My SD CARD recently has been giving me some issues. It has worked fine since I got the phone in Aug 2010. I went to access my pictures yesterday and as they started popping up, an SD card picture popped up and said please insert SD card. No pictures could be accessed. I went to phone and SD storage and it said my SD card was unmounted. I went to mount it when I saw this, pressed mount, started to mount and stopped all of a sudden. Now I have an icon of an SD card with a question mark in my taskbar. The message says" SD card damaged. You may have to reformat it." When I go to reformat it, it gives me the message "external media in unknown state". Any ideas?

Because you need to give us more than 3 hours to help you. Being impatient and bumping your thread like this makes people NOT want to help you.
i wish someone could please help me with this issue because its also happening to me, it happend twice already on two different sd cards, i even took the sd card out and put it into my Samsung Epic but still it says the sd card is blank . please help
I used to get this message way back in June 2010 when I first bought my Evo. It happened exactly as you are describing. I thought my SD card was bad. So I moved all my contents to my PC, formatted the SD card, then moved the contents back. It may have been coincidence, but it helped, for a little while, and then the exact same problem returned.
All this happened before I had ever rooted. I then rooted my phone, installed a custom rom, and have never had the problem again, even though it was the exact same SD Card.
Are either of you who are having this problem currently rooted? Or are you both stock? If you are stock, you could try rooting, and installing the stock rom. If you aren't comfortable with that, then try formatting the card, and if you have a spare microSD try that just to troubleshoot it. If you get the same result with a different card that you know is good, that will narrow it down to being related to software on the phone.

I had a similar issue, I reformatted my memory card, and the problem has not returned. It has been seven months since then.
If you are formatting it on a mac, make sure that you set the partition type correctly to MBR (Master Boot Record)

So I have searched for this error and no one seems to have an answer that fits my problem. I got an error after booting that stated SD card unexpectedly removed and gave me a triangle with an exclamation point in the center
the card works fine
it will mount up perfectly from recovery, i even pushed several roms to it and tried (cyanogen, RR, and SkyRaider) all three worked beautifully, but as soon as I boot up BAM "SD card unexpectedly removed" and I can't access anything
the computer reads it just fine so I am pretty sure there is nothing wrong with the card, also with it reading in recovery it seems there is nothing wrong with the phone
is this a problem with ROMs?
I could try to do a factory reset and see if that works... but its really annoying :\
any help?
I would try reformatting the card on your PC. The card may have a bad sector that the PC and Recovery ignore. The phone OS tends to be a little more picky about those sort of things.

This worked
Thank you, I am running good now
I noticed when I was trying to backup my SD card (pictures and whatnot) that some of the files were corrupt
I will make sure I keep a regular backup just in case its a sign of the card going bad
good thing is my titanium backups were all perfect

You are on the right path - don't forget, a good practice is to back up your SD card to your PC regularly.
Why? If (when really) the SD goes wonky, you have a PC backup of the card, and you can get another SD, format it on the PC, load the backups you made, and be back in business in short order.

My solution was to format and start over, that was the only thing that worked for me
I've gotten this a few times and it is a PITA because it resets all the notification sounds and also the phone reboots. Once it does it, the only way I can get it to work is to remove the card, put it in the PC, let Win7 do a fix and repair errors and then it will work. I tried removing and reinserting, and it would mount the card and immediately give me the 'sd card unexpectedly removed' message.
I tried reformatting, and the sd card worked for a while and then I got the 'unexpectedly removed" message again after a few days. The last time it gave me that message, I just did the repair errors thing again and it's been working without fail for about 4 days now. It seems like it's going to be an ongoing thing though.
I've been googling like a mad person trying to find a real fix for this. I saw a thread on Android central where they guy said he put his wife's card in his phone and it did the same thing; but worked fine in hers. He mentioned it might be related to the 2.2 OTA update. My problem did start after that update.
In another forum thread, someone posted that they bought a new card, and it gave them the same message. So it doesn't appear to be a bad card; which is what I originally thought. I'd hate to buy a new one and have same issue (i have a 16gb card currently).
(sorry for long post)
I have the same problem! I'm using a Verizon HTC Droid Incredible 2. Previously i've been running Froyo 2.2 over the phone with a 16 GB microSD Card without problem. Later i've been using a 2 GB microSD Card and works fine, but now i'm runnin' Gingerbread ROM From verizon with 16 GB microSD Card and the messages "SD Card unexpectedly removed" don't stop.... the card works fine over other phones... even the stereo of my car can read perfectly the card's content.
I don't know if this is a problem with the update or what????
I don't think in the versions of "Bad sectors" because i check everthing and the problem persist.
The problem appears when i'm playing music... later of that the problem appears at the camera, the file explorer or another app that tries to read the SD Card.
I reboot the phone and works fine but after a couple of minutes the same [email protected]$%&t appears again....
I'm tired of reading forums telling me that is a problem with the sectors... that is a LIE!
no matters the class of the card because i've tried with Class 2, Class 4 and works fine.
This night i'll go to the store that solds me that coursed card to change it!
if tomorrow my problem was solved i been writing at this same forum!

