I've been working on a fix since a long time now. If you are in SD community then you prolly know me. Anyway I was able to fix this on two of my phones and it's now working fine. I tested for 12 hours now. I'll also tell you in the end what problems I'm seeing. I'm also working on a fix for that.
I noticed that a service is started when the fallback happens but if I kill that service the network also dies. But if you follow the below method it safely starts a new service which kills the one that's responsible for killing the 4G/LTE
1. Upgrade your phone to 13B firmware
2. Root 13B
3. Now download Shortcut Master lite app from play store.
4. Now add these shortcuts to your favorites in the "Shortcut Master lite app". You can search for "LTE" and click on search button
LTEBandSelection
LTETestMain
LTE System Selection
CDMAPreference
5. Now click on "LTEBandSelection" and enable all the bands
6. Save and reboot
7. You now need to enable your data and turn off your wifi. Make sure your phone is in Global mode (in settings >> Mobile networks >> System select)
8. After your phone is rebooted open "Shortcut Master lite" app.
9. Now tap on LTETestMain
10. Select Click on CDMA preference.
Now choose Band preference and select "any"
P_REV_Preference ---> "MOB_P_REV 3
This is important.. EVDO Revision --> Rev A
11. "LTE Connect w/o IMS PDN" must be checked
12. Now go back to your main menu in shortcut master lit app
13. This step is very important. Changing the timeout is important. Tap on LTE System Selection
Except "Scan duration" add a couple zeros to all the other timers. For eg: T_mpsr timer (Min) is 200
The fallback timer will reset itself after reboot. But don't worry the configuration is already updated with the new value. For some reasons the shortcut master lite app is not picking it up.
14. Now finally click on "CDMA preference" in the main menu and make sure it has the value you set in Step 10
At step 10 or 13 your network should go away and your phone should complain about "no network". That's fine. just ignore the message.
Now finally reboot your phone. That's it. You should now have 4G LTE and it won't fallback to 3G or H+
The only problem is when I'm on a call my 4G LTE is dropped to 3G/H+. But after I hang up it goes back to 4G LTE after 12 seconds. I don't know if it's an expected behavior. But I'm trying to see if there is a fix for that.
I tested this with Tmobile. Make sure your APN settings are correct.

I experienced a similar problem with my Diamond I purchased on the weekend. It was a Telstra NextG model that was supposedly unlocked and able to work on all networks.
It worked with my Optus SIM as a phone but I couldn't get any Internet connectivity.
I ended up flashing it to a cooked ROM which auto-detected the Optus SIM on first boot and configured itself automatically. Internet is now working without problem.
FYI I'm now using "Dia Druitt v05" ROM and am very happy with it. Original ROM version was 1.38.
Battery life is not great but I have just changed the radio from .06 to .bs14 and will see if that makes any difference.

You should be able to download the optus/vodafone rom from HTC using the Telstra Next G diamond serial without any issues.
Otherwise, here are the steps to manually input them:
For Internet:
1. Tap on the Start button.
2. Tap on Settings.
3. Tap on Connections.
4. Tap on the Connections Icon.
5. Tap on Add a new modem connection under My Work Network.
6. Enter Optus WEB in the Enter a name for the connection field.
7. Select Cellular Line (GPRS) in the drop down list of Select a modem.
8. Tap Next.
9. Enter internet in the Access point name field.
10. Tap Next.
11. Enter in the User name field or otherwise leave it blank.
12. Enter in the Password field or otherwise leave it blank.
13. Tap Advanced.
14. Select Use server-assigned IP address.
15. Tap OK.
16. Tap Finish.
for some pdas it is compulsary to set proxy for internet connetion if so in step 9 instead of "internet" put "wap.optus.net.au" and for proxy setting put "202.139.83.152" and port "8070".
For MMS:
1. Tap on the Start button.
2. Tap on Settings.
3. Tap on Connections.
4. Tap on the Connections icon.
5. Under My ISP, tap on Add a new modem connection.
6. Enter Optus MMS in the Enter a name for the connection field.
7. Select Cellular Line (GPRS, 3G) in the drop down list of Select a modem.
8. Tap on Next.
9. Enter mms in the Access point name field. Tap on Next.
10. Enter in the User name field.

