is a signal loss, I installed this app (https://play.google.com/store/apps/details?id=com.smartandroidapps.missedcall&hl=en) to track it properly. The results are very frightening, I'm about to do a full wipe in hopes that the issue will be gone :|
ps: like you guys, I did notice that it's very most likelly to happen while the screen is off. Actually, it has never happened while the screen was on.

Hello, with IMM76D I hadn´t the common problem of losing signal when the screen going off. But, I experienced that sometimes, when I was using the phone, the signal goes off during 2 seconds or so (WIFI also).
So, I have experienced that IMM76I doesn´t resolve the problem.

IMM76D is not supposed to fix that problem. Only the screen off problem.
But it would be nice if they fixed both surely.

I never had the mobile network bug until today, but often since the 4.0.1 the wifi bug, that disconnects randomly when the phone locks (also if i selecter "always on" in the advanced settings). I still have this bug.
Today, with 4.0.4 IMM76I, when i was browsing the web, also the mobile network bug happened.
Pratically, when i was browsing (so NOT when the phone was locked) the phone completely lost signal (and so, 3G connection), and then it got it (signal) about 10 seconds later.
This has happened 2 times in about 15 minutes.
What i must think?

I've never had this issue until yesterday. I would lose reception completely after less than a one minute phone conversation. I reflashed my rom multiple times and nothing helped.. I finally restored my very first nandroid and the signal loss disappeared
Losing the wifi or 3g signal during browse the net and to get it back since 4.0.1 is not the same that the signal is gone during the phone sleeps.
Signal issue during sleep is from 4.0.4 only.
Please avoid making disturbance by misunderstanding and mix up different things.
I don't know if i'm the only one but..i noticed that the colors have improved in this last update(IMM76I).....

I still experience it as well
I noticed that it happens on mine only when I am using a Plantronics Bluetooth headset. Otherwise both 3G and Wi-Fi don't drop on IMM76I.

Nonetheless:
IMM76I was just a kernel. The kernel reverts the radio and voltage changes.
If folks really believe that IMM76I was not a fix, they could easily run an old 4.0.3 kernel. If it fixes it, then the problem is IMM76I, if it does not, then they are pointing their finger in the wrong direction.

I'll be going through the day, and then I'll go to scan something and I'll find out the hard way it's turned off. It's a pain to have to go into settings and turn it back on.
Looks like S-Beam might be turning itself off, too. I don't ever use that but it's off right now and I know I had it enabled at one point.

I know it turns itself off on my S 3 after there has been no activity for a while and it may also on the Note 2.
That's really frustrating, the thing I like about nfc is it's quick and easy. Is there any way to tell it to knock that off?

I've had the problem on the official TMO rom also. I've solved my problem by flashing the stock 7105 4.1.2 image on top (which also comes with some additional features such as NFC notification toggle). I used the stock image created by teshxx.

My wife has a Galaxy S3 and she DOES NOT seem to experience this annoyance. I have the Galaxy Note 2 and I am CONSTANTLY having to turn my S-Beam and NFC back on.
Has anyone figured out what exactly causes the auto-off of these features? My random guess, using the test case of my phone and my wife's, is that it has to do with turning your wifi on and off. I am constantly turning my wifi on and/or off depending if I'm in a known wifi area. My wife never touches the wifi setting, just always leaves it on.
Anyway, hope to hear from someone soon with a solution to this annoying little glitch in the use-case of these great features.

Having the same issue as well, using the stock ROM, rooted, I'd rather not have to flash another rom to my device, as everything else appears to be working just fine for me.

Well, for the moment ICS (any version) is making my phone unusable as a phone itself.
I saw some people complaining about the GSM hangups (including me), so if you let the phone sleep, you wont recieve any call. The caller will hear the same tones, as if you dont pick up the call. If you send an sms on a phone in such state, it will only appear after turning it on manually (often having duplicate SMS). When you turn the phone on - you wont even get a missed call, no signs of anything.
This problem is very hard to reproduce, it happens from time to time.
I've tried to flash any other ICS based rom on the phone - any has the same problem. The only rom i hadn't such issue is 2.3 stock/modified. But i want ICS!
Anyone have any thoughts, what can be causing this? I'm tired of this happening, people complain that the cant tell me something urgent, and i dont even know they wanted to.
UPD: There is a parial fix for this issue by Maruzko by installing an app that prevents deep sleep (but thats battery consuming offcourse).
I think that there should be some internal fix for this issue, only devs can help up with that.
UPD2: My opinion that problems that people have with clock not synced is somehow connected, coz when you turn on the phone in that state, you see the clock changing rapidly and stopping at wrong time, so changed title to include that also.
UPD3: There is an alternative way to fix the issue, but not much tested. Thanx to sideshow03.

Yup, me too. Currently at miui based on pac rom, havent noticed the issue, but cant be sure its really gone, or just "lucky time" until reboot.

