Hi,
I needed to flash a ROM onto my phone (prev using leedroid), however as I was going about this process (after I had backed everything up using clockworkmod recovery, and begun wiping), the phone stopped detecting my SD card
this card had been working fine for well over a year before. now error in recovery is something like E: unable to mount xxxx
it is detected using SD card reader on computer, and I have tried re-partitioning it to no avail
now the phone can boot into recovery and also the rom, but the SD card isnt detected in either, and neither now is the SIM card.
normally I would search extensively for a solution myself before posting, but I have done as much as I can, in my current situation I have no WIFI, only an internet dongle with very slow connection.
currently I assume that there is a problem with the radio but I have not had experience flashing this without using SD card in recovery
also, due to the risks involved with flashing radio, I would prefer to try to flash a new rom first, but again I have no experience flashing a new rom without SD card in recovery.
I have adb installed on my computer but not much experience using it.
Sorry for all this, but I am not the kind of guy who flashes roms regularly, the last time I did it was a while ago, and cant really remember how I rooted in the first place (I think I used unrevoked) - therefore uncertain of S-off status - although not sure what the significance of this is
please help i dont know what to do

This is what you need to do (instructions I wrote for myself in case I ever broke it again) :-
To gain access to SD card:
Put phone into fastboot mode (back button + power)
In CMD, go to dir C:\Android\android-sdk\Tools
Type: path C:\Android\android-sdk\Platform-tools)
Input "fastboot oem enableqxdm 0"
You'll need the Android SDK and once installed (along with the updated USB drivers that are available in the updates it finds) I discovered that 'Fastboot' was in a different directory to ADB so attempting to run Fastboot failed as it couldn't find ADB. My cure was to create a path to the ADB directory (as listed above). Be sure to change the path and directories to wherever you install them.
Alternatively (and far easier) search for 'Android Flasher' which is available on these forums. This has fastboot and adb installed in the same directory and is a much, much smaller download and run the above command from a CMD prompt from the directory that fastboot and adb reside in wherever you install Android Flasher to. I've never tried it this way but no reason why it shouldn't work and far easier than getting the whole SDK thing!
Yep confirmed, Android Flasher will do it. A quick search should find the program, it's a very small download compared to downloading the Android-SDK (and the Java runtime module it requires). Simply change directory in CMD to c:\your android flasher dir\data directory and run that command from there with the phone plugged in and running fastboot, fixed.

Ignore the S-Off bit, that's to do with the functions of the flasher itself, we're not actually using that.

after reflashing the ROM, the problems with the SIM went away, so no need to flash new radio!

I solved the problem with your solution (fastboot oem enableqxdm 0) then full wipe then reinstall rom
But now windows can't see my device. On my phone there is no popup menu to ask me what do i want to do when i connect the usb plug
Also, in recovery, i choose mount USB storage but windows dont see my ds card (4gb or 32gb cards)
What is the problem?

I'm afraid you have what is known as a USB brick, the fact that it asks for the Qualcomm drivers confirms it. Do a search on the forum, there is a fix for it.

hi,just got a dhd s-on.but its not detecting the s card,tried the above method but i just get waiting for device ?... can get into fastboot etc,but cant get rid of the usb brick.looks like the dhd is stopping on stock sense 3.0

Just download the correct RUU for your region from one of the post in this forum. Connect the USB cable, run the RUU exe and it flash your phone back to the OTA ROM.
I was put on the original room and then proceed boot, successfully run the STEP1, STEP2, but to the error can not enter the recovery

Rom I downloaded OTA and it still can not root, and do not receive equipment, including sdcard

His computer don't have connection with phone. It's BRICK, so how he can use RUU?????

Had the same problem and I was able to retrieve the roles of USB and SD Card making the procedures described in the tutorial past links above ...
Read carefully and make with calm ...You'll see that it is not as difficult as it sounds!

it's very strange, I have exactly the same problem since yesterday!
SD Card brick is resolved by this command in fastboot mode :
fastboot oem enableqxdm 0
But USB Brick is not resolved and my phone no longer root how can I do? I tried unsuccessfully Z4 root, is there another software to root without USB?

