I want to return my at&t m8 back to stock (currently on stock rooted with one bad mod installed) so I can give the phone to my father. I dont have S-OFF and dont care about resetting the tamper flag. I have tried many methods to do so, the first being fastboot flashing the stock recovery, then fastboot oem lock, rebooting bootloader, then running the AT&T RUU. After i tried this several times, i noticed that the stock recovery never gets flashed even though it reports 'okay':

Fastboot reboot-bootloader (both the cli command and if you go select reboot fastboot on the phone) also doesn't work, it reboots the phone normally.
Seems weird that the flash doesn't happen and that reboot-bootloader doesnt work, but nonetheless i try another method restoring a stock nandroid from http://forum.xda-developers.com/showthread.php?t=2701376 . I followed the steps, fastboot erase cache --> fastboot flash boot boot.emmc.win --> fastboot erase cache --> reboot to recovery and restore nandroid --> reboot phone. ANNDDD.... somehow I'm still on the same System/ROM as I was before all this. Also worth noting, any files I push with adb are gone after a reboot. Not sure what is going on but any help would be appreciated.
So you have root permission right ?
Try downloading the stock recovery to the device and flash it with an app called "Flashify" or sense you know how to fastboot maybe try the "dd" method using Terminal emulator to push the stock recovery assuming you know the block where recovery lives [ mmcblk0p43 ]

Yes I have root.
Unfortunately anything that has to be done from the phone that isn't in the boot loader or recovery, is not an option at this point because it's in a boot loop (makes it to lock screen and then reboots). Sometimes it stops the loops and works for awhile, so if it does I'll give this a shot. However, is there any other method via adb/Fastboot over USB or TWRP?
Do you get the phone's ID (don't post it, just say yes or no) in response when you type: fastboot devices
If not, you have a fastboot connectivity issue. Try to re-install HTC Sync and HTC drivers.
You don't need to flash stock recovery to RUU. I think you might be mixing up your guides/methods. But if you don't have fastboot connectivity (which I suspect is the issue), you won't be able to relock the bootloader and RUU, anyway.

But again, you need to relock the bootloader first.

Device id shows when i run fastboot devices . I am Positive the fastboot connectivity is not the issue here. So i am attempting to use the bootalble RUU. Since I am S-On though, I have to relock the bootloader first. When I run fastboot oem lock I get this:

This is very common, usually caused by older TWRP versions. Shouldn't effect anything you are trying to do, I don't think.
Its odd that fastboot devices works, but fastboot oem lock fails.
Try re-installing HTC drivers, and maybe re-installing your fastboot/adb package.

Tried reinstalling drivers and fastboot/adb AND tried installing it all on a Windows box (im on a mac) and the result is the same. I think its safe to rule out an issue with fastboot/adb connection. Any other ideas?

To clarify, I have tried this both natively on a Mac and on a PC.

I don't think you can flash recoveries with the bootloader locked if its locking when you fastboot oem lock,I do believe you can lock the bootloader with a custom recovery flashed before hand.

Every fastboot reboot-bootloader booted the phone normally. Even tried the "reboot fastboot" option via the device. I still did the steps as you said despite this abnormality, and I ended up in TWRP still. somehow. Tried to wipe the System, Dalvik, Cache, and Data 5 times in TWRP... the phone still boots to its old system, and all the files reappear. I also tried reflashing Chainfire's SuperSU. It reported successful, tried all the fastboot and TWRP stuff again, and the same issue prevails.
Oh, and I have tried multiple USB cables for the hell of it.

K you said you use a type of virtual machine right.
Try installing adb/fastboot directly to your Mac and run the fastboot-mac commands and hope for a better outcome

No, I dont use a virtual machine. I have adb/fastboot installed directly on my mac, and its failed. I also installed adb/fastboot on another computer I have, which runs Windows 8.1, and it fails with the same errors. I could see why you thought I meant virtual machine, so I added an edit to that post. But to clarify, I am not using a virtual machine.

Strange, fastboot problems are most often a driver issue. But I agree, if you've tried it natively on Mac and PC, it would seem that drivers is not the cause.
Out of curiosity, what ROM is running on the phone currently?

I am trying to check to see if maybe fast boot is on. I suppose it is a possibility, now I just need the phone to stay on long enough to check.

