So I was looking into roms and I downloaded ROM manager 3.0.1.1. Thats when my trouble started. I did a back up with the rom manager and it wiped my old nandroid back ups and I was stuck in bootloader/recovery and couldn't restore my back up. I do an nandroid back up about once a month and they were all gone. I did back up my sd card in december and I had an old back up on it and I flashed it and it worked.
Tonight I tired to flash an older version of clockwork (like I had before) and I'm stuck at the bootloader/recovery again. I tried to restore an old nandroid and nothing seems to work.
I'm open to suggestions.
Should I try to flash a new rom and start over ?
any way to get my old customized rom back ? (I have the clockwork back ups)

I haven't tried a different rom, but I cannot boot my current or older.
It can't load PM36IMG.zip, I tried loading one onto my card but it just aborts the installation. Is there a more current or older one one I need ?
that might be a problem. you sayed you were trying to put a PM36IMG.ZIP on but you need a PC36IMG.ZIP i believe if your on an evo. use amon-ra and and flash a new rom and see if it works. copy and paste this link into your url and it will start a download for amon-ra recovery. hope this works

also don't try to flash a clockwork nandroid backup with amon-ra or vise versa. it will mess up your phone from what ive heard
I wonder if it does not like your sd. I am thinking try a different sd or see if you can format yours.
I also do not like ROM managers. They always seem to want an upgrade and then when you do the upgrade your old backups may or may not be compatible any more. To get your old file back you would probably have to run an unerase or some type of debug program on the sd card. erased files are not really erased they just have an F5 in front of them to tell the computer that the space is availible. If you have a backup on your compter or old SD I think you should be able to that.
I did check for errors on my SD card, nothing bad there.
I put a PC36IMG.zip on my SD and when I try to install the zip from my sd it says operation aborted.
I think my entire problem is the fact that I have clockworkMod recovery v3.0.0.5. I've read (after) that anything after 2.6 is not going to play nice with my evo.
I tried downloading a stock rooted rom and I cannot flash it. Anytime I boot the phone it stays on the HTC EVO white screen, I can hold down the - and power and I can go into HBOOT/fastboot/bootloader/recovery, then it cannot find the PC36DIAG.zip, and PC36ING, and a few others.

edit: I'll try pulling everything off my card and formatting it...

I loaded a older clockwork recovery then tried to flash a different rom...
it started opening, installing, formatting, deleting, copying, then...

and when I restore a previous nand backup it appears to work then when I reboot the screen looks scrambled and freezes.

Your system can not root since it can not find /system/xbin/su su is not in bin
ln -T /system/xbin/su /system/bin/su would probably be the approach if you were running. Not sure if you could edit line 76 to change bin to xbin, copy a proper symbolic link or?
I looked in bin and there were many links to the tool box that appeared to be associated with error logging. With this in mind it seems to me that this link is part of the recovery/installation process.
I would try to correct the permissions.
I was able to do a hard reset and my phone booted up rooted still..so I restored my apps from Titanium Back up.
Right now I can shut my phone off, restart it, no problem. If I go into recovery, problem.
I need to get rid of clockwork asap, I've read about Amon_ra's recovery but I don't think I have enough knowledge yet on how to install. I've tried installing from a zip but I'm just not doing it right. I can go into folders on my phone (root explorer), should I just delete clockwork anywhere I find it ? I just don't want to go blindly deleting files, that would be dumb.
I need recovery back...this is scary

I have been into Android modding since My G1 from day ONE, I bricked a friends G1 and had to swap internal stuff and fixed it so I have never had a problem I could not solve till now, Like the guy above I cannot boot into my ROM. I have running Myn's Roms since his(their) first release with ZERO problems until RLS5. I started getting random reboots so like any other time when this happened with anyone elses ROM I did the Recovery, Wipe Dalvic Cache, Wipe Cache reboot and that was that, However today my Evo would not stop rebooting every 5-10 minutes so I wiped from scratch even my sd card was wiped, then reflashed Myn's RLS5 got it to boot once and after finishing SETUP it was on for like 2 maybe 3 minutes and again REBOOT!!! this time staying on the white screen, So I pulled the battery and went to recovery and wiped dalvic and cache rebooted and from then on It does not get past the white screen, I can still get into Fastboot and Recovery but it DOES NOT boot into the ROM, I am now Flashing MYN'S Z 2.2 and praying it boots. Do you guys think VIRUSES are now affecting OUR EVO's????
I just need to get rid of clockwork and I'm still stuck on that...

ok, I just want to be clear.
First, I'm sorry if I'm noobish
next, I'm gonna put that PC36IMG on my SD card (just anywhere on the card) then go into the bootloader, choose apply update, then boot into recovery, then done, correct ?
side note, I've read around that it should be put on the root of the card, I don't have a root folder on the card, but there is one in the internal system file (found it in root explorer).
just want to clarify before I loose everything again.
You are correct, and the "root" of the sdcard means to just be on the sdcard, not inside any folders. Also don't extract the zip file.

