OK, so for months now I have done everything from buying a new cable, battery, fully erasing both internal and external (I have a card reader I can put my micro SD card in), reformatted everything, tried using other computers to connect the phone, and the list goes on.
I never thought about this until lately, but is it possible to put a tar ROM file on my micro SD card and install it by adb sideload it in recovery? My phone has just been having too many glitches and I'm sick of it.

I have the same problem with my note 4 and I build cm13 for our device so imagine how bad it pisses me off but anyways the only fix is buying a new micro USB port for the note 4 cuz something malfunctioned with ours and there's no way to fix it.

It's possible. However, before you do that, take the card out ant put directly into a card reader for a computer. This will let you know if the card is bad, or maybe the phone's reader is bad. If the card is bad, you can try error checking it in Windows, and maybe salvage the data on the card.
But if that doesn't work, try re-formatting it. It may help, it may not. If not, new 8gb cards are only a few bucks now.

It very well could be your USB. Use an adapter as posted, then try another USB.

Over the weekend my phone has decided to not read my sd card anymore. I copied the whole card to my external drive and formatted it via the computer twice, I booted the phone back up and everything on my card was still there. I used the option in my rom (ILWT) to erase my memory card, no luck. I then formatted the card via cmd prompt today which didn't work, and then my last shot in the dark was to reformat the card via 4EXT recovery. My phone still gives me the "insert sd card" when I take a photo and all my of files are still on it. Anyone have an idea of what happened?
the sd card slot probably is bad if it won't work in recovery or a rom, could be the small cable that connects it or the tray it self. about 10~15 on ebay and a 10 min fix

Yup, card still has everything on it.
It seems more like a broken SD card to me. Have you tried another one?
You can also try some low level formatting tool like HDD Low Level Format Tool (from hddguru.com).

Have you attempted to copy off everything on the card, and then doing a full format of it on the computer (including maybe low level formating).
IF that still doesn't work, try another SD card, and if that doesn't work, I suspect the slot itself is bad.
Every time I flash anything based on the official JB 4.1.1 release, my 32GB MicroSD card (Patriot brand?) freaks out. The phone will say it's "unexpectedly removed" and then adds it back in, and removes it, over and over.
It's happened on all three JB 4.1.1 ROM's I've tried.
I thought my MicroSD was going bad so I low-level formatted it in Windows (hated doing this) and still had trouble. But ONLY with the card in the phone. Windows usually liked the card just fine.
So what I did to resolve it was use Titanium Backup Pro to "freeze" the "MTP Application". After doing that, my MicroSD works great in the phone and AFAIK, I haven't lost any functionality. So I'm not sure what the "MTP Application" is or does, but so far.... so good.
This post is just a heads up to others, in case anyone else runs into this problem.
Or in case anyone wants to tell me WTF is wrong.
Ive had this issue as well twice. Both times I had to format the sd card in my pc and it will work fine for a while. Im curious to know if you are using es file explorer? Im thinking it might have something to do with this issue.

Hmm come to think of it I just recently purchased root explorer about the time all this started but surely an app couldn't cause this issue? Heck I dont know it happens so random it hard to pinpoint the problem thats causing it. If I figure it out I will let you know

Ahh hah I duplicated the problem on my phone anyway. I used the my file app to move a rather large file from the internal sd to my external sd and afterwards it lost connection. I had to remove the sd card and install it several times to get it to mount back again. Im not sure if its the moving of the file or the my files app itself that is doing it. I also noticed the media scanner isnt running after moving the file. So I hadnt pinpointed the exact cause but im dang sure getting closer. And I dont think its a bad sd card because I tried another and it did it with it too

