I have looked around on here and on other forums but am unable to find anything with the OS-6.12.401.4 to help solve the problem all i can tell it has probably tried/has been updated to Android Marshmallow. other then that i am stuck as to where to go from there.
Any help much appreciated

What is wrong with the phone?
"Locked" in bootloader simply means the bootloader is locked, which is the normal stock condition. It doesn't indicate a problem in itself.

Well that information is remarkably vague, to the point of not being useful!

What recovery? How did you try to load it?
If you tried to flash a custom recovery like TWRP, you can't do that with a locked bootloader, by definitions (the very purpose of locking the bootloader).
But don't unlock the bootloader. You can install a complete factory image (RUU) using the following process and files: https://forum.xda-developers.com/showpost.php?p=64926626&postcount=6

by upgrade i ment do an update sorry.
by recovery i mean recovery as in im guessing the bootloader screen where it says software status modified. i just used the volume keys to go down to it and pressed the power button but the screen just reloaded back onto the software statuts modified screen.
i will have a go at the complete factory image install and post back with what happens although hopefully the phone will be working again.

I see, you mean "boot to recovery" (and in your case, stock recovery). I took "load" to mean you were trying to flash or install something.

So it either sounds like you know what RUU to use (needs to match your CID and version number), or you don't and you are guessing you will find it. Not clear which is the case.

Did you find a solution to this? I have exactly the same issue with my wife's old HTC One M8, which had been left to go flat for over two years. After recharging, it came back on with exactly the same message (I didn't even have to switch it on). It's never been modified in any way, apart from scheduled OTA updates

I gave a solution. But the RUU may not be for the M8 version you have. You have to provide specific device info, for anyone to be able to help you. Lots of versions of the M8, each with different (or no) RUU.

I have not used my HTC one M8 phone for nearly 2 years.

Cool story, bro. But seriously, you didn't ask a question or tell us what you need help with.
I'm guessing it is stuck on the bootloader screen (that you typed out and posted)?

i have the same problem. My HTC One m8 wont boot into android and its stuck in the bootloader. Even if i restart the phone it wont go to android. I have a 6.12.111.4 OS version. Could someone put a 6.12.111.4 os version here, because i want to install it! THANKS

[Q] How to restore to original HTC OS?

Hi guys, i was just wondering how do I restore my htc one x to its original os? I mean, currently it is running cynanogen mod9 and I have no idea how to restore it. When I got the phone, it's already running cynanogen. the version i saw from my phone is 1.29.707 so I downloaded RUU_ENDEAVOR_U_ICS_40_hTC_Asia_WWE_1.29.707.11_Radio_1.1204.105.14_release_260743_signed
and I tried to install it but i always get an unknown error. any ideas on what I should do?
Did you relock your bootloader ?
Hi ....one thing I have learned ...is to make backups of the device before making any changes

you should download ruu for your device: in that case you downloaded asian ruu, if you're in asia it's ok, then you must relock bootloader with "fastboot oem lock" command from cmd, and then try to flash it, if no luck, try again but be already in bootloader.
and, your hboot must be lower/the same as in ruu, you can't downgrade: so if you have 1.29.707.11 or lower is ok, in other case download another ruu. you can see from bootloader the hboot version
I've been wondering how to do this as well assuming you don't make a recovery image backup. I assume you just search for the RUU for your country.
Thanks for the replies guys, I just have one more question, if I relock the boot loader, is there a way to unlock it again in case I do something wrong? Won't it give me a bootloader loop? Thanks.
in addition to this guys,
this is what i see on the hboot bootloader

Just check the actual version that your device has.
How to, see : http://forum.xda-developers.com/showpost.php?p=32070413&postcount=3
If it is 1.29.707.11, then go for the latest version : http://forum.xda-developers.com/showpost.php?p=30906073&postcount=1852
And yes, you need to relocked bootloader in order to run the RUU, and run it in fastboot mode.
See http://forum.xda-developers.com/showthread.php?t=1859714 on how to.
THANK YOU SO MUCH guys, I've finally done it. I appreciate all your help. Thank you.:good::victory:

[Q] I need to go back to stock ROM but have no idea how

Hello all. I am a complete novice at Android phones. I have a Desire HD which I bought from a friend about 18 months ago who had installed CyanogenMod on the phone. I am now moving to Giffgaff and need to unlock the phone. I got an unlock code based on the IMEI and when I enter it, despite it saying network lock successful, the sim continues as unregistered and each time I reset the phone it asks me for the Network Unlock code again. I am told it is because of Cyanogen and I need to go back to the start.
Here is all the info I have:
Android Version : 2.3.7
Kernel Version : [email protected] #1
Mod Version : CyanogenMod 7.1.0-DesireHD
Build Number: GWK74
I have tried to follow a number of guides on this site but something always goes wrong and if I am honest I don't really get what I'm doing. I downloaded an .exe from StockRoms and that looked like it was going well but then died at the end with no specific errors.
Can anybody point me in the right directions as to how to achieve this?
Thanks!

Boot in hboot and find it out....power off the device and then boot by pressing power + volume down
Sorry I could have googled how to do that before posting - my apologies!
It says this:

Am guessing this means I am not S-OFF?
I did see this screen before when I tried to use the Simlock feature. It says this when runs:
FAT32 init OK
OPEN MCCMNC file fail
Open CID file fail
Process done, reboot device?
Of course the process is not done.

With an unlocked bootloader you won't be able to flash a RUU. Besides you need a RUU for your device and not any RUU. If not, you will need a goldcard.

Is it possible to lock my bootloader? Is that a stupid question?
I have found the "right" RUU here:

Is there anything I need to do to make this work? S-OFF maybe?

Yes, it is possible to lock your bootloader. S-Off is no longer available; AAHK, the tool to get it, was retired this morning.
Thanks all, I sense that this could be tricky overall.
Perhaps a different approach is needed - is there another way to Sim Unlock the phone?

[Q] Unable to enter recovery after using dd install method

Good day all,
I am unable to get into my TWRP recovery, have read through the forum and tried a couple of methods but still not able to get into recovery.
Below the series of events that led to this happening.
27/5/15
-wanted to install CM12.1 but needed to update my TWRP as it prompted me to have an updated version.
[backup my rom into my ext_sd card
-went to TWRP web site ( https://twrp.me/devices/htconem8gsm.html ) via computer and used the dd install method.
-believe it to be successful thus went for a recovery reboot but learnt that I was unable to get into recovery mode after waiting 30+ mins at the 'enter recovery mode' screen.
-tried to factory reset but was unable to factory reset as was unable to enter recovery.
28/5/15
-googled more and tried a few more methods.
-Tried to use a toolkit to get my CID and version number but saw I did not have the version number which was blank and read that countless others are also facing the same problem.
CID = HTC__004
-wanted to unroot and revert to stock rom but unable to get into recovery to flash the stock rom i downloaded from http://forum.xda-developers.com/showthread.php?t=2701376&highlight=version-main
-Tried http://forum.xda-developers.com/showthread.php?t=2727831 (RUU to install twrp) --> the bootloader just seems to hang at the loading of the RUU file and can't get to phrasing it after 30mins of waiting.
-force to reboot phone via pressing power button and now my phone has a error saying 'system uids inconsistent', which i pressed feeling lucky to continue using.
Tried updating twrp via playstore app which still made me unable to enter recovery.
29/5/15
-nearly giving up hope

Managed to clear cache and install 2.7.2 twrp and now able to go into recovery.
However, believe I woulkd require twrp 2.8.0 and above to flash cm12.1 and the gapps as I am getting an error when trying to flash cm12.1 nightly in my current 2.7.2 twrp that says that I am unsuccessful.
Tried to update twrp via their app this time and seems to be unable to get into recovery again.
Had cleared cache and installed twrp 2.7.2 again and now awaiting your further assistance in updating my twrp.
Thanks!