Hi, I had the Same Problems some Minutes ago. (I was using the new AmonRA Recovery to flash a new ROM).
Here are the Steps what I have done to solve my problem (Rooted ROM is required):
1.) Download Android SDK -> Then open a comand Line and change into the Tools Directory:
cd path\to\AndroidSDK\tools
2.) You need to know your CID.
Turn your Phone into fastboot mode (Pressing Back and Power On). Then in your Comand Line do:
fastboot oem boot
The log will say something like the following:
CODE
.
.
.
#INFOCID is T-Mob101
In this specific case, the CID is T-Mob101.
3.) Get and extract the the attached Rescue.zip File.
Open the File "misc.img" in a hex Editor of your Choice and change "T-MOB101" to your CID
3.) Turn your Phone again into fastboot mode (Pressing Back and Power On) then go to your Command Line and do:
fastboot oem enableqxdm 0 (This will enable your SD Card to be seen in your favorite explorer on your phone)
then "Reboot"
4.) Then get the two files to your SD-Card (email them or dropbox them or what ever).
5.) Open a Terminal Emulator (http://www.appbrain.com/app/android-terminal-emulator/jackpal.androidterm) on our Phone
enter:
su (Enter to get Superuser Rights)
cat /sdcard/flash_image > /data/flash_image (this will copy the file to /data, therefore you need root!)
cat /sdcard/misc.img > /data/misc.img (this will copy the file to /data, therefore you need root!)
chmod 755 /data/flash_image (this makes the file "flash_image" executable)
/data/flash_image misc /data/misc.img (this will execute flash_image the result will be that you overwrite your misc section)
Reboot.
Everything should be ok
Ok this is a bit weird cos my usb got bricked yesterday too but a lot of searching i have managed to get it sorted. i lost root too so used this method http://forum.xda-developers.com/showpost.php?p=9056719&postcount=15 its similar to the above but for people who have lost root.

I "S-OFF'ed" my Device some Time ago without any Problems. Today I flashed the new AmonRA via Unrevoked. Everthing went fine. After Flashing a new ROM (Oxygen) i had the mentioned USB/SD-Card Problem.
I was able to access my Device via Fastboot and get into Recovery but my SD Card was not able to be mounted anymore. (Can't mount /dev/block/mmcblk0p1 (or /dev/block/mmcblk0) (No such file or directory)).
Also i was stuck in a Bootloop after the "S-OFF Bootlogo".
After "fixing" the SD Card Problem, i was able to Boot the new Rom, access the SD Card via FIle Explorer, but my Device was not recognized properly while connected via USB. Something like "unrecognized Quallcom Device" Driver not found.
Yeah i had similar experience i had i flashed new amon ra via fastboot then backed up my leedroid2.2b rom via rom manager just cos i could rename it easily then booted into amon ra wiped and flashed latest leedroid. On the reboot it decided to reboot on the spalsh screen a few times and then the boot animation started when i booted i realised i had sd problem but wasn't aware of usb so ended up going the long way around
I was trying to flash a insertcoin ROM the other day and i failed to do so. I then just kept trying to flash the same file and wiping my cache and stuff in recovery. Then i tried another ROM but still didnt work; i formatted my sd card and after that when in recovery my phone says fail to mount sd card dev/......so on so on. I think my problem is that i have a screwed up OS im not too sure. i hope some experts can give me some suggestions to solve this problem thank you

It's one of two possibilities:
1) You've usb bricked your device. To solve try this: http://forum.xda-developers.com/showthread.php?t=691639&highlight=usb+brick
2) Mis-formatted SD. SD has to have one FAT partition (first) and a second ext3/4 partition. Both have to be primary.

try this put your htc desire in fastboot mode and instal the ruu and voala the phone is back