Yea it happened with MTP frozen and unfrozen. Ive tried freezing the my files app and every file explorer on my phone. I just cant put my finger on the cause it happens so random. But it seems me and you are the only ones having this issue so im wondering if we didnt get a bad flash when we updated. When I updated via kies it did not do a full wipe update it did what is known as a dirty flash which I never do because you end up with issues like this. So if we continue to ask each other questions we will eventually figure out what the common link as far as if its an app we are using and things like that.
So did you update via kies or odin?
If you updated via odin where did you get your firmware file file from and if you used odin was it odin on the pc or mobile odin?
And was your update a full or a dirty flash?
We are gonna figure this thing out one day lol.....
Edit.... well apparently we are not the only ones having this issue. Over on droid forums there are several s3'owners most on t mobile but anyway it appears there are a few phones out there with this problem and the only fix is to replace the phone and sd card because the phone ruins the card by unmounting it without stopping it first. My phone is still under warranty so I will be calling ATT tomorrow for them to replace my phone and throw a fit for them to replace my micro sd. I paid 48 bucks for this card and I think ATT should replace it since their faulty phone ruined the card

I had the problem with two or three different ROM flashes. So it wasn't just one ROM flash.
Each time, freezing MTP Application has resolved the issue for me.
I'm going on ~4 days now with my current ROM and no more issues since freezing MTP.
Maybe there are a couple different SD card related issues out there.
I had the same problem with ADATA and Kingston brand sd cards. I tried the Sandisk 8GB from my old phone and no problems so far.
Freezing the mtp app has seemed to slow it down. The only problem with freezing mtp is that is how the phone mounts to the pc. Mine just did it again but I unfroze mtp to hook my phone to the pc and right after I backed some stuff up and unplugged it, before I could freeze mtp again it unmounted the card. Ive also tried several different sd cards and several different sizes even tried a 2gb Samsung card with the same results. Im thinking hard about going back to ics to see if that will resolve the issue. ...

I found that my 64GB card was not recognized by the newer kernels - when I use the older kernels or stock, it is recognized, otherwise, the ROM attempts to read the card, and then states it is 'ready to be removed'.

Try reformatting the 64gb card to a fat 32 file system. 64gb cards come formatted exfat and some s3's have had trouble reading them. Put it in a pc into the card reader and open my computer. Then right click on the drive thats showing your card. Ex drive E or F. Mine is drive N. Once you right click on it click format and a box will pop up and you will see the box that says exfat. Click the drop down box and change it to fat32 then click format and your done. Once it finishes put it in your phone and it should recognize it...

I have just purchased 64 GB Class 10 Samsung micro SD card with a SD adapter and USB adapter.
I plugged it in to my S4 and everything seemed OK. Copied all of the contents from my old SD. Everything was working fine until I restarted my phone. After restart all of the files were present by could not access them. Some of the files and folders had ~ as prefix. Plugged the card in my PC and the files were corrupted. Recopied everything and tried several times again. The same outcome. Reformatted the card to exFAT and to FAT32 both from my phone or my PC using several available programs. No success. Whenever I unmount the SD card or restart the phone the files get corrupted. So I figured the card is broken. So I decided to test it only on my PC. Copied lots of stuff to it (music, movies etc.) and started using it on my PC. I have mounted/unmounted it on my PC probably 100 times and it works just fine. So that means that the card is fine (or am I wrong???). Also tried it to plug it with the usb adapter into my TV. It says that usb disk is empty.
The files only seem to get corrupted when I place the card in my S4 and unmount it/restart the phone. The card works just fine on my PC.
Does anyone else have this issue?
I am running a rooted 4.3 Android OS.

You have to partition the card into 32GiB chunks. So make two, 32GiB partitions and it will work fine in your S4, except it will show you have two external sd cards.

If you don't want to make two seperate partitions on your external microSD card then do the
following:
Remove your external microSD and plug it into a card reader and or the adapter which came
with the card then plug it into your computer.
Make an empty folder on desktop of computer and copy all files and folders from the microsd
card into the empty folder.
Format your external microsd using this windows utility and copy all the files/folders back onto
the card from your computer and replace it into your phone.

By doing the above your external microSD card should work fine in your phone.