I have the same 'No response to a call' issue after flashing AT&T UCLJ3 rooted on a Rogers i927R Glide.
Essentially, after the Glide has been idle for a while, not sure how long, at least 30 mins I think,
there is no response from the Glide when calling it, and it goes to voice mail, but not straight away though.
I can hear a ringing tone from the phone I'm calling from, but the Glide just sits there in sleep mode.
Try several calls but the Glide does not wake up.
Haven't tried sending it an SMS when this has happens though.
If I then wake up the Glide manually, and then put it into standby again and call it, it responds as normal and starts ringing.
The thing is, it doesn't happen every time, sometimes the phone could be idle for 2 hours and still respond to a call, others times it doesn't after say 30mins on standby.
Hard to troubleshoot as its so intermittent.
This did not seem to happen on stock Rogers GB.

Sad thing - cm and other jb roms higher than 4.0.4 have the issue aswell
Currently have the 312 min value with interactive governor - this does not help, some deep sleep problem not frequency related

It doesnt appear to be just a Glide problem with ICS from internet searching but there doesnt appear to be a solution anywhere.
I found a reference to using an App called Wake Lock which some have found may fix it but at the cost of some battery life.

Ive installed it but not activated it yet.
Still testing with WiFi sleep policy set to "Only when plugged in", where as previously I had it set to "Always".
Anyways, there are devices on ICS that do not have such issue. May this be connected only with the tegra2 platform? Even have no idea how is this issue called to search.. sleep of death? As i know, its the state, when device does not wake at all, even after pressing power. Not our case.

Good point, maybe it is to do with the Tegra 2 platform or the sub revision of this ICS 4.0.4.
Yes, 'sleep of death' seems something different, when ieven the power button does not wake it.
I think our issue is referred to as not coming out of 'deep sleep' when a call/sms is supposed to wake it up.
Yesterday I had WiFi sleep policy set to be on only when on charge.
So all day I was on 3G data not WiFI. I called the Glide intermittently through out the day and it always woke up.
Today going back to normal setting of WiFi Always on to stay connected to an access point when in sleep mode. Will see how that goes.

I'm running CM10.1 and not having this issue.
I did notice something though.
Hypothesis: Each ICS leak came with a different baseband. So there is an I927UCLG9 baseband, a I927UCLH2 baseband, and a I927UCLJ3 baseband. UCLJ3 introduced some ABI change that the libsec-ril we're using in AOSP ROMs doesn't know how to handle (or it straight up just has a bug).
I currently have the I927UCLH2 baseband as I never flashed UCLJ3.
Could someone more wizardlike than me whip up some baseband-only Odin images for testing?

"Sleep of death" already has an established meaning. It means there's an intermittent whole-system freeze that only happens when the device is in standby, giving a "dead" appearance. It's been the bane of Kindle Fire developers for some time now.
I'd call this "no radio wake" personally.

Nice term, tottaly agree.
I just can say, that I have tested the lite rom, based on 4.0.4, that has a baseband chooser in aroma installer. I've tried running it with a baseband from 2.3.6 stock rom (that was my only hope), that did not give any difference, radio still did freeze Anyway, I cant be sure I did everything right that time, and your way of solving this must be tested.
Also, my thoughts are about some ATT frequency specific things, as I cant say everybody here is complaining (most are from North America, right?). May this be caused by using glide outside of US and Canada?

That's quite possible, as I'm on AT&T towers even though I don't have service through them (I'm with a prepaid MVNO) and I'm not having problems.

When you turn the phone on - you wont even get a missed call, no signs of anything.
This problem is very hard to reproduce, it happens from time to time.
I've tried to flash any other ICS based rom on the phone - any has the same problem. The only rom i hadn't such issue is 2.3 stock/modified. But i want ICS!
Anyone have any thoughts, what can be causing this? I'm tired of this happening, people complain that the cant tell me something urgent, and i dont even know they wanted to.

I'm having this issue too ;(
My problem: randomly stumbled upon the notorious phantom menace radio signal antenna drop that plagues jelly bean owners. My particular case ; if i opted to turn off "data" to save power etc then every five minutes the antenna would drop for maybe thirty seconds then come back (all actions monitored via gs battery monitor app).
What I tried previous to my supposed "fix ":
-I'm running cross platform roms currently synergy, knowing that I figured it was my d2att recovery image which I later flashed, it fixed my build.prop but that was it
-flashed back to stock recovery... No go (fyi I've owned the phone since it was new back in August, ran jb for months with no issues until several weeks ago when this began
-flashed and wiped many roms several times over with no fix
-flashed every Att fix possible along with the pages of thread research that went with it
- worked about every angle of network settings I could've imagined
It wasn't until recently I had decided to disable the phones stock power saving switch when I realized my antenna was stable once again! Upon toggling it back on the problem will resume.
update... this hasn't fixed **** as of now. don't know what happened it was working fine now it's back to its same ****.smh....
the only choice I have is to leave data on :_(

have you tried a different modem? try downgrading or updating respectively

I recently discovered that this trick does work but it's intermittent. in order to stabilize the antenna I believe the connection had to of held a steady 4g signal for a brief period before killing power save them the data. but once that's accomplished its good until power is cycled off. it's weird, can anyone make sense of this?

that honestly sounds like a defective device. thats just my thoughts
I think so it's just odd that it went this long before acting up
