Suddenly my phone got hang and then restart. After that oneplus logo comes and goes twice or more then black screen.
I entered into recovery to clear cache data but there was only one option " Format Data" I did format but no luck, same condition
Now what to do? How can I flash firmware? or which process should I follow

Can you boot to bootloader? With the phone off, hold vol-down while pressing power on. If you see a menu screen show up, navigate to Recovery. Tell us how far you can go.

So you wiped, then installed a rom zip, then tried reboot but it gets stuck at the white HTC screen? Do you see any error messages in the recovery menu?
It's possible your emmc chip went bad. Follow the steps here that start with "To be sure that you really are effected by the emmc issue..."
i just try hard reset , wipe cache, data but nothing... when i enter in recovery appears a error message : E: can't open /cache/recovery/command ...
I would try to put the original rom whit adb but in the cmd the device appears connect but offline ...
Sounds like another busted emmc chip story that's it when I get my replacement I think I'm gonna be too scared to root it

I have tried a hard reset with the volume up button + power button until vibrates then switching screen lock on and off. I see the Erasing user data...Erasing cache... come up then I let go and all it does is gives me the acer logo and proceeds to bootup like normal. I have tried about 100 times lol

I have tried the update.zip thing which didn't work (see below)

Sounds like someone relocked the boot loader with a custom recovery installed. It'll say unlocked if it is.
You could let it boot android and see if you can still adb in and get it unlocked again, if its failing to boot android and recovery you bought a tablet someone bricked.

Yeah I think it's bricked luckily the guy at the pawn shop said I could take it back within 30 days since I purchased it with the unknown password on it.
I managed to get a 32gb Nexus 7 from craigslist for $170 today so happy with that.

I have the same problem with a Acer Iconia A100 wich has user code...Tryed to hard reset it,I can get into recovery menu, but once the recovery menu appears the device won't take the wipe command.
I can scroll with vol buttons through comands:
-reboot system now,
-apply update from external storage
-wipe data/ factory reset
-wipe chache partition
-apply update from chache
it's just like I am not pressing the confirm button,but all butons are ok
any one has a fix for this problem ???????????

I'm having the exact same issue. I'll let you know what I figure out. I've had mine rooted previously, but have since done a stock ICS update, which removed root. I let the kid play with it for a year and now it's in the exact state you describe. I'm hoping to get it going so I don't need to buy him another tablet!

I need some help with my OnePlus 3 running the latest or close to latest Android 8 OTA stock rom. It started half year ago, suddenly the phone was in a boot loop. I switched it on and off and I managed it to start again, doing a factory reset and wiping cache data. Then I reentered my details and selected a password. It was working for some days again, but I struggled to enter the right password after a while. Not because I'm stupid, it's pretty easy and I did it again and again. Then it was accepted and the phone was running again. I backuped my data and wiped it again just to check whats going on with that password. After restart I reentered all my data with password 0000. After restarting, it started again having trouble to enter the right password... not accepted. Switching on and off and then suddenly 0000 was accepted.... I don't know, I was tired to use this phone anymore and left it on my desktop.
Now half a year later. I want to fix this somehow and I thought, ok nothing to lose. I try that new Android 9 beta with adb sideload. Before that I'll do a full wipe loosing all my data (no problem). The wiping bar stucks at 99% and then it says "Format failed" with the only option "Yes". So stuck there, not started to sideload so far.
Update 1:
If I select that "Yes", I'm back in that erasing menu, with 3 options factory reset, wiping cache and erasing everything... if I select the last option again, it takes a second saying "Format failed"... I could do this forever. But if I select another option like wiping data something strange happens. It opens up a password screen asking for password... none of my passwords are working there. It is starting to decrypt... but stops with vibration and I'm back in that password screen. Only option that is left is "Forget password". If I select that, it is warning me, that I'll lose all my data. I say "Yes" and it's rebooting with a Chinese screen with a bar stucking at 99% again. That after a while the screen turns black. Phone is rebooting into that Chinese screen again, animating the bar to 100% again, switching itself off. If I start normal, Android logo appears, thats it, no OnePlus circle is coming up. Then I did it once more... "Chinese init/wiping loop" again. So I can restart normally having this or restart in recovery to do all the things as before.
Update 2:
After showing up the Chinese init/wipe loop again it restarted but now with red/white OnePlus circle animation. As you read this, it's still rotating...
Are there any thoughts about it... has this phone a hardware damage or what can I do? I appreciate any ideas.