I do not think someone saying it is bad sectors means they are lying.
I think they are merely trying to help you determine what the problem could possibly be.

did you format the card and try again as suggested earlier? That usually clears up any issues with sd cards unless they're fubar'd. Just make sure you back your card up first.
Whenever my card does this I put it into my Windows computer, right click on the card and go to properties/tools/error checking/ and check both boxes and let it run. It fixes my issues every time without formatting the card.
I had that problem a while back with one of Verizon's updates after 2.2. Reformatted card. worked fine in other phones but still said same thing everytine I put it back in the dinc. Ended up getting a new SD card then all was fine.

If you have an ext extension on your sd use G-Parted (google it) to format your ext2/3/4 partition.
Hello, XDA commnity. I'm really hoping you guys can come through for me once again, and I apologize if this has already been discussed before.
Anyway, a little while ago, while rebooting my phone (a rooted Droid 2 running Liberty 1.5 if that helps) I got a message in the notification bar that my SD card was damaged.
Please tell me if I can recover all my card's contents without having to format the card. I have a lot of important stuff on there (including two nandroid backup images) and I absolutely MUST recover everything. Please have some good news for me!

This happened to me this morning. I crapped my pants, remounted my SD card in Settings > Storage and it mounted just fine. As soon as it did I managed to back up everything on the card.
Failing that, try booting into Ubuntu if you've got it handy and try reading the card from there. I've had memory cards/sticks which Windows has reported dead but worked just fine on Ubuntu before.
Hope this helps.
I keep getting an error saying "Blank SD card" I have formatted it on the phone, and even though windows. I am not sure what went wrong, or what to do next. Can someone please help me...
Make sure the write protection tab is not engaged. Some cards have them. Absent that, does the same thing happen with a different card?

I had that problem once, my phone would not read the card. I actually went through a couple steps as you did like reformat. But then I just blew out the card holder and stuck it back in and it worked. Not sure if it was lucky or not. Kind of brought me back to my original Nintendo days.
If you go to menu-settings-SD & phone storage and scroll down under utilities, is the SD card mounted? I had a few instances where the SD card "unmounted itself" after a reboot and never thought to look there.
well i flashed a kernel and didnt like it so i went into recovery and began flashing my nandroid backup. from there it was all going fine but stopped in the middle and said, oops something went wrong. after that i tried to get back into my sd to see if i had another backup. i then realized that my sd card was unmounted and i tried to mount it but to no avail. this is where i began to freak out because everything was wiped and the backup didnt work and i couldnt boot to anything except the bootloader and recovery. i finally got adb working and was able to flash cm9 directly to the phone so i have my phone working again YAY! so phone isnt brickd, maybe just sd card.
so is there anyway to try and recover my sd card to get my stuff back? i havent tried to format it yet because i want my pictures and all that :/ still need to get a card reader and try it in a pc
Captain Throwback has a thread about fixing the SD mount on the EVO if that is the problem. Have you tried a different card or tried your card in a different phone?

i havent had a chance to do either of those but i tried using the adb fastboot commands to makes the sd card recognized and mount again but it didnt work. gotta find another sd card or something or a card reader. any suggestions other than that?
Sorry but if the captain's thread didn't fix it than your card is most likely bad. I would try it in another device before trying to format it just in case. Maybe someone else will chime in with some other ideas.

