Updated stock M8 to 6 - red triangle exclamation - One (M8) Q&A, Help & Troubleshooting

I'm trying to fix my HTC One m8 phone. I tried updating it to the latest software update and I am now stuck on an infinite loop with a red triangle and exclamation mark.
I've never done anything to the phone (it's completely stock as far as I'm aware) and I've only ever updated via the settings menu.
I'm really not sure what to do from here.. I'm completely confused as I've never rooted a phone but it's now useless. How can I get it to work again? From the internet I've worked out how to get to the menu.. which gives me the following options:
Reboot system now
Reboot to bootloader
Apply update from ADB
Apply update from SD card
Wipe data / factory reset (I tried this option.. didn't help)
Wipe cache partition (tried doing this.. didin't help)
Power off
it also says:
handle_cota_install: No CW files.(There is other text but it goes off screen)
When I went into bootloader it says software status: official then ***LOCKED***" also S-ON
Does anyone have any idea on what to do?

[Q] Desire stuck in bootloader loop after hboot downgrade - HELP!!

I have serious problems with my phone that I could really use some help with as at the moment I'm completely screwed - I've already spent hours reading forums but have yet to find advice that will help. Here's whats happened:
I have an A8181 HTC Desire, and I took an OTA update yesterday that bricked my wifi. At this point I thought it would be easiest to root my phone, and after checking that my hboot version was 0.93, I read advice that said to downgrade this to 0.83, which I duly did.
Now, my phone is stuck in a bootloader loop - I get the 3 androids on skatebooards with FASTBOOT in red, and I can choose from:
BOOTLOADER
REBOOT
REBOOT BOOTLOADER
POWER DOWN
If I select bootloader I then get:
FASTBOOT
RECOVERY
CLEAR STORAGE
SIMLOCK
I can access recovery and apply update.zip roms successfully (I've only tried official HTC 2.2 roms as the phone isnt rooted), but after doing so it just goes back to the bootloader loop.
If I select FASTBOOT then I'm just back at the first screen.
I've tried clearing cache and factory reset aswell, but I still can't get past this 2 screen loop.
It's like the bootloader just can't load whatever rom is installed.
Because the phone isn't booting up, I can't use adb or any other pc tools as the device is not detected on my pc - although it does show up in device manager on windows.
I'm close to sending this back to try a warrantly claim, although I'm worried that by trying to fix this I may have invalidated this.
I'd really appreciate any help or advice as to how I can get out of this and get my stock 2.2 back.

If your computer using windows, have you installed hboot drivers?
If you have those installed your pc should detect the phone.

You shouldn't have had to downgrade your bootloader. If you can, go back to the newer.
The wifi issue, was widespread on desires, I had it also after a "pushed" update from HTC (european model, unlocked). I downloaded a stock 2.29 froyo. Had to make a gold card also. Took several tries, wipe cache, but eventually successful.
Then, I rooted, and went to CyanogenMod7 rom with clockwork recovery.

[Q] [HELP REQUEST] E: Can't mount /cache/recovery/command

I hope i'm posting this in the right section but I'm having a major problem. After draining the battery on my Gnex, I brought it home and put it on the charger. It came up with the battery charging symbol for awhile then when it booted it showed the Google splash screen for about a minute. Then rebooted, showed the google image again and went into CWM Recovery with the following message.
E: Can't mount /cache/recovery/command
E: Can't mount /cache/recovery/log
E: Can't open /cache/recovery/log
E: Can't mount /cache/recovery/last_log
E: Can't open /cache/recovery/last_log
When I tried to wipe data and cache and factory reset it comes up saying it's completed but then says the same error message as above underneath it.
I've tried full factory reset with the GNex toolkit and still nothing.
I also can't mount the cache in CWM Recovery, it just says Error mounting /cache!
I'm at a loss at this point. I hope someone knows what to do. It seems that if I can get into fastboot and CWM Recovery there should be a way out but I can't find one. Thanks in advance for any advice you guys have.

try flashing CWM again? wiping dalvik or fixing permissions perhaps
OK, clicking Wipe Dalvik gets me to the Click yes to wipe Dalvik but also gives this error:
E: unknown volume for path [/sd-ext]
But then when I click yes it says Dalvik Cache wiped.
Fixing permissions just says Complete with no problems.
Reflashing CWM completes fine but still the same error as before when I try to boot.