12. Tap Advanced.

22. Enter 61.88.190.10 in the Gateway field.
23. Enter 9201 or Permanent, non-secure in the Port number field.
24. Enter http://mmsc.optus.com.au:8002 in the Server Address field.
25. Select My ISP in the Connect via drop down selection.
26. Select WAP 2.0 in the WAP Version drop down selection.
27. Tap on OK.
28. Tap on Set As Default.
29. Tap OK.
If setting above doesnt work change the port address on step 23 to "8070" then try again.
PS. I just did a search on google for "Optus MMS settings" and copied and paste those instructions.
Optus Rom Update
I've got the same problem. My Touch Diamond is from Telstra, but I'm with Optus. I can send/recieve sms, but not mms.
Can someone post up the latest rom for Optus users? I can'd download it from the HTC website as the serial number on the phone will only download the Telstra one.
Hey guys, I'm really sorry to bring this up again (after all this time) but I'm having issues getting the optus mms to work and I can't for the life of my work out why. I'd tried using all the suggested settings and it still refuses to work (the mobile internet is and has been working flawlessly for some time now).
I was just wondering if any of you guys actually managed to get the mms working or if you all just ended up flashing with a cooked rom.
I had a problem accessing 3G when installing a new rom. After flashing a few roms and finding out that roms were not the issue, as well as applying a few radio updates and changing radios and a google on the internet, I found out what was the problem. The access point was not set up. Defrost's rom based on cyanogen mod had the massive APN List preloaded, hence I never ran into that problem. However, most roms did not.
Here's how to set it up
Creating a new access point
When you need to add another data connection on your phone, obtain the access point name and settings (including username and password if required) from your mobile operator.
1) From the Home screen, press MENU, and then tap Settings > Wireless & networks.
2) Tap Mobile networks > Access Point Names.
3) On the APNs screen, press MENU and then tap New APN.
4) On the Edit access point screen, enter the APN settings. Tap an APN setting item to edit it.
5) Go to this site and find your mobile network company name
6) Modify the settings as said
7) Press MENU and then tap Save.
You should now have working 3G!

I thought you already had a thread on this and a mod said to post this in general?
If you think its apn problem, install the rom it works on and then download apn backup and restore. Use that to backup apns and then when you install a rom you want, use the same app to reinstall the apns you backed up.

My friend Jeremy helped me with getting rid of the "Hands Free Activation" pop-up and I thought I would share it. It especially comes in handy when you are flashed to another carrier. If you are on one of the new custom ZVB Roms you probably don't have to worry about it. All you have to do is:
1) Get a Root File Browser .... I like Rom Tool Box
2) Go to "System" folder
3) Go to "App" folder .... (not Apps)
4) Scroll down and find "OmaDmclient.apk" and rename it to "OmaDmclient.bak"
5) Reboot and you should not get that aggravating ass pop up any more

No Probelem....The guy I use to use to flash phones figured it out....he was the only one (entire USA) that knew how to flash the OG to Verizon and get full data (3G). He quit flashing phones from where he was living so now I can't get in touch with him...he could flash any freakin phone..

With Page Plus and Straight Talk you have to have certain APN's set. Is your APN correctly set for Page Plus?

I found this in my files, used it long ago and am not sure if it still is applicable but there may some information in it that may be able to lead you in the right direction......

3G data speeds (HOW TO)
This guide is to get evdo data or 3g data speeds on phones flashed to page plus cellular this is not a beginers guide by no means this guide is assuming both spc codes are set to 000000.
things you will need:
1.A donor phone of your own that will ota program and one that you can swap the esn number to that of the target phone. I used a samsung i500
2.Target phone that you own
3.qpst and dfs 4.0.0.4 demo
4.drivers for both phones
Pre-Setup:
1.pull battery out of the target phone and LEAVE it out
2.place that esn number on the donor phone
3.Deactivate then reactivate the target phones esn number
4.OTA program the donor phone that has the swapped esn number on it.
5.Verify that 3g speeds are working on the donor phone
Phase 1:
1.Place donor phone in diagnostic mode and plug into computer
2.Open qpst configuration add the port
3.Go to start clients then efs explorer
4.Open the DMU folder and copy the 10.key file to your computer then exit qpst
Phase 2:
1.Open DFS and select the port that the phone is on
2.Send the spc you should see a device unlocked message in the log.
3.Go to the programming tab then the nam tab and read that page.
4.Read the Data tab and Mobile IP tab as well.
5.Do not close the dfs tab
6.Revert esn back to normal or smash the donor phone into a lot of pieces
Final Phase: (Non-Verizon and Sprint)
1.Power on target phone and place in diagnostic mode and plug into computer
2.Open qpst configuration add the port
3.Go to start clients then efs explorer
4.Now on the Left side of the efs explorer right click the screen go to new and directory
5.Name the directory "DMU" without the quotation marks then place the 10.key file into that directory by drag and drop then exit qpst
6.Now go back into dfs do not add the target phone yet and go to mobile ip nothing else on the screen should change
7.Now select the port the target phone is on in dfs and send the spc code
8.Write the nam tab to target phone after verifing it doesnt say home only then go to data tab and check the box next to pwd on the ppp section only which will change the pwd to hex format then write the data tab
9.Go to the mobile ip tab and right click the red stop sign looking thing on the second bubble and enable it and set active profile to 1 then click "write current profile settings" and click the write button on the left side of the screen
10.Now go back to the Nam tab and load the prl file I prefer using 00002 but you can experiment
Final Phase: (Verizon 4gLTE)
1.Power on target phone and place in diagnostic mode and plug into computer
2.Open qpst configuration add the port
3.Go to start clients then efs explorer
4.Now on the Left side of the efs explorer right click the screen go to new and directory
5.Name the directory "DMU" without the quotation marks then place the 10.key file into that directory by drag and drop then exit qpst
6.Now go back into dfs do not add the target phone yet
7.Now select the port the target phone is on in dfs and send the spc code

