I think in all the flashing I kept the phone plugged in while rebooting into CWM.
As stated in the thread:
Warning Do not keep phone plugged in while rebooting into CWM, phone may loose the ability to boot into cwm, reinstalling the root and CWM does not fix it. Also xparts does not boot inot CWM. full wipe is needed.
Can anyone help me with this? How do I perform a full wipe? I tried to wipe with fastboot, also tried
to flash another stock image with flashtool. How can I get cwm recovery back to work.
I had the same problem a few days ago. After flashing the existenz ultra update I kept usb plugged in and recovery was completely gone.
I reflashed .67 stock rom via flashtool.

You have to close flashtool BEFORE you use DooMLoRD_Easy-Rooting-Toolkit_v18 !
After the root, I flashed NUT's "lockeddualrecovery2.6.3-RELEASE.windows.zip" : http://forum.xda-developers.com/showthread.php?t=2288118
and everything worked fine again.
For the rooting process you'll need "unknown sources" and "usb debugging" checked.

i unlocked bootloader on my nexus 7 2013 and tried to root it following this guide http://nexus7root.com/new-nexus-7-root/how-to-root-new-nexus-7-2013/ im running android 5.0 on it so i used the supersu 2.30 like it said once it started to reboot it got stuck in boot animation ( 4 colored dots swirling) abd i tried wipeing everything i tried reformat i tried factory reset im at a loss please help i have twrp on it and can get into it ido not have usb debugging turned on yet the device registers in device manager as nexus 7 but wont show up in libraries ( i run win7) i have downloaded a stock rom of 5.0.1 for my device ive downloaded android sdkive flashed i just rewiped dalvik, cache, data, system, and factory reset then went to reboot the system it said no OS the only thing on it is twrp atm
Flash the 5.0.1 firmware in fastboot. Then try to boot into system. If successful, try to gain root.
If it is recognized in the bootloader, use the factory image from Google for 5.0.1. Once you've confirmed you can boot, put it back in bootloader and run Chainfire Auto Root.
Hi there. Here I have my t989 with stable cm11 by Sultanxda and twrp 2.7.0.0 installed.
Everything was fine until I wanted to try 5.0.1.
I got into twrp and did the usual wipe and install ParanoidAndroid 5.0. It failed.
Ok stuff happens so I just went restore my backup no big deal.
Now when I reboot back my phone looks as it were before BUT it reboots after 2-3 minutes in every time it boots back up.
So I thought the power button has problem but soon I figured it is not because my phone won't self reboot in recovery and download mode.
I went back to twrp and try to wipe it clean and install cm11 instead of restoring to my previous backup.
Wipe says successful, install says successful, reboot system. And only to find myself still in my old Rom and nothing was wiped.
All my apps are still there blahblahblah. I tried to update my twrp through twrp manager but phone reboot before it finishes installing.
I grab my odiin and try to update recovery there. Odin says success. Goes to recovery only to find its still in 2.7.0.0
Download a stock .tar and flash it with Odin. Odin says success, boot up and still in my old cm11 rom...
Now I went out of option but ask for help. I have done flashing for 3 years with this phone and I have never gotten this problem before.

yesterday my beloved galaxy nexus started rebooting continuously. I did not even touched it. The loop is continuing still now without any reason. Usually, in this occasions, I've performed a clean/hard reset (I don't know the correct term), wiping system, data, cache and dalvik partitions, flashing a new rom and bla bla..
The very strage thing today is that I'm not able to do that. I'm using the TWRP custom recovery: if I wipe something or perform a factory reset (standard in twrp) it starts the process, gives me at the end the advice that everything has gone well, it even allows me to flash a new custom rom and gapps but when I reboot, nothing has changed. It reboots in the previous system, nothing erased and again with the reboot loop.
I've just tried also using fastboot interface. Any command I launched performed its task, the phone gives a positive ending but when I reboot again, it's like nothing happened.
Specs: Galaxy Nexus Maguro, GSM version. Actually running a 4.4.4 rom (SlimKat) with its own kernel. A lot of customization (writing/formatting) in the past 2 years.

My phone reboots in 10 sec after he boots. I can not install flashify and not even flash cwm recovery.

I'm having the same problem here, it seems that It's not saving any change I try to make, as you've described it says that every task was performed ok but after rebooting it has all my apks, data etc. even afer wiping.
The only thing I can make to keep it ON, is to enter in SAFE MODE, then I can save my pictures and data but I can't flash anything (well I can but it make no change).
FACTS:
-If you start the phone and you activate USB debugging, after rebooting it's not active anymore
-It shutsdown after 10 secons entering in a boot loop (unless you use SAFE MODE, that way you can keep it on)
-If you wipe data, wipe cache, flash boot, flash bootloader, flash system, unlock boot loader, root it, restore, flash stock + unroot (using nexus root toolkit v2.0.5) or use any flash option of fastboot via command line including update, you will get task perfomed ok but nothing is changing; after rebooting It shutsdown after 10 secons entering in a boot loop and all my data including APKs, pictures etc are still in the device.
-You can boot in Download mode, Recovery mode

I had some old lineageOS installed. So, yesterday i got into a bootloop with my old S4 mini and the first thing i did was to reboot in Twrp recovery and wipe cache and dalvik but i also wiped data and system. Now my phone is in state were it can only boot in download, odin recognizes it and i am able to flash trwp but i cannot boot into it. Adb doesn't recognise my phone. Well, maybe it did once but no more.
I know i probably erased something i shouldn't so thats why i cant even load into recovery although Odin shows it got flashed properly. But thats not my problem. What i cant find are instructions on how to wipe everything and start installing from zero, just like i did when i installed LineageOS years ago. Currently i am downloading the stock Rom, if i manage to flash it with Odin then i can continue from there.