Update 3:
Before following tnsmani's advice to install TWRP I did something that makes things worse. Because I couldn't wait, I sideloaded the new Android 9 zip without any problems in sideload mode. It was installed and the loading circle was coming up. It took a while and finally the phone switched off. Now the phone is booting up to a new rounded edge OnePlus splash screen without any animation and It's not possible to start the old recovery mode anymore. So I can't connect to adb or fastboot to install that TWRP. All what is coming up are different menus without the options I need. I attached two new pictures to show them. But I can't establish a connection to my pc anymore. No devices showing up.
Update 4:
Ok, it seems that fastboot is communicating when I'm in that green/red menu which says start but my command:
fastboot flash recovery twrp-3.2.3-10-oneplus3.img
fails with the output:
target reported max download size of 440401920 bytes
erasing 'recovery'...
FAILED (remote: Partition erase is not allowed)
finished. total time: 0.019s

All I want to have is a booting up final or beta stock rom working as before, without any specials.

Is fastboot/bootloader working or not? If it is, simply flash the TWRP first and then proceed with ROM.
If the MSM Download Tool succeeded, you should be able to boot. If not, it means that it did not succeed. Try it a few more times since for some people, repeated attempts are required before the phone boots.

What I noticed from your SS is that your bootloader is locked. So no way you can flash or run a custom recovery til the time you actually unlock your bootloader.
I will advise to unlock bootloader first, flash TWRP fixed version (somewhere in LOS thread is the link), "format" /data partition, flash your desired OOS Rom and then try again and see if it works.

Boot into the bootloader and on the PC enter this command: fastboot oem unlock.
This should unlock the bootloader. Then flash TWRP, boot back into it and flash ROM etc.

1. Formated data via twrp
2. Cleaned Dalvic and Cache
3. Install RMM, no Verit and Magisk zip files (without going into recovery )
4. Then then system reboot
And since then I am stuck on Samsung logo screen with blue led flashing. I cannot do anything now tried holding power button nothing.
Its been 40 minutes now and I am freaking out.

I was able to enter download mode and restore stock OS. But now how do I proceed with rooting and installing twrp? Not sure what I did wrong

Sounds like issue with disk quota management
Use this instead of using no-verity zip to disable disk quota.

You will have to re-name the file to add 'quota ' to the file name.
Long story short, I tried to flash a global ROM on my Mi phone. However, after a few unsuccessful attempts at doing so, my phone is now stuck in the fast boot screen.
I tried all I know and what I can find online, but the problem still persists, where when holding the power button it would boot to main power screen and then make a buzz and go back to the fastboot screen.
Have tried:
- Resetting through recovery mode (pow + vol up)
- plugging phone into my computer to reflash the ROM (error remote: unknown command) / on my phone it would switch out of fastboot and instead display this message: "press any key/button to shutdown"
- pow + vol down (held for more than 1 minute)
- pow (held for more than 5 minutes, everytime it would boot and then boot back into fastboot mode with the buzz in between)
- ADB console commands:
- fastboot reboot
- fastboot device
- tried using MiPCSuite, plugged my phone in, but application didn't detect my phone
- running all applicable applications as mentioned above as admin
- when resetting phone progress bar would stop at 1.03% then it would say reset completed (I don't think did reset the phone, but whatever...)
- tried different ROM versions, Global and China (Once again it would either switch out of fastboot on screen OR in the flash tool it would display the error "unknown command" which I assume would be something to do with the communication between my PC and the device itself)
At this point I don't know what else to do, watched every tut on youtube, read pretty much all the forums I could find related to this issue.
Any help would be much appreciated!

I whould try to update firmware, then recovery (wipe all at this point) and at the end ROM (format data, wipe dalvik and cache at this point), always worked for me. But - the phone should be unlocked for this

I have tried going into recovery mode and wiping all data, and trying to update the firmware via the flash tool. But as I said above I can't it gives me the error "error: FAILED(remote:unknown command)" and since I can't get into my phone, I can't use mipcsuite either or enable and settings.
Note - wipe data and format data is not the same
I've stuck with some stupid error ("erase all data" message and reboot to twrp) for hours until I formated data