Having issues try getting 1x first on lte phones you will need a sim card unactivated by verizon and it should pop up after a minute or two should never take longer than 15 minutes the first time
make sure you have it set to nv only on lte handsets.
Need a guide for a specific model name it I'll write it for you step by step for a small fee.
Note I have not tested this on any apple product so cannot support them.
Between getting 1x and then going to target phone must be on airplane mode after esn on account is switched back

ANOTHER EDIT : SOMETHING I FORGOT ABOUT YOUR MMS
Steps to MMS (picture mail) configuration: This is only one way, but it works...
1. Open the messaging app and press menu and select settings.
2. Uncheck EVERY item in the settings and exit the app with the back button.
3. Open your browser and go to the URL "mms.ihavegreat.info" and download
4. Pull down the top of your screen and wait for the app to finish downloading
5. Tap the app to install and let it authorize and setup
6. Remove the stock messaging app from homescreen and replace with this new one.....(may also disable the original message app)
7. Reboot the phone and you should have working MMS

I already have root and have tried idones app to unlock, but for some reason, the unlock seems not to be working. The app says the phone is unlocked but when I insert a GSM sim card into the phone, it doesn't get any signal and the hands free activation loops. I was advised to go into settings -> mobile networks -> GSM, but the mobile networks setting disappears when I put in a GSM sim. It comes back if I put my Sprint sim is put back in. I also select GSM on that while my Sprint sim is in but when I take it out, that menu disappears. Is my phone unlocked or is it still locked? If its still locked, what steps can I take to unlock it given my current build and android version?
Idone should b able to help . Go to dialer, dial *#0011# ,
then in service mode press menu > back , then press menu > key input and type "Q" and press enter.
Then press menu > key input and type "0000" thats zero , 4 times .

- Band Selection
* Make these selections above
* *Make sure you use the more button in the upper right corner to go back if needed(Back Key on phone will exit menu)

* Reboot
* Go to settings/Wireless & networks/more and it will allow you to add the correct APN
* Change preferred network type to LTE

After all those steps, the Service Mode Menu still looks the same, how do I get to the "UE Settings and Info"? It's not in my settings menu

Back on the S8 I was able to add "dun" to the end of the apn data profile for tethering but they are grayed out on the S9. I tried shortcut master and while I can get to it I just can't edit it. Anyone know of a way to do so? I'd like to have unlimited tethering and also if there's a way to turn off the throttling that would be great as well.

Nothing yet. As far as I know, you could flash the U firmware and that would allow it. I have been looking through some adb commands as well, trying to see if we could do it that way.
I went to add one and was finally able to select that one I added and I made sure to put the "dun" at the end and so far so good

How are you trying to edit the value? I was able to change it on my Sprint S9+ using Shortcut Master Lite.
There's a thread in the 9+ forum where one person said he had to try several times (using SML) before it actually worked.

I actually have it working now, using a modified version of the ABD command.

2) Edit
3) Sprint MSL for mine is 000000
4) APN Settings
5a) "APN" and "APN protocol" should be editable. Make a minor change, save your APN.
5b) Then change them back, save the APN again. Do this for each APN you want full access to.
6) Open the Shortcut Master Lite and open LTEFunctionalityTest
7) Open LTE APN Setting
8) If it's greyed out, close everything and then restart from #6.
9) Edit APN Type, add ",dun" to the end.

I had to do this a third time after Factory Resetting my phone, the second set of steps let me do this very quickly.
Do not update your PRL or Profile in System Update, this will wipe out your changes.
This..............
1) Dialer App, ##3282# (DATA)
2) Edit
3) Sprint MSL for mine is 000000
4) APN Settings

Trying to get hotspot bypass working... PDAnet works, but is spotty. I have edited my APN's successfully (I had to rename before I could edit). As well as added the ADB commands. I sort of have hotspot working...
When I disable M.data and enable hotspot AND am connected to WiFi, it enables and allows me to re-enable M.data. Hotpot will continue to work indefinately.... until I leave the WiFi connection. Then the Sprint M.data connects and tells me i am not subscribed to Hotspot service. If I am not in a WiFi area when I re-enable M.data, then it instantly goes to the hotspot subscription error.
EDIT: I just reconnected my phone to check settings in adb. The settings were reset to defaults:
tether_dun_required is '1'
tether_dun_apm is 'null'
If you turn on tethering while connected to wifi, you're not really tethering. You're just sharing the WiFi connection.

I could not share the WiFi connection before I added dun to the APNs.
The real problem appears to be that the ADB settings edits are being reset somehow.
not working with Android 9 (PIE) update
Has any been able to figure out how to open the com.test.LTEfunctionality on Android Pie
I was able to edit the apn before using Quick Shortcut maker but now the com.test.LTEfunctionality cannot be opened.
Any update?
has anybody informed the developer of the app that it needs to be updated for PIE?
Apn settings greyed out
Turn off mobile network and connect to a wifi network(i used my printer wifi network which doesnt internet . I dont know if u can use any wifi or not) i restarted my phone a used mastershortcut and apn is editable again