Did you tried flashing stock RUU?
If your are still usb bricked after flashing RUU/pb99.img then try the steps below i fix mine that way.
after i unlocked my sd card via fastboot command"fastboot oem enableqxdm 0" sd card recognized by phone and i can browse it via file explorer.
note-you can access your sdcard only via file explorer nothing else so after execute the command download a file explorer from android market.
then i copied the visionary+(it gave me temporary root access which is needed to work this whole thing out).
root explorer,terminal emulator apk and those usb bricked update file(image_flash,mtd0.img) to sdcard from pc via card reader and installed them.
before copy those update file on sd card i edited the mtd0.img to put correct cid n rom info via hex editor.
after that i copied those update file in phone memory
/data/data/ and set permission for r/w and execute via root explorer.
after that i simply typed this command in terminal emulator
"su
/data/data/flash_image misc /data/data/mtd0.img"
Rebooted my phone and voila usb brick is gone htc sync,usb tethering,sd card mount,charge everything is working and now i am rooted and s-off again
Note-you don't need the rageagainstthecage file which only need if you try to get root access via terminal emulator which is lengthy n complicated but with visionary+ you can get root access with 1 click and it's much easier than typing command in T.E.

It's not that simple if your misc.img is corrupted that fast boot command only enable your sd-card if it's blocked it's not gonna fix your corrupted misc.img so i had to flash the pb99.img to get back to stock rom get through the process i described earlier.
well in order to carry out these tasks ill need my phone to be detected by the computer right? I think this is why i cant get into my phone cuz when every i try adb shell it says devices not detected.
That's why you need to do exactly what i did as you are now stuck on stock rom with usb brick.

It seems you didn't get me either.i was usb bricked twice on s-off device first time that command worked but second time it didn't that's why i had to unroot it and go through this long process.there are other people like me who was s-offed but that fastboot command didn't work for them either for some reason.any way lets stop the arguing here it's not gonna help the thread starter.it worked for you fine but it didn't work for me second time

Don't type adb shell, just fastboot enable etc etc. in once, in that way it won't work
ps: you still have to do the cd C:/android-sdk etc.
But if you go into fasboot and if it says fastboot usb after connecting it's no problem just do what i said than
Right, basically I think I have pretty much messed up my phone, yeah yeah I know what your all thinking, seen this before.. you most likely have, I have read all the pages and tried to search for people with similar problems to me to save asking the same thing again, however I keep finding answer's that take me back to another problem on my phone.
anyway long story short, my phone wont get past bootloader. i can still get to recovery, but that does not help much as i get about 1 minute before it restarts if i plug it into my pc when in this mode, unless i format the system.

am able to format Boot, System, Data and Cache.
However when i try format SDcard i get the message

I thought maybe trying to re-root my desire using unrevoked, if that would have been an option, with the latest Clockwork recovery, but it will not find my phone in fastboot. However after a few attempts of plugging in my phone ,AMDesire Recovery finds my phone and can get it to wipe the dalvic cache, sd:ext partition, create a partition, try to wipe sd:ext partition and it says "error : run 'wipe ext' via adb.
I thought at looking at the DeBricking page on here, but that seems a lot of code, and when trying to find the right downloads i to even enter all that code i have got myself a bit lost.
any help whatssoever, would be greatful,
also i apologise for this if it is in the wrong forum but as i am pretty much a new user, i am unable to post in many places.

Try this command in fastboot mode: "fastboot oem enableqxdm 0"
Search in the forums to set up ADB/fastboot properly in your PC.

In order to install Android SDK you need JRE. You do not have to install all the software available in SDK you have only to download the tools.