Would this work for the Incredible as well? I have the same issue (tried to flash clockwork recover but can't get into it on the phone). Also cannot get into the ROM, so stuck at the boot screen and nearly every command brings me back to it...

I have been busy today with a nasty problem, i hope you anyone can help me out.
I was running JDROID's custom rom with great satisfaction, had it backupped. The trouble started when i downloaded an other rom and forgot to place the rom (.zip) on my sdcard before i flashed the boot.img via flashboot.
By that time i didnt knew how to use ADB commands, so i decided that i would forget about the new rom and i would restore my working backup. (im using CWM)
After restoring that backup a bootloop occured, so tried lots of things like wiping the cache. Also ive tried to restore the backup after lots of full wipes ect. Even installing JDROID's latest rom witch i had on my sdcard dindt help. Still had a bootloop.
I managed to use ADB when my phone was switched off, and i pushed a rom.zip to sdcard. But CWM cant seem to find it.
My question is this: in CWM there is an option to "mount USB storage". When i use it i get the message "unable to open ums lunfile". I have tried formatting the sdcard, whereby i deleted my backups still didnt work.
What whould you guys try next?

Let me first apologize for all of the information, but I wanted to give you guys as much to go on as possible.
TLDWR - Can't flash roms, can't decrypt phone, can't wipe data, can't mount internal storage, can't use ODIN....where next?
Here is what I got:

Android vers - 4.2.2
Cyanogenmod vers - 10.1.0-d2spr
Was getting flaky reception when I was on 10.0.0 nightly, decided to update to 10.1.2. Downloaded the the required files through Rom Manager, rebooted into CWM Recovery version 6.0.3.1 (non-touch) and did the install (dirty)....booted up and the ROM itself worked, but all my apps were getting "stopped unexpectedly". I figured, ok, did a dirty update so stuff isn't working; I'll redo it and this time wipe everything (after backup of course). When I went to do this, I noticed that it did not mount my sdcard (internal storage, not external sd card). I thought was odd, tried manually mounting, didn't work, tried rebooting recovery and that didn't work. Thought maybe it was a fluke, tried wiping and reflashing 4.2.2 (CM 10.1.2).....it went very quickly. Rebooted, and all of my stuff is gone, which is good, but now it won't let me restore apps.
At this point, I decided I'd had enough of trying to upgrade, so I decided to downgrade back to my old CM 10.0.0....long story short, the flash went quickly, booted up, things worked, but still had the same problem with the apps. I could see the data on the internal storage when the phone was booted, but it never saw it in recovery mode. After doing some reading, I saw where the filesystem changed when going to 4.2.2 so that you can use the new profiles feature. This seems to be what caused CWM Recovery to not see my internal storage anymore.
I thought well, maybe it's because I am back on 10.0.0, so I tried downloading and flashing 10.1.0. It did the same quick flash and showed after boot that I was running on 10.1.0, but I still couldn't mount internal storage (sdcard) in Recovery. I said screw it, reinstalled my apps and decided to truck on with how it was. Short version here - installed mobileiron and touchdown, ran through the encryption in mobileiron, etc etc...mobileiron didn't recognize touchdown and I could not get LEAP for wifi to work.
Decided I would try to decrypt and completely wipe phone. Removed Mobileiron as administrator, tried to decrypt - could not. Tried tweaking security settings, reboots, adb mount and format - all to no avail. Tried wiping in recovery....this failed because it cannot read my storage (because it can't mount it and it is now encrypted)!
With the phone booted up and running adb shell, I can get to the main file system, but my sdcard, data, data/media directories all look empty. I can't mount sdcard through ADB either.
When all of this failed, I looked to ODIN to fix my woes...no luck here either. Downloaded the stock Sprint ROM (4.1.1 L710VPBLJ7 L710SPRBLJ7 ) and tried flashing it but I get hung up on the "Setup Connection"...in ODIN. I have tried multiple drivers, usb ports, battery pulls, etc...I even tried from another PC.
I know this is a lot of information and I apologize again. If you read through all that, thanks for being thorough...if not, I will answer your questions, but hopefully won't have to repeat a bunch of info again.
My end all questions are - where do I go from here? What other options do I have? Is there any repair place I might be able to take this to that they might be able to decrypt/flash the phone to stock for me?

Where did you download your stock Rom from? Did you FULLY uninstall drivers from Windows, before reinstalling?