Your hboot is the original that came with the phone/4.4.2
You definitely need to update. I would recommend not just your hboot but the whole firmware package. And what you listed as firmware isn't firmware, it's your ROM.
Boot into bootloader, and post the readout from it here. I, or someone else here, will be able to direct you on how to update it with that information.

HTC One M8s - com.android.systemui

Sorry for posting in the M9 section when I have an M8s but I thought you still may be able to help with my issue. A couple of days ago my phone developed an problem where it stated that com.android.systemui has stopped unexpectedly, initially this went after pressing 'ok' a couple of times but now it's just stuck in a loop and reappears every time I press ok. I've looked online and followed the recommendations of clearing the partition cache and booting in safe mode, i've even done a factory reset but this still hasn't cured it. Does anyone know where I go from here? the phone is now unusable with the constant prompt, I only got it in September and have no idea what has caused the fault. I'd like to just install fresh firmware on it via my laptop but not sure how/if this is possible. The phone is on EE if that makes a difference.
Thanks

HTC One M8 always starts into bootloader

Hello guys,
I'm pretty new to the forum, thats why I'm not sure if theres another topic as this one.
Anyway I've bought an HTC One M8 from a guy on the internet. Well, I've received it today and tried to start it. The first thing after the HTC-Boot-Logo is the Bootloader. I've tried to factory reset the phone - didn't help.

So, since I can't enter the OS, I don't know how to flash a new OS or a custom recovery. The only thing that works is the bootloader.
I appreciate any help, thanks in advance!

So, since I can't enter the OS, I don't know how to flash a new OS or a custom recovery. The only thing that works is the bootloader.
I appreciate any help, thanks in advance!

Post the results of fastboot getvar all here and we should be able to figure out the proper firmware/RUU for you to flash. Edit out your IMEI and serial# first before posting it.
Stuck in fastboot is the expected result after relocking the bootloader, as it expects RUU to be run.
Provide the getvar details requested by xunholyx in the previous post, and we can direct you further.
If no RUU for your version, the other option is to unlock the bootloader again, install custom recovery TWRP, and restore a stock TWRP back (or alternately a custom ROM).
But we need more info in order to determine which way forward, what files to use, etc.

Hello guys,
I'm pretty new to the forum, thats why I'm not sure if theres another topic as this one.
Anyway I've bought an HTC One M8 from a guy on the internet. Well, I've received it today and tried to start it. The first thing after the HTC-Boot-Logo is the Bootloader. I've tried to factory reset the phone - didn't help.
Some other information:
Software status: Official
RELOCKED
M8_UL PVT SHIP S-ON
HBOOT 3.19.0.0000

run the ruu maybe this can fix your problem . if you flashed ruu and still facing the same problem maybe its your phone battery problem or board issue try to put it on charge then try to turn it on again .

The advice (run RUU, if his version has one) has already been given. It's not value added to give that advice again, until we have more info about the device (getvar data already requested twice); since the OP gave basically zero specific details.

Okay, I tried to flash TWRP and to unlock the bootloader again. nothing worked. And if I want to get more information with getvar there is nothing showing up - it's just loading forever... So I decided to change the Motherboard of my HTC, and well.. Now it doesn't work anymore... But still: thanks for trying to help guys!

Wow, you were probably not justified in trying to replace hardware; and probably an overly hasty decision. It was probably a fairly simple matter to recover via software related solutions. You probably were just not following the steps correctly.

Too late now, it looks like you ruined the phone. But FYI, you can't flash TWRP with a locked bootloader; it's simply impossible. So unlocking the bootloader should have been your main focus.
Fastboot not connecting is usually an easy matter to troubleshoot. Usually a matter of drivers on the PC, or a different cable, different USB port on the computer.
You also can't unlock the bootloader without fastboot. So that was probably the reason why your attempts to unlock the bootloader failed. Again, it probably would have been easy to get fastboot working, then either RUU (after giving us the getvar info); or unlock bootloader and install TWRP.
But probably too late for any of that, now.
