After installing a system update phone stayed on recovey boot. can't boot on normal. Tried wipe and restore did not help. Then make a factory reset. Now says "Your device does not appear to be rooted. Install Super SU now? This will root your device." Still looped on recovery screen... What can I do?
never take an ota with custom recovery. But since is sounds like you did that..what you will be easiest to do (if you can get into download mode), is tot the phone back to stock.

I tried to factory format my phone through TWRP, then when I tried to restart again, it never comes back.
I still have access to TWRP, but when I install zip files I downloaded, it always failed. I tried to use Odin, it failed to flash an image in.
Plz help me.

What format option did you select in TWRP and hope you didn't accidently remove your OS? The device could fail to boot if you don't have an OS installed. You may be able to flash a complete stock restore image via TWRP. Would you recall what Android version you were on before the brick? Also what is your phone model and carrier?

Mine was ATT S3 I747 with Android 4.3 system. I did wipe my OS during recovery. That caused the whole problem. Then I use TWRP to flash a system and a kernel in. I was unable to flash bootloader and modem in; I did not know what is the problem. The weird thing is that I successfully start my phone and used it for a while. When I rebooted it, it dies again. I tried to install the some boot update in this January. It never works again. Any advise?

You can try flashing a complete stock system to get your phone back to working condition. Since you already had 4.3, flashing zips often fails if there is a bootloader incompatibility. The stock restore should do it. Please go to the following thread ....

Download the stock restore zip to your PC. Use another device or something to load it to a (micro) SD card. Insert the card into your phone. Boot into recovery mode and flash from TWRP. When done, wipe your cache/dalvik and reboot to system. Let's know if this works for you.
So two days ago I thought about installing Vision X rom on my Note 3 (T Mobile).
I downloaded the ROM and then backed up my current ROM (nandroid). I had Phillz touch recovery then.
Then I thought about going to TWRP and thus I downloaded the latest zip for my phone and flashed it via Phillz upon trying to reboot into recovery it got stuck in the screen "Rebooting Recovery...".
When I restarted it would still try to reboot into recovery and get stuck. I got out of that by going into download mode and then selecting "Restart Phone".
After that I looked for solutions and upon failing to find any I thought about going back to stock and starting from ground zero. I managed to flash the latest stock using Odin however I forgot to do a factory reset before hand.
When I tried to do a factory reset in my, now stock, phone it failed and then got stuck trying to get into recovery. It would flash "Rebooting Recovery..." then restart and flash that in a loop.
I have lost my root as well in the process and trying to root using Odin and CF-Root just get's me back in the recovery boot loop. I can use the phone OK however this is not a stable situation.
I'm sure I've made some serious errors in the tinkering and would like to get my recovery back. I really love Custom Roms. Any kind and knowledgeable souls out there to help me in my cause?

Go to the Playstore and download TWRP Coordinator and then
use it to install the latest TWRP recovery for your phone.
Once you can get back into TWRP recovery you need to flash
the cf-autoroot for your phone. Be sure you flash the CORRECT
version of cf-autoroot for your phone's model.

The app is constantly crashing!
It would start for a second then close. I tried the TWRP manager as well with the same result.
I've lost root access when I Odined back to stock, won't I need root access to install new recoveries?
Sounds like you flashed the stock 4.4 firmware and now need recovery to root. Flash this TWRP file using Odin http://goo.im/devs/OpenRecovery/hltetmo/openrecovery-twrp-2.7.0.1-hltetmo-4.4.img.tar
Once you flash it, you need to boot into recovery to install supersu to root.
Switch off the Galaxy Note 3 (remove the battery and reinsert if needed)
Next, press and hold simultaneously Volume Up, Home and Power.Release the buttons when the screen flickers and wait for the TWRP menu.
Select Reboot, then System and install Supersu for root.
Once booted, open Supersu to update it and grant permission.

I have a Canadian HTC m9 , bootloader unlock, Twrp , rooted.
i was going to do a OTA to get the fix for the camera and heat issue. however on the process to revert back i end up stuck on the HTC splash screen with recover twrp acess .
what i did is did flash my backup file before the root and tried flashing stock recovery file. but no luck.
i tried flashing again the twrp and still no luck. and everytime i tried to reboot it shows NO OS installed.
any suggestion on what i can do to fix it? im kind new in HTC, i read upon the forum and not sure where to get RUU or the right ROM or if i should flash does stuff..... can i get some help?
I would suggest you flash the latest recovery through fastboot (2.8.6.4) from here https://basketbuild.com/devs/Captain_Throwback/One (M9)/Recovery and try to flash a custom rom or run an RUU if there is any for your variant

push via adb a rom.zip to your sdcard and flash it via twrp recovery
yes it did fix it, i was able to boot the phone up again however, can i still flash my original back up so that i can get OTA? i tried using Venon ROM and according to them i can get the OTA but when i tried to update it no update available and i know there is one. i check my build # it still the old one, 1.32.401.17 CL506785 release key.
any idea?

Yep and if it works flash blu-spark twrp it should be fine.
Yeah, I went through a lot of steps, but basically install stock recovery v2 and let it restart. Once it succeeds or fails, it will go back to normal, and you can flash whichever recovery/rom combination you prefer. I don't have the official url, but this one seems legit:

You should never install an update through the phone's update setting while using a custom recovery. The updates are designed to work with the stock recovery only.
If you can still boot into and use TWRP, use it to flash the full ROM of OOS 4.0.3.
If you can't boot into TWRP, try to see if fastboot works and manually sideload a working ROM in.
Otherwise, you're going to have to use the unbrick guide. Your phone doesn't have to be completely bricked to use it. It simply reverts the phone to a truly factory-fresh state (relocks bootloader, wipes everything, etc. - At least method 2 does anyways).
I got stuck in the same situation some time ago, and in order to solve it I had to flash the stock recovery, let it restart, it tried to update (and failed) and then it booted to system again. Hopefully that way you can avoid installing everything again. Once the device boots you can try to update with the stock recovery and revert to twrp if you want.