Missed your other question the first time I read your post....Yes, I uninstalled the drivers, rebooted, reinstalled the drivers. Tried connecting on another PC as well.
Ok, little bit of an update.
I went through Recovery and was messing around trying to mount and wipe...I couldn't mount data or sdcard, so I figured I couldn't just format them either. WRONG! I was able to format /data and /data/media, which then let me mount /data. After doing this, I rebooted recovery and I could mount sdcard...so I did and formatted it.
Now, I had my old version of CM 10.0.0 on my external sd card, so I installed that and booted up...WOOT - back to blank CM 10.0.0!
Now I've got all my data and apps saved to a folder on my PC, if I can just figure out a way to put them back on the phones storage and restore from my backup...

It's sounds as if you should start a totally clean install, then backup your downloaded apps with Titanium Backup after you get ROM setup again.

I couldn't get my backup to restore correctly....I kept getting an error restoring /data. Luckily I had an older backup from several months further and was able to restore that. After doing so, I had to remove my google accounts, readd them, and reinstall my mobile iron client for work and I am now good to go!
Man, that was a nightmare.....thought I had soft-bricked my phone.

Unfortunately I do have the same issue. The problem is I have not backed this up previously. Any suggestions?
I have a D855K (Telstra Australia). I recently upgrading to 10i, using the firmware: LG-D855 A6KG D85510i 00_6 16GB Hong Kong http://csmgdl.lgmobile.com/dn/downloader.dev?fileKey=FW436606432109876543213/D85510I_00.kdz
Before the upgrade, I backed up in LG PC Suite, then imaged with a full wipe, and then restored the backup via LG PC Suite again.
I now can't take photos or videos with the native camera app (haven't tried any third party apps yet). Photos just fail silently, and videos say "Failure to write". I've done some googling, and people having complained about 4.4.x breaking the ability to write to SD cards, but I don't have an SD card in my device. It's just internal storage. Also note that I haven't rooted the phone since the firmware update.
Has anyone else had this issue, or can provide any advice at all? I'm going to get a third-party camera app to see if that can at least let me take some pics if I need to in the meantime, but I would like a fix not a workaround.

I just installed Camera360 and it has the same issue. With photos, both apps save a thumbnail with nothing in it, then warn that the album has changed and you need to refresh the folder. When I do, the folder is empty.
Initially I was running 10d, then then 10b because of a boot loop issue. So it could be a range of things causing this
The Camera360 app works fine (ie can save pictures) if I change the folder to DCIM, instead of DCIM/Camera.
I have used Purpledrake to root my device, and installed a Terminal app. I can navigate into DCIM, but can not do anything at all with the Camera folder. I can't navigate into it, delete it, chmod or chown it. It appears to have all the same permissions as every other folder under the emulated sdcard, but refuses to do anything. I haven't tried it via adb but I doubt it would make any difference.
Any thoughts would be greatly appreciated.
I managed to run e2fsck, and it appears that my data partition has some corruption. But e2fsck can't fix it unless the partition is unmounted. I set up adb but couldn't get to a point where the partition (/dev/block/platform/msm_sdcc.1/by-name/userdata) was not in use. I booted into recovery (adb reboot --bnr_recovery) and it still mounts the /data partition, and on top of that I don't seem to be able to elevate to root. I am only new to the Android ecosystem, so please forgive any ignorance. My google-fu doesn't appear be very helpful either. Any suggestions welcome.
Have you got the nandroid backup?? If so, How about restoring it back?... Have you tried it?? Hope it works...

I was under the impression I couldn't do a nandroid backup until I had a recovery rom installed, which isn't available for the D855 yet... since the partition appears to be corrupted, I suspect formatting the data partition would solve the problem. But I don't really want to lose the customisations I've done. I wouldn't be surprised if the LG PC Suite backup is like an image of the partition, which means it might restore the corruption if I used it.

Tethered twrp recovery could have you saved your nandroid backup which then you be go back to fully functional status of your phone... Sorry I am new to LG G3 myself coming from galaxy s4.
I looked into those recovery boots but it seemed a bit more complicated than I could be bothered with at the time. The thing is, my phone is still fully operational. The only thing is that the Camera folder appears to be corrupt. Even just changing the path that the default camera app saves to would fix the visible issue, however the corrupt filesystem means I could be in for a lot more pain if I can't sort that out. I'm hoping that someone can point me in the right direction to be able to use e2fsck on the data partition (I think all I need to be able to do is unmount the partition so it's not in use). Otherwise, formatting the data partition will likely be the solution, which I'm trying to avoid.

