Both mine AND my girlfriends gs2 have this problem. When the phone is charging, the touchscreen is not reading the correct point of where I touch. Anyone else having this problem?
Yeah we all are. There is something wrong with the square adapter that came with the phone. Try plugging it into a PC to charge or using a different adapter. It works fine.
I noticed this as well, but only with non-OEM adapters.
And I thought it was only me. Get the same problem only with cheap chargers.
OEM, MT4G, Transformer, Touchpad, Galaxy Player, Ipad & iPhone chargers all do not experience the problem.

I don't and I also have this problem.
I believe its the square Samsung charging adapter.
Powered by the SGSII....
Ditto here, it behaves Ok when charged from my laptop, PC and Motorola charger (came with my Milestone), but definitely quirky when charged from the square little adapter that came with the SGS-T989D.

Same here, but it does change when I use another Samsung adapter like the one from my Vibrant it'll actually work once i use that one.

This is happening to me. I am using a Quikcharger for Samsung which I bought from Sears. Anyway to fix this? Because the OEM square charger + data cable is to short to use in bed.

it only happens with defective charging cables
how i know?... i just accidentally crippled a pin of my existing charger, and now even though it still charges, it causes all kind of random weird things in my phone.
so i simply stopped using it, and changed to another charging cable

So you are saying the OEM wire is defective? Has anyone used the cube with a different wire or vice versa?
I just bought this new charger from Sears and it still gives me a quirky screen

This is a known issue with the SGS2. Any inconsistencies or issues with the power supply via AC will cause the screen to tweak out. Possible causes are a bad cable, bad power source (outlet), or improperly inserted microUSB connector. If you try a number of times and cannot get the cable to connect properly, return it or get a new one.

it's not a SGS2 only issue, it happens to all the other phones i've owned before

When I charge phone the only thing that works is the power button and track ball....the screen is unresponsive to touch but as soon as I unplug it, everything is back to normal....can anyone help me...it is a rooted incredible

I recall reading about someone who had this issue with one (but not all) of their el-cheapo MonoPrice chargers. Maybe a grounding issue with the charger cable (beyond the grounding issue the phone had universally until 2.2 update). So, questions for you...
Have you tried more than one charger, and if so, does the phone behave this way on them all? If not, are you using a MonoPrice charger by any chance?
yep - are you using the OEM charger and cable? Mine works fine with I use the HTC charger, but not when I use another microusb charger I have.

Never thought off that, I'm traveling and had to get a cheap charger...when I get home I'll use my oem and see what happens, thanks guys...

My 2 cents ... I am on my 3rd incredible and have found all 3 do get slower while charging, and I guess you might say 'unresponsive' but 'delayed response intermittently' might be a better phrasing. I suspect that part of the issue is that the battery/phone gets warmer while the charging is going on, and if it gets to a heat issue, the phone may start clocking down a bit.
When my Mytouch 3g Slide is plugged in the touch screen goes crazy. It isn't accurate and starts clicking everything. Could it be my charger? 5.2DC - 500mA

I'm guessing that it is because of something along the lines of the electricity from the charger shorting to the touch screen, and since the touch screen works by using the electrical signals in your fingers, the electricity is screwing with the digitizer. I doubt its a charger issue, probably something within the phone. You could try using another charger. Does it happen when you plug your USB cable in?

Really? Anyone that does any troubleshooting knows that if problem is unknown that one must start eliminating possible problems.
@shefsalad: If the phone is fully functional before plugging it in, then the charger must be the issue. Now since the charger is two piece, it must be the cord or the charging unit. You had it right with asking to plug it into a computer. If it works fine plugged into a computer, the cable is fine and its the charging unit. If you'd like to further put suspicions to rest, see if you can borrow another persons charging unit or see if tmobile can lend you one for a minute.