Running stock rooted, with one mod installed (wifi tethering). It is the 50% of the problem because it is causing the reboot loop i mentioned earlier.
******* Update **********
"Fast Boot" is turned on in battery/power settings. However, I turned it off, rebooted, and its back on. Tried multiple times and it won't stick. Thanks to all sticking with helping me out here, even as we approach 3 pages

Are you absolutely sure the bootloader is currently unlocked and you didn't previously re lock it, does it state so in the bootloader screen?
If you can still get into twrp then try flashing a nandroid back to stock from the stock backup thread. I'd also be trying a different cable regardless of whether you think the one you're using is okay.

Bootloader says unlocked... so I assume it is unlocked. I suppose I could go through the unlock process again, but it definitely says "unlocked". Also TWRP flashing/restoring/anything does absolutely nothing. And I have tried 3 cables and 2 computers.
*** CLARIFICATION ****
I can get into TWRP fine, and I can restore, flash ROMS, whatever... BUT even though they report okay with 0 errors, everything stays the same, i.e. nothing in TWRP actually works. Same with Fastboot.

I have flashed the eUI 5.8.19S ROM properly and it was working fine. After that , I might have done mistake with TWRP .
Now , I am in bootloop .
My phone is recognized in fastboot mode.
I am able to flash recovery image also ,
But , after that when I am trying to reboot fastboot is giving me error message in phone :

After getting the above error for couple of times , I have flashed the boot image by

Hi,
TL;DR: Bootloader unlocked. "fastboot boot twrp.img" worked fine until I upgraded to OxygenOS 5.1.4 (from 5.0.4). Now "fastboot boot twrp.img" just does nothing.
I had a OxygenOS system (Android 8.0, I believe it was OOS 5.0.4), rooted with Magisk. Everything was fine. Stock recovery, I wanted to change it as little as possible. I could use twrp-3.2.1-x_blu_spark_v8.70-op5_op5t.img perfectly, via "flashboot boot /path/to/img".
Today I decided to upgrade. I had an OTA notification for months, for OOS 5.1.4 (Android 8.1). I made a full backup with TWRP. Read about latest ROMs, seems 5.1.6 has some Bluetooth issues.
I opened the 5.1.4 OTA notification, curious to read the Changelog. A message said I had a rooted phone, and the update would install a full version instead of the OTA. Also, I'd lose root. Nothing I didn't expect, but I still wanted to flash with TWRP. Unfortunately, the updater had already downloaded the ROM and suddenly restarted. I did not want to interrupt the update, so let it go through.
Phone booted on OOS 5.1.4 without problems. Unrooted, of course. I went into fastboot mode, and tried loading TWRP with "flashboot boot /path/to/twrp-3.2.1-x_blu_spark_v8.70-op5_op5t.img". The image gets downloaded, and boot takes about 10s (most of which is a warning message that my bootloader is unlocked). After that, I just drop back to the 1+ bootloader screen. If I then try repeating the same "flashboot boot ..." command, I get the error "FAILED (remote: 'Failed to load/authenticate boot image: 00000050')".
I tried:
Several TWRP versions, from Blu_Spark, Codeworkx and siankatabg. Don't remember all of them, but at least the latest ones, i.e. twrp-3.2.3-x_blu_spark_v9.85_treble-op5_op5t.img, twrp-3.2.3-0-20180822-codeworkx-dumpling.img and TWRP_3.2.3-Pie_siankatabg_OP5.img.
Removing the screen pattern, lock screen None.
Upgrading to OOS 5.1.6 via stock recovery.
Different USB cable.
I can boot the phone to the OS just fine. But without TWRP I cannot flash Magisk, to root the phone. I have not tried flashing TWRP to recovery, because at least I know stock recovery works and don't want to lose that.
This is what I have from device-info (did not change from before the upgrade, when TWRP was working):

This is how I'm trying to boot TWRP. From the OS, I hold the power button, press Reboot, then choose Bootloader (I have advanced reboot enabled). That takes me to a pretty screen with a menu and options to start the OS, boot to recovery, or restart bootloader. I can select using the volume up and volume down keys, but I just ignore the options and type the fastboot commands:

The 10s is basically the Android warning message saying the boot loader is unlocked and the data cannot be trusted. I get that when booting to normal OS as well. After that warning goes away, instead of getting TWRP, I just get a black screen with "1+ Bootloader".
In that screen, I can still type fastboot commands, but now I have a different error:

I'm at a loss here. Can someone help? I've considered trying out the RedWolf alternative thing, but am worried it might mess with encryption or something. I could flash TWRP, but really worry that 1) if it doesn't work, I won't have any recovery at all, and 2) I'd like to keep the stock recovery as a fallback.
This could perhaps be more of a question for the developers? I posted here because I was not allowed to post in the "OnePlus 5T ROMs, Kernels, Recoveries, & Other Development" section.
I am new to flashing custom roms, and I got lineage running but I wanted to try Evo X, when I tried flashing through lineage Recovery it booted back into lineage, ok, so I erased all datat through lineage recovery and installed TWRP. I got that on but within a few clicks of messing around I found myself with no recovery or OS and im only stuck in fastboot, I have no idea how to get out of it. mods take this post down if posted in the wrong community. I just though this would be the best place to post it. I am new to XDA, I have these ADB logs from the command I was trying to run

Try the following command :
fastboot flash boot lineage-recovery.img
Once that is done, boot to recovery from fastboot and try flashing the rom you were trying to.

I guess you should try msm tool then
Go for msm tool. You can find a tutorial for 8pro on YouTube. Follow the same steps( obviously with the correct op9 pro firmware.

I have gotten msm tool and my firmware, but nothing is showing up in msm when my device is connected
ive linked a screenshot of what my msm tool looks like, im just so desperate to get my phone back
Power off phone, position mouse cursor o er start button on msm tool, hold down vol up and down simultaneously, plug phone into computer wait a quick second and press start. That should work.

Instead of going for msm tool, when your stuck in fastboot, after successfully flashing an .img (Twrp, losrecovery ) use:
"fastboot --set-active=b" (when you're active in slot a)
Then reboot to recovery.
Basically it flashes okay but to the other slot. Happened to me too. I am now booted into the system on slot b. Hope it helps

Lol, i'm not that old found the answer myself in a forum after 2 days of being stuck in fastboot.

I think you should start over and use "fastboot flashing unlock" instead of fastboot oem unlock.
Follow the link in that thread. Droidwin has the best tutorials for oneplus you'll find. Never had this error myself, so can't say for sure what to do.
Redmi note 8 (unlocked bootloader) stuck on "Your system has been destroyed" (I was trying to install PixelExperience 12, but was not able to boot into the PitchBlack recovery itself). Unable to boot to recovery or fastboot (no button combination is working).
If connected via USB (while just charging too), it just keeps rebooting & shows "Your system has been destroyed".
When disconnected from external power supply, it shuts down automatically after a few seconds, but when I try to boot into the bootloader (or try to turn it on), it again goes to the system destroyed screen.
Here are the set of commands I ran just after unlocking the bootloader:

What I believe is that I forgot to disable vbeta verification before flashing the new recovery (when I tried flashing a new recovery in my POCO, I got an error regarding this, but this time none of that happened). Is there any way to do it now? without having to go into EDL mode & stuff?

to avoid problems on ginkgo you have to flash VBMETA then TWRP then the recovery you want
now try to flash the attached VBMETA using :
fastboot --disable-verity --disable-verification flash vbmeta vbmeta.img
it will fix "System destroyed", if not you have to use EDL

Umm.. that's where the problem is for me, I am unable to boot into fastboot mode. It juts goes back to the system destroyed screen no matter what button combination I try .

did you try the comand while "system destroyed" is displayed ?
No.. but I tried connecting using adb and it did not work. I think I event tried "fastboot devices" which too didn't show anything .
But I'll try again now to confirm this.

Thank you @loopypalm , I was able to boot into fastboot somehow. Now the device is working fine on MIUI 12.

I think you should try some custom roms
miui is not the optimal choice unfortunately ...

It wasn't my phone so just bringing it back to working condition is pretty awesome for me . On my phone I keep trying new ROMS (just tried Projet Elixir)

Just keep trying pressing the fastboot button combination.. It took me around 2 days

same here, it's not booting to fastboot even tho I tried the combination when "Your system has been destroyed" was displayed ;-;

I got it to working condition this way only, unfortunately.