No expert here but have you tried restore upgrade errors with pc suite. Hopefully will fix your problems and it doesn't wipe your phone. So you will just have to reroot afterwards.

A good suggestion! I tried to use that feature a week or so ago when I have my phone in a reboot loop. Unfortunately it only said "You have the latest version of the software, go away". It was a little politer than that, of course, but my response was not polite at all. I will give it a go again tomorrow when I have access to the same computer I was doing it with before, and see how I go. I'm not hopeful that it will help, but worth a shot.

Was that using restore upgrade errors tho or software update? It worked for me when I was having continuous f/cs when I bootlooped I had to use the flashtool. Can't wait for a custom recovery
It was the Restore Upgrade Errors option. Part of the problem is that there have been no OTA updates for my phone. Checking that XML file at LG shows no firmware at all. I think that cripples the repair functionality a bit.

Ah ok, bummer. How about flashing the kdz again or a different one without wiping, that might do it, its sort of the same process.

Unfortunately I don't think the OS partition is the problem. I don't have force quits or the like happening. It's the data partition that has some corruption. I can run e2fsck in no-change mode and it tells about a range of issues. But I can't fix them unless the partition is not in use. Doing anything that keeps the data partition intact, eg flashing the firmware, is not likely to resolve the problem.
Well, I'm flashing the firmware again. I changed the runtime from Davlik to ART after some reading, and the phone stuck at the LG boot logo. I will not restore the backup I did last week, in the hopes that it doesn't screw up the data partition again. At least I get to have some fun tweaking the OS again I guess...
And I can successfully take photos again!
I am still interested to hear if anyone knows of a non-destructive way to fix this kind of issue. e2fsck is on the device, but I can't seem to run it. I'm using Linux knowledge here, but is there a way to set a dirty flag on the partition so that the OS automatically runs e2fsck on next boot?

May you tell us how did you fixed it?
As per my post directly before the one you quoted, I had to re-flash my phone to recover from changing the runtime from Davlik to ART (it got stuck at the boot logo). I chose not to restore my backup, which may have re-introduced the problem. A full wipe of a phone will fix a lot of problems... but as I said, it would be nice to know of a less destructive way to do it.

Probably a bit late but adding this in case anyone else has the issue.
To fix it on a rooted device, enter TWRP recovery, go to Wipe > Advanced Wipe
Select Internal Storage. and swipe to wipe. (this will delete everything on the Internal "sdcard". That is not apps or settings, but it is saved files and some game files etc.

I was running a RR rom and got a msg about an update for my ROM. So i thought i would try it out. I made a full nandroid back up and after that updated my ROM. After the update my homebutton didnt work anymore. So i thought, lets just restore my back up again. I did a fully factory wipe, i flashed TWRP again on my OP3 because for some reason after the factory wipe i got the Cynogen recovery mode. So i put back TWRP, put my back up on my phone from my laptop, but when i went to "restore" nothing was there. I tried to move my back up from the "sdcard" directory to the root directory but i got an error which didnt specify what the problem was.
Can any1 help me restore my OP3 again?
so when u went to TWRP -> Restore, there was not backup found to restore? but when u check ur phone through file manager its there?

Thats correct. When i go in the file manager, i see the backup in the sdcard directory along side the TWRP directory. I tried to move it with the file manager, but it gave me an error which was not specefied.

thats already mounted under the tab "mount". Or did you mean in the terminal?

ok now i extracted all the files from the tar files, but what is the next step? I tried putting them on my OP3, but that does nothing. My OP3 still doesnt see a back up.
Another thing is something i saw is that in TWRP, when i shut down my OP3, TWRP says that there is no OS installed. Is that normal?
No that is not normal, the extraction is there so u can reinstall things, I will have to take a look at it again, do u know what all partitions u had backed up and did u move the backup location?
Lately backup in twrp recovery sucks.
Yesterday, i want to test another rom. So i wipe cache/dalvik & backup in twrp. I select all except rom img (3gb) and cache (zero). Try the new rom. Does not work for me. So wipe all except sd card & restore my backup.

I use only twrp 3.0.4.1, so it can not be a conflict in different twrp's versions. It's not the first time....
If you wipe youre phone, including sd card, go to recovery, backup and choose the smallest size file to backup. Recovery creates youre backup folder in TWRP directory. After that you can erase the that backup.
I flashed back oxygen (OMG I forgot how ugly oxygen was) last version, so I got a working phone again but ofc without all my media. I just find it weird that I make a full backup (I had everything checked) and I can't do anything with it.
I did full factory reset, so maybe that's the reason my OS got deleted aswell. What partition has the OS installed?
When I reboot my OP3, I get a msg saying that dm-verity has not started in enforcingmode and may not work properly. What does that mean?