Then there's a good chance it's the charging unit, like the other guy said, try to get another one, maybe a warranty replacement from tmobile. It would also be good to try a different unit as well as a different cable entirely, as the problem could still be with the usb cable. We want to narrow it down as much as possible.
@guitarist5122, no need to be so hostile, I was simply projecting a theory. Somehow, electricity is flowing over to the digitizer with the phone plugged into ac power. This is why I asked him if it happened with just the usb cable plugged into his computer. If it didn't, then its possible that either the charging unit is defective, giving the device too much electricity, which is resulting in it discharging in the digitizer or elsewhere, or it could also be that the wiring within the phone is slightly messed up, so that the small voltage of usb doesn't affect it, but full AC power overloads it. Now all he needs to do is try using a different AC unit to determine the problem. However, the problem could still lie in the usb cable, the problem could just not be emerging until full AC power is fed through it, so it would be ideal for him to try an entirely different charger, with a different unit and a different usb cable. This would eliminate all possible problems. So, I think my troubleshooting skills are just fine, thanks.

No hostility. Just pointing out that i see advice coming from little knowledge disguised as solid advice. Nothing wrong with advising people with what knowledge we do have, but mention that the advice is based on little knowledge and may be inaccurate.
I must point out the flawed advice. It simply cant be the cable since the cable just transfers electricity. In this instance, it could only be the cable if it had a bad connector (the standard A or micro B) like maybe a bad ground. If that was the case the problem would carry over to the pc.
Also, it has nothing to do with AC simply because the phone does not recieve AC, but rather DC. The charger converts AC to DC. DC is the same current coming from the laptop as well. If i actually think about it, i dont know a charger that supplys AC to a device (especially a chargable one).
I experienced this when I tried to plug my Motorola Milestone into my Dad's Blackberry 9700 charger. It charges just fine, but the screen goes wacky when I touch it. Other chargers such as the one from Motorola, or my Nexus One charger, did not have this problem. So it appeared to be some sort of odd grounding issue.
I also have this issue happening, but only with the cheap aftermarket charger. It also does it to my girl's motorola Cliq. It's simply the charger, get a new one if it bugs you, or unplug it when you want to use it.

Have the same problem with Desire HD
I'm using HTC charger and cable, it also happened when connecting to pc so I'm suspecting the cable.
I'll try a different charger and cable then I'll try the original charger with the new cable. The problem is that this issue is happening sporadically so it will be difficult to test.
Definitely the charger despite being the one that came with the phone

I bought a local manufactured phone. And when i plugged in as for the first charge it went crazy when trying to use the phone. But it does even when USB is plugged in and not switched on.v but works fine when cable not plugged in. And when i tried my brothers Samsung Galaxy Nexus charger it worked like a charm with no problem. It's just matter of hours(2/3hrs) after i bought it.
I've had the phone since Monday. Last night, now that we have a stock kernel, I went ahead and rooted my phone.
Today, on three occasions (requiring reboot) my phone had what I can only describe as a touchscreen freak-out. The image twitches and jumps around even when not being touched. When it is touched, it's entirely unusable. Apps not even close to your finger open and close with a quickness. I've already sent a few very fun swype text messages, as you might imagine.
I migrated from the OG Epic and never had any issue like this. Lack of responsiveness when dirty, but not total madness. Has anyone else had anything like this happen? Is it just a matter of needing to get a new phone?

It's a known issue. Some people claim to have good results with a different cable, some with a different charger. One theory is that the wall charger is over-rated and that extra juice is making it go wonky. USB charging doesn't seem to be affected by it. The effect seems to decrease the closer to 100% charge you get. It's a known problem on a wide variety of devices from a wide variety of manufacturers and aside from your cable and/or charger your own local power may have something to do with it. A software update can fix it as it has in the past on a wide variety of devices. Not everyone has this problem all the time. YMMV
I would suggest buying an OEM blackberry charger from Amazon for like 7 bucks. That's what I used on my epic and what I'm using now on my epic touch. The charger that came with this phone is still sitting in the box.

