My phone is having issues with accessing the SD card.
When I open gallery or music, I get a message that says to insert SD card.
I can browse the card using a file manager however.
I have mounted/unmounted, removed, inserted, formatted and the card is till not recognized.

What ROM are you running?
Try Rescan Media
Description
Rescan Media is a simple app will rescan your mobile phone for new files that usually happens on a system re-boot.
It only calls the android media scanner.
Reboot into recovery and clear the cache and dalvik cache. That should fix it.

Sorry for being vague..
I'm running stock Verizon/HTC 2.3.3.
I have installed that app.
I wiped the caches.
I still get errors in Gallery and Music.
Try clearing each apps data?

Did you try popping it in and out? Happened to me once.
Sure did. I did it several times with no luck.
I have always been suspicious of the shutdown + battery pull technique (leaving it out for at least a solid minute) and yet oddly, it seems to work for things like this sometimes. It certainly can't hurt. And if you see the media scanning process begin on start up, you may be good. Maybe try that?
You mayb have to reformat your SD card. Backup your sd to a computer. Go to settings/sd card and phone settings/unmount sd than reformat. Than restore your backup. Reformatting usually fixes SD card issues.

Before you reformat, make sure you don't have a file with a name that has a "period" at the beginning like ".nomedia" at the root of your sd card. Folders are fine, but a .nomedia file stops that section from being scanned, so if its placed on the root of the sd, nothing gets scanned

Many of my apps wont update from the market. It downloads them and them and begins to install the update and then I get a red triangle with and ! in the middle and it says update unsuccessful? What is going on? I have about 305mb of internal and a couple gigs external. Please help!

First thing to try would be to clear data of the market. Go to the manage applications section and press the clear data and reboot. If that doesnt work then move all the apps you have on your sd card over to the phone. Then use a file explorer to delete the .android_secure folder. Don't worry, if you move all your apps over you wont lose any apps or data with the apps. Then reboot. Once you have rebooted you can move all of them back to the sd card. Once you start downloading again it will create a new android_secure folder Then you will be good to go. That folder gets corrupted during flashing, bad downloads, etc.

First off thank you for your response! I ended up doing the ladder of the two options but now it wont let me move the apps back to my sd card? I have my SD card partioned and for some reason when I try to move the app to my SD card with Titanium back up it looks like it moves it but when I click on the app again it still gives me the option to move to sd card? Can you help me with this?
Have you tried any apps from the market that will allow you to move besides titanium? Maybe a2sd or app admin yet? Try that and let me know.

I had this problem as well. It was with apps that are on my SD card only. The apps that stay on my phone memory never had problems updating. What I did to work around the not updating issue was, unmount my SD card. Then go back into market and download the apps that wouldn't update. Since your sd card is unmounted it will say "Install" instead of update for all the apps that are on the sd card, but they will still be in the "My Apps" section of the market. After you download the latest versions, remount the sd card and move the apps back over with app3sd or something similar. Hope this helps.
I was connected my phone to my PC for development. In the process i mounted my SD card to my pc on and off to transfer a few files back and forth. Now since then the 'media scanner' is constantly running. It never shuts off. I've never had such a problem before. I thought maybe my work pc here corrupted my SD card, so i transferred all the data to the local storage and formatted the card. After reboot, the same problem. If the SD card is removed, there are no issues.
Any ideas... my phone battery is just drained!
Even if it is formatted this can happen with a faulty/messed up sd card

i can still see the data on my sd card. maybe it didnt cleanly disconnect from the work pc and corrupted some bytes on the card. no way to know for sure until i get home and plug it into a PC that isnt windows xp.
is there a way to determine if the sd card is faulty all the sudden? Can i just run the check disk tool in windows?
I fixed my own problem. After running check disk on windows 7 did not report any errors, i found the Media Storage app in settings and cleared the data. After clearing the data, the problem went away.

hi having this problem also, could you tell me where the media storage app (is this in the Android phone?) and how do you clear the data? Is it clearing the cache from everything on the phone? not too sure please show us if you can....(-:

I have an Xperia Arc S with original GB firmware. Every time I copy something onto the SD card using some file manager or network share it does not show the files in any apps. I have to reboot the phone. Is there any way to force a refresh without a reboot?
Have you tried un mounting card then re mount it.
It is in settings think it is in storage

I just installed ics 8.2 and having difficulties in opening the gallery. keep getting error stating that no external storage available at the same time i could see my directories from my sd card loading behind the errror message.. i could play my music from my sd card via stock player and all.. any idea guys?
Use quickpic
Try clearing data and cache of Gallery app. No such issue for me.
Settings>Apps>All>Gallery>Clear Data & Clear Cache

First solution above – use Quicpick
Second one – try to clear Gallery's cache and data (using Applications manager) - it **may** help.
I didnt have any issues with gallery either in 8.2.
I'd tried clearing gallery data and cache still d same thing.. currently using Quickpic at the moment

I suggest you do a format of your SD card in recovery and then re-flash you ROM (no full wipe). That should definitely fix this. Copy everything off your SD card, format it in recovery, and put everything back. Delete com.android.gallery/com.android.gallery3d folders in the 'Android' folder in SD card.
Also run 'fix permissions' in recovery after doing this.