Don't format it until you try it in a card reader. There's a chance the card is corrupted, but I've also had cards not read on my phone, at all, then pop it in a card reader, and Windows reads it without a problem
yeh i will definitely do that thanks. im really hoping it just glitched out and my phone isnt reading it and can save everything. no idea how it happened still. just restoring a nandroid backup and it said there was an error then no sd card :/
so got a card reader and popped it into the pc and it was all there now formatting the card to exFAT and going to see if it works
so its still saying there is no sd card... i know it works because it read on the computer fine. any ideas of why its not showing up in recovery or on my phone? and ideas how to fix?
I don't think the Evo can read exFAT. I'd go with FAT32.
Been working fine for over a month now all of a sudden the SD card appears to be randomly mounting/unmounting itself and has become unreadable on my phone.
I've put the SD card in my PC and it works fine so I suspect my phone has developed a fault. I hope I don't have to get an exchange as it's otherwise faultless and I'm dreading getting a unit that can't be S-Off'd
I haven't modified or installed anything prior to the fault developing,
Any tips would be most welcome.
PS It's a genuine Transcend Premium 300x 64GB bought from MyMemory.

I've not tried a reset yet but it's been as good as gold until about an hour ago when it suddenly started flashing between "preparing SD card" & "SD card unexpectedly removed"

well, a simple solution is to enter recovery and see if the sdcard works there if this way it works, change the rom/perform factory reset, if this doesn't, well, you might try to flash everything again, but i's possible that the sd slot is broken...
i wish you all the luck, let me know
I would copy EVERYTHING off the card to a PC and do a format and try a different file system. I would has perform the format on the phone, to make sure its gets formatted to the correct file systems.
Long story short, It sounds like you memory card is starting to get Corrupted. I have to SanDisk cards that started doing that in the biginning and ended up not being read at all on PC nor the Phone.
Also while its doing that, youll also notice the battery start to drain more, at leasst thats what happended on my Note 3 when I had the san disk cards going corrupt.
Ended up buying new Card and now everything is fine.
I doubt is the card reader built in phone, more then likely its your memory card going corrupt.

I agree most likely the card. I'd download an sd card format tool and do a full format, don't use Windows format then do full check for bad sectors. I had a similar problem and it was the card at fault, but strangely worked ok on the pc.

Thanks for all the advice.
I could read the card in recovery but it failed to format there.
I tried to format in windows but my only options were NTFS or exFAT, no FAT32. But this failed anyway also.
Downloaded a SD format application but this said the card was write protected and to move the lock switch, obviously MicroSD has no switch like standard SD.
So next I deleted all the files on the card and ran h2testw test. This failed and reported card is probably faulty.
Then I checked the contents of the card again and all the original files were back again, so it's definitely 'Donald Ducked'. I was able to get all my files backed up and fortunately I had my photos already thanks to Dropbox & G+.
So I will now look at getting MyMemory to replace it but my fear is they will want me to return the faulty card but it has sensitive information on it that I can't delete. Any ideas on how I can wipe it?

How did you attach the card to the pc? Try a usb card reader and then try again. If still problems download mini partition tool (Google) and delete the partition. You can then wipe it properly.
I reformatted the SD card from the settings within the phone and since then it been working fine :\
make sure you're using 2.0USB also..
when i connect my phone to charger
Its been few days i am noticing when i conmect my phone to charger it says 'sd card removed unexpectedly or either it suddenly stops showing content from my phone, then i have to go to settings unmount/mount the sd card frkm storage option and then it shows the content even when charging, or sometjmes it doesn'fshow option for that so on that case i have to restart my phone. What could be the possible reason and solution to that. It was fine on 4.4.4 now i am on 5.0.2 all stock htc one m8 h/k edition with sim unlock and s-off via sunshine.