I use an OEM blackberry charger and still get the freakout
I've been using the charger that came with a bluetooth ear piece, and I have no issues whatsoever.
Although, I normally charge through USB.
I have a VZW Galaxy Nexus and it won't charge while it's on,but when I turn it off it charges. Also It will charge while powered on when its connected to my VZW branded car charger. Anyone know why this is?
In settings>battery does it show charging(USB) or charging(AC)?

I bought one micro usb cable that looked high quality but didn't charge at all when in use and charged very slowly when not in use.

Yeah I've tried two different cables. Haven't tried the stock one since I don't have it.
Although not all cables can carry data, I thought any cable should be able to charge?

Try on a different computer, maybe your computers USB ports are malfunctioning or the GNex overloads them when the phone is on?
Not sure what the problem is. I've tried two dfferent computers usb ports several different outlets and two different cables. The only time it truly charges is when I have it hooked up to my car charger
I'm having a similar problem with the GSM version. I had the original Galaxy S and had to replace it because over time it completely stopped charging no matter what cable I used. Is this an issue with Samsung phones? Now My Galaxy Nexus seems to only charge with certain cables and some cables that it originally worked with no longer charge it. I've tested the cables with other devices and they seem to work. I'm wondering if I should begin the lengthy process of shipping it off to the UK for warranty service. Anyone have a link to a tutorial on how to flash everything back to stock and relock the bootloader?

So today my Galaxy Nexus (GSM) has started doing the same as the OP. I did not charge at all last night on the charger. I turned it off. Took it downstairs to another charger and plugged it in and noticed that it was charging. I figured it just needed a reboot. I turned it back on and left if on the second charger and it quit charging. Now I've moved to a third charger and the same thing is happening....no charge except while turned off.
I don't think I have done anything to harm the phone--the only thing I've ever done to it is swap the battery to the larger version. I'll try to switch the battery back to the OEM when I get home from work to see if that makes a difference. In the mean time, does anyone have any ideas on this? The phone is only ~6 months old, would I be wrong to say that the warranty would take care of this? Thanks.

My SGS2 did this as well. I had to tweak the USB connector on my phone. I bent it slightly toward the screen and all was well, never had to mess with it again.

This is true, the outer pins on a USB are for charging, positive and negative. The inner two are data in, data out. At the very least, a cable will always have positive and negative, so all cables will charge unless there's a break in either positive or negative or both. If there's a break in data in or out, it should still charge but it won't communicate with a PC or something like that.
The issue OP is talking about sounds rom or kernel related. Nandroid > Try something else. Hope for the best.
I've noticed that on 4.1 my USB charging worked fine. Now that I am on 4.1.1 it still charges over USB, but at a decreased rate. In other words, before 4.1.1 it easily charged over night and now it will only charge to about 80%. Does anyone have any ideas why that might be?

I just did the same thing, and it worked. Gnex was only charging when it was off, now, it's fine. I had to do the same thing to my old Fascinate for the same issue.

Having the exact same problem, won't charge with the original adapter as well as spare adapters, also wont charge through USB. Car charger is the only thing charging it right now.

Could you explain further what you mean by "bent slightly toward the screen?" are you talking on the phone or on the cord? If on the cord, then the whole metal connector? Or just something inside.
I have the stock charger for my Nexus, and just recently it has stopped charging.
But i noticed that if i pushed up on the charger cord that it will charge..this is very annoying. What happened? Did the slot just get pushed out of place? If so, how do i fix this?

Yes I have.. it turns out I have one charger that for some reason works.. It's alot bigger than my other ones, but also a lot shorter.
I had the same problem with my captivate and currently my Gnex. I went out and bought a blackberry data transfer cable and it works without a problem. I wonder if Samsung charge cables short out or break quickly.

If you've tried a few different cables and chargers and still have the problem then you definitely should get a warranty replacement. I had the same problem and it got progressively worse to the point where I could hardly ever get it to charge. Something was wrong with the USB header because even if I unplugged it the phone still thought it was charging. There's a flat metal contact point in the middle that can be bent slightly. You can try moving it, but that's not a good fix... metal fatigue and all that.