This reads to me like you have a corrupt Ext...did you attempt to flash any alignment.zip before this happened?? Every time I try to flash an alignment, I also get this error message...hence I don't bother anymore...
My suggestion is to remove your SD and place in a card reader, then format your Ext using GParted. When you boot to recovery again, all should be sweet.
Just a thought.
dont use any EXT partition at this stage.. format ur sd card with single FAT32 partition and then try
Thanks to all of you trying to help me, I think i may need to mention that my phone does not seem to be able to read any sd card, not just the one i was using, tried my 16 gb, 8 gb, and 4gb. same message comes up.
going to try gparted later, and also have another go at creating gold card (what driver should be on set for this to work, Androidbootloader or MY HTC ? )
hopefully by the end of tonight i will have a working phone .

this is an usb brick, you should install android sdk, go into cmd
1. type in cd C:/android-sdk-windows/tools atleast if you have it there
go into hboot, press fastboot and connect your phone to your pc, it should say fastboot usb. then enter fastboot enable etc..(this enables the sd card read ability) after that go to the link i gave you, flash the update.zip and ur usb brick is gone,

this sounds too simple and i still am having problems, do i not need a gold card?
Command prompt and typing in the commands you say, i continue to get an error message saying deice not found, seriously, i bet i am doing one thing wrong.. that's too simple to notice?!?!
i've never heard that you need a goldcard for a usb brick, it may sound dumb but have you installed htc sync? if so uninstall it and i you haven't install and uninstall for the drivers, usually a usb brick is solved in 4-5min, so maybe you missed out something, because when i had my first usb-brick i entered adb shell-.- you just have to type the command in once
just tried same on my notebook, no htc sync installed , Still Says error : device not found. am guessing i am pretty much screwed.
not got my hopes up too high just yet, but i think it's actually looking up a bit now, conected to phone, typed in the commands given, it can read sd card, installed update.zip. now i dunno if i should go back to backup, or try install everything again,

Hello, I was happily using a previous miui build and updated using ROM Manager and it was going fine until the phone stopped being able to recognise the SD card whatsoever, so I tried looking that up, and was finding things about reformatting memory cards, trying different ones etc. I tried all these things to no avail, I am now trying to RUU it back to a stock Sense ROM in the hope that will fix the SD card problem but when I try to connect it via USB to the PC it isn't recognised at all. I've tried installing HTC Sync etc. I've no idea what to do anymore?

Retarded post: Download a terminal program and: For this, do 'fastboot oem enableqxdm 0' in fastboot mode. Strike that.
Can you boot in Fastboot ? Back arrow + power.
Is your phone visible to the computer in fastboot ?
Don't jump into restoring an RUU as you will really be in trouble then!
Trust me, I've bricked mine and gone down this road to put it right and it can be done.
Get hold of Android-SDK (Google it), get that installed and prepare to learn. In my case I managed to restore SD card functionality and thought I had won, then discovered that I still had a USB brick. Nevertheless, many hours reading and several vodkas later I had it fully restored.
If you still have clockwork recovery on the phone (I'm assuming you're using that?) then it's a pretty easy fix, don't panic! And above all, don't attempt to go back to stock as then you'll still have a knackered phone but with no root to recover it (although I doubt you can if your phone can't read it, unless you learn the fastboot stuff and flash the RUU before actually fixing the problem).
The thread linked gives some idea, there's also a lengthier one, search 'USB brick'.
And good point by paul.c, that would be far simpler but I've never tried it myself - although esentially that is indeed the command required to get the SD card working again.
Just beware that getting the SD card working may not be the end of your woes, you may then find (as I did) that USB functionality is broken and bluetooth doesn't work, classic 'USB brick' symptoms. Again, pretty easily fixed.

Yes, I can get to fastboot ok and the phone's recognised in it. I've been trying to use the command prompt & do 'adb devices' but that's not working it states 'adb' is not recognized...etc. Should I be doing the 'fastboot oem enableqxdm 0' in a terminal on the phone or in the SDK on my computer ?

Adb only works when running a rom. You have to run this comment in android sdk on your computer.

So for Android SDK should you be properly booted or should you be in the Fastboot screen?

If you have to issue fastboot commands your phone has to be in fastboot.

