This also appears when i try to flash genocide.
What Am I Doing Wrong?

Sounds like a corrupt memory card. Back up what you can to your desktop and format the card from the desktop. Transfer your files back and try again.
It could also be in combination with bad downloads. Though, I never get why dev's on XDA don't provide md5 hashes for their files. Makes determining a bad download or not guess work.
formatting now. I'll let you know what happens when im done with everything.
It Got even worse now.
My sd card doesn't appear on my phone, but it appears when I put it in th computer & in clockwork.

Edit: I refreshed the phone and the sd card loaded.
BUT, the ROM still didnt download

last nite my phone rebooted on its own,into recovery.now my phone wont see my card.tried formatting in cpu,and phone neither work.files still show when in the cpu. But it wont let me format. Any help would be greatly appreciated..........

Is the card mounting as read only, or is it not recognized at all?
it says unavailable in settings.but when i put it into the comp., i can see all the files.
edit:when i boot the phone,the preparing sd card message goes thru.but about 5 secs. later it umount and wont remount. tried it from recovery, and it mounts.boot up,and it umonts again........i was running myns.rls5.using a2sd,if that helps.
You probably have a corrupt file on the card. Put the card in a pc and copy all files to your hard drive. It will probably give you an error with one of the files and you can then see which file is corrupt. Delete the file off of your card and you should be good to go.

Try running files to check for a corrupt file on your PC.
If not, you can always format it and start over.

If i click file,it says could not find but the files are visible...weird......i cant engage tje files at all,but they are visible.but when i put it into the phone the card is unmounted after about 5vsec.

The WinAmp app tends to do this to cards as well, if you have it, or any other apps installed, make sure that they don't have some sort of automounting built into the app, as it jacks with the phone.

It was the Download it said it completed but it really didn't
I started having this problem, first it was every so often until it became everything I downloaded and moved to my phone that was larger than 75 meg. It turned out to be bad usb ports on my computer. Using my laptop I had no issues but when I used the desktop to transfer roms, videos ect they would all be corrupt. Hope this is not happening to you. Even files I moved to external drives ended up being corrupt. Try moving a large video file and see if you get pixelation and freezing, if you do you may be starting to have the same issues I started having.
Yeah, that usually means the zip you're trying to use to flash is bad. Delete and redownload.
If you're downloading directly to your phone; try doing it to a computer first and then copy it over to the sdcard after.

Have you partitioned your SD card lately?.... does it say that when you try to wipe your data also?

If so the only way to really fix it is to ruu back to stock and then reroot......if you have an slcd dinc I have the ruin you need.... let me know if you want to try and ill give you the file and directions..... I had a similar problem awhile back and it was the simplest way to fix it

Thanks....I have tried RUU, but I keep getting error and it can't mount "/data/" VZW is sending a "new" phone after I called CS and Tech Support this morning - I have learned to NEVER go to the store for "help".
I will backup my card when I get the new phone and them reformat the card.

Actually both....I am not entirely convinced that the PB13IMG.zip file got pushed properly...even though I did not get an error...maybe you can answer....does it get pushed to "-s" (sd card) or another location?
I am on a mac (I have windows also, but setting up adb was much quicker on mac) and I don't have the time for Windows setup, even with the Setup for Dummies that I found.

If your SD is fat32 I have one you just put on the root of you SD no need door adb just place it on the root of you SD but you have to make sure its formatted fat32 rename it PB31IMG.zip go into hboot wait on it to read your SD if you did everything right it will find it and ask you to press up on the volume to update do that and wait till it says update is done b4 you touch anything.....

I am trying to wipe my virtual SD card on my LTE Galaxy Nexus and it is acting weird.
If I do a factory reset via the settings on the phone, it says it erases all of the files on the SD card. When I hook the phone up to my computer and view the files in explorer, it shows a stock directory structure and root. However, if I try and copy over a file that existed on the SD card before formatting, it says the file is already there and asks me if I want to replace it. If I view the SD card in root explorer, it shows all of the old folders and files, even though they dont show up in windows explorer. Also, if I boot into Clockwork and choose a .zip to flash, it shows all of the old folders and files as well.
I have tried using the super wipe lite script that comes with Android revolution. I have also factory reset the phone a few times via the settings. I previously had Android Revolution installed and I am currently running GummyNex 0.4.2.
No matter what I do, I cannot for the life of me get the SD card to truly reformat.
Am I doing something wrong? Is there an ADB command I can do to reformat it?

Try formatting with cwm recovery. Reboot into recovery, go to mounts and storage, and select format sdcard

There isn't any "SD Card" partition.
EDIT: Format /data cannot be done in adb, if you really want to do so just make a CWMR flashable zip with

in the updater-script.

bumping this... same issue and its driving me crazy. i can't see any of my files unless i use a file explorer (or if i'm in recovery)

If you are having an issue finding your files from your PC it's due to the way Android exposes your "internal sdcard" via MTP. Download an app from the Market called SDrescan which will force the media services on your phone to re-index your files on the phone. Then they will show up on your PC.

I do not recommend doing this indeed. To be failsafe just delete everything in the /sdcard folder with an explorer app.
I use
fastboot /w
To do a full wipe. This will wipe /sdcard.

Are you on a ROM and/or modified kernel? If you are using Franco's kernel he has made modifications to how the "sdcard" works.

yeah, i'm on the latest CND and using franco's latest too... but i've flashed other kernals and had the same problem (but are you saying this might have started the first time i flashed a franco kernal?)
anyway to fix it that you know of?
I'm having a problem downloading the latest version (8.0) of Sabsa Prime, there is something wrong with the file for me, Clockwork mod says:

Winrar is also giving me an error message when i try to unzip the file:

This has happen before with other Sabsa Prime updates but not all, for example, v6.0 worked for me.
I've tried to disable my virus protection and redownload the file three times, but it hasn't helped.
Could someone who have downloaded the file get it mirrored for me or something?

If it doesn't work, try again with another PC / device.
Run a memory test to test ram. If it shows errors try restarting and see if they go away. Also clear out any temporary folders and delete the current download and clear recycle bin. Your pc is writing corrupt data to drive, which is usually ram or bad sectors on drive.

Thanks, I restarted my computer and did a disk check, it didn't find anything but I was able to download the file without problems, I'm doing a memory check right now to see if there is anything wrong with my RAM.