Just a slight update to the problem. It appears that no matter what I do as far as using the tool kits to return to stock or reflash stock images, CWM recovery is still always there and the Bootloader version also doesn't change. I've tried both 4.0.1 and 4.0.2 and the bootloader version is always PRIMEKL01.
I'm assuming if there was a way to get into the sdcard memory from windows with fastboot, that would be the solution to my problems but searching for if that's even possible has turned up nothing :-(
I think i'm genuinely screwed at this point.

If you flash cwm then you HAVE to boot into Android at least once before entering cwm or it will give you the error messages. Just boot into Android, then enter cwm and everything will be fine. This is explained in my ToolKit option for flashing cwm.

Can't boot into android whatsoever. If I hit power like I normally would it shows the google image and the little unlocked lock for about a minute. Then reboots, shows that again and goes right into CWM with the error from the first post about not mounting /cache

Sorry I think I jumped the gun when replying originally. Cwm will not mount properly until after you have booted into Android at least once after flashing it so thats out until you have fixed Android. You cannot access the sdcard from fastboot it needs adb to push/pull files which in turn needs your phone booted into Android.
What Android build is your phone on? Is it stock or custom? What exact modifications have you made to the phone in terms of flashing roms/kernels/radios/bootloaders?
Can you enter fastboot mode?

So you just restored a Stock Rom to your phone via odin?
If so does it still not boot into Android?

Have you used the GNex ToolKit to flash back to recovery (using that method) already? Im not sure what odin flashes but I know exactly what the google stock image flashes.
You could try relocking the bootloader, then unlocking it again which will force a full wipe of you phone and may fix something.
You mentioned that the bootloader version doesnt change after flashing but it wont as its the same version across the different stock builds.
If you flash a google stock image via the ToolKit (option 8 and download *THIS* image to use if you have an LTE Model) and let it reboot then see what happens. The procedure will flash the stock recovery back to your device so keep a note of what happens after it reboots and if it ends up back in recovery as it did before then it will be a screen with a little android man with a red triangle in the middle.
If your phone still isnt booting after all that then maybe you need to consider relocking the bootloader and sending it back under warranty.

OK so I tried unlocking and locking. No change. Did the GNex toolkit option 8. Completed successfully but it didn't reboot the phone afterwards. Just stayed on the fastboot screen with this on the bottom: Fastboot Status - OKAYPRIMEKL01 highlighted in blue.

So it didnt reboot after it finished flashing?
Did you try pressing the power button when the top of the fastboot screen says 'START' to force the reboot?
The last thing I can think of (if the above didnt do anything) is to try booting the phone with one of my insecure boot images to check if theres a problem with the boot partition:
Enter fastboot mode on your phone and start the ToolKit (v5.3)
Choose option 20 to open a new command prompt
In the new command prompt type
fastboot-toolkit boot root\boot_lte_ICL53F_insecure.img

followed by return.
This will attempt to boot the phone using the insecure boot image of the ToolKit.
What happens with that (exactly the same as before or anything different)?

OK, I tried that and it completed. Rebooted the phone but unfortunately same scenario. Boots twice then into CWM with the same list of errors mounting cache. :-\

One thing I should mention is when I say I unlocked and locked the bootloader and it didn't change, I mean it stays unlocked no matter what I do. It'll come up and say its locked or unlocked but as soon as the phone reboots its always unlocked. It seems like whatever is happening nothing can actually write to the part of the phones memory it needs to so nothing sticks. If that makes any sense, i'm just guessing.

So even though you said you flashed the google stock image earlier it still has cwm on even though it should have been overwritten with the stock recovery.. very wierd.

All the procedures must be completing (at least in the eyes of your pc and phone) or you would get error messages from the ToolKit when flashing. As you said before its like nothing is being written to the phone even though it says it is.
One thing is to try flashing a newer bootloader in case thats been corrupted in some way (if we can).
Download the latest bootloader *HERE* and extract the .img file from the downloaded zip file. Copy the PRIMELA03.img file to the 'put_img_files_to_flash_here' folder in the ToolKit and flash it using option 12. Make sure you select FLASH (2) and flash it to the BOOTLOADER partition (2).
After the flash fastboot should restart and if it went on correctly then the Bootloader version will now say PRIMELA03.
If it changed then try reflashing the stock google image as before. If it didnt change the bootloader then I really am out of ideas.