Why does the card need to be divided in 2*32GB in the first place? Isn't the S4 suppose to support 64 GB cards?
Your suggestion seemed to isolate only part of the problem. It remains only the restart. What can be associated with corrupting data on sd card and android restart?

Thank you for your reply. Unfortunately it didn't work. I tried similar thing with other software also (Minitool partition wizard, Panasonic SD formatter, etc.). When I unmount/mount or restart data corrupts

No, it depends on your kernel if it supports a certain FS. I am not sure why your files are being corrupted other than one, your ROM isn't unmounting your card properly on restart or two, your card has issues. Again, I am not sure.

All the stock S4's come with the same kernel, so if your S4 is stock then it's not a kernel issue.
Have you tried any other external microsd cards, if not that's what I would do next.
Most of the better microsd cards have either a 2-3 year or a lifetime warranty.

I currently use my old 8GB sd card with no problems whatsoever. My S4 is rooted. Does that change the kernel? Should I unroot?

Been working fine for over a month now all of a sudden the SD card appears to be randomly mounting/unmounting itself and has become unreadable on my phone.
I've put the SD card in my PC and it works fine so I suspect my phone has developed a fault. I hope I don't have to get an exchange as it's otherwise faultless and I'm dreading getting a unit that can't be S-Off'd
I haven't modified or installed anything prior to the fault developing,
Any tips would be most welcome.
PS It's a genuine Transcend Premium 300x 64GB bought from MyMemory.

well, a simple solution is to enter recovery and see if the sdcard works there if this way it works, change the rom/perform factory reset, if this doesn't, well, you might try to flash everything again, but i's possible that the sd slot is broken...

I would copy EVERYTHING off the card to a PC and do a format and try a different file system. I would has perform the format on the phone, to make sure its gets formatted to the correct file systems.
Long story short, It sounds like you memory card is starting to get Corrupted. I have to SanDisk cards that started doing that in the biginning and ended up not being read at all on PC nor the Phone.
Also while its doing that, youll also notice the battery start to drain more, at leasst thats what happended on my Note 3 when I had the san disk cards going corrupt.
Ended up buying new Card and now everything is fine.
I doubt is the card reader built in phone, more then likely its your memory card going corrupt.

I agree most likely the card. I'd download an sd card format tool and do a full format, don't use Windows format then do full check for bad sectors. I had a similar problem and it was the card at fault, but strangely worked ok on the pc.

I have been through many a Samsung phones and always had corrupt sd cards in exfat. One day I saw a thread that said convert to fat32 and have never had a problem since.

I could read the card in recovery but it failed to format there.
I tried to format in windows but my only options were NTFS or exFAT, no FAT32. But this failed anyway also.
Downloaded a SD format application but this said the card was write protected and to move the lock switch, obviously MicroSD has no switch like standard SD.
So next I deleted all the files on the card and ran h2testw test. This failed and reported card is probably faulty.
Then I checked the contents of the card again and all the original files were back again, so it's definitely 'Donald Ducked'. I was able to get all my files backed up and fortunately I had my photos already thanks to Dropbox & G+.
So I will now look at getting MyMemory to replace it but my fear is they will want me to return the faulty card but it has sensitive information on it that I can't delete. Any ideas on how I can wipe it?

How did you attach the card to the pc? Try a usb card reader and then try again. If still problems download mini partition tool (Google) and delete the partition. You can then wipe it properly.
I reformatted the SD card from the settings within the phone and since then it been working fine :\
make sure you're using 2.0USB also..
when i connect my phone to charger
Its been few days i am noticing when i conmect my phone to charger it says 'sd card removed unexpectedly or either it suddenly stops showing content from my phone, then i have to go to settings unmount/mount the sd card frkm storage option and then it shows the content even when charging, or sometjmes it doesn'fshow option for that so on that case i have to restart my phone. What could be the possible reason and solution to that. It was fine on 4.4.4 now i am on 5.0.2 all stock htc one m8 h/k edition with sim unlock and s-off via sunshine.