My SDK is working fine for commands such as 'ADB Devices' now but when I go into Fastboot and give the command 'fastboot' it states that it's not recognized! Do you have to have S-Off for this to work?
Well... you start your phone.. by pressing.. Vol down and power.. than connect your phone with usb cable to pc ... open command prompt.. got to your sdk folder "platform-tools' and than enter ADB or Fastboot command..
But when you connect your phone to PC.. check if ADB drivers are installed.. go to device manager and check if your phone is recognised as ADB interface under Android Phone

Ah, I just looked in my platform-tools folder and there was adb etc. but I could not find fastboot. Where can I download that?
In the device manager, it's under 'Android USB Devices' as 'My HTC' is that ok?
OK, I have the SDK working fine, but when I go to this ( http://ks25738.kimsufi.com/misc/index.php ) website to get the .img file I need I can't figure out my kernel/ROM number - in the settings it says:

You are once again trying to flash an RUU. Do not do this without first fixing your SD card problems.
fastboot oem enableqxdm 0
and also
fastboot oem eraseconfig
Need more details on where you are at. If you don't fix the issues you have then flashing an RUU could leave you in a much worse situation than you are now, and where you are now is easily fixed!
Parva4 is right.. you should 1st try enabling your sd card....
what you need to do is...
first download adb drivers for your device...
power on your phone into fastboot.. by pressing volume down and power.. connect usb cable to pc.. and in device manager.. update driver to adb driver.. while in hboot.. and than run that fastboot command which Parva4 mentioned.
I sorted it out, thanks to all who helped!
I did the fastboot enableqxd 0 command to unlock the SD card then once that was able to be used I flashed back to a nand backup I had made minutes before I did the flash that went wrong and now it's working fine!

i unmounted my SD Card with CWM... And now i restarted... Because SD doenst work in OS i tried to start recovery, but i can only start Amon RA... And Mount doesnt work **** !!! What should i do ??

Remove the card and try to connect it to PC using the microSD card reader.
Don't worry too much! But yeah, take it out, connect it to your PC (through a card reader or whatever) and see if it recognises it. Hopefully this is the case.
If not, you might have to reformat it... but please don't do this just yet! Wait a few more posts and see if anyone else has a bit more experience on the matter

Once u have sdk installed use these commands

fastboot oem eraseconfig

Open cmd type fastboot then the two commands
But make sure Ur sdk path is correct else cmd won't find the location

I have a german desire hboot 0.93 slcd and it had tmobile.branding before i rooted anf installed oxygen...
The problem is that i somehow have to boot cwm and then mount it over that...

Yes the commands I gave u for fastboot will sort this issue buddy install android sdk then once install run fastboot from cmd on Ur MAC then the 2 commands
Ps u need Plug Ur device to MAC boot into fastboot before u start the commands

I'm having similar problems, SD card won't mount when in phone either in recovery or normal.
The SD card can be seen when in a USB reader in the PC
I've followed your instructions below and they exectued OK, but I still can't see the SD Card.
Any help would be greatly appreciated.

Aha - sorted it myself!
I checked out another SD card and it worked fine. So I put my (broken) SD card into my old Hero and it was recognised OK... Strange I thought as I expected it not to work.
As it was visible from the Hero, I was then allowed by ROM Manager to Partition it. Can't remember exact values but it was approx 512 & 0.
Once this had been completed, and although it had problems mounting on reboot to perform the partition, it seemed to work.
After re-installing it in my Desire I was able to use the SD card both from the phone and recovery.
Thinking back, the problem arose when I flashed the EXT4 Align Flashable.zip as part of my planned move to InsertCoin CM7/A2SD+ STABLE. I'd flashed the EXT4 zip, but my SD card was not then visible to flash the ROM.
Perhaps someone more knowledgeable might correct me, but I think I should've partitioned my SD card prior to flashing, but there was little info on this procedure.
Anyway, hopefully my fix may help someone else out there and I'm now running my planned aforementioned ROM.