I tried that. Says flash complete both on the fastboot screen and GNex Toolkit but when it comes back on the bootloader version doesn't change. Still PRIMEKL01. I can't believe it but I think this is my first truly bricked android phone. If only there was a way to get into the memory of the phone through fastboot or odin or something maybe we could repartition it somehow. But it seems like the parts of memory I need fixed are just completely inaccessible at this point.

Unfortunately, it goes through all that saying its complete but when it boots back up nothing changes. CWM is still there. Bootloader still unlocked.

HTC One M9 Marshmallow - Tried to get rid of red text, now only boots to bootloader

Hi, recently I converted my international HTC One M9 to Developer Edition so that I could receive the Marshmallow OTA. This worked and I had everything setup, but whenever I booted the phone I was shown a red warning text. After surfing the internet for a while, I found a file to flash through ADB that should fix the problem. The file stated that it was for the Developer Edition, so compatibility should have been fine. Unfortunately, the first time I flashed it, it said error, please flush immediately and so I flashed it again. The second time it took much longer and then said it had been a success. However, when I went to reboot, it went straight to bootloader, and from there I can access download and recovery, but rebooting only takes me back to the bootloader. I have tried the hard reset key combo to wipe the cache and also 'fastboot erase cache' but it is still stuck.
My CID and MID is BS_US001 and 0PJA11000.
Software status: Official.
Unlocked and S-OFF.
Any help would be greatly appreciated as this is my daily driver and I need it online soon . I will probably try a factory reset and if that doesn't work maybe RUU it again

Just tried a factory reset and it went straight to bootloader so I have now started to RUU it again using 0PJAIMG in the SD Card. Fingers crossed :/
i did flash dev ed 2.11 and then OTA all way to Dev 3.35.617.12 MM, i will try on the afternoon to root with twrp 2.8.7.1beta and SUperSU 2.65.
just flash 3.35.617.12-Firmware-noredtext-aboot-only.zip if you are right now on this firmware , you will get the msg (90 pre-update FAIL) do not worry about this MSG as this is actually a success msg . all you have to do after that is to just use this command (fastboot reboot) . all done your m9 device will reboot into normal mode with no red text warning.

unlocked bootloader before rooting
after unlocking bootloader, its now stuck in unlock successful reboot in 5 seconds loop if i try to access bootloader
cant root for the app used to just flash a rom on because of the 5 second loop
i downloaded the WW_ZE551ML_2.15.40.13_20150506 raw file to abd side load (thinking this was what i needed to fix everything
cant apply anything because of the 5 second loop
phones current state
all cache and data wiped and just redid all the new settings and unlocked developer

update: new issue: reached to .156* and tried using twrp to apply the superzen.
android goes to recovery and then shows the sleeping android with the red triangle so twrp cant complete its steps to install superzen

M8 appears bricked no rom or recovery menu

Tried to remove cynagenmod from my htc and return to stock android settings and messed up somewhere along the line, on powering on my phone it loads straight into the boot loader, navigating to >HBOOT then pressing >RECOVERY makes the HTC logo appear for a moment before returning to the first bootloader screen.
[software status modified, re locked, security warning, M8-UL PVT SHIP S-ON]
The rom has been deleted along with recovery, I am not an expert in using fastboot tried using it for a bit and had no success also tried installing htc sync manager to see if it would help but the installer for the sync manager would crash my computer every time (windows 10). Looking for some help as what to do not sure if it can be fixed or should I bin it thanks.
I would RUU if it was me in your position.

To properly troubleshoot where you went wrong, and how to solve it, you will need to describe the exact steps of how you tried to return to stock.
It seems you wiped things in recovery, expecting that to return you to stock? If so, that isn't the case as you now know, without flashing a ROM, TWRP backup, etc.
Similarly, relocking the bootloader renders the phone unable to boot, and wipes the phone; as it expects RUU to be run. But you haven't explained if you were trying to run an RUU, which one, details of your phone version, etc.
